                        <title>HSS Presents Research at 2024 AAOS Annual Meeting</title>

                        <guid>https://news.hss.edu/hss-presents-research-at-2024-aaos-annual-meeting/</guid><pp:caseid>620443</pp:caseid><description><![CDATA[<p><span style="padding:0in;">At this year’s American Academy of Orthopaedic Surgeons (AAOS) Annual Meeting, HSS physicians and scientists presented exciting new research related to clinical advancements for care and patient outcomes in orthopedics.</span></p><p><span style="padding:0in;">Research highlights include the evaluation of artificial intelligence (AI) chatbots to provide accurate answers to medical questions about spine surgery, and two retrospective studies analyzing outcomes in both knee and hip replacement surgeries performed with robotic assistance.&nbsp;</span></p><p><a href="https://news.hss.edu/hss-research-evaluates-whether-ai-chatbots-provide-reliable-medical-information/" target="_blank"><span style="padding:0in;"><strong>HSS Research Evaluates Whether AI Chatbots Provide Reliable Medical Information</strong></span></a></p><p><span style="padding:0in;">A study co-authored by </span><a href="https://www.hss.edu/physicians_qureshi-sheeraz.asp"><span style="padding:0in;">Sheeraz Qureshi, MD, MBA</span></a><span style="padding:0in;">, co-chief of HSS Spine and colleagues identified nine frequently asked questions about cervical spine surgery, which they considered to be of particular clinical relevance. Question topics ranged from the benefits and drawbacks of different surgical approaches to side effects and recovery after surgery. The questions were inputted one at a time into ChatGPT version 3.5. Two experts in cervical spine surgery who were not involved in designing the questions rated the chatbot’s responses on accuracy, appropriateness, and readability. On average, the responses received a score of 8.1/10, with a 3.9/5 for accuracy and a 2.2/3 for appropriateness.</span><span> </span><span style="padding:0in;">The main drawback the reviewers noted was that ChatGPT failed to provide comprehensive responses, often omitting important factors. AI chatbots are more accurate than expected when asked to answer medical questions about spine surgery, however patients still need to use extreme caution when turning to these tools for help with medical decision-making.</span></p><p><a href="https://news.hss.edu/new-hss-studies-highlight-the-benefits-of-robotic-assisted-joint-replacement-surgery/" target="_blank"><span style="padding:0in;"><strong>New HSS Studies Highlight the Benefits of Robotic-Assisted Joint Replacement Surgery</strong></span></a></p><p><span style="padding:0in;">HSS investigators presented two retrospective studies analyzing outcomes in both knee and hip replacement surgeries performed with robotic assistance.</span></p><p><span style="padding:0in;">The first </span><span>study co-authored by HSS hip and knee surgeon </span><a href="https://www.hss.edu/physicians_jerabek-seth.asp"><span>Seth A. Jerabek, MD</span></a><span>, and colleagues found that the use of a surgical robot during total knee replacement surgery reduced the need for manipulation under anesthesia, an outpatient procedure that may be prescribed when physical therapy after surgery becomes too painful for the patient due to joint stiffness caused by scar tissue.</span></p><p><span>The second study led by HSS hip and knee surgeon </span><a href="https://www.hss.edu/physicians_gausden-elizabeth.asp"><span>Elizabeth B.&nbsp; Gausden, MD, MPH</span></a><span> and colleagues compared short-term outcomes (e.g., rates of dislocation, re-operation, revision, and patient-reported outcome measures) of the anterior versus the posterior approach in robotic-assisted total hip replacement surgery. The investigators found that hip replacement has excellent outcomes with each respective approach when performed by surgeons with extensive experience in that approach. CAMMISA, JR., MD</strong></span></p><p>Frank P. Cammisa, Jr., MD, is Chief Emeritus of HSS Spine and a Professor of Clinical Surgery at Weill Cornell Medical College. He specializes in the surgical treatment of spinal disorders. His areas of expertise include spinal reconstructive surgery, minimally invasive surgery, athletic spinal injuries, and motion preservation techniques. TSAI</strong></span></p><p><span>Joe Tsai is a co-founder and executive vice chairman of Alibaba Group. He is a member of the board of directors of Alibaba Group and its financial services affiliate, Ant Group.</span></p><p><span>Mr. Tsai is governor of the Brooklyn Nets, alternate governor of the New York Liberty, and chairman of the teams’ home arena, Barclays Center. Ms. Wu Tsai is the governor of the WNBA's New York Liberty and, along with her husband, is an owner of the NBA's Brooklyn Nets and the NLL's San Diego Seals. In the next few years there will be increased pressure to move orthopedics, and a portion of spine care, to the ASC setting. Naturally, this will decrease costs, and therefore increase value. Centers will be forced to "lean" their processes, whether with enhanced recovery pathways or expedited processes, in order to decrease length of stay, shorten operative times and decrease the time it takes for patients to recover from operations all while upholding quality.</span></span></p><p><span style="background-color:rgb(255,255,255);color:#000000;"><span style="text-align:start;">He continued, “</span></span><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">Centers will have to be creative and efficient in working within and leveraging their multidisciplinary teams and skill sets in order to deliver greater efficiency in care delivery, and decrease the costs for overall episodes of care for a given operation. Such innovations have been a boon to orthopedic surgery, as engineers, scientists and inventors design systems to improve accuracy and safety in the operating room. As technologies evolve and studies demonstrate the benefits, increasing numbers of orthopedic surgeons are using robotics, computer navigation, augmented reality and smart sensors to improve patient care.