The study, published in Science Immunology, identifies a new role for specialized skin immune cells, called Langerhans cells, in shaping the development of lymphatic vessels—an essential part of the body’s immune network.</p><p>Lymphatic vessels act as communication highways, carrying signals from tissues to the immune system to alert it to infection, injury, or disease. When this system does not function properly, the body may struggle to fight infections, respond to vaccines, repair tissue, or control inflammation.</p><p>The new findings show that Langerhans cells play a key role during early life by helping lymphatic vessels grow and develop properly in the skin. This early “setup” has long-term consequences, influencing how well the immune system functions later in life.</p><p>“Our study shows that immune cells in the skin do much more than respond to threats—they help build the infrastructure the immune system depends on,” said senior author <a href="https://www.hss.edu/profiles/research/theresa-lu">Theresa T. Lu, MD, PhD</a>, who holds<span> </span>the St. Giles Chair for Research in the HSS Research Institute and is a member of Pediatric Rheumatology at HSS. “If this process is disrupted early in life, it can have lasting effects on immune health. Our results also underscore the critical role of skin health in regulating lymphatic vessel development and immune system function in health and disease.”</p><p>“These findings advance our understanding of how early-life biological processes influence long-term immune function,” explained <span>JiHyun Sim</span>, PhD, <span>first author and Postdoctoral Fellow at HSS</span>. “We are hoping to further understand if this physiologic process is disrupted to contribute to autoimmune disease.”</p><p><strong>Key Findings</strong></p><ul style="list-style-type:disc;"><li data-list-item-id="ea0581f7ccb19c074037353d6c12cd9e7"><strong>Early-life development matters:</strong> Proper formation of lymphatic vessels in childhood is essential for strong immune responses in adulthood.</li><li data-list-item-id="ec7ccb42616025c65ae8942e23b561b8f"><strong>New role for Langerhans cells:</strong> Beyond their known immune functions, these cells help guide the growth and programming of lymphatic vessels.</li><li data-list-item-id="e1656640b38c53f727033c51472cc1c55"><strong>Long-term impact:</strong> When Langerhans cells are reduced or impaired early in life, the immune system may be less effective later on.</li></ul><p><strong>Implications for Patients and Providers</strong></p><p>The research suggests that conditions affecting skin health in early life—such as severe sunburn, burns, or certain autoimmune diseases like lupus—could interfere with Langerhans cells and, in turn, disrupt lymphatic development. This may contribute to weaker immune responses later in life, including reduced effectiveness of vaccines, increased susceptibility to infections, impaired healing, and potential autoimmune complications.</p><p>“These findings highlight the importance of protecting skin health early in life,” said Dr. Lu. “They also open the door to new approaches that could strengthen immune function by targeting the lymphatic system.”</p><p><strong>A New Way of Thinking About Immune Health</strong></p><p>The study adds to a growing body of evidence showing that early-life health and environmental factors can shape disease risk in adulthood. While current therapies largely focus on controlling inflammation, the mechanisms that drive the abnormal growth of synovial tissue—and its invasive portion, known as pannus, which erodes cartilage and bone—have remained poorly understood.</p><p><strong>A New Model of Tissue Growth in RA</strong></p><p>Using advanced spatial transcriptomics to analyze human tissue samples, the HSS research team discovered that <i>SPP1</i><span>ʰⁱ</span>macrophages cluster within fibrin-rich niches in the synovium. Fibrin, a protein typically involved in blood clotting and wound healing, appears to act as a temporary scaffold that supports new tissue formation.</p><p>Within these niches, the macrophages were found to:</p><ul style="list-style-type:disc;"><li data-list-item-id="ee6984bd9a342ee4643137a10c934eb27">break down fibrin scaffolds through enzymatic activity and cellular uptake;</li><li data-list-item-id="e73e0a06f81b3a40dae35d2dd629454d0">stimulate nearby fibroblasts, key structural cells, to proliferate; and</li><li data-list-item-id="e8f94ea5f741be8a405d66fa9012f021c">promote tissue remodeling and expansion independent of traditional inflammatory pathways.