Rodeo, MD</a>, sports medicine surgeon at HSS, highlights the promise of <span>regenerative medicine and the importance of advancing research responsibly.&nbsp;</span></p><p style="text-align:start;">It’s hard to miss the buzz around regenerative medicine. Clinics tout “miracle injections” and “next generation cell therapies” across websites, glossy advertisements, and social media, promising to heal joints, reverse arthritis, regrow cartilage, and even restore youth. For patients in pain, or simply hoping to turn back the clock, it can feel as if the future of medicine has already arrived.</p><p style="text-align:start;">But as an orthopedic surgeon and researcher, I have to say what the ads won’t: Regenerative medicine isn’t there yet. The research team shared their study results today in a poster presentation at the American Academy of Orthopaedic Surgeons 2025 Annual Meeting.&nbsp;<br><br>After a knee injury, such as an ACL tear or rupture, immune cells travel to lymph nodes, where they interact with other cells that support and regulate inflammatory responses in the joint. This activity plays a critical role in healing, but prolonged immune cell activity and inflammation can promote osteoarthritis.&nbsp;<br><br>“Orthopedic surgeons actively debate the optimal timing of ACL reconstruction after an ACL injury,” says senior study author <a href="https://www.hss.edu/physicians_rodeo-scott.asp" target="_blank">Scott Rodeo, MD</a>, Vice Chair of Orthopaedic Research, co-director of the Orthopaedic Soft Tissue Research Program, and director of the Center for Regenerative Medicine at HSS. “The original thought for delaying surgery was to let inflammation resolve since operating on a highly inflamed knee can lead to postoperative stiffness, but how long to wait has been unclear.”&nbsp;<br><br>Dr. Rodeo and the HSS research team, including Theresa T. Lu, MD, PhD, St. Giles Chair for Research in the HSS Research Institute and pediatric rheumatologist, investigated the timing question in preclinical models of ACL reconstruction. In total, 16 models underwent immediate reconstruction after ACL rupture, 16 underwent surgery a week after injury, and 16 had no surgery. After four weeks, the researchers used advanced techniques to identify and measure the number of immune cells present in the spleen and iliac lymph nodes that drain from the knee.&nbsp;<br><br>The researchers observed an increase in several immune cell populations in the two surgical groups compared to the no-surgery group. Immune cell counts were significantly higher in the group that had delayed surgery compared to the group that underwent immediate surgery.&nbsp;<br><br>The investigators also assessed cartilage changes associated with osteoarthritis in the preclinical models. The group that had delayed surgery showed more severe and extensive signs of cartilage destruction than the group that had immediate surgery.&nbsp;<br><br>“Overall, our preclinical study suggests that immediate ACL reconstruction may lower the risk of developing knee arthritis, tying in with existing studies showing that chronic, unresolved inflammation leads to progressive arthritis after knee injury,” Dr. Rodeo says. “These results inform ongoing human studies at HSS in which we are evaluating the optimal timing of surgery and other factors that affect the role of immune cells and inflammatory responses in ligament healing. In these studies, we are evaluating changes in immune cells and inflammation in lymph nodes using magnetic resonance imaging, joint fluids and blood.”&nbsp;<br><br>Authors: Thomas M. A new trial led by the Center for Regenerative Medicine at Hospital for Special Surgery (HSS) with a sub-site at Emory University will investigate whether using concentrated bone marrow aspirate (cBMA) can help patients to heal more effectively and prevent PTOA after revision surgery. This translational research study comes from HSS’ new Center for Regenerative Medicine-based trial to utilize a cell-based therapy.</span></p><p style="margin-left:0in;"><span>The new single-blind, randomized, pilot, clinical trial, part of the Arthritis Foundation 2023 FastOA Interventional Pilot Studies, will harness the expertise of surgeons, radiologists, biostatisticians, and cellular and molecular biologists to determine whether cBMA may prevent the chronic inflammation that can lead to pain and stiffness following surgery, and can also contribute to post-traumatic osteoarthritis. Previous studies have suggested that cBMA injections may improve surgical outcomes, but this approach has not yet been evaluated in rigorous clinical trials in patients undergoing ACL revision surgery.</span></p><p style="margin-left:0in;"><span>The cBMA used in these procedures is the patient’s own and is collected from the pelvic bones. The participating patients will be randomized to receive either cBMA injections at the time of revision surgery or standard care.</span></p><p style="margin-left:0in;"><span>The goal of the trial is not only to establish whether the addition of cBMA reduces the risk of PTOA, but also to understand the underlying mechanisms of cBMA therapy and to look for biomarkers in the joint microenvironment that might help to refine the approach and further improve outcomes. Given the lack of effective non-surgical interventions for the prevention and treatment of PTOA, this study is expected to provide valuable insights that could guide care in the future.</span></p><p style="margin-left:0in;"><span>To determine whether any differences observed between the two arms of the trial are clinically meaningful, the primary measure of the treatment’s success will be the patients' self-reported pain levels 12 months after surgery. This will be determined by using the Knee Injury and Osteoarthritis Outcome Score (KOOS)&nbsp;for pain. The researchers hypothesize that cBMA’s functionally relevant immune-modulatory and disease-modifying effects depend on the fluid’s cellular and molecular composition. Talk to the player, see what happened, get them in right away, and evaluate them. I work with my colleague Bob Hotchkiss, our hand and wrist expert and get all the imaging studies. Dr. Hotchkiss did the surgery and did a wonderful job as far as restoring anatomy. But the biggest issue in the recovery process is getting the motion back. Fingers and hands tend to get stiff," said Dr. Rodeo.</span></span></p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:left;">“If you put him in a cast for three months, the bone would heal, but then he couldn't move his fingers. But if you move faster than the bone is healed, then that alignment issue that you are trying to protect becomes compromised. The Jericho High School quarterback was diagnosed with non-Hodgkin’s T-cell lymphoma, a rare type of cancer.</span></span></p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">During his cancer treatment, Brandt had two goals: to get well and to play football again. As soon as he could, he resumed training while receiving chemotherapy treatment, setting up a makeshift gym in his hospital room. Side effects of the grueling treatment weren’t Brandt’s only challenge. At one point, he had to overcome neuropathy in his legs that made walking difficult. He dealt with listeria and appendicitis that required another four-week hospitalization.</span></span></p><p style="text-align:start;">Brandt received the “Honored Hero” award from the Leukemia & Lymphoma Society, which pays tribute to individuals who inspire others with their stories of strength and courage.</p><p style="text-align:start;">For his outstanding character and determination, Brandt previously received the USA Football “Heart of a Giant” award presented by HSS and the New York Giants. The award recognizes high school football players in the tri-state area who demonstrate an unparalleled work ethic, extraordinary dedication and a passion for the game.</p><p style="text-align:start;">In 2022, Brandt &nbsp;faced another challenge that brought him to HSS. Many who know him call him a hero, including his mom Abby, who was with him every step of the way. “He’s a warrior, he’s my rock star,” she said. “I’m the proudest mom in the world.”</p><p>For his outstanding character and determination, Brandt received the USA Football “Heart of a Giant” award presented by <a href="https://www.hss.edu">Hospital for Special Surgery</a> (HSS) and the New York Giants. The award recognizes high school football players in the tri-state area who demonstrate an unparalleled work ethic, extraordinary dedication and a passion for the game.</p><p>In late 2022, Brandt, who will turn 18 in April, faced another challenge that brought him to HSS. The cancer treatment, which included steroids, had contributed to the development of a condition called osteonecrosis in his right knee. Resulting damage to the cartilage and bone made it difficult for him to engage in weight-bearing activities such as running and climbing stairs. His best bet to repair the damage was a cartilage transplant.