</p><p><span style="color:#000000;">According to Dr. Dowdell, i</span>n spine surgery, surgical robots, advanced imaging technology and computer-assisted navigation provide enhanced precision, accuracy and predictability. “The system enables the surgeon to make very small incisions that preserve muscles and other structures surrounding the spine, so patients generally experience less pain after surgery, a shorter hospital stay and a quicker return to activities compared to traditional open surge<span style="color:#000000;">ry.”&nbsp;</span></p><p>The technologies are especially useful for more challenging cases—patients whose anatomy differs from the norm— according to Dr. Sokunbi. He advise<span style="color:#000000;">d any</span>one considering spine surgery to carefully check the physician’s credentials and choose a hospital in which a high number of spine surgeries are performed. He <span style="color:#000000;">noted that a g</span>ood rapport with the treating physician, good communication and trust are also essential.</p><p>Dr. Vigdorchik performed the first knee replacement using augmented reality (AR) in the US in 202<span style="color:#000000;">0, stating “It</span>’s like a car that has the speedometer projected on the windshield, so you don’t have to look down when you’re driving<span style="color:#e74c3c;">.</span>” Taking the car analogy one step further, he comp<span style="color:#000000;">ared the AR knee replacement platform to GPS. “Everyone knows how to drive home, but you use GPS technology to find the best and most efficient route to take. D</span>r. Vigdorchik <span style="color:#000000;">added t</span>hat the augmented reality platform allows for an ultraprecise surgery.</p><p>HSS spine surgeons have also begun to use augmented reality technology. The spine system consists of a headset and the elements of a traditional computer navigation system. During surgery, it accurately determines the position of surgical instruments in real time and superimposes them on the patient’s CT scan. “The technology allows us to visualize the patient’s 3D spinal anatomy during surgery to accurately guide instruments and place surgical implants, such as pedicle screws, while looking directly at the patient rather than at a separate computer screen,” explai<span style="color:#000000;">ned Dr. Cam</span>misa “Improved control and visualization of the patient’s anatomy and critical structures can lead to a more precise, efficient surgery and can enhance safety.”</p><p>An innovation in the growing field of remote patient monitoring, it was the first implantable device approved to collect data on an individual’s progress after a knee replacement. Although the technology does not preclude office visits altogether, it enables the doctor to actively monitor a patient’s recovery with real-world, objective data to supplement his or her care. and recovery. Remote monitoring can be especially useful during the early post-operative perio<span style="color:#000000;">d, said Dr. Sculco,</span> who performed the first procedure at HSS. “In the first several weeks following knee replacement, hard work is required from the patient. The research was presented today at the American Academy of Orthopaedic Surgeons (AAOS) Annual Meeting in Las Vegas. &nbsp;</span></p><p><span>Many spinal surgeries require the use of implants called pedicle screws to stabilize the spine. Precise positioning of these screws is critical for a successful outcome. “Two-dimensional biplanar radiographs [BPR] have been the gold standard to confirm pedicle screw placement in spine fusion surgery for many years. However, when utilizing BPR, there is the potential that a two-dimensional image alone will not properly demonstrate the successful placement of a screw,” said </span><a href="https://www.hss.edu/physicians_lebl-darren.asp"><span>Darren Lebl, MD</span></a><span>, a spine surgeon at HSS and principal investigator of the study. “As an alternative, three-dimensional intraoperative imaging systems are now available, offering improved visualization.”</span></p><p><span>Dr. Lebl and colleagues set out to compare the accuracy of BPR versus 3D imaging when assessing intraoperative pedicle screw placement. “Our study is the first to compare the differences in intraoperative biplanar radiography and 3D imaging for pedicle screw accuracy in thoracic and lumbar cases using robotic technology,” Dr. Lebl noted.</span></p><p><span>Investigators analyzed data from 103 patients who underwent spinal fusion by a single surgeon from 2019 to 2022. Pedicle screw placement was assessed with both intraoperative BPR and 3D imaging in each case.</span></p><p><span>“CT scans taken after surgery were compared to the findings of intraoperative BPR and 3D imaging to detect either false-positive or false-negative readings,” explained <strong>Fedan Avrumova, BS</strong>, an HSS clinical research coordinator who presented the study at the AAOS meeting. “False positive findings are instances when BPR imaging suggests the screw was not in an acceptable position, while in fact a more advanced 3D image (intraoperative 3D scan or postoperative CT scan) showed the screw to be in an acceptable position. Conversely, a false negative instance was when a BPR image led one to believe or looked as though the screw was in an acceptable position, when in fact a more advanced 3D image or post-operative CT scan showed that it was in fact not acceptable.”