</li></ul><p>“This work suggests that rheumatoid arthritis is not only driven by inflammation, but also by dysregulated tissue repair processes,” said <a href="https://www.hss.edu/profiles/research/laura-donlin">Laura Donlin, PhD</a>, senior author and scientist at the HSS Research Institute. “We are seeing a coordinated interaction between immune cells, structural cells, and the extracellular matrix that fuels abnormal growth.”</p><p><strong>Beyond Fibrosis: A Distinct Pathway</strong></p><p>Interestingly, while <i>SPP1</i><span>ʰⁱ</span>macrophages resemble cells involved in fibrotic diseases such as lung or liver fibrosis, the RA tissue environment lacks the dense collagen buildup characteristic of fibrosis. Instead, the researchers describe this process as “pro-generative” tissue remodeling, more closely resembling wound healing gone awry.</p><p><strong>Therapeutic Implications</strong></p><p>The findings also suggest that targeting <i>SPP1</i><span>ʰⁱ</span>macrophages and their signaling pathways could represent a new therapeutic approach. The study highlights the role of IL-6 signaling in sustaining these cells, offering further insight into why IL-6–targeting therapies, already used in RA, may be particularly effective in certain patient populations.</p><p>“Current treatments focus on suppressing inflammation, but our findings point to additional pathways that drive disease progression,” said Dr. Donlin. “Targeting these tissue remodeling processes could open the door to more precise and effective therapies.”</p><p><strong>Broader Impact</strong></p><p>The study’s findings may extend beyond RA. General practitioners (GPs) within England's National Health Service (NHS) are not providing the level of support needed by people living with frailty, says the report, which outlines a suite of recommendations aimed at a more consistent, community-focused and patient-centric approach to frailty in England.</span></p><p><span style="text-align:start;">The lack of a structured approach is also true in rheumatology, according to </span><a href="https://www.hss.edu/profiles/doctors/sarah-lieber" target="_blank"><span style="text-align:start;">Sarah B. They made these presentations at ACR Convergence 2025, the annual meeting of the American College of Rheumatology, held in Chicago from October 24-29.&nbsp;<br><br>"Social determinants of health — such as food insecurity, low health literacy, limited language proficiency, and limited education — are responsible for more than 70 percent of adverse health outcomes and drive inequities in care," explained Jillian Rose-Smith, PhD, MPH, MSW, Vice President and Chief Equity Officer at HSS. "Rheumatologic conditions are unpredictable and unrelenting. If a patient's physician gave them an expensive biologic medication, for example, treatment will fail if the patient doesn't have a refrigerator to keep it in. That's why it's so important for practice guidelines to align with patients' lived experiences."&nbsp;<br><br><strong>Study: Predictors of 30-Day Readmissions in Pregnant Patients with Systemic Lupus Erythematosus: The Impact of Social Determinants of Health&nbsp;</strong><br><br>Prior research on pregnancy outcomes in people with lupus has focused primarily on clinical risk factors and racial disparities. HSS investigators led the largest national study to date examining how SDOH affect hospital readmissions in pregnant women with lupus, who face disproportionately higher maternal health risks. Using an AI model called the Explainable Boosting Machine, which is capable of differentiating the effects of interconnected social and medical factors, the researchers analyzed hospital data from 28 U.S. states (2011-2021) to explore the influence of SDOH on 30-day hospital readmission rates among patients with lupus.&nbsp;<br><br>Among more than 66,000 women with lupus who were admitted to the hospital for pregnancy, 2,469 were readmitted within 30 days of discharge. Women who lived in lower-income neighborhoods and those with public insurance (Medicaid or Medicare) were much more likely to be readmitted within 30 days than those with higher incomes or private insurance. While these SDOH have been associated with poorer health outcomes in people with other medical conditions, their impact is magnified in patients with lupus, especially during pregnancy.&nbsp;<br><br>Women with serious medical issues — such as pre-eclampsia, chronic kidney disease, hypertension, anemia, and vascular complications — were also more likely to be readmitted.&nbsp;<br><br>The investigators were surprised to learn that readmitted patients were younger than those who were not readmitted — the opposite of what usually happens during pregnancy, where older mothers tend to have more complications. "This finding suggests that younger women with lupus may have more aggressive disease or severe disability, even though they're at an age when pregnancy is usually safer," said Sandhya Kannayiram, MD, an HSS rheumatology fellow and the study's lead author.