</p><p><a href="https://www.hss.edu/physicians_rodeo-scott.asp">Scott Rodeo, MD</a>, vice chair of Orthopaedic Research, co-director of the Orthopaedic Soft Tissue Research Program at HSS, and Head Team Physician for the New York Giants performed a surgery known as OATS - osteochondral autograft&nbsp;transfer surgery. “The procedure consisted&nbsp;of taking a piece of cartilage and bone from a non-weightbearing portion of his right knee and transplanting it to the area of the lesion,” Dr. Rodeo explained. “Because of the size of the area to be treated, I used a similar graft from his left&nbsp;knee as well. We also used some bone graft from the iliac crest near the hip.”&nbsp;</p><p>Abby Morgan said she was grateful that the procedure could be scheduled quickly. “Dr. Rodeo made magic happen, and Brandt had the surgery on December 29. I wanted to expedite it because Brandt had some big milestones ahead and the recovery would take time,” she explained. “On March 23, he will receive his last maintenance cancer treatment and ring a bell at the hospital to mark the occasion, and he didn’t want to be on crutches. Later that evening, he will receive the “Honored Hero” award from the Leukemia & Lymphoma Society. Spring break is in April, and he wants to enjoy it, something he hasn’t been able to do in the past two and a half years.”</p><p>HSS staff who got to know Brandt, even briefly, were impressed and inspired by his positive attitude and perseverance. Dr. Rodeo said he was struck by Brandt’s unwavering commitment to achieve his goal to play football again.</p><p><strong>Stephen Melancon, PA-C</strong>, a certified physician assistant who works with Dr. Rodeo, said he was impressed by the way Brandt confronted a challenge that many adults would have trouble grappling with emotionally and physically. Now Brandt has a social media presence to help other young people diagnosed with cancer.</p><p>“For someone his age, with all that he’s gone through, not only to still be able to participate in sports, which is great, but to put himself out there and make himself a resource for other people – it proves he’s an exceptional young individual with incredible drive and outstanding character,” Melancon said.</p><p>“Brandt is truly the definition of exceptional.&nbsp;I don’t know anyone quite like him,” said <strong>Robbie Tran</strong>, senior director of Global Sports Marketing at HSS.&nbsp;“When faced with adversity, he set his mind on a goal and despite multiple setbacks fought until he accomplished what he set out to. He is an inspiration to many,” added Tran, who met Brandt when he received the Heart of a Giant award.</p><p>“He really has the heart of a giant,” said <a href="https://www.hss.edu/physicians_taylor-samuel.asp">Samuel Taylor, MD</a>, a sports medicine surgeon at HSS and Associate Team Physician for the New York Giants, who presented the award to Brandt. “He refused to let cancer stop him. In the process he inspired his friends, family, teammates, and his community.”</p><p>Brandt, who went home from HSS on New Years Eve, continues to make progress following his knee surgery. He is still in physical therapy, which is expected to last 4 to 6 months, and the prognosis is good. At this time, no additional surgery is planned, and he should be able to return to most activities, Dr. Rodeo said.&nbsp;<span>&nbsp;</span></p><p>Although football may no longer be in his future, Brandt still has dreams and goals. He’s looking forward to college and a possible career in sports management. In the meantime, he is dedicated to helping other young people with cancer who contact him on social media or via a website he has created called “<a href="https://sites.google.com/view/comebackkids/home">Comeback Kids</a>.”</p><p>The site was born out of the kindness and generosity of a teenager who has been through a lot, as he seeks to encourage others, candidly describing his long journey from the moment he was diagnosed with cancer. He provides heartfelt advice on ways to cope and maintain a positive attitude. He communicates sincere empathy to comfort young people facing similar challenges.&nbsp;<span> &nbsp;</span></p><p>“When I was undergoing treatment, it was helpful to talk to someone who had been in that situation and who would be honest with me,” Brandt explained. “Being able to help others makes me feel like a better person. These are conditions that affect the muscles, bones and joints. Our patients often participate in a clinical trial to help generate knowledge that will improve their own treatment or will one day benefit people with the same condition. As a foremost center for orthopedic surgery, we conduct research on new technologies and techniques such as robotic-assisted, less invasive spine surgery and joint replacement; the use of a biologic treatment during rotator cuff surgery to enhance healing, and a new technique for ACL repair, to name a few. Basic science research in the laboratory at HSS lays the groundwork for many of our clinical trials, and we have a robust infrastructure in place. This synergy between basic science researchers and those conducting clinical trials is one of our greatest strengths. In the area of rheumatology, HSS physicians and investigators are involved in vital research to find new and better treatment approaches to improve quality of life for people with rheumatoid arthritis, lupus, scleroderma and other autoimmune diseases. HSS is leading the way in clinical trials focusing on pain management after orthopedic surgery. Researchers have published a number of studies on ‘multimodal analgesia,’ which targets multiple pain pathways to reduce the amount of opioid medication needed after surgery. Patient registries are a critical part of our research at HSS. These extensive databases enable us to track patients and evaluate outcomes over the long term. More than 70 registries follow well over 100,000 HSS patients. Rodeo, MD</span></p>]]></description><content:encoded><![CDATA[<p>The HSS Research Institute is dedicated to identifying mechanisms and developing effective approaches for the prevention, diagnosis and treatment of orthopaedic and rheumatic conditions. This week, I discussed innovations in rotator cuff repair, specifically the role of cell therapy, at the American Academy of Orthopaedic Surgeons 2022 Annual Meeting. I outlined what’s currently available, what questions we are currently investigating and what the future looks like.<br><br>Rotator cuff tears are one of the most common musculoskeletal conditions, becoming more prevalent as people age. While most tears need to be treated surgically, healing is an imperfect and slow process. The surgery itself does not reverse the muscle atrophy (thinning or loss of muscle tissue) but involves re-attaching tendon to the bone. Up to 15 to 20 percent of patients may have incomplete or failed healing. The most common issues post-surgery are pain and weakness. To date, there is no approved biologic therapy to improve rotator cuff repair.<br><br>There is a real unmet clinical need to raise the success rate of this repair. More and more older individuals want to get back to an active lifestyle through activities such as tennis and swimming, which put strain on the shoulder. Approximately 200,000 to 250,000 rotator cuff repairs are done each year, with over 1,000 performed at HSS.<br><br>The <a href="https://www.hss.edu/regenerative-medicine.asp" target="_blank">HSS Center for Regenerative Medicine</a>, of which my lab is a part, has a robust research program that investigates the basic cellular and molecular mechanisms of healing damaged tissue. Our multidisciplinary group of scientists and clinicians are trying to understand the underlying reasons why tendons tear and why they don’t heal. We carry out basic science models of tendon repair for conditions including overuse tendonitis and rotator cuff tears.<br><br>Cell therapy has tremendous potential to regenerate tissue. We know that tendons are not highly cellular. We need to determine how to stimulate cells that are intrinsic in tissues or inject a cell population to jumpstart healing. The term “stem cell” is often used, but a more accurate description is a “connective-tissue progenitor cell”, which is a bit more limited in its ability to regenerate tissue. A true stem cell is a rare and highly specialized cell, and we know that true stem cells are present in many tissues. We have discovered that stimulation of these intrinsic stem cells can improve tissue healing and regeneration. Learning how to best leverage these existing cells may greatly improve the healing of rotator cuff tears.<br><br>Additionally, we are continuing to identify ways to characterize cell populations. The cells we can use currently derive from fat or bone marrow and are heterogeneous and highly variable. If we are able to sort them, we can culture only the desired cells and remove undesired cells. This would be a big breakthrough in cell therapy research.<br><br>We need to know which cells are best, where they should be sourced from and what doses to use. I believe there also needs to be changes in the regulatory environment so relevant clinical trials can occur.<br><br>At HSS, it is important that our lab research mirrors clinical work so that we can bring our findings directly to patients. The HSS Center for Regenerative Medicine evaluates a range of cell therapies that seek to heal tissue that has been damaged by injury or disease in order to improve effectiveness of new treatment options.