</span></p><p><span>Postoperative CT imaging revealed a clinically significant number of patients who had false-negative and false-positive screw placement readings on BPR. However, screw position shown on intraoperative 3D imaging was found to be much more accurate, Avrumova added.&nbsp;&nbsp;</span></p><p><span>“Based on our study, BPR imaging may lead one to think a screw is acceptable when in fact it is not, and also may miss many screws that are not in fact acceptable. In our study, it was approximately one percent of cases where this occurred. However, for surgeons and centers that implant hundreds and thousands of screws per year, this is going to result in a significant clinical impact for many people,” Dr. Lebl noted. “Even one misplaced screw can have a significant impact for a patient, a surgeon, and a hospital system. AR navigation in spine surgery superimposes 3D images of a patient’s anatomy onto the surgeon’s view of the operative field. Critical elements include a preoperative CT scan of the patient’s spine, which is used to plan and perform the surgery, and an augmented reality headset worn during the procedure.</span></p><p><span style="background-color:rgb(254,254,254);"><span>The system Dr. Cammisa uses consists of a near-eye-display headset and the elements of a traditional navigation system. During surgery, it accurately determines the position of surgical instruments in real time and </span></span><span>superimposes them on the patient's CT data. </span><span style="background-color:rgb(254,254,254);"><span>It has been likened to giving surgeons “x-ray vision,” enabling them to see structures underneath the skin.</span></span></p><p><span>“The technology allows us to visualize the patient’s 3D spinal anatomy during surgery to accurately guide instruments and place surgical implants, such as pedicle screws, while looking directly at the patient rather than at a separate computer screen,” Dr. Cammisa explained. “Improved control and visualization of the patient’s anatomy and critical structures can lead to a more precise, efficient surgery and can enhance safety."&nbsp;</span></p><p><span>He notes that the technology allows for a highly personalized surgery that can be used for most spinal procedures requiring the use of implants. Girardi, MD</span></a><span>, spine surgeon at HSS.</span></p><p><span>“I had seen Dr. Girardi for some follow up and started some mild jogging. Within four months or so I was back to running," said Mordos. Mordos has completed more than 75 marathons over the course of his life, and post-surgery he is focused on staying active to remain healthy. “Dr. Girardi recommended from the get-go that I stay active in order to encourage healing,” said Mordos concluding, “The thought of being able to get out there felt like a certainty. Investigators also compared outcomes for patients with predominant neck pain with those with predominant arm pain and equal neck and arm pain.</p><p>The study found that cervical disc replacement leads to comparable improvement in neck pain and disability in patients presenting with neck pain greater than arm pain.</p><p>“Understanding the best treatment for neck pain (irrespective of type of surgical or nonsurgical intervention) is difficult. In general, when we perform surgical interventions in the cervical spine, we often tell patients that arm pain is more likely to get better than neck pain. Furthermore, if neck pain is a major component of a patient’s symptoms, then surgeons will often suggest fusion as the best option,” said Dr. Qureshi.</p><p>He added, “I was pleasantly surprised that patients who underwent cervical disc replacement did indeed have improvement in their neck pain (as well as their arm pain).                         <guid>https://news.hss.edu/sure-signs-you-need-to-see-a-spine-specialist/</guid><pp:caseid>532246</pp:caseid><description><![CDATA[<p>Eat This, Not That! featuring Angelie Mascarinas, MD, and Sariah Khormaee, MD, PhD</p>]]></description><content:encoded><![CDATA[<p>Eat This, Not That! discusses signs that indicate the need to see a spine specialist according to experts including HSS Florida physiatrist <a href="https://www.hss.edu/physicians_mascarinas-angelie.asp">Angelie Mascarinas, MD</a>, and HSS spine surgeon <a href="https://www.hss.edu/physicians_khormaee-sariah.asp">Sariah Khormaee, MD, PhD</a>.<br><br>According to Dr. Mascarinas, “Certain issues associated with the spine (i.e. lumbar disc herniations or lumbar spine nerve compression) can cause referred pain or numbness (pins/needles sensation) into the buttocks or legs and can also cause leg weakness.”<br><br>“An unaddressed issue such as a large lumbar disc herniation can cause bowel/bladder issues if there is significant stenosis (narrowing in the spine) due to the disc herniation," she added.<br><br>She explained, “Numbness or weakness in the leg or foot can indicate nerve compression in the lumbar spine. This can be assessed by determining where there is difficulty when performing a calf raise,<span>&nbsp; </span>difficulty when walking on your heels, or if your foot drops or slaps the ground while walking.”</p><p>Dr. Mascarinas noted another sign is if pain persists for 4-6 weeks. “70% of cases of disc herniations in the general population have significant improvement of back and leg pain within 4-6 weeks. Therefore, if symptoms are not improving, a spine specialist can evaluate symptoms and perform a physical exam, obtain x-rays or more advanced imaging if indicated, and offer other treatments like medications, physical therapy, or injections that may help alleviate symptoms.”