Yoon, MD</p>]]></description><content:encoded><![CDATA[<p>In a recent episode of the Newswise Curious by Nature podcast, <a href="https://www.hss.edu/profiles/doctors/edward-yoon" target="_blank">Edward Yoon, MD</a>, chief of interventional radiology at HSS, explains a breakthrough treatment designed for a very specific and often overlooked type of back pain.</p><p style="text-align:justify;" dir="ltr">Dr. Yoon discusses <a href="https://www.hss.edu/health-library/conditions-and-treatments/intracept-procedure-basivertebral-nerve-ablation" target="_blank">basivertebral nerve ablation</a>, a minimally invasive procedure that targets vertebrogenic back pain, a condition linked to inflammation in the endplates of the spine. This inflammation shows up on MRI as Modic changes, making it easier to identify who can benefit from the procedure. Yoon, MD</p>]]></description><content:encoded><![CDATA[<p><span style="text-align:left;">The Monmouth Journal reports that after years of disabling back pain, a young Hazlet mom finally got her life back thanks to an innovative nonsurgical procedure called </span><a href="https://www.hss.edu/health-library/conditions-and-treatments/intracept-procedure-basivertebral-nerve-ablation" target="_blank"><span style="text-align:left;">basivertebral nerve ablation</span></a><span style="text-align:left;"> at HSS. Also known as BVN ablation or the Intracept procedure, the treatment applies heat to disrupt a specific spinal nerve that is transmitting pain signals to the brain.</span></p><p><span style="text-align:left;">Rosie Amoroso, 32, had suffered from low back pain for years, but it got progressively worse after giving birth three years ago. She said the pain became so severe, she often found it impossible to sleep.</span></p><p><span style="text-align:left;">“My back would be so bad, I couldn't pick up my son," Amoroso said. "I couldn't do basic things around the house, and I was putting a lot of pressure on my husband. He was very supportive but had a long commute to New York City for work every day.”</span></p><p><span style="text-align:left;">Desperate for relief, Amoroso consulted with nine different doctors, tried physical therapy and medication, received numerous epidural spinal injections, and underwent repeated radiofrequency ablation procedures. Nothing helped.</span><br><br><span style="text-align:left;">The unrelenting pain wasn’t her only challenge. While seeking care, Amoroso said she dealt with doctors who made her feel “very dismissed,” suffered through uncomfortable procedures that provided no relief, and spent thousands of dollars in insurance co-payments.</span></p><p><span style="text-align:left;">Still, she refused to give up hope.</span><br><br><span style="text-align:left;">“I was ready to do anything to eliminate the pain, even if it meant spine surgery,” she recalled. At that point, she made an appointment at HSS in Manhattan. After reviewing her medical history and examining her, he referred her to Edward S. Yoon, MD, chief of the division of interventional radiology at HSS.&nbsp;<br><br>Dr. Yoon determined that Rosie was a candidate for BVN ablation and performed the procedure in May 2025. The treatment has changed her life. She is now pain-free and deeply grateful to Dr. Yoon, not only for relieving her suffering but also for demonstrating exceptional compassion and understanding. “I felt like he went above and beyond, like he really cared about me,” she recalls.&nbsp;<br><br><strong>Basivertebral Nerve Ablation&nbsp;</strong><br><br>A minimally invasive nonsurgical procedure, BVN ablation uses radiofrequency energy to ablate—or shut down—the basivertebral nerve in the spine to prevent it from sending pain signals to the brain.&nbsp;<br><br>“BVN ablation is a targeted treatment for a specific type of chronic lower back pain called vertebrogenic pain, which results from damage to vertebral endplates. A vertebral endplate is the interface between the spinal disc and the bony portion of a spine vertebra,” Dr. Yoon explains. “Microfractures or other damage to a vertebral endplate leads to inflammation, which causes the adjacent basivertebral nerves to transmit pain signals to the brain.”&nbsp;<br><br>Patients who find relief with BVN ablation often describe pain in the middle of their lower back that is made worse by physical activity, prolonged sitting, bending forward, or when bending down and lifting objects, Dr. Yoon explains.&nbsp;<br><br>To accurately diagnose vertebrogenic pain and confirm that a patient is a candidate for BVN ablation, doctors look for specific changes on a patient’s MRI, known as Modic changes, that occur with vertebral endplate inflammation.