                        <title>Searching for New Cell-Therapy Innovations in Rotator Cuff Repair</title>

Rodeo, MD</span></p>]]></description><content:encoded><![CDATA[<p>The HSS Research Institute is dedicated to identifying mechanisms and developing effective approaches for the prevention, diagnosis and treatment of orthopaedic and rheumatic conditions. This week, I discussed innovations in rotator cuff repair, specifically the role of cell therapy, at the American Academy of Orthopaedic Surgeons 2022 Annual Meeting. I outlined what’s currently available, what questions we are currently investigating and what the future looks like.<br><br>Rotator cuff tears are one of the most common musculoskeletal conditions, becoming more prevalent as people age. While most tears need to be treated surgically, healing is an imperfect and slow process. The surgery itself does not reverse the muscle atrophy (thinning or loss of muscle tissue) but involves re-attaching tendon to the bone. Up to 15 to 20 percent of patients may have incomplete or failed healing. The most common issues post-surgery are pain and weakness. To date, there is no approved biologic therapy to improve rotator cuff repair.<br><br>There is a real unmet clinical need to raise the success rate of this repair. More and more older individuals want to get back to an active lifestyle through activities such as tennis and swimming, which put strain on the shoulder. Approximately 200,000 to 250,000 rotator cuff repairs are done each year, with over 1,000 performed at HSS.<br><br>The <a href="https://www.hss.edu/regenerative-medicine.asp" target="_blank">HSS Center for Regenerative Medicine</a>, of which my lab is a part, has a robust research program that investigates the basic cellular and molecular mechanisms of healing damaged tissue. Our multidisciplinary group of scientists and clinicians are trying to understand the underlying reasons why tendons tear and why they don’t heal. We carry out basic science models of tendon repair for conditions including overuse tendonitis and rotator cuff tears.<br><br>Cell therapy has tremendous potential to regenerate tissue. We know that tendons are not highly cellular. We need to determine how to stimulate cells that are intrinsic in tissues or inject a cell population to jumpstart healing. The term “stem cell” is often used, but a more accurate description is a “connective-tissue progenitor cell”, which is a bit more limited in its ability to regenerate tissue. A true stem cell is a rare and highly specialized cell, and we know that true stem cells are present in many tissues. We have discovered that stimulation of these intrinsic stem cells can improve tissue healing and regeneration. Learning how to best leverage these existing cells may greatly improve the healing of rotator cuff tears.<br><br>Additionally, we are continuing to identify ways to characterize cell populations. The cells we can use currently derive from fat or bone marrow and are heterogeneous and highly variable. If we are able to sort them, we can culture only the desired cells and remove undesired cells. This would be a big breakthrough in cell therapy research.<br><br>We need to know which cells are best, where they should be sourced from and what doses to use. I believe there also needs to be changes in the regulatory environment so relevant clinical trials can occur.<br><br>At HSS, it is important that our lab research mirrors clinical work so that we can bring our findings directly to patients. The HSS Center for Regenerative Medicine evaluates a range of cell therapies that seek to heal tissue that has been damaged by injury or disease in order to improve effectiveness of new treatment options.<br><br>We are currently running two clinical trials testing cell therapy for patients undergoing rotator cuff repair. One is a <a href="https://clinicaltrials.gov/ct2/show/NCT04057833">Phase 1B clinical trial</a> that evaluates the safety and feasibility of injecting E-CEL UVEC cells at the end of the operation; these are proprietary allogeneic human umbilical vein endothelial cells produced under cGMP and cGTP regulations. Angiocrine Bioscience is providing support for this study.