&nbsp;<br><br>"Social determinants are vital, independent indicators of health outcomes in pregnant women with lupus. Addressing these factors, along with medical risk factors, is crucial for improving outcomes in this vulnerable group," she continued. The study also demonstrates that AI-powered models can aid clinicians in identifying patients who need extra support after hospital discharge so they can implement proactive, equitable interventions — such as connecting patients with resources for housing, childcare, and transportation, scheduling follow-up appointments before hospital discharge, offering telehealth services, and engaging community health workers.&nbsp;<br><br><strong>Study: Addressing Perceived Discrimination in Lupus Care: Enhancing Patient Empowerment and Health Equity Through Education and Support&nbsp;</strong><br><br>Systemic lupus erythematosus disproportionately affects Black, Latino, and Asian populations. Perceived racial discrimination in health care is a major concern among people with lupus and results in lower trust in physicians, less treatment adherence, and worsened health outcomes. HSS investigators from Charla de Lupus (Lupus Chat)®, a peer support program established over 20 years ago, conducted a survey to identify patients' self-reported experiences with perceived discrimination and healthcare navigation.&nbsp;<br><br>The idea for the survey stemmed from the Hospital's Community Health Needs Assessment, which highlighted discrimination and biases in health care identified by people living with lupus. People with lupus were surveyed before (37 patients) and after (23 patients) six monthly virtual support group sessions that provided education about recognizing and intervening when discrimination occurs in healthcare settings.&nbsp;<br><br>After the educational sessions, there was a measurable decline in perceived discrimination. Fewer patients felt they were treated with less courtesy and less respect or received poorer service, and fewer patients said that doctors or nurses acted superior.&nbsp;<br><br>A small number reported increases in feeling unheard or judged as unintelligent. "These increases reflect a new awareness and ability to recognize instances of bias, giving participants the language and confidence to name experiences they might not have previously identified," explained Kimberly Cabrera, Senior Program Associate for Charla de Lupus and the study's lead author. "Patients who took part in the virtual meetings reported feeling more empowered to advocate for themselves and experienced greater openness in communication with their providers, increasing their sense of trust in the healthcare system."&nbsp;<br><br>The study researchers also shared consented audio recordings from participants' focus groups with rheumatologists at HSS and nearby hospitals. Many were grateful to hear the patients' feedback and expressed an intent to integrate interventions to mitigate perceived discrimination and implicit bias concepts into their practices. "The result was not only a decrease in perceived discrimination among patients, but a greater mutual understanding between patients and providers — a reminder that progress is possible when what is valuable to patients is at the center, and both are willing to listen and work together," concluded Ms. Cabrera.&nbsp;<br><br><strong>Presentation: Implementing Social Determinants of Health Screening in Rheumatology&nbsp;</strong><br><br>Dr. Rose-Smith and her colleague Melissa Flores, MPH, MSW, Assistant Director, Office of Health Equity, will deliver a presentation outlining strategies for healthcare providers to implement SDOH interventions in their rheumatology practices. This session aims to equip rheumatology providers with the knowledge and resources needed to enhance care delivery and patient outcomes for underserved populations and ultimately promote a more inclusive, equitable care environment.&nbsp;<br><br>The presentation is designed to improve SDOH screening not only by rheumatologists, but also allied healthcare professionals such as nurses, physical and occupational therapists, social workers, and public health administrators — anyone in the care continuum with whom patients might share SDOH information. "You can give them the best advice and have the best care plan, but it falls apart in the end if you don't link patients with the appropriate resources or address what's going on in the places where they live, work, and play 95 percent of the time," said Dr. Rose-Smith.