I quickly went from being a recruited athlete to play basketball in college to being unable to walk down the halls at my high school.”</p><p style="text-align:start;">He continued, “I had spine surgery along with restoration of mobility, and over time, was fortunate to resume normal living and return to sports. It was a hallmark moment for me, going from being disabled back to normal. I had been interested in being a doctor for some time but following the accident, I was inspired to help others recover just as I had.                         <title>HSS Surgeon Performs World’s First Implant of a Stemmed Elliptical Head Prosthesis for Anatomic Shoulder Replacement</title>

Blaine, MD</a>, a shoulder and elbow surgeon at HSS has performed the world’s first implantations of a stemmed elliptical head prosthesis for anatomic shoulder replacement surgery. The breakthrough procedure provides the most anatomic solution for patients with arthritis, and was performed in May at HSS in Stamford, CT. HSS has a collaborative relationship with Stamford Health.</p><p>This innovative surgery revolutionizes conventional anatomical stemmed total shoulder replacement, using an elliptical-shaped head which is anatomically matched to the ovoid shape of the ball-and-socket joint in the human shoulder. The stemmed elliptical head enables surgeons to readily convert an anatomic total shoulder replacement prosthesis to a reverse shoulder replacement prosthesis, should a revision surgery ever be needed in the future. Dr. Blaine is the first surgeon in the world to use this advanced technology, placing an elliptical head on a stemmed implant.</p><p>Anatomic total shoulder replacement arthroplasty is often advised for patients with osteoarthritis. This surgery replaces the ball and socket with artificial components, allowing younger patients to stay active and play sports. Reverse shoulder replacement is advised when patients have both arthritis and a deficient rotator cuff. The natural positions of the ball and socket are switched. <span style="background-color:white;">A metal ball implant is placed where the patient's own natural socket was, and a plastic socket implant is placed on the top of the humeral head.</span></p><p>For the past 50 years, since they were first introduced by Dr. Charles Neer in the 1970s, the heads of the prostheses used in shoulder replacement surgeries have been spherically shaped. In the last several years however, two manufacturers have introduced elliptical heads which may improve range of motion and healthy shoulder functionality over spherical heads, according to some biomechanical and clinical studies.</p><p>“The main reason for implanting a convertible prosthesis in a patient’s shoulder is that an anatomic shoulder replacement provides a more normal shoulder than a reverse prosthesis in active patients.<span>&nbsp; </span>However, if a patient ever had an injury or tear of the rotator cuff in the future, we now have better options with a stemmed prosthesis,” explains Dr. Blaine. “Up until now, we haven’t had convertibility from a standard ball-and-socket prosthesis to a reverse prosthesis. The stemmed elliptical head prosthesis for anatomic replacement gives us the ability to make this conversion in a more expedient way without having to remove the stem.”</p><p>“One of our primary goals at HSS is to reproduce or recapitulate the human anatomy,” Dr. Blaine adds. “The normal anatomy of the shoulder joint is an egg-shaped head, not a sphere. This new approach allows us to get closer to replicating the normal function of the human shoulder so patients can remain active well into their later years.”</p><p>These first procedures using elliptical head prostheses at HSS Stamford have been successful, and the technology is now being used at multiple centers throughout the U.S. Dr. Blaine also reports that HSS will soon be conducting a motion analysis study in conjunction with other institutions to compare shoulder replacement patients who received oval-shaped heads versus those who received spherical heads.</p>]]></description><category><![CDATA[pressrelease,Blaine,shoulder-replacement,Innovation,Surgery,Stamford Health,Stamford]]></category>