&nbsp;<span> </span>The other is a <a href="https://clinicaltrials.gov/ct2/show/NCT03332238">Phase II interventional clinical trial</a> that has a similar design except surgeons take adipose tissue from the abdomen at the beginning of the operation, isolate stromal vascular fraction cells (SVFCs) from this tissue, and then inject it into the tendon and muscle after rotator cuff repair. This trial will determine if the use of these cells can enhance outcomes for patients undergoing rotator cuff repair.<br><br>The marketing for stem cells continues to stay ahead of the science. HSS laboratories are testing whether injecting cells derived from human umbilical veins into the muscle and tendon of an injured rotator cuff will stimulate stem cell activity and promote better repair after surgery. Surgeons have made a concerted effort to minimize and sometimes eliminate the use of opioids after surgery because of the serious side effects and unanticipated consequences of prolonged opioid usage after surgery. In place of opioids, surgeons will now often use multimodal anesthesia. For example, prescribing acetaminophen alongside an anti-inflammatory medication can mitigate pain when used in conjunction with nerve blocks. Ice is also an important aspect of pain control. Altchek, MD</a>, sports medicine surgeon and founding medical director of HSS Florida, discussing the types, causes, symptoms and treatment of rotator cuff tears.</p><p>Dr. Altchek explained there are two major groups of tears, partial (some of the tendon remains attached to the bone at the tear site) or full thickness (none of the tendon remains attached). Both types can be caused by one or a combination of factors, including trauma (such as a fall), impingement from a bone spur on part of the shoulder blade (acromion), or degeneration due to tissue aging.</p><p>Symptoms of a rotator cuff tear are shoulder pain and a sense of weakness, particularly when performing overhead activities like throwing a ball. Several other shoulder conditions, including arthritis, tendonitis, and disorders of the biceps tendons can exhibit similar symptoms. Diagnosis requires an MRI scan or ultrasound, with an MRI considered the gold standard.</p><p>Dr Altchek cited most patients with partial tears can be managed with conservative treatment, such as nonsteroidal anti-inflammatory medication, cortisone injection or platelet rich plasma (PRP), and physical therapy. In contrast, people with full thickness rotator cuff tears have the best chance at remaining active with an arthroscopic rotator cuff repair. Blaine, MD</a>, discussed the latest techniques being used to treat and repair various shoulder injuries.</p><p>&ldquo;For tendon type injuries, I think what we&rsquo;ve found is unfortunately in the shoulder, tendons don&rsquo;t heal. If&nbsp;you tear your rotator cuff and it&rsquo;s a full tear, a complete tear, that tear is not going to heal. That&rsquo;s changed&nbsp;our approach. While physical therapy can be incredibly&nbsp;important for partial tears, labral tears, and a variety of different things, when it becomes a full tear we&rsquo;re much more likely to talk about surgery at an earlier point. We also have some newer techniques to repair tendons. There are times when the tendon tear is so large that you can&rsquo;t fully reconstruct it with the patient's own tissue. So there has been work with graft procedures and biologics to be able to try and reconstruct a tendon that doesn&rsquo;t exist anymore. We&rsquo;re not totally there yet, but I think we&rsquo;re making some great advances and it does give us opportunities we didn&rsquo;t have before.&rdquo;</p><p>Dr. Blaine continued, &ldquo;On the arthritis side, we also have been able to make a lot of advances in that area. <p>Dr. Fu explained the key takeaway from the study, noting, &ldquo;The most important result is that there appears to be an association between a delay from the time of initial rotator cuff tear diagnosis to the time of rotator cuff repair, and the need for subsequent revision rotator cuff surgery due to repair failure. We found that a delay of more than 12 months from rotator cuff tear diagnosis to rotator cuff repair was associated with a significant increase in the rate of undergoing revision surgery in the future.&rdquo;</p>