<br><br>We are currently running two clinical trials testing cell therapy for patients undergoing rotator cuff repair. One is a <a href="https://clinicaltrials.gov/ct2/show/NCT04057833">Phase 1B clinical trial</a> that evaluates the safety and feasibility of injecting E-CEL UVEC cells at the end of the operation; these are proprietary allogeneic human umbilical vein endothelial cells produced under cGMP and cGTP regulations. Angiocrine Bioscience is providing support for this study.&nbsp;<span> </span>The other is a <a href="https://clinicaltrials.gov/ct2/show/NCT03332238">Phase II interventional clinical trial</a> that has a similar design except surgeons take adipose tissue from the abdomen at the beginning of the operation, isolate stromal vascular fraction cells (SVFCs) from this tissue, and then inject it into the tendon and muscle after rotator cuff repair. This trial will determine if the use of these cells can enhance outcomes for patients undergoing rotator cuff repair.<br><br>The marketing for stem cells continues to stay ahead of the science. Goodman, MD</span></a><span>, rheumatologist, and </span><a href="https://www.hss.edu/physicians_figgie-mark.asp"><span>Mark P. Figgie, MD</span></a><span>, chief emeritus of the Surgical Arthritis Service, found that Black patients were less likely than white patients to be completely satisfied with the process of care following knee or hip replacement. Socioeconomic status was not found to play a role in patient satisfaction. The researchers reviewed the Press Ganey survey responses of more than 4,600 patients who had surgery at HSS from July 2010 to February 2012. Individuals having a primary knee or hip replacement who resided in New York, New Jersey or Connecticut were included in the study. Although Black patients indicated they were less satisfied with the process of care right after surgery, there was no difference in satisfaction with their joint replacement outcome. More research is needed to investigate other factors, such as perceived staff courtesy and baseline pain and function, to understand why disparities exist to achieve a high level of patient satisfaction for everyone.</span></p><p><a href="https://news.hss.edu/study-finds-cementless-knee-replacement-outcomes-comparable-to-standard-knee-implant-less-time-needed-in-or/"><span><strong>Study Finds Cementless Knee Replacement Outcomes Comparable to Standard Knee Implant, Less Time Needed in OR</strong></span></a></p><p><span>In a study comparing outcomes of a modern cementless knee implant to the standard knee implant that requires bone cement for fixation, HSS hip and knee surgeons </span><a href="https://www.hss.edu/physicians_westrich-geoffrey.asp"><span>Geoffrey H. Westrich, MD</span></a><span>,</span> <a href="https://www.hss.edu/physicians_chalmers-brian.asp"><span>Brian P. Rodeo, MD</a>, sports medicine surgeon, and <a href="https://www.hss.edu/physicians_baxi-naimish.asp">Naimish Baxi, MD</a>, physiatrist.</p><p>Dr. Rodeo noted that regenerative medicine has the potential to revolutionize patient care, but the field is still in its early stages. He added, more research is needed to identify what types of treatments work best for a particular condition and for a given patient.<br /><br />Dr. Rodeo explained, “We can potentially use regenerative medicine therapies for conditions that don’t heal well on their own, such as osteoarthritis, chronic tendonitis, degenerative disc disease in the spine and other injuries. I tell patients these types of treatments can work well for pain relief. However, in terms of regenerating damaged tissue such as cartilage, we’re not there yet.”<br /><br />Some studies have shown platelet-rich plasma (PRP) treatment to be effective in the treatment of chronic tendon injuries and mild to moderate knee osteoarthritis. Dr. Baxi explained, “Although the exact mechanism of action is unclear, evidence suggests that the platelets amplify growth factors that promote tissue repair and healing.”<br /><br />He continued, “Patient selection is key. For someone with severe bone-on-bone arthritis, for example, PRP may not be the most effective treatment option.” He added that a thorough patient history, a discussion of symptoms, and diagnostic imaging tests such as x-rays, MRI and ultrasound are critical to make the diagnosis and determine if PRP would be a good option.<br /><br />Dr. Rodeo underscored vigilance when considering regenerative medicine treatment options. He explained, “The marketing is ahead of the science. Unscrupulous providers and clinics are marketing largely unproven treatments, targeting patients who hope to find relief for a difficult-to-treat problem. People sometimes pay exorbitant sums of money for these unregulated and potentially harmful treatments, possibly delaying a proven treatment that could be beneficial.”<br /><br />At HSS, a number of regenerative medicine studies are under way and enrolling patients. Dr. Rodeo is leading one such clinical trial to see if the injection of certain cells contained in adipose (fat) tissue during rotator cuff surgery can enhance healing. The challenge lies in identifying materials that are effective enough to be cost-effective.&rdquo;</span></span></span></p><p><span><span><span>He noted that more research is needed to provide better understanding of the biology of the materials used in hybrid soft tissue implants, including how the cells interact with the synthetic material, whether the cells proliferate and how the cells can be stimulated to produce new matrix and new proteins.</span></span></span></p><p><span><span><span>Dr. Rodeo continued, &ldquo;Another big area would be how can we implant these synthetic materials with exogenous cells. Could we take some stem cell or progenitor cell population outside the body and apply that to these materials and then apply the material?&rdquo;</span></span></span></p><p><span><span><span>He added, &ldquo;That has exciting potential because we put these materials in the body [and] we rely on the body&rsquo;s own host cells to infiltrate the material. There is a lot we do not know about the cell types that infiltrate the material, how those cells interact with the material. This week, I discussed new and exciting ways we are looking into the prevention of arthritis after knee ligament injury in several sessions at the American Academy of Orthopaedic Surgeons 2021 annual meeting.</span></span></p><p><span><span>Four bands of connective tissue called ligaments support the knee joint: the anterior cruciate ligament (ACL), posterior cruciate ligament, medial collateral ligament and lateral collateral ligament. Ligament damage, such as a tear from a sports injury, makes the knee joint unstable. About half of patients who experience injury to the ACL develop degeneration of cartilage that can lead to <a href="https://www.hss.edu/conditions_knee-arthritis-overview.asp" style="text-decoration:none">knee arthritis</a> over about 15 years. This late effect can be particularly life-altering for young athletes.</span></span></p><p><span><span>HSS research is driven by the fact that we are seeing an increasing number of patients with these problems. Surgery can fix the initial problem of a torn ACL or cartilage and stabilize an injured knee, but we are striving to develop new treatments to prevent post-traumatic arthritis that happens later.</span></span></p><p><span><span>Our researchers have unparalleled access to patient data and tissue samples, making HSS a living laboratory for translating basic research insights into cutting-edge treatments. We&rsquo;re investigating ways to prevent the development of arthritis after knee ligament injury from three different angles. First, we are analyzing changes at the tissue and cellular level in basic laboratory studies and with use of state-of-the-art quantitative magnetic resonance imaging (MRI) techniques that provide unparalleled information on changes in the microscopic structure of cartilage, going far beyond the diagnostic capability of standard MRIs.</span></span></p><p><span><span>Second, we are conducting biomechanical studies using knees from human cadavers, simulating typical gait and loads on joint surfaces and seeing how injury to ligaments and cartilage damage affects movement of the joint. Simulator technology makes it possible to perform thousands of repetitive movements in a much shorter period than observing changes in patients. The biomechanical studies are being paired with 3D computer modeling, helping us to better understand and predict abnormal stresses on the cartilage that lead to arthritis. There are numerous factors that can lead to post-traumatic arthritis, and our hope is that these models will provide insights into why some patients develop arthritis while others do not.</span></span></p><p><span><span>Third, I am leading a new, multi-institutional clinical research study evaluating the use of <a href="https://www.hss.edu/condition-list_prp-injections.asp" style="text-decoration:none">platelet-rich plasma</a> (PRP) injections as a potential treatment for reducing the risk of developing knee arthritis after an ACL injury. PRP injections contain platelets concentrated from the patient&rsquo;s own blood. These platelets provide a rich source of anti-inflammatory chemicals and growth factors known to stimulate repair processes. We want to determine if it can combat the inflammation that is a big culprit behind progressive degeneration in the joint.