&nbsp;<br><br>The U.S. Centers for Medicare & Medicaid Services (CMS) and Joint Commission have issued regulatory requirements and other guidelines mandating healthcare providers to collect SDOH data. Building on an SDOH session presented at the ACR meeting in 2023, participants in the 2025 session will learn how to incorporate SDOH assessments into clinical workflows, identify key barriers to care related to social factors, and leverage data to identify disparities and develop targeted interventions.&nbsp;<br><br>The session will explore an example of a successful SDOH implementation and intervention process at HSS. The Hospital automated its SDOH process and launched an onsite food pantry last March as a result of SDOH screening, in collaboration with Food Bank NYC. Rheumatology patients who are identified as being food insecure during SDOH screening will be directed to the food pantry to receive a bag of groceries, which is especially important for those who need to take their medicine with food. "By asking open-ended questions about SDOH, we invite patients to share this part of themselves with us so we can better provide high-quality care and meet their needs," said Ms. Flores.&nbsp;<br><br>SDOH is also a value driver because it improves patient retention, patient satisfaction, and clinical outcomes. Louis Bridges, Jr., MD, PhD</p>]]></description><content:encoded><![CDATA[<p style="margin-left:0px;text-align:start;">The New York Times reports that The Food and Drug Administration recently approved a medical device that offers new hope to patients incapacitated by <a href="https://www.hss.edu/health-library/conditions-and-treatments/list/rheumatoid-arthritis" target="_blank">rheumatoid arthritis</a>.</p><p style="margin-left:0px;text-align:start;">The condition is usually managed with medications. The device represents a radical departure from standard care, tapping the power of the brain and nervous system to tamp down the uncontrolled inflammation that leads to the debilitating autoimmune disease.</p><p style="margin-left:0px;text-align:start;">The inch-long device is surgically implanted into the neck, where it sits in a pod wrapped around the vagus nerve, which some scientists believe is the longest nerve in the body. The device electrically stimulates the nerve for one minute each day.</p><p style="margin-left:0px;text-align:start;">The stimulation can turn off crippling inflammation and “reset” the immune system, research has shown. Most drugs used to treat rheumatoid arthritis suppress the immune system, leaving patients vulnerable to serious infections.</p><p><span style="text-align:start;">According to </span><a href="https://www.hss.edu/profiles/doctors/louis-bridges" target="_blank">S. Patients with pre-existing psoriasis who experienced worsened disease after starting ICI treatment all saw partial or complete improvement with apremilast.</p><p>However, the medication was less effective for <a href="https://www.hss.edu/health-library/conditions-and-treatments/list/psoriatic-arthritis" target="_blank">psoriatic arthritis</a> (PsA) flares, with some patients showing no response, and six of 21 participants discontinued treatment due to side effects. But that could not be farther from the truth. “While it is true that patients should not perform exercises that may worsen their pain, there are many safe exercises for such patients, including water aerobics and low impact activities such as walking,” he said.</p><p style="margin-left:auto;text-align:start;">Additionally, appropriate lifestyle changes can make a huge difference for patients with rheumatic diseases. These include avoiding smoking, maintaining a healthy weight and eating a healthy diet.</p><p style="margin-left:auto;text-align:start;">“Evidence suggests that the Mediterranean diet, which has been shown to reduce cardiovascular disease, may have beneficial effects on rheumatic diseases,” Dr. Bridges said. The institution is collaborating with medical centers and other organizations to advance the quality and value of musculoskeletal care and to make world-class HSS care more widely accessible nationally and internationally. </span><a href="http://www.hss.edu"><span>www.hss.edu</span></a><span>.</span></p>]]></pp:boilerplate><description><![CDATA[<p>Hospital for Special Surgery (HSS) presented a research study at the European Alliance of Associations for Rheumatology (EULAR) Annual Meeting showing that an artificial intelligence (AI)-based model can predict readmissions of pregnant women with lupus by looking at patients’ social factors and clinical comorbidities.