<p>Dr. Fu added, &ldquo;A significant percentage of patients I see in the office with rotator cuff tears can still be successfully treated non-surgically, and there are factors that influence their prognosis such as age, tear size, activity level, muscle quality, etc. Based on the results of this study and our clinical experience, however, patients that have failed non-surgical treatment are advised that delaying rotator cuff repair may be associated with worse outcomes. Ideally, rotator cuff repair should be performed within 12 months of their rotator cuff tear diagnosis to optimize the chances of restoring shoulder function, relieving pain, and returning to the activities they love."</p>

<p><span><span>The revision rate for patients with rotator cuff tears who delayed surgery for 12 months or more was 15.2 percent, versus 9.9 percent for those who had surgery within six weeks and 8.3 percent for those who received surgery within the routine period of six weeks to 12 months of diagnosis.</span></span></p>

<p><span><span>&ldquo;Patients who have not responded to non-operative approaches for rotator cuff tears frequently ask how long they can delay surgery,&rdquo; says <a href="https://www.hss.edu/physicians_fu-michael.asp">Michael C. Fu, MD, MHS</a>, a sports medicine surgeon at HSS and lead author of the study. &ldquo;Our research was designed to investigate the relationship between delaying surgery and the need for subsequent revision due to failure of the initial rotator cuff repair.&rdquo; The <a href="https://pubmed.ncbi.nlm.nih.gov/33002182/">study</a> was published online first on October 1 in the journal <i>Orthopedics</i>.</span></span></p>

<p><span><span>The rotator cuff is a set of four muscles around the shoulder that keeps the joint centered in order to allow the upper arm to rotate. <a href="https://www.hss.edu/condition-list_rotator-cuff-injuries.asp">Rotator cuff tears</a> are common, affecting more than three million people in the United States every year. Most people recover with physical therapy, activity modification, and potentially injections. However, more severe injuries with significant symptoms that continue to affect activities and sleep despite non-surgical approaches require surgical repair. Surgeons typically perform rotator cuff surgery using minimally invasive <a href="https://www.hss.edu/condition-list_shoulder-arthroscopy.asp">shoulder arthroscopy</a>. Recovery involves using an arm sling for four to six weeks and several months of rehabilitation.</span></span></p>

<p><span><span>For their study, the investigators examined de-identified patient records in Humana&rsquo;s national private-payer insurance claims database. They identified 2,759 patients diagnosed with partial or complete rotator cuff tears who subsequently underwent arthroscopic repair between 2007 and 2016 and grouped them according to the period from diagnosis to initial surgery.</span></span></p>

<p><span><span>At a minimum follow-up of five years, 1,510 patients (55 percent) had early repair within six weeks, 1,104 patients (40 percent) underwent routine repair between six weeks and 12 months and 145 patients (5.3 percent) received delayed repair more than 12 months after diagnosis.</span></span></p>

<p><span><span>Overall, the revision surgery rate was 9.6 percent within five years of diagnosis. The differences in revision rates between the delayed repair and both early and routine repair groups were statistically significant. Delayed repair beyond 12 months of diagnosis was associated with an increased risk of undergoing revision surgery compared to routine repair, even after controlling for age and other health conditions, with an odds ratio of 1.97 (p = 0.009).</span></span></p>

<p><span><span>&ldquo;These findings are now informing our discussions with patients about the benefits of surgery and the risks associated with delays,&rdquo; Dr. Fu says. &ldquo;Future prospective studies would benefit from including additional data, such as the exact time from rotator cuff injury to surgery, range of motion, imaging results and return to sport outcomes.&rdquo;</span></span></p>]]></description><category><![CDATA[pressrelease,Fu,Sports Medicine,rotator-cuff-tear,Research Clinical]]></category>

<p><span><span>When asked why of all the salvage procedures SCR was chosen for study, Dr. Dines said, <span>&ldquo;We see a significant number of patients with complicated rotator cuff tears or re-tears that may not heal if operated on using standard rotator cuff repair techniques. Prior to the invention of the SCR, many patients required a reverse shoulder replacement.&rdquo; </span></span></span></p>