&nbsp;<br><br>Dr. Yoon notes that basivertebral nerve ablation is not useful for treating low back pain caused by conditions that have not affected the endplates, such as a herniated disc.&nbsp;<br><br>Delighted with her remarkable recovery, Rosie expresses immense gratitude for regaining her quality of life since having the procedure. “Before the treatment, I barely slept two hours a night. Now I can sleep through the night,” she says. “I can pick up my son, take him to the jungle gym, go down the slide with him. It’s a big deal.” To add to her happiness, Rosie recently learned that she and her husband are expecting their second child.&nbsp;<br><br>BVN ablation is an outpatient treatment often covered by insurance plans. Koch, PhD, as founding Director of the Applied Sciences Institute and Vice Chair of Research in Radiology. The appointments are effective April 1.&nbsp;<br><br>As Director of the forthcoming Applied Sciences Institute, Dr. Koch will oversee the development and adoption of novel technologies that can advance musculoskeletal care for the benefit of patients worldwide. Among his responsibilities, he will lead the evaluation and deployment of innovations emerging from HSS research, guide multidisciplinary teams driving advancements in medical devices and digital health, and provide strategic guidance on incorporating artificial intelligence (AI) and advanced informatics into technology development. In his role as Vice Chair of Research in Radiology, he will direct imaging research initiatives and champion the translation of imaging research findings into clinical practice at HSS.&nbsp;<br><br>Across both roles, Dr. Koch will cultivate strategic partnerships with industry leaders and academic institutions. He will also work to strengthen interdisciplinary collaboration within HSS, helping to align HSS’s world-class expertise across radiology, engineering, clinical care, and digital health with the goal of advancing HSS’s research and innovation evolution.&nbsp;<br><br>"Dr. Koch’s appointment is one of many actions we’re taking to reinvigorate our world-class research with greater strategy and intention—creating the structure and leadership needed to turn groundbreaking ideas into real-world impact," said <strong>Suzanne A. "Additionally, the new Applied Sciences Institute will represent a new era in musculoskeletal science at HSS, where discovery, technology development and clinical application are more closely aligned to advance patient care."&nbsp;<br><br>A visionary physicist specializing in magnetic resonance imaging, Dr. Koch joins HSS from the Medical College of Wisconsin, where he held roles of increasing academic responsibility and leadership since 2014. Most recently, he was Vice Chair of Research in the Department of Radiology and Executive Director of the Center for Imaging Research. Previously, he worked as a scientist at GE Healthcare, where he focused on the development of new MRI technologies.&nbsp;<br><br>It was during his time at GE that Dr. Koch began an almost 20-year-long collaboration with <a href="https://www.hss.edu/physicians_potter-hollis.asp" target="_blank">Hollis G. Potter, MD</a>, Chairman of the Department of Radiology and Imaging and Coleman Chair in MRI Research at HSS, that led to the creation and validation of a new MRI technique called 3D multi-spectral imaging. This groundbreaking advance improves the view of metal implants and surrounding tissue, helping orthopedic specialists diagnose whether issues arising after joint replacements may be caused by infections, loosening, fracture or soft tissue abnormalities. GE Healthcare launched the innovation, called MAVRIC SL, worldwide in 2013.&nbsp;<br><br>“I’m excited and honored to join HSS, an organization that consistently punches above its weight in developing groundbreaking imaging technologies and driving innovations in orthopedic care,” said Dr. Koch. “I look forward to continuing my long-standing collaboration with Dr. Potter and expanding partnerships across HSS. He has published 80 peer-reviewed research papers over his career and holds patents on more than 20 inventions. Dr. Koch’s research into developing new imaging technologies has been funded by several grants from the National Institutes of Health, the Department of Defence, industry partners and foundations. In response to high demand from patients on Long Island,&nbsp;close to 18,000 square feet were added, nearly doubling the facility size to 37,000 square feet. Westrich, MD</p>]]></description><content:encoded><![CDATA[<p><i>Radiology Today</i> featured an update on HSS research on MRI & adverse tissue reactions in asymptomatic hip patients and included insight from authors <a href="https://www.hss.edu/physicians_potter-hollis.asp">Hollis G. Potter, MD</a>, Chairman, Department of Radiology and Imaging, and <a href="https://www.hss.edu/research-staff_koff-matt.asp">Matthew F. Koff, PhD</a>, assistant scientist at HSS.