&nbsp;<br><br>Systemic lupus erythematosus (SLE), commonly known as lupus, is a chronic autoimmune disease in which the immune system attacks the individual’s own healthy tissue, causing pain, inflammation, and eventually damage to various organs. It commonly affects young women, and pregnancy is a particularly vulnerable period for this high-risk population. Although lupus-related medical complications affect pregnancy outcomes, social determinants and economic conditions, known as social determinants of health (SDOH), may also be important in shaping maternal health outcomes.&nbsp;<br><br>“Pregnant women with lupus have five times higher maternal mortality compared to those without lupus,” said Sandhya Shri Kannayiram, MBBS, MD, rheumatology fellow at HSS and principal author of the study. “There is little data on how social factors, including Income, Insurance, housing stability, access to transportation, availability of utilities, and literacy, along with clinical comorbidities, affect pregnancy outcomes in individuals with lupus.”&nbsp;<br><br>To better understand how SDOH impact readmissions for pregnant patients with SLE, the team used ten-year data (from 2011 to 2021) from the National Readmissions Database of the US, encompassing approximately 66,000 hospitalizations in the United States, and counted the number of readmissions within 30 days of discharge during pregnancy and delivery. “We found that approximately 2,500 [pregnant women with lupus] were readmitted to the hospital within 30 days,” said Dr. Kannayiram.&nbsp;<br><br>“We used a supervised ‘glass box’ machine learning model called the Explainable Boosting Machine (EBM), which helped predict who is likely to return to the hospital within 30 days,” said Dr. Kannayiram. She explained that EBM is a form of machine learning with a type of AI that learns from data to make predictions. “It’s an AI model that can be trained on the dataset to predict outcomes with transparency and accuracy compared to traditional statistical models,” she added.&nbsp;<br><br>The team specifically examined the major social determinants from the database that may influence readmissions during pregnancy in SLE, including Income and insurance status (public, private, or self-pay). Dr. Kannayiram explained that the results obtained showed patients living in lower-income neighborhoods were nearly twice as likely to be readmitted compared to those from wealthier areas, and those with Medicaid or Medicare insurance were more likely to be readmitted within 30 days. Patients discharged to locations other than home, such as rehabilitation facilities or nursing homes, were also more likely to return. However, they also found unexpected correlations. “We found that the size of hospitals was highly related to readmissions, rather than the location, such as cities or rural areas,” said Dr. Kannayiram, with large hospitals being the ones with more readmissions. Additionally, unlike most pregnancy studies, where older mothers are usually at higher risk, this study found that younger women with lupus were more likely to be readmitted. Dr. Kannayiram said that this result may be related to severe lupus in younger individuals since about half of the readmitted patients had Medicare coverage, which may suggest a high prevalence of disability or kidney disease within that younger population.&nbsp;<br><br>By using the predictive AI model, the team also discovered that income and insurance status were among the top five predictors of 30-day readmission during pregnancy and delivery-related hospitalizations, explained <a href="https://www.hss.edu/profiles/doctors/bella-mehta" target="_blank">Bella Mehta, MBBS, MS, MD</a>, rheumatologist at HSS and lead author of the study. She noted that these results could inform targeted interventions, such as policy actions to advocate for improved Medicaid and Medicare coverage, as well as post-discharge care coordination. “Integrating SDOH screening into prenatal care for SLE patients, and incorporating a multidisciplinary team of social workers, could reduce preventable readmissions and improve maternal outcomes,” added Dr. Mehta.&nbsp;<br><br>According to Dr. Mehta, in future studies, the team plans to analyze in detail how individual-level social factors, such as housing instability, food insecurity, education, mental health, transportation access, and patient demographics, interact with clinical comorbidities to determine pregnancy outcomes for women with SLE. “This study underscores the vital intersection between clinical care and structural inequality,” said Dr. Mehta. “The lupus community and broader maternal health initiatives must not only consider disease management but also address the social factors that influence outcomes.”