</p><p>Dr. Mascarinas said persistent back pain after a minor accident can be an issue. "Persistent back pain following a seemingly minor incident could be a sign of a lumbar fracture, and an x-ray or MRI would be helpful to confirm diagnosis. Especially for those with a history of osteopenia or osteoporosis whose bones are more fragile, minor trauma can cause lumbar fractures."</p><p>Dr. Khormaee explained signs of a rare condition, "If you have tingling or numbness in the groin area and lose the ability to go to the bathroom normally, this can be a sign of what is called cauda equina syndrome. This condition occurs when there is compression on the nerves at the base of the spinal column.”</p><p>Dr. Khormaee said agility changes in fingers and hands may be a sign to see a spine specialist. She explained, "The vertebrae in your upper back and neck connect your spinal column to your arms, hands and fingers. Changes in the area can affect your ability to carry out tasks that require the use of your hands. "People may notice that they have problems with buttons or zippers. Another thing that I sometimes hear from patients is that their handwriting has changed. Similar to the concerns seen in the lower spine, these symptoms can be caused by a number of different types of spinal problems. I see two aspects of the clinical significance of these results: One, it is vital information for surgical counselling as patients often inquire about the rates of reoperation, and two, even though the rates are statistically greater after revision discectomy versus primary (6% vs. 5% for SRD and 10% vs. 6% for SLF), these are not major differences looking from the clinical standpoint. The study is sponsored by Centinel Spine, LLC, and Hospital for Special Surgery (HSS) in New York City, is one of the centers participating in the research.</span></p><p><span>Cervical disc disease, or </span><a href="https://www.hss.edu/condition-list_degenerative-disc-disease.asp"><span>degenerative disc disease</span></a><span>, is the progressive deterioration of spinal discs and arthritic changes in the facet joints. During this procedure, surgeons will test the safety and efficacy of removing discs at two levels and implanting an artificial disc device at each level of the cervical spine between the vertebrae where the disc was removed. The artificial disc will serve to stabilize the spine while preserving greater motion than a conventional fusion surgery.</span></p><p><span>Previous patients who underwent artificial disc replacement reported greater improvements in their pain relief, mobility, and activities of daily living than those who underwent spinal fusion surgery, the standard treatment. It is less invasive but as effective as spinal fusion surgery, however the procedure using this particular device has not been validated through a clinical trial to date.</span></p><p><span>“The concern with doing a fusion type operation is that over the long term the other discs have to accommodate for the motion lost at the fused level,” said </span><a href="https://www.hss.edu/physicians_lebl-darren.asp"><span>Darren Lebl, MD</span></a><span>, a minimally invasive spine surgeon who is leading the research at HSS. Over time, the increased mechanical demand on the surrounding vertebrae can make the discs in the region more vulnerable to additional degeneration.</span></p><p><span>“We do know very clearly that fusion is limiting motion,” Dr. Lebl added. “The concept of the two-level disc replacement is to preserve motion with two moving mechanical devices.”</span></p><p><span>To be eligible for participation, patients must be between the ages of 18 and 69 years and have received a diagnosis of radiculopathy (impingement of the nerves in the spinal column) or myelopathy with radiculopathy, a more severe form of damage to the spinal cord involving vertebrae in the neck, defined at cervical discs C3 through C7. Patients must also be experiencing symptoms associated with disc problems, such as neck or arm pain, decreased muscle strength and abnormal reflexes that has not responded well to conservative treatments such as rest, heat, physical therapy and pain relievers.</span></p><p><span>The trial will include at least 390 patients once enrollment is complete. In response to high demand from patients on Long Island,&nbsp;close to 18,000 square feet were added, nearly doubling the facility size to 37,000 square feet. As a result, Dr. Cammisa started working with the National Hockey League Players’ Association throughout the process to explain the available data for ADR.</span></p><p><span>“We went back to some of the original work we did, in terms of the mechanics and results and what kinds of athletes have gotten it over the years,” said Dr. Cammisa. “It hadn’t been done [before], but I came to the conclusion with that data that it was reasonable in a hockey player to do a disc replacement. I spoke to the league about the reasons why, and I wrote a report.”</span></p><p><span>Dr. Cammisa noted that the operation does entail some risks and remains viable for only a limited range of injuries.</span></p><p><span>He added, “Now there’s a precedent. Hopefully they’ll continue to have great careers. Cunningham, MD, PhD</a>, spine surgeon at HSS.<br><br>According to Dr. Cunningham, the condition is most often diagnosed between the ages of 10 and 15 when the spine is still developing, but it can affect people throughout the lifespan. Adults are much more likely to experience pain, not from the curvature itself but from the strain it puts on certain muscles and joints.<br><br>He said, “Scoliosis pain in adults can affect the lower back and also cause pain shooting down the leg. If there's pressure on certain nerves, your legs may also feel weak.”<br><br>“The only way to diagnose scoliosis definitively is through an X-ray,” noted Dr. Cunningham.