</span></span></p><p><span><span>We plan to recruit patients with ACL injuries. Patients will be randomly assigned to receive one PRP injection at diagnosis and another during surgery to repair a torn ACL or to receive placebo injections instead and will be followed for two years.</span></span></p><p><span><span>Joint fluid samples will be analyzed over time to look for inflammatory markers by using state-of-the-art laboratory techniques and quantitative MRI techniques that will allow us to detect biochemical changes in cartilage tissue within a year or two, much faster than the development of changes on routine x-rays, which can take up to 15 years. We will combine this information with patients&rsquo; symptoms and measures of function, pain and strength.</span></span></p><p><span><span>PRP has great potential, but one of the biggest challenges to date is its variability. As a blood product, the components vary greatly between patients and even on different days from the same patient. To address this issue, we will analyze a small sample of each patient&rsquo;s PRP in the lab to look for common characteristics that lead to a positive response in warding off the development of arthritis.</span></span></p><p><span><span>We are proud to be leading multiple avenues of research into new ways to prevent knee arthritis in patients with knee ligament injuries.</span></span></p><p><span><span>- <a href="https://www.hss.edu/physicians_rodeo-scott.asp" style="text-decoration:none">Scott A. Callahan, MD</a>, sports medicine physician, about the challenges that athletes and medical professionals are facing during the Olympic games.</span></span></p><p><span><span>Dr. Callahan explained the importance of being vaccinated against COVID-19, but when that&rsquo;s not possible, &ldquo;it will be important for those at the Olympics to remember the mitigation strategies that have proven important in prevention,&rdquo; she advised.</span></span></p><p><span><span>On the importance of a COVID-19 vaccine, &ldquo;The truth is, the risk for serious illness from COVID is so much greater than the risk for serious side effects from a vaccine. That to me is the take-home message for all the athletes,&rdquo; noted Dr. Callahan.</span></span></p><p><span><span>Dr. Rodeo agreed, &ldquo;We need to continue to follow the data. And luckily, there's a huge international effort to accumulate more data. I think that you made the two important points. Number one, first, these risks &mdash; the side effects of the blood clots in females and myocarditis in men &mdash; although concerning, they're uncommon. In fact, there's still some question as to whether they're even higher than the normal rate you'd expect from any other vaccine or just in the underlying population. Number two, all that said and all that aside, the risk for illness from COVID is much higher than the risk for these side effects.&rdquo;</span></span></p><p><span><span>Dr. Callahan praised the efforts of those on-site managing the safety and logistics for the athletes. She continued, &ldquo;I think we have learned, as you said, the more testing, the better, and really on a daily basis is probably your best way to go, because we know you can be infectious and be in the presymptomatic or asymptomatic phase. Rodeo, MD</a>, sports medicine surgeon at HSS.</span></span></p><p><span><span>According to Dr. Rodeo, there are some limited data to support a possible use of biologic agents for elbow conditions. He explained the most common elbow problems in athletes are attritional soft tissue injuries due to repetitive use.</span></span></p><p><span><span>He continued, &ldquo;Symptoms associated with these conditions are often due to the associated inflammatory response and blood-based agents, such as PRP, may have a positive role via their production of immunomodulatory and anti-inflammatory mediators. Rodeo, MD</a>.</p><p>Dr. Rodeo, who is also a team physician for the New York Giants since 2015, has participated in medical reviews that have examined the vast database of injuries from the combine from 1987-2000. Players who had anterior cruciate ligament (ACL) surgery in college did not play fewer games or have shorter careers, but those who had meniscus procedures before entering the NFL played about two fewer seasons.</p><p>Dr. Rodeo explained why meniscus issues can be red flags for NFL teams. &ldquo;Meniscus injuries often lead to progressive arthritis over time,&rdquo; he said. &ldquo;They are important shock absorbers for the knee. Loss of meniscus does increase the risk of degenerative changes in the knee. In particular, the lateral meniscus, which is the outer side&hellip;And we don&rsquo;t have great solutions. These projects reflect the institution’s expertise in basic, translational and clinical research, and clinical care. Research in mice infected with SARS-CoV, a coronavirus similar to the one that causes COVID-19, has suggested that plasmacytoid dendritic cell precursors (pDCs) are key to the immune response to infection. These pDCs activate macrophages, which in turn secrete cytokines. In the SARS-CoV research, depletion of pDCs appeared to protect the mice from lethal lung injury. Using blood samples from donors, the investigators will study the pathway by which pDCs activate macrophages and look at ways to therapeutically block that process.</span></p><p><strong>Inhibiting RNA Polymerase II Transcription Complexes in Macrophages to Target COVID-19–Associated Cytokine Storm</strong></p><p><span><strong>Principal Investigator:</strong> </span><a href="https://www.hss.edu/research-staff_rogatsky-inez.asp"><span>Inez Rogatsky, PhD</span></a></p><p><span><strong>Co-Investigators:</strong> Steven Josefowicz, PhD, and Robert P. Fisher, MD, PhD</span></p><p>This pilot project will dissect the role of macrophages in SARS-CoV-2-induced acute respiratory distress syndrome (ARDS), the main driver of COVID-19-associated mortality. We will test small-molecule inhibitors of RNA Polymerase II (Pol II) transcription complexes for their ability to modulate type I interferon and inflammatory pathways in monocytes/macrophages. This research will be done using cultured macrophages as well as donor blood and blood from COVID-19 patients.</p><p><span><strong>Mechanisms of Cytokine Storm in Patients with COVID-19</strong></span></p><p><span><strong>Principal Investigator:</strong> </span><a href="https://www.hss.edu/physicians_crow-mary.asp"><span>Mary K. The project aims to characterize immune cell populations seen in COVID-19 patients who experience cytokine storm and compare them to those patients who do not, to compare the immune response before and after patients are given the anti-inflammatory drug anakinra, and to identify measures that suggest patients are more likely to decline and eventually require mechanical ventilation. This study will evaluate the response to and recovery from total knee arthroplasty (TKA) in people who have antibodies to COVID-19 — a marker of exposure. This study will include both patients who have COVID-19 antibodies and those who don’t, to act as controls. Patients will be followed for six weeks after surgery and evaluated for the presence of certain immune markers in the blood, as well as symptoms of inflammation including pain and stiffness in the joint.</span></p><p><span><strong>SARS-CoV-2 Exposure and the Role of Vitamin D Among Hospital Employees</strong></span></p><p><span><strong>Principal Investigator:</strong> </span><a href="https://www.hss.edu/physicians_stein-emily.asp"><span>Emily M. This study will investigate vitamin D status and associated immune markers as risk factors for COVID-19 infection in a cohort of healthcare workers. Healthcare workers are at higher risk of contracting COVID-19 than the general population, making them a good group to study. Vitamin D is critical for immune function and is known to be protective against respiratory-tract infection and tuberculosis. As elective surgeries resume, those with prior exposure to SARS-CoV-2 will inevitably present for treatment, and some may have perioperative management considerations related to their risk of deep-vein thrombosis. This project will use, a noninvasive device that can determine clotting risks, to investigate whether people who have had COVID-19 have a more dysfunctional endothelium preoperatively and at 24 hours after surgery. <p><span><span>Dr. Kinderknecht noted, &ldquo;What has been developed is an infectious disease emergency response team, that is an entity the NFL mandated which involves several of us. We look at our protocols for cleaning. Everybody is screened every day for symptoms. Players and what we call tier 1 &ndash; or the people who are going to be directly related to the players are tested regularly. A lot of mechanisms are in place to ensure that the work environment we create is as COVID free as possible.&rdquo;</span></span></p>

