In an update shared by the Calgary Flames, team rehabilitation sports physiotherapist Scott Cyr described Dr. Su as "one of the world's leading specialists" in hip resurfacing surgery.

Just one week after surgery, Huberdeau reported that his recovery was progressing well and that his body was responding positively to treatment.

Resurfacing — an alternative to a full hip replacement that involves shaving damaged bone and cartilage from the femur, capping that bone with metal and popping it back into a lined socket — has high-profile test cases in tennis and pro wrestling.

“I think the thing that got me,” Kane said, “was the fact that Dr. Su believes that I won’t have to retire because of my hip now.”

The belief, held by the player, the team and its medical staff, is still that Kane can be that person, even at 35. It’s shared by Dr. Su, who has performed thousands of hip resurfacings over nearly two decades, including . Success stories are important to him; the procedure accounts for less than 1 percent of all yearly hip replacements in the United States, and he believes that number should be higher, especially, he says, now that lower-quality implants and less committed surgeons are no longer in the mix. 

Kane, in the six months since his surgery, “has reset the whole recovery timeline” — and not just for hockey players.  “(Kane) really is, like, kind of freakish,” Dr. Su told The Athletic. “He’s just blowing it out of the water. Things that we expected to be accomplished by three months, he did in six weeks. Things that we expected in four months, he did at two months. He’s basically accelerated the timeline by almost half.”

The study found that patients with unilateral hip resurfacing arthroplasty had higher cobalt and chromium serum ion levels than those with unilateral ceramic on polyethylene bearings. Additionally, more patients who had a hip resurfacing arthroplasty developed adverse local tissue reactions (ALTR) or metallosis on MRI than did patients who got ceramic on polyethylene bearings.

Dr. Potter said, “While the majority of these high functioning individuals with HRA [hip resurfacing arthroplasty] had no adverse local tissue reaction, longitudinal yearly evaluation disclosed that more patients who received a HRA developed ALTR or metallosis on MRI than patients who had ceramic on polyethylene bearings (hazard ratio 4.8 (95% CI 1.2-18.4); p=0.02).”

Dr. Su explained, “This study highlights the value in using MRI to evaluate patients with metal-on-metal hip resurfacing. It demonstrates that metal levels or clinical performance do not always correlate with MRI findings of adverse local tissue reaction, and therefore all of this information is valuable in the evaluation of such a patient. Just as I would not recommend the sole use of metal ion levels in deciding management of a patient, similarly, I would not utilize MR information alone to make decisions. All of the information—clinical picture, metal ions, and MR information—should be used in concert in managing the patient with a metal-on-metal hip resurfacing."

Brinkley injured her hip in a helicopter crash 26 years ago, and was told 12 years ago that she would require surgery. Due to her busy schedule, Brinkley delayed the procedure until now.

“She had an accident where basically the hip was pulled upon and that probably tore some of the structures and the cartilage in the joint. She was able to maintain herself for years and years and exercise through it but it got to a point where the cartilage wear was too great,” said Dr. Su. He continued, “She started experiencing stiffness, limitations of motion, sudden pain that would come on where she almost would feel like the leg couldn't support her. Because of a little bit of down time during the pandemic, not having her usual schedule, it ended up being a really good time for her to be able to undertake this recovery. She felt comfortable with the hospital, with the protocols we have in place.” Dr. Su added, “Her condition was posttraumatic so she's on the younger side for a hip replacement and making an incredibly fast recovery.”

Thomas' hip was bone-on-bone, resulting in relentless pain and the favoring of his right side which constantly compromised his balance. “Before the surgery, you could see he was clearly favoring his right side; and that it was painful," said Dr. Su. "It was natural to avoid pushing off and landing off [the left]. Four months post-op, we are seeing higher loads and seeing symmetry between the right and left." Dr. Su added, “Now, he's playing like he's eight months out from surgery, not just four. He's such a hard worker. He's moving quickly, and able to jump and pivot."

Dr. Su has routinely executed a hip resurfacing procedure on patients, including professional athletes, but Thomas was a far different case: younger, athletic and trying to restore a level of explosiveness that would allow him to compete with the best athletes in the world. The surgery included a unique kind of artificial joint to account for Thomas' age and need to regain movement. "The goal was to give him a smooth joint surface so he doesn't feel pain and has better mobility," explained Dr. Su. "We have more confidence for athletes like Isaiah to get back to full activity."

Dr. Su explained that HSS has cancelled all non-essential orthopedic surgeries out of an abundance of caution with the goal of ensuring safety for patients and staff.

“It is something that is going to be continually re-evaluated, but the COVID-19 pandemic is certainly going to affect orthopedic care for the next several months, both because of the need to postpone and reschedule operations, but also because its overall impact on the economy. Academic and industry meetings have also been cancelled, so innovations and research in the orthopedic arena may also be delayed,” he added.

Ortho Spine News spoke to Edwin P. Su, MD, hip and knee surgeon at HSS, who is the director of the Adult Reconstruction and Joint Replacement (ARJR) service portion of the lab. Dr. Su noted the lab now has 4 dry/wet stations that can accommodate 4 to 12 trainees. “The facility is open to all residents and is under the guidance of a coordinator who communicates with trainees on the available specimens and times. Trainees may also include physician assistants and nurses who come in for practice on suturing or on the examination of anatomy,” said Dr. Su.

Dr. Su added, “As for residents, they have an anatomical dissection curriculum where they each select a joint or a body part to work on. Afterwards, they present their experiences to other residents. Whether handling power instruments, working with the latest arthroscopic simulators, or trying out different surgical approaches, trainees at our lab have many unique opportunities. They can, for example, view the anatomical structures around the hip joint, perform a surgical approach while looking at the relevant neurovascular structures, and see a true-to-life representation of what they will encounter in the OR.”