<br><br>Physical therapy and exercise can be included in a treatment plan. Dr. Cunningham said, “While specific exercises must be geared to each individual, the general aim is to strengthen the muscles that support your spine.”<br><br>He continued, “If one of your symptoms is pain, doctors may also order other tests to see if you have another medical condition. The institution is collaborating with medical centers and other organizations to advance the quality and value of musculoskeletal care and to make world-class HSS care more widely accessible nationally and internationally. </span><a href="http://www.hss.edu"><span>www.hss.edu</span></a><span>.</span></p>]]></pp:boilerplate><description><![CDATA[<p><span><span>A scientist at Hospital for Special Surgery (HSS), <a href="https://www.hss.edu/research-staff_dahia-chitra.asp" style="text-decoration:underline">Chitra Dahia, PhD</a>, has received two National Institutes of Health (NIH) grants totaling $6 million for translational research aimed at understanding the root cause of disc degeneration and chronic back pain in order to develop treatment options that do not rely on addictive pain-relieving medications.</span></span></p><p><span><span>This funding will support Dr. Dahia&rsquo;s <em>Spine Development and Regeneration Lab to</em> study the potential of two important developmental molecules in the reversal of age-related intervertebral disc (IVD) pathologies and in the treatment of chronic back pain.</span></span></p><p><span><span>&ldquo;Approximately 70 percent of the U.S. population is affected by chronic back pain, negatively impacting longevity and quality of life,&rdquo; said Dr. Dahia. &ldquo;There is currently no treatment available for disc degeneration, which is a big concern when you look at the world&rsquo;s aging population. With the lack of treatment options, people can become dependent on strong pain relievers such as opioids.&rdquo;</span></span></p><p><span><span>&ldquo;It is an incredible honor for Dr. Dahia to receive these two NIH grants,&rdquo; said <a href="https://www.hss.edu/physicians_ivashkiv-lionel.asp" style="text-decoration:underline">Lionel B. Ivashkiv, MD</a>, chief scientific officer at HSS. &ldquo;Being awarded an NIH grant is the highest metric of scientific quality and impact. The application itself is peer-reviewed and quite rigorous.&rdquo;</span></span></p><p><span><span>Dr. Dahia received the first 5-year grant for $2.8 million from the National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS)/NIH R01 to study the role of the sonic hedgehog (SHH)/Brachyury axis in the maintenance of the postnatal IVD. This work aims to identify the role of Brachyury, a critical developmental molecule first reported in the nucleus pulposus cells of postnatal disc by Dr. Dahia herself in 2009, in the growth and maintenance of the postnatal IVD, how its age-related loss causes disc pathologies and peripheral sensitization and pain, and whether this can be prevented. The study findings will also identify targets to reverse painful disc pathology and help regenerate the disc.</span></span></p><p><span><span>She was then awarded the National Institute of Aging (NIA)/NIH R01 5-year grant for $3.3 million to study the role of developmental signaling pathways in the maintenance of spinal discs. The study is focused on understanding the crosstalk between major developmental signaling pathways interacting with SHH signaling (also first shown by Dr. Dahia in 2009) in temporal regulation of the different components of the IVD and their beneficial effect in maintenance and activation of disc progenitor cells. The research intends to discover if there is a potential for disc regeneration using its own cells, and is being done in collaboration with <a href="https://www.hss.edu/physicians_albert-todd.asp" style="text-decoration:underline">Todd J. Albert, MD</a>, spine surgeon and surgeon-in-chief emeritus of HSS. Dr. Albert and his team will test Dr. Dahia&rsquo;s findings in clinical conditions.</span></span></p><p><span><span>&ldquo;Initially, we found that these crucial developmental molecules are expressed in very young spinal discs, and recently we observed that they decline with age and disc pathology, hence we are now evaluating their function from stages of growth and maturation to aging,&rdquo; explained Dr. Dahia.</span></span></p><p><span><span>Dr. Dahia has committed her career to developing a full understanding of disc biology as they grow, mature, and age to identify potential avenues for therapeutics. She previously received an NIH grant in 2014 to study how the Wnt/SHH signaling loop controls IVD growth and differentiation. When Dr. Dahia began her research in disc biology over a decade ago, she was one of the first to study the beneficial role of developing molecules in the postnatal spine. Now, this is being adapted in other musculoskeletal research in a broader sense.</span></span></p><p><span><span>According to Dr. Dahia, other researchers have tried to use stem cells to solve these issues, but the cells are unable to survive due to the harsh microenvironment of the disc. &ldquo;Our lab&rsquo;s process is unique, as we aim understand the cellular and molecular basis of disc pathology and know how to intersect it, using our unique and novel genetic strategies,&rdquo; she noted.</span></span></p><p><span><span>Dr. Dahia&rsquo;s <em>Spine Development and Regeneration Lab</em> at HSS concentrates on understanding the role of key developmental cell signaling pathways and molecules in the development and homeostasis of various musculoskeletal tissues including the IVD, spinal column, growth plate, and tendon. ]]></description><content:encoded><![CDATA[<p><span><span><span>ABC 7 reports the story of HSS patient Ruth Torres, who will run the 2021 New York City Marathon after receiving minimally invasive surgery performed by <a href="https://www.hss.edu/physicians_qureshi-sheeraz.asp" style="text-decoration:underline">Sheeraz Qureshi, MD, MBA</a>, to address her lumbar spinal stenosis.