&nbsp;<br><br>Poster details&nbsp;<br><br>Title: Predictors of 30-Day Readmissions in Pregnant Patients with Systemic Lupus Erythematosus: The Role of Social Determinants of Health&nbsp;<br>Authors: Sandhya Shri Kannayiram, Yiyuan Wu, Lisa Sammaritano, Michael Lockshin, Rich Caruna, D. Although usually accompanied by joint pain, swelling and deterioration, the level of inflammation varies from patient to patient, potentially requiring customized treatment plans. “What is surprising to us is that there are some patients who have clinical remission, yet continue to experience pain and show radiographic progression of the disease with high levels of synovial inflammation, while others have high disease activity with many swollen and tender joints and little inflammation in the synovial tissue” said </span><a href="https://www.hss.edu/profiles/doctors/susan-goodman"><span>Susan M. Goodman, MD</span></a><span>, rheumatologist at HSS and principal investigator of the study. She explained that even when standard tests suggest the disease is under control, the joint fluid can still show high levels of inflammation. “By analyzing synovial fluid characteristics, we expected it might give us some insight into the disparity between disease progression and apparent remission,” she added.</span></p><p><span>The team analyzed samples from 64 patients with RA, 90.6% of whom were women, and with a median age of 65.3 ±11.7 years. The physicians collected synovial fluid samples to analyze and compare with the synovial tissue biopsy counterpart, which defines the inflammatory profile of the disease.</span></p><p><span>Dr. Goodman and the team measured the total white blood cell count, as well as the percentage of neutrophils, lymphocytes and monocytes in each synovial fluid sample. They then compared those findings to two additional key pieces of clinical information: the patient’s disease activity (such as joint swelling, pain, etc.) and the level of inflammation seen in the tissue.</span></p><p><span>Their analysis revealed that the total white blood cell count in the synovial fluid did not correlate with disease activity, however, the counting of individual cell types gave insights into tissue inflammation. As such, a higher percentage of neutrophils was associated with increased inflammation in the tissue, while a higher percentage of monocytes was associated with lower inflammation in the tissue.</span></p><p><span>Because the white blood cell counts in synovial fluid provided distinct and different information from standard disease activity measures and correlated well with tissue inflammation, the team believes it could help guide the selection of personalized anti-inflammatory therapies more efficiently.</span></p><p><span>“Synovial fluid extraction is already a routine part of care, but until now, it has never been used to analyze disease state in this way,” explained Dr. Goodman. In other words, the approach could be adopted without placing additional burden on healthcare providers.</span></p><p><span>"It's important that we're gaining new methods of investigating patients with RA,” Dr. Goodman added, “since choosing the appropriate therapy and choosing it in an efficient way is clearly the best way to achieve the best outcomes.”</span></p><p><span>According to Dr. Goodman, the next phase of this study, led by HSS rheumatologists </span><a href="https://www.hss.edu/profiles/doctors/melanie-smith"><span>Melanie H. Salmon, MD</a>, a rheumatologist and the Collette Kean Research Chair at HSS and D. Ware Branch, MD, an obstetrician/gynecologist at University of Utah Health. It is the first clinical trial to evaluate a biologic therapy to prevent serious adverse outcomes in pregnant women with APS and their babies. The study results were published online in <a href="https://www.sciencedirect.com/science/article/abs/pii/S0003496725002432" target="_blank">Annals of the Rheumatic Disease</a>s.&nbsp;<br><br>“The IMPACT study represents a bold and very successful partnership over many years between government, industry, foundations, and academic health research institutions,” said Dr. Salmon, the senior study author. “It is exciting to see our preclinical work in the laboratory, which began more than a decade ago, translate into such promising results for patients. I hope our findings will allow the drug to become widely available so that more pregnant women with APS and high-risk pregnancies can benefit.”&nbsp;<br><br>APS is a rare autoimmune disorder affecting up to 0.05% of people. It is frequently associated with lupus, a disease most prevalent in women during their reproductive years. In APS, the body produces autoantibodies that react with blood vessels and circulating cells, causing dangerous blood clots throughout the body, which can lead to strokes, heart attacks, and phlebitis. During pregnancy, APS raises the risk of serious complications arising from issues with the placenta, including fetal death, preeclampsia, and restricted fetal growth.