I am working with a team of HSS hip and knee surgeons and our biomechanics department to run computer simulations of hip and knee replacement. These models help us determine individualized patient targets for ligament tensioning and balancing precisely for knee replacement. Our robotic and computer navigation technologies allow us to execute surgical plans and target specific alignment and balance of the knee with great precision."</span></p><p><span style="background-color:rgb(255,255,255);color:#000000;"><span style="text-align:start;">Dr. Kahlenberg added, We’ve made great strides in speeding up patient recovery from joint replacement surgery. Also, new surgical techniques and the use of regional anesthesia instead of general anesthesia lowers the risk of complications and side effects. We like to get patients up and moving right after surgery; most can go home the same day or after a one-night stay. As a bone trauma specialist at HSS, I specialize in injuries involving fractures from accidents, falls and other traumatic events.”<br><br>He continued, “Given that most traumatic injuries are unforeseen events, they can happen to anyone at any time. We see these injuries most often among patients in two age categories. The first spike occurs among those aged 15 to 30, the most active age segment. These tend to be higher energy injuries, more common among men, who tend to take greater risks which can lead to injuries. The second spike is among those 60 to 80 years old. As you might guess, this group is more prone to loss of balance and falls, often resulting in hip and other fractures.”<br><br>According to Dr. Grose, “What I enjoy most about my job is seeing patients that I’ve treated for severe injuries return to my office months later in an improved state, with increased mobility. I get as much joy in dealing with the onslaught of elderly patients who have fallen, helping families navigate mom’s or dad’s situation. We all fall apart at some point; it’s the human condition. Blatz, MD</a>, physiatrist at HSS, and <strong>John Giametta</strong>, physical therapist with HSS Sports Rehab at Chelsea Piers in Stamford.<br><br>According to Dr. Blatz, “If you’ve been relatively inactive for a period of time, and we’ve all been hibernating over the winter, you really want to get out there and run around on the first day.”<br><br>He added, “But a lot of times what people do is they overdo it, especially on that first day or week or even month.”<br><br>“One of the main things is just trying to slowly ramp up your activity regimen. So, if you were running five miles a day in the fall but stopped in the winter, you wouldn’t want to start running as far right from the get-go,” Dr. Blatz explained.<br><br>According to Dr. Blatz, the necessity of warm-ups and cool downs stem from tissues throughout the body taking time to adapt to changing conditions. Tissues such as muscles or the cardiovascular system, which haven’t been used in a while, will take a while to become “perfused” with good blood flow and ready supply of nutrients.<br><br>Giametta agreed with the necessity of warming up before exercises as a way to avoid injury and ensure health.<br><br>He said, “People always ask me what kind of warm up to do, and what I usually recommend is some sort of dynamic warm-up that kind of simulates what you’re going to do.”<br><br>According to Giametta, passive stretching before activities is no longer recommended, and Dr. Blatz explained that an ideal warm-up should be rigorous enough to break a light sweat.<br><br>Dr. Blatz underscored, “If you start to modify your gait while running or modify your stroke while swimming because of a pain you should stop.”<br><br>“If you start to limp because of discomfort, you should call it quits because that’s going to throw off your entire biomechanics and could cause a separate injury,” he noted.<br><br>Read the full article at <a href="https://westfaironline.com/146423/from-desk-jockey-to-weekend-athlete-preparing-for-a-new-sport-regimen/">Westfaironline.com</a>. In addition to injuries, pain is common among aging athletes and from degenerative joints and tendons, but it isn’t normal and should be addressed.”<br><br>He continued, “When we age we find muscles and joints become stiffer and lose elasticity in tendons which can lead to higher injury rates. I usually tell people that normal stiffness lasts for no longer than 15-20 minutes and goes away once they have loosened up. Persistent stiffness, that lasts for more than an hour in the morning, is more of a concern because it can be correlated to an inflammatory issue. Swelling, redness or warmth of joints are all also things to look out for that may be signs of a problem.”<br><br>“Loss of strength, function or the inability to put weight on joints means you want to see a doctor right away. Any injury with symptoms and pain that is getting worse after a few days you would also want to have looked at,” Dr. Sgrignoli explained.<br><br>Dr. Sgrignoli recommended incorporating a dynamic warm-up before exercise. He noted, “You are more likely to get hurt by jumping right into a sport with a cold, tight muscle. Any amount of stretching is most effective when your muscles are warm. Immediate symptoms are pain and swelling in the knee. In most cases, we recommend surgery as the best way to stabilize the knee joint and resume an active lifestyle.”