</span></span></span></p><p><span><span><span>In 2018, Dr. Qureshi performed a minimally invasive surgery to relieve the compression on Torres' nerves and restore stability.</span></span></span></p><p><span><span><span>According to Dr. Qureshi, &ldquo;While that is a very common problem we see, what made Ruth's case particularly unique was that her goal was to get back to running and so we needed to do something that would not only allow us to correct her problem but give her the longevity and the ability to run the distances she wanted to run.&rdquo;</span></span></span></p><p><span><span><span>He explained, &ldquo;We made a small opening on the side of her spine. We used image guidance during surgery that allowed us to be able to see in three dimensions, Ruth's spine, without having to make big incisions and dissecting muscles. We then placed a spacer and screws to provide stability for her spine.&rdquo;</span></span></span></p><p><span><span><span>Torres noted, &ldquo;I made a decision that I'm going to run the New York City Marathon every year until I can't run anymore.&rdquo;</span></span></span></p><p><span><span><span>She continued, &ldquo;I am excited and scared. I feel that my training is going to get me to the finish line. My back is definitely not going to prevent me from running. ]]></description><content:encoded><![CDATA[<p><span><span><span>Medscape reports on HSS study results presented at the American Society of Bone and Mineral Research (ASBMR) annual meeting by senior author <a href="https://www.hss.edu/physicians_stein-emily.asp" style="text-decoration:underline">Emily Margaret Stein, MD, MS</a>, endocrinologist and bone specialist, finding levels of bone formation biomarkers were decreased for postmenopausal women who received an epidural steroid injection (ESI) in the lumbar spine to treat back and leg pain arising from a compressed nerve in the spine.</span></span></span></p><p><span><span><span>Dr. Stein and Shannon Clare, research assistant and first author of the study, cited &ldquo;Our results are notable because we found that the duration of suppression of bone formation extended beyond 12 weeks, a far longer duration than seen previously with intra-articular injections [of glucocorticoids].&rdquo;</span></span></span></p><p><span><span><span>According to the authors, women are not typically screened for osteopenia or osteoporosis before ESI. However, Dr. Stein and Clare noted, &ldquo;Our results suggest that physicians should consider screening women for osteoporosis who receive ESI, particularly those who are treated with multiple doses. Sandhu, MD, MBA</a>, co-chief of HSS Spine.</span></span></p><p><span><span>Dr. Sandhu explained by using specialized goggles, he is able to see through tissue surrounding a patient&rsquo;s spine and provide more precise and faster surgeries and, by extension, quicker recovery with less chance of infection.</span></span></p><p><span><span>He cited the technology allows the surgeon to see an overlay of data pulled from MRIs [magnetic resonance imaging] and other technology superimposed on the patient&rsquo;s spine, as well as written data visible in the corners of his visual field. He said the visible terrain means he can pinpoint areas without having to expose more markers along the spinal landscape, as is the case in more traditional surgery.</span></span></p><p><span><span>Dr. Sandhu saw HSS patient Paul Solis-Cohen and determined he needed lumbar decompression surgery involving a laminectomy and spinal fusion of the L4 and L5 vertebrae.</span></span></p><p><span><span>&ldquo;I went into the OR [operating room] with pain down my leg and I was wheeled out pain free,&rdquo; noted Solis-Cohen.</span></span></p><p><span><span>HSS patient Brynn Blystone, who was living with degenerative disc disease, underscored the success of her surgery.</span></span></p><p><span><span>&ldquo;He [Dr. Sandhu] broke things down and told me what was going on and how he could attack it. Dowdell III, MD</a>, spine surgeon at HSS.</span></span></p><p><span><span>Dr. Dowdell explained, &ldquo;This hump typically affects the area of the spine called the upper thoracic spine. When you have this condition, your head appears to jut forward because the upper thoracic spine has an excessive curvature.&rdquo;</span></span></p><p><span><span>Dr. Dowdell said the number one cause of the hump is bad posture. &ldquo;Unfortunately, this becomes a chicken and egg scenario as bad posture places more pressure on the spine, which can cause worsening of the curvature which further reinforces the existing bad posture,&rdquo; he added.</span></span></p><p><span><span>Lifestyle changes can help supplement a treatment plan. Dr. Dowdell recommended physical activity, a back brace for advanced cases, anti-inflammatory medication if the pain is significant, and treatment of underlying conditions. &ldquo;This is also important to prevent further worsening&ndash;osteoporosis is the major condition that can worsen kyphosis if untreated,&rdquo; he noted.</span></span></p><p><span><span>Practicing proper posture is also key. Dr. Dowdell explained, &ldquo;This prevents further stress being placed on your spine, decreasing the likelihood of degeneration and worsening of your hump. ]]></description><content:encoded><![CDATA[<p><span><span><em>Autumn Years</em> reports HSS patient Michael Nestler had a successful spinal fusion and removal of a bone spur performed by spine surgeon&nbsp;<a href="https://www.hss.edu/physicians_dowdell-james.asp" style="text-decoration:underline">James E. Dowdell III, MD</a>.