<p><span><span>Dr. Rodeo added, &ldquo;We take a lot of precautious to help our players get back in to it. Although they&rsquo;re in shape from running, they may not be in football shape by having done football drills. Its carefully monitoring how they respond to their practices. We&rsquo;ll carefully talk to players about symptoms, and really monitor how they do. There&rsquo;s no one size fits all - each player is different. The important thing is daily check-ins with the players, daily monitoring by our athletic training staff and our physicians.&rdquo;</span></span></p>

<p>&ldquo;We go out and see the player. We talk to them, get a quick sense of where they are with injuries and where they are hurt. And so we&rsquo;ll do an examination right there. Sometimes your best exam is right there on the field, which I&rsquo;ll look at the injured area to make a quick, but accurate assessment,&rdquo; explained Dr. Rodeo. &ldquo;We definitely have distinct protocols for different injuries and standard ways for evaluating. The first, our primary concern is the players&rsquo; health and safety. So we go through a thorough evaluation. And if a player can be cleared to go back, great! Let them go back. This may directly impact recovery from orthopedic surgical procedures,&rdquo; he noted.&nbsp;</span></span></span></span></span></span></p>

<p><span><span><span><span><span><span>The authors wrote there is evidence suggesting the SARS infection leads to deficits in both muscle strength and endurance, likely due to the proinflammatory effects of the viral infection and the deconditioning that occurs during the convalescent period. The reduced functional capacity of these patients corresponded with decreases in several indices of health-related quality of life.</span></span></span></span></span></span></p>