</p><p>The article also featured commentary on the clinical implications from <a href="https://www.hss.edu/physicians_bostrom-mathias.asp">Mathias P. Bostrom, MD</a>, chief of the Adult Reconstruction and Joint Replacement Service at HSS, and <a href="https://www.hss.edu/physicians_westrich-geoffrey.asp">Geoffrey H. Westrich, MD</a>, hip and knee surgeon at HSS.</p><p>According to Dr. Potter, “At the end of the day, we can’t cure osteoarthritis, but we can treat it. As the population gets older, more people will require hip and knee implants.”</p><p>She continued that joint replacements also are going into younger people, many of whom lead active lifestyles, “which means the demands on the implants are greater. Noting the newer materials used in hip replacements are considered safer. However, some patients still have adverse reactions.</p><p>According to Dr. Bostrom, “Where you have dissimilar materials touching each other, you could have problems.”</p><p>Dr. Koff explained that patients’ genetic makeup typically determines whether they will have an adverse tissue reaction, “but, unfortunately, there’s no way to predict whether someone is genetically predisposed.”</p><p>Dr. Potter and colleagues have been working on software that makes MRI the go-to tool to detect adverse tissue reactions in total hip arthroplasty and hip resurfacing, which is an alternative to completely replacing the hip.</p><p>Drs. Potter, Koff, and colleagues published a December 2021 study in <i>Clinical Orthopaedics and Related Research</i> finding that MRI identifies adverse tissue reactions in asymptomatic individuals after hip resurfacing arthroplasty. Previous studies have been limited to metal-on-metal implants.</p><p>The HSS researchers determined MRI identified adverse local tissue reactions in 25% of patients who had hip resurfacing arthroplasty. Dr. Potter said the finding was surprising because the patients who had reactions had symptoms similar to, or even less severe than, patients with ceramic-on-polyethylene and metal-on-polyethylene total hip replacements.</p><p>“We found that patients can be completely asymptomatic and have high-functioning hip scores while harboring reactions that could start to destroy the soft tissues around the hip,” Dr. Potter noted. “If you have an adverse tissue reaction, and it goes unchecked, it could destroy the tissues in half your pelvis.”</p><p>Dr. Koff added that the benefit of finding adverse reactions early cannot be underestimated. “If a patient has an adverse tissue reaction and you don’t find it early, if you have to do a revision, the revision surgery is going to take much longer,” he says. “It’s a lot more challenging, the recovery is longer, and there is a greater cost associated with every aspect of it, including the rehabilitation.”</p><p>Dr. Bostrom noted the HSS study is cutting edge and groundbreaking. “It will revolutionize the way we assess total joint replacement. I don’t think you can understate that.”</p><p>Dr. Westrich agreed that it would be worth following high-risk hip replacement patients with MRI but to exercise caution. “It would be extremely costly for the hospital, the patient, and the insurance carrier, and, in this era of cost containment, we have to pick carefully what tests we’re going to have. If the hip has no cobalt or chrome, that’s not the patient I would order an MRI for,” he said.</p><p>Dr. Westrich underscored that not all radiologists are able to use MRI to evaluate hip implants as successfully as Dr. Potter and those at HSS. “With MRIs, it’s very dependent on who’s doing it and whether they are dedicated to figuring out the best methods of doing metal suppression MRIs,” he added.</p><p>He continued, “MRI quality and technique vary tremendously from location to location. The results suggest MRI is uniquely capable of assessing soft-tissue complications and that the use of MRI should be included as part of routine follow-up protocol for</span></span><span> </span><span style="color:#272725;"><span>hip replacement patients, as</span></span><span> an annual clinical assessment dependent on survey or blood metal ion testing alone may not detect complications</span><span style="color:#272725;"><span>.</span></span></p><p><span>The study, published yesterday in </span><i><span>Clinical Orthopaedics and Related Research<strong>,</strong></span></i><span> is the first to evaluate the presence and risk of local tissue complications after hip replacements made of different materials in patients, including those who are asymptomatic. Previous studies have been limited to metal-on-metal implants.