<p><span><span><span>He added, &ldquo;While reverse shoulder replacements work great in many patients, they don&rsquo;t last forever so using them in younger patients becomes problematic.&nbsp;Finding a solution to this significant clinical problem was something I was very interested in. When SCR came about it provided hope for these most difficult situations. There was a lot of excitement initially. However, with any newer technique, it is always important to step back after doing some of them to rigorously analyze the results to make sure that patients are benefitting from the technique as much as we expect them to. I think this was the main impetus for the study&hellip;..to critically analyze our results which will help us do a better job selecting patients who are good candidates for the procedure and to be able to counsel them better with regards to postoperative expectations.&rdquo;</span></span></span></p>

<p><span><span><span>Regarding additional work to be done in this area,&nbsp;<span><span>Dr. Dines noted,</span></span> &ldquo;Future studies will focus on three areas: 1. Improving the repair construct: while the overall concept of the SCR is the same for all surgeons, people have slightly different ways to fix the graft or even use different grafts. We are doing studies in our biomechanics lab to try to figure out the best/strongest biomechanical construct for the SCR. 2. Delineating indications: we are getting more granular with our data to figure out which patients are the best candidates for the procedure. Are their certain variables that make a given patient a better or worse candidate for the procedure? 3. Long term outcomes: because this is a relatively new procedure, studying the longer term results is critical.&rdquo;</span></span></span></p>

<p><span><span><span><span>Dr. Dines and colleagues looked at data from 72 patients who had undergone arthroscopic superior capsular reconstruction with dermal allograft at HSS between 2012 and 2017. Almost half of the patients in the study had a history of failed rotator cuff repair. According to data collected via patient-reported outcome surveys including Single Assessment Numeric Evaluation (SANE), American Shoulder and Elbow Surgeons (ASES) and University of California at Los Angeles, most of the patients experienced significant improvements in forward elevation, abduction, and external rotation after survey.</span></span></span></span></p>

<p><span><span><span><span>&ldquo;This technique has proven to be effective for patients with irreparable tears,&rdquo;&nbsp;said Dr. Dines. &ldquo;As we continue to do more outcomes research, we become more precise at predicting the best candidates for each technique. While not for everyone, this has emerged as a great option for patients who may not be right for a reverse shoulder replacement.&rdquo;</span></span></span></span></p>

<p><span><span><span><span>Dr. Dines and his colleagues plan on future research to look at long-term outcomes and results.</span></span></span></span></p>

Complete rotator cuff tears are generally repaired by arthroscopic surgery. While surgical techniques have vastly improved over the years, a high re-tear rate still exists and some tears are not even able to be repaired primarily. For these cases, several salvage procedures exist. One is superior capsular reconstruction (SCR) which uses a graft to reconstruct the superior capsule of the shoulder in the region of the irreparable rotator cuff.</span></span></p>

<p><span><span>A new retrospective study from Hospital for Special Surgery (HSS) evaluated 72 patients who underwent arthroscopic SCR with dermal allograft to treat symptomatic irreparable rotator cuff tears. All patients were operated on at HSS between 2012 and 2017. Nearly 42% of the patients had a history of failed rotator cuff repair.</span></span></p>

<p><span><span>After surgery, the overwhelming majority of patients demonstrated significant improvements in forward elevation, abduction and external rotation through their pre- and post-operative active range of motion. At follow-up appointments, patients completed various patient-reported outcome surveys including Single Assessment Numeric Evaluation (SANE), American Shoulder and Elbow Surgeons (ASES), and University of California at Los Angeles (UCLA). Patient scores were high, averaging 80%, 78% and 85%, respectively.</span></span></p>

<p><span><span>Five patients required a second operation &ndash; no other complications were recorded.</span></span></p>

<p><span><span>&ldquo;This technique has proven to be effective for patients with irreparable tears,&rdquo; said <a href="https://www.hss.edu/physicians_dines-joshua.asp">Joshua S. Dines, MD</a>, HSS sports medicine surgeon and senior study author. &ldquo;As we continue to do more outcomes research, we become more precise at predicting the best candidates for each technique. While not for everyone, this has emerged as a great option for patients who may not be right for a reverse shoulder replacement.&rdquo;</span></span></p>