<p><span><span><span><span><span><span>&ldquo;Another important point is the immune dysfunction and altered inflammatory response in COVID-19 patients. Surgery, of course, also induces an inflammatory response, and the stress response to surgery when combined with altered immune function in COVID-19 patients may lead to surgical complications. An altered immune/inflammatory response may have a profound effect on wound healing,&rdquo; said Dr. Rodeo.</span></span></span></span></span></span></p>

Much of that attention has been directed toward head injuries and protecting quarterbacks. Additionally, wide receivers topped the list, while cornerbacks and safeties ranked second and third, respectively.</p>

<p><em>Forbes </em>spoke to <a href="https://www.hss.edu/physicians_rodeo-scott.asp">Scott A. Rodeo, MD</a>, sports medicine surgeon at HSS, who&nbsp;discussed the impact of injuries and injury prevention in the league. &ldquo;Injury susceptibility is multifactorial, and careful analysis of injuries requires distinguishing injuries that may be due to overuse versus injury due to contact,&rdquo; he noted. &ldquo;It is well-established that muscle strain injuries (such as a hamstring injury) occur in sprinting athletes. Contact that occurs when an individual is running at a high rate of speed likely plays a role as a risk factor for injury."</p>

<p>While there was an uptick of concussions between 2018 and 2019, Dr. Rodeo is encouraged by the progress made by the league when treating head injuries. &ldquo;Although the trend in concussion numbers is encouraging, more important is the recognition that these numbers reflect the increased attention by the NFL on concussion detection, early evaluation and efforts to improve player awareness,&rdquo; he said.&nbsp;&ldquo;These efforts, combined with rule changes and equipment modifications to mitigate concussion risk, should continue to improve player safety.&rdquo;</p>

]]></description><content:encoded><![CDATA[<p>Health Day reports on the findings of a study which suggest humans have the ability to restore cartilage in the joints, by using a molecular process similar to one that allows a salamander to grow a new limb. Additionally, researchers found the capability exists in a "gradient" - with it being greatest in the ankle, less apparent in the knee, and lowest in the hip.</p>

<p><a href="https://www.hss.edu/physicians_rodeo-scott.asp">Scott A. Rodeo, MD</a>, sports medicine surgeon at HSS, who was not involved in the study, provided commentary on the results, inquiring if this could explain why osteoarthritis is common in the knees, hips, but not the ankles. "Its been assumed that it's related to the biomechanics of the joints," said Dr. Rodeo. However, this study suggests there might be intrinsic differences in the joints' ability to repair cartilage, he added.</p>

<p>Additionally, Dr. Rodeo inquired whether this human capacity can translate into new treatments for arthritis, noting "Can we better understand the basic biology, and harness it?" Furthermore, he posed, "Can we learn lessons from animals that do regenerate tissue, and apply that to humans?"</p>

As the safety and efficacy of these approaches are further defined, changes in the regulatory environment at the FDA level may also aid progress.</p><p>“Orthopedic surgeons must be well informed when discussing biologic agents with patients," added Dr. Rodeo. "It is paramount that orthopedic surgeons provide leadership in this area and work toward developing practice guidelines and policies for the use of biologic agents. Rodeo, MD</a>, a sports medicine surgeon and clinician-scientist at Hospital for Special Surgery (HSS) has launched a clinical trial of an experimental engineered cell therapy that aims to promote better healing of <a href="https://www.hss.edu/condition-list_rotator-cuff-injuries.asp">rotator cuff tears</a> after surgery.</p>

<p>The <a href="https://www.hss.edu/condition-list_rotator-cuff-injuries.asp">rotator cuff</a> is a collection of muscles and tendons at the shoulder joint that allows it to move. Overuse from repetitive overhead activities like swimming, tennis, or pitching a baseball, or <a href="https://www.hss.edu/playbook/tips-to-avoid-rotator-cuff-injuries-2">acute injuries</a>, can cause tears in the muscles or tendons of the rotator cuff.</p>

<p>&ldquo;Arthroscopic shoulder surgery for rotator cuff tears is effective for relieving pain, restoring function and improving quality of life, but up to 25% of patients show incomplete or failed healing on imaging,&rdquo; says Dr. Rodeo. &ldquo;The frequency of incomplete or failed healing is more pronounced in patients over the age of 60. That matters because more people today want to stay active as they age.&rdquo;</p>

<p>Many tissues in the body, including muscles and tendons, harbor stem cells that have the ability to form new tissues. The new clinical trial will test whether injecting an experimental cell therapy into both the muscle and the tendon of an injured rotator cuff will stimulate stem cell activity and promote better repair.</p>

<p>The engineered cell therapy product is E-CEL UVEC<span>&reg;</span>, produced by San Diego based <a href="https://angiocrinebioscience.com/">Angiocrine Bioscience</a>, Inc. The investigational product consists of endothelial cells derived from the walls of the blood vessels of human umbilical cords that have been genetically modified to maintain their stability and their regenerative properties. The US Food and Drug Administration has granted the cell therapy Investigational New Drug (IND) status.</p>

<p>In a <a href="https://news.hss.edu/new-basic-science-study-finds-muscle-derived-activated-endothelial-cells-may-be-promising-for-treatment-of-soft-tissue-injuries/">previous basic science study</a> led by Dr. Rodeo, the cell therapy showed accelerated healing and an increase in the strength of the tendon attachment at the repair site, with no observable side effects, compared to surgery alone.</p>