</span></p><p><a href="https://www.hss.edu/conditions_revision-total-hip-replacement-overview.asp"><span style="color:#0070c0;"><span>Hip replacement</span></span></a><span> </span><span style="color:#272725;"><span>parts are made of plastic, ceramic, metal or a combination of these materials. The components move against each other and wear down slowly through regular use. Small wear particles can break free from the hip replacement material over time and damage local joint tissues. MRI is a non-invasive imaging technique that uses a large magnet and radiofrequency energy to produce digital images without the use of ionizing radiation. MRI is often used in orthopedics to visualize the soft tissues around total hip replacements that are affected by the small wear particles.</span></span></p><p><span style="color:#222222;"><span>“We found that patients can be completely asymptomatic and have high-functioning hip scores while harboring reactions that could start to destroy the soft tissues around the hip,” said</span></span><span> </span><a href="https://www.hss.edu/physicians_potter-hollis.asp"><span style="color:#0070c0;"><span>Hollis G. Delayed detection results in unnecessary pain, longer and more complicated revision operations and more challenging recoveries.”</span></span></p><p><span style="color:#272725;"><span><strong>Matthew F. Koff PhD</strong>, the lead study author said, “The study also indicates that MRI is clinically useful for patients who receive a ceramic-on-polyethylene or metal-on-polyethylene implant, in addition to patients who received a hip resurfacing arthroplasty.”</span></span></p><p><span style="color:#272725;"><span>Drs. Potter and Koff and</span></span><span> </span><span style="color:#272725;"><span>HSS colleagues prospectively evaluated 206 patients (243 hip replacements) at least one year after having hip resurfacing arthroplasty or total hip replacement. The researchers examined the volume of joint tissue reaction using MRI images, levels of metal particles in the blood and</span></span><span> </span><span style="color:#272725;"><span>patients' responses on a survey called the Hip Disability and Osteoarthritis Outcome Score annually over the course of 3 years. The researchers compared results for patients who received hip resurfacing arthroplasty (HRA) with patients who received ceramic-on-polyethylene and metal-on-polyethylene total hip replacements.</span></span></p><p><span style="color:#272725;"><span>During the study, MRI identified adverse local tissue reactions in 25 percent of patients who had hip resurfacing arthroplasty. This finding was surprising, since these patients reported symptoms similar to or less severe than patients with ceramic-on-polyethylene and metal-on-polyethylene total hip replacements implants. Patients who received hip resurfacing arthroplasty had a significantly larger volume of joint tissue reaction on MRI, a nearly five-times-higher risk of having tissue complications than patients who had a ceramic-on-polyethylene total hip replacement. ]]></description><content:encoded><![CDATA[<p>Wall New Jersey Patch reports Ken Huack, a 94 year-old World War II veteran spent the first few weeks of the COVID-19 pandemic largely unaffected making sure he kept up his exercise regimen while the world was on lockdown. One day he overexerted himself on his exercise bicycle and began to experience pain. Due to compression in his spine causing excruciating pain, simple activities such as walking or even laughing without pain were out of the question.</p><p>Huack&rsquo;s pain worsened over the next several months and he came to see <a href="https://www.hss.edu/physicians_yoon-edward.asp">Edward S. Yoon, MD</a>, chief of interventional radiology at HSS. Dr. Yoon performed an innovative procedure called a kyphoplasty in which acrylic cement was injected into Huack&rsquo;s spinal fracture.</p><p>"It's one of the most satisfying procedures I perform. Patients come in not being able to stand or walk and immediately after the procedure, they don't have any pain," said Dr. Yoon. "My colleagues and I are huge proponents of this procedure. Sometimes patients are told by doctors that there's nothing you can do [for an ailment] and just take pain medication, which is not a good solution for an underlying problem. I'm glad he is now able to be up and about."</p><p>Huack is now enjoying a variety of activities that he participated in such as walking on the boardwalk, eating blueberry pancakes for breakfast, doing chair exercises with his aide and watching the golf game on TV. Potter, MD</a>, chairman, Department of Radiology and Imaging at HSS, and others, discuss recent advancements in 3D imaging that have allowed radiologists and orthopedic surgeons to work increasingly in tandem to better treat patients with musculoskeletal conditions.</p>