<br><br>For meniscus tears, “There are two menisci inside each knee joint. They are soft cushions on the inside of the knees that protect the cartilage. Meniscus tears are very common with all types of activity. A torn meniscus can be categorized as traumatic (acute) or degenerative (associated with aging or wear and tear).”<br><br>On the topic of chronic knee pain, she explained, “Many of the patients we see at HSS with knee pain are suffering early arthritis, which we also call cartilage softening or thinning. Sandhu, MD, MBA</a>, co-chief of HSS Spine.</span></span></p><p><span><span>Dr. Sandhu explained by using specialized goggles, he is able to see through tissue surrounding a patient&rsquo;s spine and provide more precise and faster surgeries and, by extension, quicker recovery with less chance of infection.</span></span></p><p><span><span>He cited the technology allows the surgeon to see an overlay of data pulled from MRIs [magnetic resonance imaging] and other technology superimposed on the patient&rsquo;s spine, as well as written data visible in the corners of his visual field. He said the visible terrain means he can pinpoint areas without having to expose more markers along the spinal landscape, as is the case in more traditional surgery.</span></span></p><p><span><span>Dr. Sandhu saw HSS patient Paul Solis-Cohen and determined he needed lumbar decompression surgery involving a laminectomy and spinal fusion of the L4 and L5 vertebrae.</span></span></p><p><span><span>&ldquo;I went into the OR [operating room] with pain down my leg and I was wheeled out pain free,&rdquo; noted Solis-Cohen.</span></span></p><p><span><span>HSS patient Brynn Blystone, who was living with degenerative disc disease, underscored the success of her surgery.</span></span></p><p><span><span>&ldquo;He [Dr. Sandhu] broke things down and told me what was going on and how he could attack it. He landed parts in <i>A Chorus Line</i> and <i>Jerome Robbins&rsquo; Broadway</i>. While he was performing, he realized he wanted to take new path and pursue medicine.</span></span></p><p><span><span>In between his shows, Dr. Grose said he began watching City College of New York classes on cable TV. &ldquo;Tuesdays were science day and I found myself watching chemistry between shows. I thought, this is kind of weird,&rdquo; he noted.</span></span></p><p><span><span>After returning to Syracuse to teach a workshop, he took a biology class and orthopedic elective that sealed his fate to becoming an orthopedic surgeon.</span></span></p><p><span><span>Dr. Grose cited medicine and music are not that different. &ldquo;Doing a joint replacement,&rdquo; he said, &ldquo;is like playing something classical. There&rsquo;s a step to everything. How perfectly can you play the notes that are already written for you?&rdquo;</span></span></p><p><span><span>And when it comes to fixing fractures, Dr. Grose compared it to choreography and performance. &ldquo;It lets you listen to the patient the same way you pay attention to the audience. A headset fitted with AR eyeglasses enables surgeons to determine the position of surgical tools in real time by superimposing the patient&rsquo;s CT scan or x-ray images over the surgical site, and projects 3D navigation data onto the surgeon&rsquo;s retina.</p><p>According to <a href="https://www.hss.edu/physicians_sandhu-harvinder.asp">Harvinder S. Sandhu, MD, MBA</a>, co-chief of HSS Spine, the technology saves time and makes the surgeon&rsquo;s job easier. &ldquo;It makes surgery very precise,&rdquo; said Dr. Sandhu. &ldquo;The technology allows us to see through the tissue. It&rsquo;s a little like a GPS system &mdash; it tells us where we are and gives us a roadmap of where we need to go.&rdquo;</p><p>Dr. Sandhu performed the first such surgery at Stamford Health&rsquo;s Bennett Medical Center earlier this year. He said that while computer technology has long been used in surgery, &ldquo;Up until now it involved looking back and forth from a computer screen with a two-dimensional view and the patient. Now we can concentrate on the patient and the 3D image rather than having to look at a screen.&rdquo; Dr. Sandhu saw a demonstration of AR navigation technology several years ago, when prototypes included wearing a helmet. Spine surgeons equipped with &ldquo;x-ray vision&rdquo; have the capability to see through tissue surrounding a patient&rsquo;s spine to provide more precise and faster surgeries. On January 28<sup>th</sup>, Dr. Sandhu performed the first surgery using this technology at HSS Stamford Health.</span></span></span></span></p><p><span><span><span><span>Augmented reality (AR) refers to digital information that is superimposed on a user&rsquo;s view of the physical environment comparable to a car manufacturer that offers an AR app utilizing 3D imagery to help potential buyers learn more about a car&rsquo;s design, features, energy use and other details.