<p>In the new clinical trial, patients with full-thickness rotator cuff tears who undergo arthroscopic surgical repair will receive two injections of the cell therapy: one into the tendon and one into the adjacent muscle. The study is open-label, meaning that all volunteers will know that they received the cell therapy.</p>

<p>The primary objective of the trial is to evaluate the safety of the cell therapy over 12 months. Dr. Rodeo and his research team will use blood tests to check whether the experimental cells have migrated away from the rotator cuff repair site. The investigators will also evaluate patients&rsquo; symptoms as well as shoulder strength, motion and healing as measured by MRI, ultrasound and patients&rsquo; reports about their progress.</p>

<p>&ldquo;Improving patients&rsquo; symptoms with standard rotator cuff surgery is good, but strength improvements are less predictable,&rdquo; Dr. Rodeo says. &ldquo;Identification of new ways to improve healing and muscle function would represent a significant advance in the treatment of rotator cuff tendon tears.&rdquo;</p>

<p>&ldquo;We continue to enjoy working with Dr. Rodeo and his team on this long-term collaborative effort and look forward to providing support for this exciting new therapeutic application of E-CEL UVEC cells,&rdquo; commented Paul Finnegan, MD, Angiocrine&rsquo;s CEO.</p>

<p>The study opened September 1, 2019, with the goal of recruiting up to 20 patients ages 45 to 70 with full-thickness rotator cuff tears confirmed by physical examination and MRI. Eligible participants must already have tried and failed standard non-operative approaches, including a minimum of three months of physical therapy, oral anti-inflammatory medications, or a steroid injection.</p>

                        <title>What Everyone Should Know About ACL Surgery</title>

On behalf of Hospital for Special Surgery, the world&rsquo;s largest academic medical center specializing in musculoskeletal health, we the surgeons of the HSS Sports Medicine Institute have drafted the following guidelines for people who are considering ACL treatment.</p>

<p>Reconstruction is the current standard of care surgical treatment for tears of the anterior cruciate ligament (ACL). This procedure typically uses a graft, or a piece of tissue, placed in the knee minimally invasively, using small incisions. About 300,000 ACL reconstructions are performed annually in the United States with well documented rates of success.</p>

<p>ACL repair is an older technique that involved suturing (sewing) torn ACL tissue, not rebuilding it with a graft. ACL repair was performed in the 1970s at select institutions, including Hospital for Special Surgery, but was abandoned due to unacceptably high failure rates of up to 50%.</p>

<p>Today, ACL repair has been modernized and can be performed through a minimally invasive approach. Proponents suggest that modern ACL repair techniques may be performed safely and may lead to a quicker recovery than ACL reconstruction. However, the limited studies to date raise questions that must be discussed in detail with your surgeon.</p>

<h2><strong>1. It is critically important to get ACL surgery right the first time.</strong></h2>

<p>When ACL surgery fails, surgeons must do a revision procedure to correct or revise the problems that were not fully addressed the first time. Revision requires reconstructive ACL surgery with a graft. (ACL repairs cannot be done as revisions.) Redoing ACL surgery may result in higher rates of failure, lower rates of return to sports, and increased risk of osteoarthritis.</p>

<p>The importance of successful initial ACL surgery holds true for people of all ages, but it is particularly critical for young athletes, for whom a failed surgery can have devastating consequences, including years lost from sport in the short term as well as chronic pain and loss of function in the long term.</p>

<h2><strong>2. Failure rates for ACL repair appear to be significantly higher than those for ACL reconstruction in people of all ages.</strong></h2>

<p>Current studies suggest that modern ACL repair techniques have a failure rate that may be five to ten times higher than that of ACL reconstruction<sup>1,2,3</sup>. One major recent study in a prominent sports medicine journal found that arthroscopic ACL repair has unacceptable outcomes in young athletes, with short-term failure rates of close to 50%<sup>2</sup>.</p>

<p>It has been suggested that ACL repair may be appropriate for older adults who have a type of ACL tear called a proximal avulsion and who do not participate in &ldquo;cutting and pivoting&rdquo; sports such as basketball, volleyball, and soccer. For these patients, failure rates in the short term appear to be much higher than ACL reconstruction in this age group<sup>1,2,3</sup>.</p>

<h2><strong>3. There is still much we don&rsquo;t know about ACL repair.</strong></h2>

<p>There has been no scientific evidence that shows a difference in postoperative pain between ACL repair and reconstruction. Rehabilitation and return-to-play protocols for ACL repair have not yet been established. Proper guidelines for rehabilitation and return to sports are essential to keep people safe from re-injury.<br />

]]></description><content:encoded><![CDATA[<p><em>The New York Times</em> reports many people have become intrigued by the use of stem cell treatments for damaged joints. While some people have claimed this to have helped, there is no clear evidence that these treatments work, and the safety has yet to be established. Most researchers feel the efforts to sell therapies involving adult stem cells, which can develop into different types of cells to replenish tissue, have gotten way ahead of the science. The popularity of these treatments has prompted academic centers to offer similar procedures, as they are accompanied by efforts to collect data to determine their efficiency.</p>

<p><em>The New York Times</em> spoke to <a href="https://www.hss.edu/physicians_rodeo-scott.asp">Scott A. Rodeo, MD</a>, orthopedic surgeon at HSS, where these treatments are being studied, who explained, &ldquo;&lsquo;Modest&rsquo; data suggested that platelets might ease the pain in arthritic or inflamed knees and elbows.&rdquo; Dr. Rodeo added, &ldquo;There&rsquo;s great potential,&rdquo; however, &ldquo;we are just not there yet.&rdquo;</p>

                        <title>New Basic Science Study Finds Muscle-Derived Activated Endothelial Cells May Be Promising for Treatment of Soft Tissue Injuries</title>

Cell-based approaches have shown potential in improving tendon-to-bone healing in these tears.</p>

<p>A new basic science study led by <a href="https://www.hss.edu/physicians_rodeo-scott.asp">Scott A. Rodeo, MD</a>, clinician-scientist at HSS, looked to evaluate the effects of the novel population of muscle-derived activated endothelial cells (mECs) implanted at the repair site to analyze the gene expression comprehensively by means of RNA sequencing.&nbsp;</p>