&nbsp;<br><br>Previous research led by Dr. Salmon identified lupus anticoagulant (LA), an autoantibody produced by some APS patients, as the strongest predictor of poor pregnancy outcomes in APS patients. Historically, 39% to 86% of pregnant women with APS who are LA positive have faced severe complications, despite treatment with standard-of-care blood thinners like heparin and aspirin. These patients have the greatest need for better treatments.&nbsp;<br><br>Dr. Salmon’s preclinical research in experimental models showed that inflammation in the developing placenta, not blood clots, was the main cause of pregnancy complications in APS. This pivotal discovery led her team to investigate whether TNF-alpha inhibitors, a class of drugs used to treat inflammatory conditions like rheumatoid arthritis, Crohn’s disease and plaque psoriasis, could offer better protection to APS pregnancies than blood thinners alone.&nbsp;<br><br>The IMPACT trial enrolled 51 pregnant women ages 18 to 40 with high-risk APS, defined as positive for lupus anticoagulant. All participants were treated with certolizumab, a TNF-alpha inhibitor that does not cross the placenta, in addition to standard-of-care heparin and aspirin, starting at week eight of pregnancy. Medication was stopped at week 28, the point at which the placenta is well-developed. The unique design of the IMPACT study made it possible for pregnant patients from 16 states and one Canadian province to participate.&nbsp;<br><br>The 20% complication rate in patients treated with certolizumab was dramatically lower than their prior pregnancies in which 69% to 79% had severe adverse outcomes, despite standard treatment with heparin and aspirin. The average gestational of age at delivery was 36.5 weeks in certolizumab-treated pregnancies, compared to 24 weeks in patients’ prior pregnancies. Among IMPACT participants, 93% brought home a healthy baby—a remarkable improvement over the 38% survival rate for their previous pregnancies. Notably, none experienced serious infections or lupus flares, a concern when the study began.&nbsp;<br><br>The trial supports the concept derived from basic laboratory studies that targeting inflammation, rather than blood clotting, is effective in preventing pregnancy complications in high-risk pregnant patients with APS. It also shows the power of collaboration between rheumatologists and obstetricians.&nbsp;<br><br>“Our study heralds a new era for trials with biologics to prevent adverse pregnancy outcomes. We have shown that it is possible to gain the confidence of regulators and the trust of pregnant women who will enroll in clinical trials,” said Dr. Salmon. “We are grateful to the patients and their care providers, who were committed partners in this trial, and to our funders who watched our trial with anticipation and excitement,” said Dr. Salmon.&nbsp;<br><br>“These results also open a window for studying whether TNF-alpha blockade could help prevent preeclampsia in women without autoimmune disorders,” she added. “Preeclampsia is the most common cause of morbidity and mortality of pregnant women and their babies, and there are currently no effective therapies.”&nbsp;<br><br>The study was supported by funding from the National Institutes of Health, the Lupus Foundation of America, the Morris and Alma Schapiro Fund, the James R. and Jo Scott Research Endowment at the University of Utah and UCB Inc., the maker of certolizumab (Cimzia).&nbsp;<br>&nbsp;</p>]]></description><category><![CDATA[pressrelease,Salmon,Rheumatology,APS,antiphospholipid-syndrome]]></category>

But that’s by design: You need a little fire to defend yourself. Where you can run into trouble is if those flames keep smoldering even after you’ve healed.</span></p><p><span style="text-align:start;">The key difference here is between acute and chronic inflammation. With the former, your immune cells detect a threat and spurt out substances that widen your blood vessels, allowing more blood (and immune cells) to get to the affected area—an essential process for fighting germs and recovering from physical trauma. After they’ve dealt with the danger, those cells typically retreat, and your symptoms die down. But with the long-lasting version of inflammation, that response keeps percolating. At low levels, it isn’t always something you’ll notice (though it may play a role in fatigue, GI issues, and joint pain); it might only be detectable via a blood test or skin biopsy, Dr. Bridges explained.                         <title>HSS Participates in Promising Phase 3 Lupus Trial of Investigational Therapy DZP</title>