</span></span></p><p><span><span>The pain that Mr. Nestler experienced as a result of a bone spur pressing on his spine was mostly felt in his foot. Dr. Dowdell explained, &ldquo;It&rsquo;s not uncommon for a lower back problem to masquerade as a hip, knee or ankle problem.&rdquo;</span></span></p><p><span><span>Mr. Nestler benefited from the latest innovations in spine surgery: robot-assisted minimally invasive surgery and computer navigation. Advances in surgical techniques and technology allow for a more precise and targeted surgery, according to Dr. Dowdell.</span></span></p><p><span><span>&ldquo;The use of very small incisions preserves muscles and other structures surrounding the spine, so patients generally experience less pain after surgery, a shorter hospital stay and a quicker return to activities compared to traditional open surgery,&rdquo; he noted.</span></span></p><p><span><span>Since the surgery, Mr. Nestler is now pain-free and back to participating in many of his favorite activities.</span></span></p><p><span><span><a href="https://www.hss.edu/physicians_lebl-darren.asp" style="text-decoration:underline">Darren R. Increasing numbers of hospitals and spine surgeons are adopting the technology, which aims to enhance precision, accuracy and predictability, according to</span> <a href="https://www.hss.edu/physicians_lebl-darren.asp" style="text-decoration:underline"><span>Darren R. Lebl, MD, MBA</span></a><span>, a spine surgeon at Hospital for Special Surgery (HSS).</span></span></span></span></p><p><span><span><span><span><span><span style="padding:0in"><span>He believes the technologies have reached a tipping point. &ldquo;I think they&rsquo;re good enough to greatly improve what we do in terms of minimally invasive surgery,&rdquo; said Dr. Lebl. &ldquo;In my own practice, I&rsquo;ve seen the benefits for patients in terms of a shorter hospital stay, quicker mobility and decreased healing times.&rdquo;</span></span> </span></span></span></span></span></p><p><span><span><span><span><span><span style="padding:0in"><span>The main barriers to implementing the new technologies include additional time in the operating room (OR) and concerns regarding the accurate placement of pedicle screws. Many spinal surgeries require the use of hardware such as pedicle screws to stabilize the spine, and precise positioning is critical.</span></span> </span></span></span></span></span></p><p><span><span><span><span><span><span style="padding:0in"><span>Dr. Lebl</span></span> <span><span>and colleagues at HSS launched a study to evaluate the accuracy of robotic- and navigation-assisted technology in screw placement. They also set out to determine the extra time needed in the OR, noting that there is a learning curve for a surgical team when implementing a new system.</span></span> </span></span></span></span></span></p><p><span><span><span><span style="padding:0in">The study, &ldquo;K-wireless Robotic and Navigation Assisted Pedicle Screw Placement in Adult Degenerative Spinal Surgery,&rdquo; was presented today at the American Academy of Orthopaedic Surgeons (AAOS) 2021 annual meeting. The results were also published in</span> <a href="https://jss.amegroups.com/article/view/5317/html" style="text-decoration:underline"><span style="padding:0in">The Journal of Spine Surgery</span></a> <span style="padding:0in">in June 2021.</span></span></span></span></p><p><span><span><span><span>&ldquo;Our main finding w</span><span style="padding:0in">as that the screw positioning was very accurate, and the additional time needed to adopt these technologies into practice was small,&rdquo; Dr. Lebl noted. &ldquo;In our study, the speed of robotic screw placement showed improvement within the first several cases.&rdquo;</span></span></span></span></p><p><span><span><span><span><span><span style="padding:0in"><span>The research included 65 adult patients (mean age of 59) with lumbar degenerative pathology who underwent a robotic-navigated posterior spinal fusion from June 2019 to July 2020. A total of 364 instrumented pedicle screw implants were planned robotically. A single orthopedic surgeon performed the surgeries using the Medtronic Mazor X Stealth Edition system.</span></span></span></span></span></span></span></p><p><span><span><span><span><span><span style="padding:0in"><span>The robotic/navigation time in the OR, screw placement times and positioning times were recorded. Following screw placement, all patients had intraoperative 3D fluoroscopy, and screw trajectory was compared to a preoperative CT image. Reasons for conversion from robotic navigation to k-wire or freehand technique were recorded.</span></span> </span></span></span></span></span></p><p><span><span><span><span><span><span style="padding:0in"><span>&ldquo;The spine surgeon in the study had more than 10 years of experience and had placed thousands of screws with previous techniques, such that he had alternatives if it was determined that robotic-assisted placement of a screw would not be possible,&rdquo; said <strong>Fedan Avrumova, BS</strong>, a research assistant in the HSS Spine Service. &ldquo;It is important to note that the robot can be a valuable &lsquo;assistant&rsquo; but does not replace surgical experience and good judgment.&rdquo;</span></span></span></span></span></span></span></p><p><span><span><span><span><span><span style="padding:0in"><span>The mean time per screw placement was 3.6 minutes robotically versus 3.7 minutes freehand, with a significant decrease in time between the first 10 and last 10 cases: 5.5 minutes versus 2.3 minutes. Setting up the robotic navigation system in the OR showed a decrease in time between the first 10 and last 10 cases: 9 minutes versus 6 minutes.