<p>Through cell tracking, the researchers found that there were green fluorescent protein (GFP)-positive viable mECs at the repair site of the supraspinatus tendon (SST) at three days with decreasing positive cell numbers at seven days.</p>

<p>This is the first study to show that activated endothelial cells derived from muscle tissue can enhance healing of tendon-to-bone in as early as 14 days.</p>

<p>"This pre-clinical data demonstrates the potential for these cells to stimulate the intrinsic progenitor cells in tendons. Ultimately, umbilical vein endothelial cells can now be used in a clinical trial of patients undergoing rotator cuff repair," said Dr. Rodeo, who adds that further studies are needed to identify the ideal patient, timing of dosing, and best way to deliver the cells to treat various types of tissue injuries.</p>

<p>This paper received the Charles Neer award from the American Shoulder and Elbow Surgeons Society (ASES) and was presented at ASES 2019 Specialty Day as part of the American Academy of Orthopedic Surgeons conference.</p>

<p>HSS sports medicine surgeon <a href="https://www.hss.edu/physicians_rodeo-scott.asp">Scott A. Rodeo, MD</a>, who did not treat Wentz, noted how coaches and physicians measure a player's benchmarks throughout the rehabilitation process.</p>

<p>"You try to measure function… Running, planting, pivoting, deceleration, change of direction. We do specific tests. A triple-hop test, or how someone lands from a jump. We can do some videotape analysis to look at movement patterns to get some objective information to quantify function," said Dr. Rodeo.</p>

<p>Additionally, Dr. Rodeo explained that they also use GPS to measure information on the volume and speed of an athlete's run.</p>

                        <title>HSS Researchers Launch Study to Look for Clues to Meniscal Healing After Surgery</title>

                        <guid>https://news.hss.edu/hss-researchers-launch-study-to-look-for-clues-to-meniscal-healing-after-surgery/</guid><pp:caseid>321372</pp:caseid><description><![CDATA[<p>Researchers at Hospital for Special Surgery in New York City are launching a new study to identify inflammatory molecules that might help improve treatment of people with certain knee injuries.</p>

<p>The goals of the study, which is now enrolling patients, is to find biomarkers correlated with the development of <a href="https://www.hss.edu/condition-list_osteoarthritis.asp">osteoarthritis</a> of the joint after surgery to fix traumatic tears of the meniscus.&nbsp;This horseshoe-shaped layer of cartilage cushions the bones of the knee during weight-bearing activities like running and jumping.</p>

<p>More than one million Americans annually undergo meniscal surgery, making it among the most common orthopedic procedures performed in the United States. The injuries generally occur in young athletes who experience violent trauma to the knee, although older people can suffer them without such trauma, as well. However, patients who undergo the procedure often develop osteoarthritis of the joint&mdash;a painful, degenerative condition that to date has no cure or particularly effective treatment.</p>

<p>"The bottom line is, this is a very common orthopedic surgery and the results are not very good," said <a href="https://www.hss.edu/physicians_altchek-david.asp">David W. Altchek, MD</a>, sports medicine surgeon and co-chief emeritus of HSS Sports Medicine, and medical director for the New York Mets. "It seems like the surgery makes a small number of people better, some have an indifferent response and, unfortunately, a significant number of people get worse."</p>

<p>"We don&rsquo;t really know much about the biology of how the meniscus heals. We kind of let pain be the guide," added <a href="https://www.hss.edu/research-staff_mendias-christopher.asp">Christopher Mendias, PhD, ATC</a>, associate scientist at HSS, who is helping conduct the study. "We want to be able to say, 'Is it healing well, and is there a correlation between doing poorly clinically and elevated levels of pro-inflammatory biomarkers or that we can detect with advanced imaging?'"</p>

<p>The HSS study will have several components. Drs. Altchek, Mendias and their colleagues will take samples of synovial fluid--a viscous substance that lubricates joints--from 28 patients (14 men and 14 women) aged 18 to 45 undergoing a procedure called partial meniscectomy in which surgeons shave away frayed pieces of the cartilage. Those samples will be screened for the presence of proteins associated with inflammation, molecules such as interleukin 1 beta, which is known to play a role in osteoarthritis, as well as other previously unidentified substances.</p>

<p>The researchers also will use an advanced form of magnetic resonance imaging to track meniscal healing over time, and will match that information with the biomarker activity to look for patterns of protein activity that predict either successful healing or poor outcomes.&nbsp;&nbsp;</p>

<p>"This whole idea of precision medicine is just kind of catching on in orthopedics now," Dr. Mendias said. Ideally, a biomarker panel could help surgeons determine which patients will be most likely to benefit from particular meniscal procedures, and what kind of rehabilitation--and even prehabilitation--routine they should undergo. "It could also help us identify therapies to block those inflammatory molecules and reduce the risks of osteoarthritis later in life," he said.</p>

<p>"The hypothesis is that there is a degenerative effect afterward, as a result of the injury or the surgery, or both&mdash;and that biologic therapy might slow that down," Dr. Altchek said. He added that there is "a tremendous amount of excitement" in the field of orthopedics about new biologic therapies, including platelet rich plasma and stem cells, that are now being used to treat tendon injuries and certain other ailments. Whether they might work to treat or even prevent the development of osteoarthritis after knee surgery is unclear. "This project is an important step in the direction" of answering that question, he said.</p>

<p><a href="https://www.hss.edu/physicians_rodeo-scott.asp">Scott A. Rodeo, MD</a>, sports medicine surgeon, co-chief emeritus of HSS Sports Medicine, and co-director of the Tissue Engineering, Regeneration, and Repair Program, will be contributing patients to the study.</p>

<p>"A common clinical observation is that there is tremendous variability in the rate and risk of progressive degenerative changes in patients undergoing surgery for meniscus injury," said Dr. Rodeo, who also is head team physician for the New York Giants. "Some patients will do well for decades with no further problems, while there is a subset of patients who develop relatively rapid progression of degenerative changes. In addition to biomechanical factors, it is likely that the &lsquo;biologic milieu&rsquo; in the joint is a major factor accounting for these discrepant outcomes. In this study we will characterize inflammatory mediators, matrix degrading enzymes, and a host of other cytokines that likely play an important role in joint homeostasis. Such information will not only help us gain a better understanding of the underlying mechanism of joint degeneration in patients with meniscus injury, but, importantly, may suggest therapeutic targets to allow development of innovative treatments."</p>