Fernandez, MD, PhD</span></a><span style="text-align:start;">,<strong> </strong></span>participated in the multicenter Phase 3 clinical trial PHOENYCS GO, which evaluated dapirolizumab pegol (DZP) as a treatment for people with moderate-to-severe systemic lupus erythematosus (SLE). In fall 2024, UCB and Biogen announced positive results, presented at the <a href="https://acrabstracts.org/abstract/dapirolizumab-pegol-demonstrated-significant-improvement-in-systemic-lupus-erythematosus-disease-activity-efficacy-and-safety-results-of-a-phase-3-trial/" target="_blank">American College of Rheumatology</a> conference in Washington, D.C.</p><p>The study found that adding DZP to standard care led to greater improvements in disease activity, fewer severe flare-ups, and reduced corticosteroid use compared to placebo.                         <title>Study Shows Knee Osteoarthritis Causes More Pain and Inflammation in Women</title>

The work indicates that different genes and biological processes may cause knee OA to be worse in women compared to men and suggests that women may benefit from personalized treatments that target those specific mechanisms.</span></p><p><span>“These findings give some reassurance to female patients that there are physiological reasons why they are having more pain,” says </span><a href="https://www.hss.edu/physicians_mehta-bella.asp"><span>Bella Mehta, MBBS, MS</span></a><span>, rheumatologist at HSS and lead author of the study. “Our work may also help us develop better therapies, especially for women with more severe symptoms.”</span></p><p><span>The study enrolled 135 patients with knee OA who were scheduled to have total knee replacement. A total of 82 participants were female. Researchers analyzed patient-reported symptoms of pain, histology (tissue samples removed during surgery and examined under the microscope) and genetic markers of inflammation in the blood.</span></p><p><span>The study found that women with knee OA had higher markers of inflammation in their blood, increased tissue inflammation and widespread cartilage loss. This corresponds with the study’s findings that women reported higher levels of pain, even at night when at rest.</span></p><p><span>The findings also revealed specific genetic expression patterns in women that notably increased synovial tissue inflammation. This occurs when the synovium, the thin layer of tissue above the cartilage that helps lubricate the knee joint, becomes inflamed and painful.</span></p><p><span>“Traditionally, it was thought that knee osteoarthritis was caused by years of significant wear and tear on the cartilage in the knee joint, but synovial inflammation is now known to play a big role in this condition,” notes Dr. Mehta.</span></p><p><span>Knee osteoarthritis is the most common type of arthritis in the world. Most people over the age of 65 have some form of it, with around 70% to 80% experiencing symptoms of pain and stiffness.</span></p><p><span>Pain is typically reported more frequently in women with knee OA than in men. Dr. Mehta suggests several theories as to why, including:</span></p><p><span>-Mechanical differences between men and women. A woman’s pelvis or hip bone is shaped differently than men so they can accommodate having babies. This has implications on the knee joint, which may increase osteoarthritis and pain.</span></p><p><span>-Hormonal differences in estrogen and progesterone after menopause, which may make women more prone to osteoarthritis.</span></p><p><span>-Differences in genetics. Women are born with two X chromosomes, whereas men have an X and a Y chromosome. The second X chromosome expresses certain genes that increase the likelihood for women to have osteoarthritis.</span></p><p><span>“It’s not that women complain of more pain. There are objective, physiological differences that we have seen between men and women that can explain this,” says Dr. Mehta. “For example, they have increased markers of inflammation in their blood, inflammatory gene expression and more mechanical stress and cartilage loss in the knee, all contributing to more pain even at rest.”</span></p><p><span>Currently, first-line treatments for people with knee OA include medications to manage pain, and exercise to strengthen the muscles and ligaments around the knees. However, if there is no improvement, surgery is recommended.</span></p><p><span>Researchers continue to look for ways to improve treatment options for knee OA. “There may be certain immunomodulatory agents that target the genetic mechanisms driving tissue inflammation in women, but more research is needed to better understand these genes and their role in accelerating knee osteoarthritis in females,” says Dr. Mehta. &nbsp;</span></p>]]></description><category><![CDATA[pressrelease,Rheumatology,Research Clinical,Mehta,knee-replacement,osteoarthritis]]></category>