</span></span> </span></span></span></span></span></p><p><span><span><span><span style="padding:0in">&ldquo;Many spine surgeons recognize the value of the technology, but the concern has been the additional time needed in the OR,&rdquo; Dr. Lebl said. &ldquo;As we got further along in our study, we found that it actually saved time in the OR. The benefits of the technology easily outweigh the additional time needed in the early implementation of the system because the accuracy is unmatched.&rdquo;</span></span></span></span></p><p><span><span><span><span><span><span style="padding:0in"><span>&ldquo;When we evaluated outcomes, we found zero complications and no revision surgeries were needed,&rdquo; Ms. Avrumova noted. &ldquo;The surgical team learned how to use the robot successfully and became more efficient with robotic implantation when compared to conventional minimally invasive and conventional open techniques. In addition, the accuracy was as good as, if not better than, open or conventional minimally invasive techniques, which we confirmed with a 3D imaging study in every patient during surgery.&rdquo;</span></span></span></span></span></span></span></p><p><span><span><span><span><span><span style="padding:0in"><span>&ldquo;Just a few years ago, there were two FDA-approved robots to assist in spine surgery. Experts at Hospital for Special Surgery (HSS) have been exploring ways to increase the efficacy and efficiency of these procedures, to improve both short-term and long-term patient outcomes and bring down costs.</span></span></p><p><span><span>Today at the American Academy of Orthopaedic Surgeons (AAOS) 2021 annual meeting, details of an enhanced recovery pathway developed for these patients were presented and the improvements that patients saw after being treated with this approach were described. The results of the investigation into its effectiveness were also <a href="https://www.thespinejournalonline.com/article/S1529-9430(21)00004-8/fulltext" style="text-decoration:underline">published</a> in January 2021 in <em>The Spine Journal</em>.</span></span></p><p><span><span>&ldquo;In today&rsquo;s healthcare environment, we&rsquo;re all looking for ways to decrease costs and improve the quality of care we deliver,&rdquo; says HSS spine surgeon and principal investigator <a href="https://www.hss.edu/physicians_kim-han-jo.asp" style="text-decoration:underline">Han Jo Kim, MD</a>. &ldquo;We wanted to see which factors were modifiable in the care of these patients, to help us decrease the time spent in the operating room and patients&rsquo; length of stay and minimize complication rates.&rdquo;</span></span></p><p><span><span>The team started using its enhanced recovery pathway in February 2019. The study reports data from 40 patients who underwent fusions of at least five vertebrae in the lower back using the new protocol. Although spinal fusions of five or more vertebrae are considered complex procedures, they are relatively common and used to treat a number of conditions.</span></span></p><p><span><span>&ldquo;Traditionally, these patients stayed in the hospital for at least seven or eight days,&rdquo; Dr. Kim says. &ldquo;This procedure has a high reported complication rate, and a high rate of revision surgeries, 90-day readmissions and dependence on rehabilitation facilities upon discharge. For those reasons, we thought it would be an important operation to evaluate when we started targeting methods to improve complex spine surgery.&rdquo;</span></span></p><p><span><span>One key component of the new treatment pathway was always using the same surgical team, including surgical technicians, nurses and anesthesiologists. After surgery, the same physical therapy team cared for all the patients in the study, and patients were mobilized three times a day. Rigorous patient selection was also important for ensuring that patients could fully benefit from the optimization.</span></span></p><p><span><span>During surgery, the team focused on minimizing blood loss and keeping the time spent in the operating room as short as possible. When feasible, powered surgical instruments were used. The surgeries were done in four stages of one hour each, so that all members of the team would know how to anticipate the next phase of the operation.</span></span></p><p><span><span>Several changes were made postoperatively as well. The regimen for pain medication was modified to reduce the complications seen from narcotics. The patients were prompted to get out of bed the same day as their surgery, instead of waiting until the next day. And physical therapy sessions were increased from twice a day to three times a day.</span></span></p><p><span><span>When compared with matched patients treated without the optimized pathway, the study population achieved remarkable results. No patients went to the intensive care unit following surgery, compared with a usual rate of 30%. Hospital stays were decreased from an average of 7.3 days to 4.5 days. A higher rate of patients were discharged home versus to a rehabilitation facility (95% versus 65%). Overall patient outcomes at one year were equivocal.</span></span></p><p><span><span>&ldquo;HSS is uniquely positioned to execute a program of this kind, because everyone here is dedicated to orthopedic surgery and care. That enables us to create and initiate optimized pathways,&rdquo; Dr. Kim says. &ldquo;Not every hospital has the infrastructure and resources to build this type of program, but we wanted to share what we did so other centers could model it. Ultimately, the goal is to improve the quality of care that we provide for patients and to decrease the costs by instituting such pathways across multiple healthcare systems in the country that care for complex spine patients.&rdquo;</span></span></p>]]></description><category><![CDATA[pressrelease,Kimh,SpineBack,Spine,Spine Care Institute,Spine/Back,Research Clinical]]></category>