</span></span></span></span></p><p><span><span><span><span>The AR technology allows HSS-Stamford Health collaborative surgeons to visualize the anatomy of a patient&rsquo;s spine in 3D during surgery and track progress in real time via a transparent display headset fitted with AR eyeglasses. The headset enables surgeons to determine the position of surgical tools in real time by superimposing the patient&rsquo;s CT scan or x-ray images over the surgical site to guide the surgeon during the procedure. The headset projects 3D navigation data onto the surgeon&rsquo;s retina, allowing them to look at the patient and review the data simultaneously without having to shift their focus back and forth to a remote screen.</span></span></span></span></p><p><span><span><span><span>&ldquo;The spine system&rsquo;s augmented reality platform enables ultraprecise alignment and positioning of spinal implants through enhanced visualization capabilities,&rdquo; explains Dr.</span></span> <span><span>Sandhu. &ldquo;The spinal implants can be placed with much less invasive surgery, as we don&rsquo;t have to retract or move muscles and tissue to see a patient&rsquo;s full spine.&rdquo;</span></span></span></span></p><p><span><span><span><span>Dr. Sandhu notes that smaller incisions mean faster recovery times and improved outcomes for patients. He also says the system gives surgeons the ability to course correct or alter their approach if needed during procedures.</span></span></span></span></p><p><span><span><b><span><span>Patient Experiences Immediate Relief</span></span></b></span></span></p><p><span><span><span><span><span><span><span>Paul Solis-Cohen had been experiencing numbness in his left leg and foot along with sciatic pain for two years. When the pain overwhelmed the daily activities of the active 72-year-old owner of Catskill Art Supply, he finally decided to seek treatment.</span></span></span></span></span></span></span></p><p><span><span><span><span><span><span><span>&ldquo;I own a farm in Shokan, New York. Between lifting 50-pound bales of hay, splitting, and stacking wood, riding horses, and playing tennis over the years, I eventually just overdid it with my back,&rdquo; he says.</span></span></span></span></span></span></span></p><p><span><span><span><span><span><span><span>HSS is well known in the Catskill and Hudson Valley regions. Paul researched HSS surgeons, found Dr. Sandhu and met with him in New York City to review his case and MRI results. Dr. Sandhu determined Paul would need lumbar decompression surgery involving a laminectomy and spinal fusion of the L4 and L5 vertebrae. Dr. Sandhu performed the surgery in late February at HSS Orthopedics at Stamford Health, using the new AR-based system.</span></span></span></span></span></span></span></p><p><span><span><span><span><span><span><span>&ldquo;I felt immediate relief from my sciatic pain right after surgery. The pressure on my nerves was totally gone,&rdquo; he recalls. &ldquo;I was up and moving very soon thereafter and have been walking a lot. I can&rsquo;t wait to accelerate the rehab process.&rdquo;</span></span></span></span></span></span></span></p><p><span><span><span><span><span><span><span>Paul felt assured that HSS Orthopedics at Stamford Health was strictly adhering to all COVID-19 safety protocols, so he had no concerns about any health risks surrounding his spine surgery. The location at 372 Danbury Road is the fifth facility that is part of the HSS Rehabilitation at Stamford Health collaborative, established in 2015.</span></span></p><p><span><span>The facility is open Monday through Friday from 8 a.m. to 5 p.m. and has new equipment including a seated elliptical recumbent cross trainer, a rebounder, hi-lo treatment tables, an upright stationary bike, an upper-body ergometer, a ballet barre and TRX straps. Stamford Health and HSS will provide outpatient physical therapy services, expanding the two organizations’ collaboration to provide local, high quality orthopedic care to patients across Southwestern Connecticut.&nbsp;</p>

<p>"By extending our outpatient rehabilitation services to 75 Holly Hill Lane in Greenwich, patients can access the highest quality care at four convenient locations," said <strong>Justin Clark, PT, DPT,</strong> Director of Rehabilitation Services, HSS Orthopedics at Stamford Health. "At each site, we specialize in treating patients of all ages and activity levels. It’s also important to point out that we care for patients who may be anticipating surgery, have already had surgery or who are trying to avoid surgery altogether."</p>

<p>The new 1,200-square-foot rehabilitation center, located on the second floor, features two full-time physical therapists and new equipment to fit the needs of all patients.