<p>Although CT has been the primary type of orthopedic 3D imaging, MRI has begun to emerge as another important imaging modality within the specialty. According to Dr. Potter, MRI 3D imaging was made especially relevant through the incorporation of parallel imaging, which shortened scan acquisition time. &ldquo;We are now working on things like deep learning de-noising algorithms, which use artificial intelligence to take some of the noise away from MRI images and further shorten scan time,&rdquo; she explained.</p>

<p>Aside from CT and MRI, other scanning modalities also incorporate 3D imaging, such as the whole-body surface photographic system made by 3dMD, which Dr. Potter explained is especially efficacious for patients with scoliosis. The photographic system tracks changes in the spinal curvature without use of an x-ray and provides an option for reducing the burden of ionizing radiation, which is particularly important for pediatric patients, she noted.</p>

<p>In the context of orthopedic oncology surgery, determining the size of the tumor mass can be as important as determining what structures and tissue the mass may encroach upon. For example, Dr. Potter cited a tumor surgeon who needs to know whether a tumor in the thigh might be right up against the sciatic nerve. &ldquo;Going into surgery, the surgeon wants to know exactly what he or she is going to encounter when they open the patient up, so 3D imaging can be helpful in assessing tumor encroachment on vital neurovascular structures or muscle.&rdquo;</p>