As the safety and efficacy of these approaches are further defined, changes in the regulatory environment at the FDA level may also aid progress.</p><p>“Orthopedic surgeons must be well informed when discussing biologic agents with patients," added Dr. Rodeo. "It is paramount that orthopedic surgeons provide leadership in this area and work toward developing practice guidelines and policies for the use of biologic agents. Rodeo, MD</a>, a sports medicine surgeon and clinician-scientist at Hospital for Special Surgery (HSS) has launched a clinical trial of an experimental engineered cell therapy that aims to promote better healing of <a href="https://www.hss.edu/condition-list_rotator-cuff-injuries.asp">rotator cuff tears</a> after surgery.</p>

<p>The <a href="https://www.hss.edu/condition-list_rotator-cuff-injuries.asp">rotator cuff</a> is a collection of muscles and tendons at the shoulder joint that allows it to move. Overuse from repetitive overhead activities like swimming, tennis, or pitching a baseball, or <a href="https://www.hss.edu/playbook/tips-to-avoid-rotator-cuff-injuries-2">acute injuries</a>, can cause tears in the muscles or tendons of the rotator cuff.</p>

<p>&ldquo;Arthroscopic shoulder surgery for rotator cuff tears is effective for relieving pain, restoring function and improving quality of life, but up to 25% of patients show incomplete or failed healing on imaging,&rdquo; says Dr. Rodeo. &ldquo;The frequency of incomplete or failed healing is more pronounced in patients over the age of 60. That matters because more people today want to stay active as they age.&rdquo;</p>

<p>Many tissues in the body, including muscles and tendons, harbor stem cells that have the ability to form new tissues. The new clinical trial will test whether injecting an experimental cell therapy into both the muscle and the tendon of an injured rotator cuff will stimulate stem cell activity and promote better repair.</p>

<p>The engineered cell therapy product is E-CEL UVEC<span>&reg;</span>, produced by San Diego based <a href="https://angiocrinebioscience.com/">Angiocrine Bioscience</a>, Inc. The investigational product consists of endothelial cells derived from the walls of the blood vessels of human umbilical cords that have been genetically modified to maintain their stability and their regenerative properties. The US Food and Drug Administration has granted the cell therapy Investigational New Drug (IND) status.</p>

<p>In a <a href="https://news.hss.edu/new-basic-science-study-finds-muscle-derived-activated-endothelial-cells-may-be-promising-for-treatment-of-soft-tissue-injuries/">previous basic science study</a> led by Dr. Rodeo, the cell therapy showed accelerated healing and an increase in the strength of the tendon attachment at the repair site, with no observable side effects, compared to surgery alone.</p>

<p>In the new clinical trial, patients with full-thickness rotator cuff tears who undergo arthroscopic surgical repair will receive two injections of the cell therapy: one into the tendon and one into the adjacent muscle. The study is open-label, meaning that all volunteers will know that they received the cell therapy.</p>

<p>The primary objective of the trial is to evaluate the safety of the cell therapy over 12 months. Dr. Rodeo and his research team will use blood tests to check whether the experimental cells have migrated away from the rotator cuff repair site. The investigators will also evaluate patients&rsquo; symptoms as well as shoulder strength, motion and healing as measured by MRI, ultrasound and patients&rsquo; reports about their progress.</p>

<p>&ldquo;Improving patients&rsquo; symptoms with standard rotator cuff surgery is good, but strength improvements are less predictable,&rdquo; Dr. Rodeo says. &ldquo;Identification of new ways to improve healing and muscle function would represent a significant advance in the treatment of rotator cuff tendon tears.&rdquo;</p>

<p>The study opened September 1, 2019, with the goal of recruiting up to 20 patients ages 45 to 70 with full-thickness rotator cuff tears confirmed by physical examination and MRI. Eligible participants must already have tried and failed standard non-operative approaches, including a minimum of three months of physical therapy, oral anti-inflammatory medications, or a steroid injection.</p>