<p>Forward looking to the future era for imaging, Dr. Potter noted techniques like preoperative virtual arthroscopy and preoperative operative planning may remove a lot of the guesswork of surgery. &ldquo;The concept of using fiducial markers in the operating room, potentially with robotic-assisted surgery, is going to streamline things and decrease anesthesia time, decrease operative time and overall improve operative recovery and the surgical outcome.&rdquo;</p>

It tends to strike young, otherwise healthy adults, usually on one side of their body. For many, it happens suddenly with no known cause. The pain usually subsides within two or three weeks and is followed by muscle atrophy and severe weakness.</span></p><p><span>Twenty-nine-year old Lucy Schwartzman is participating in the HSS study. She clearly remembers the day in&nbsp;April of this year when she was struck by severe pain in her right shoulder that radiated into her neck and down her arm to her elbow. After a few weeks, the extreme pain improved, but she started to experience another problem. “Three weeks in, I went to fill up my water container and realized I couldn’t move my arm; I couldn’t reach forward,” she recalls. “I felt like my upper arm was paralyzed.” Ms. Schwartzman says she still can’t move her arm above her elbow, and it has affected her ability to perform basic activities, such as putting on a jacket or washing her hair.</span></p><p><span>“Patients experience damage to the peripheral nerves that control movement in the shoulders and arms,” explains </span><a href="https://www.hss.edu/physicians_sneag-darryl.asp"><span>Darryl Sneag, MD</span></a><span>, director, Peripheral Nerve, MRI, at HSS and principal investigator. “In our study, we will use magnetic resonance neurography to obtain high resolution images of those nerves. Our goal is to discover biomarkers to detect and monitor PTS. We will also use ultrasound imaging and check patients’ blood for inflammatory markers.”</span></p><p><span>Parsonage-Turner Syndrome is named for Drs. Maurice Parsonage and John Aldren Turner, who described 136 cases in the </span><i><span>Lancet</span></i><span> medical journal in 1948. The disease is also called neuralgic amyotrophy or brachial neuritis.</span></p><p><span>Although the exact cause is not known, it is believed PTS may arise following a viral or bacterial infection, an injury, surgery, strenuous exercise or a medical procedure. There is no cure or specific treatment, although doctors usually prescribe pain medication, steroids and physical therapy to help relieve symptoms. Sometimes patients receive intravenous immunoglobulin.</span></p><p><span>Although an estimated 80 to 90 percent of people eventually recover and regain function of their arm, it could take several years. Full strength may not return for some patients, and a certain number will continue to experience mild pain.</span></p><p><span>The discovery of a biomarker for PTS could pave the way for clinical trials to determine which treatment works best and open the possibility of finding new treatments down the road, according to Dr. Sneag.</span></p><p><span>“Our research aims to understand if imaging and serologic findings in PTS can serve as reliable, noninvasive markers of motor and functional recovery,” he notes. “In addition to facilitating clinical trials of possible treatments, early identification of the nerves affected by PTS could assist in determining if surgery could help patients.”</span></p><p><span>Previous PTS research by Dr. Sneag and colleagues identified a specific abnormality in brachial plexus branches and other peripheral nerves. “Using high-resolution MR neurography and ultrasound techniques, we have routinely identified one or more hourglass-like constrictions of nerves in patients with Parsonage-Turner Syndrome and have detected denervation changes in muscles they supply,” he explains. “The constrictions actually make the nerve look like an hourglass on the digital image. We perform almost 100 MR neurography cases monthly at HSS and have not seen this in any other nerve condition, so we believe these constrictions are unique to PTS.”</span></p><p><span>Patients enrolled in the new HSS study will have imaging tests at their initial consultation and at follow-up intervals of three, six and nine months. Dr. Sneag and colleagues hypothesize that a greater number of or more severe constrictions in affected nerves will correlate with worse muscle function and lower patient-reported outcome scores at each time interval. The MRI findings will be correlated with standard electromyography (EMG) tests that measure muscle function and the nerves that control them.</span></p><p><span>Using imaging to zero in on hourglass-like constrictions in affected nerves also aids in surgical planning, according to Dr. Sneag. “Images obtained by magnetic resonance neurography can enable surgeons to do a very targeted surgery. Instead of exploring the entire nerve in an arm, the images can pinpoint the location of the constriction,” he says. “Think of a coaxial cable. When you cut it, you see a cross section of multiple fibers. In a damaged nerve, constrictions may not involve the entire nerve, but just one or more fibers within the nerve. With MRN showing the exact location of the constriction, the surgeon wouldn’t have to spend hours opening and exploring the nerve to find it.”</span></p><p><span>The National Center for Advancing Translational Sciences of the NIH awarded the $275,000 grant for the HSS study, which is expected to take two years. Investigators are recruiting patients 18 and older who have been diagnosed with Parsonage-Turner Syndrome. Efforts are under way to include competitive surfing in the Olympics. However, the number of reports on <a href="https://www.hss.edu/playbook/surfing-safety-tips/">surfing-related injuries</a> is limited and does not mirror the trend in popularity, according to researchers at Hospital for Special Surgery (HSS).</p>

<p><a href="https://www.hss.edu/physicians_greditzer-harry.asp">Harry &ldquo;Tate&rdquo; Greditzer, IV, MD</a>, radiologist at HSS and avid surfer himself, launched a study to determine the kinds of orthopedic injuries a recreational surfer might sustain and how often he or she required surgery. &ldquo;The primary purpose of the study was to characterize MRI patterns of acute surfing-related injury at HSS, an urban musculoskeletal hospital,&rdquo; Dr. Greditzer said. &ldquo;Secondarily, the purpose was to report the proportion of those injuries that required orthopedic surgical intervention.&rdquo;</p>

<p>Dr. Greditzer and colleagues found that shoulder injuries were the most common, followed by knee injuries. In the study, 17% of patients needed surgery for any type of surfing injury. The research was published online in the journal <em>Sports Health</em> on November 5.</p>

<p>The investigators noted that previous surfing studies from different regions of the world have described the incidence of injuries along with the type, location, and severity, but the results have been highly variable. Most prior studies have also utilized imaging modalities that are readily available in an emergency setting, but did not focus on high resolution soft tissue imaging modalities such as MRI, as in the HSS study.</p>

<p>HSS investigators conducted a retrospective review of medical records to identify patients with surfing-related injuries who came to HSS for treatment between January 1, 2009 and August 1, 2018. The researchers analyzed the data and reported on the body part injured, diagnosis, and surgical versus nonsurgical treatment.</p>

<p>The search yielded 109 patients with surfing-related injuries who had MRIs. A total of 90 patients came to HSS within six months of their injury and were included in the final analysis. The median age was 36, with patients ranging in age from 12 to 66. Three-quarters of the patients were male.</p>

<p>Acute surfing injuries were diagnosed with an MRI in 72% of study patients. The following injuries were reported:</p>

<ul>
<li>Shoulder: 46% of surfing injuries</li>
<li>Knee: 28%</li>
<li>Foot or ankle: 9%</li>
<li>Spine: 6%</li>
<li>Elbow: 6%</li>
<li>Other (rib fracture; muscle strain or muscle laceration): 5%</li>
</ul>

<p>&ldquo;Although prior studies have shown that injuries related to surfing are primarily found in the head or lower extremities, our study found upper extremity injuries to be more common,&rdquo; said <a href="https://www.hss.edu/physicians_fabricant-peter.asp">Peter D. Fabricant, MD, MPH</a>, pediatric orthopedic surgeon at HSS and study author. &ldquo;The majority of upper extremity injuries occurred at the shoulder with anterior shoulder dislocation being the most common diagnosis. Of the presenting knee injuries, a torn medial collateral ligament was the most frequent.&rdquo;</p>

<p>Only 17% of all the surfing injuries required surgery at HSS. This percentage may overestimate the number of surfing injuries in general that require surgery, the study authors noted. Tertiary care referral centers such as HSS tend to see a greater proportion of serious injuries requiring surgery.</p>

<p>The study authors noted that the overall diagnostic quality provided by MRI helps to elucidate the most serious soft tissue surfing injuries. However, the exclusive use of this imaging modality for diagnosis was a limitation of the study, as it would not account for other musculoskeletal surfing injuries that do not require an MRI.</p>

<p>&ldquo;When compared to other extreme sports, surfing seems relatively safe,&rdquo; said Dr. Greditzer. &ldquo;However, it&rsquo;s important to keep in mind that our study looked at <em>recreational</em> surfers. We did not include professional surfers, so the patients in our study were not able to generate as much speed, get barreled, or launch into the air like a professional or amateur can, where the potential for injury is much higher.&rdquo;</p>

<p>Dr. Greditzer, who has been surfing for more than 20 years, commented on injury prevention for beginning surfers. He says being a good swimmer is the most important attribute for anyone thinking of taking up the sport. It&rsquo;s a concern for health professionals, as well. Children are more sensitive to the effects of radiation over time, and that&rsquo;s why Hospital for Special Surgery (HSS) recently acquired a fourth 2D/3D orthopedic imaging system that produces significantly less radiation compared to standard x-rays.</p>

<p>The lower-dose imaging technology is now available at <a href="https://www.hss.edu/hss-westchester.asp">HSS Westchester</a> in White Plains, New York. The other imaging systems are at the main hospital campus in Manhattan, including one at the <a href="http://www.hss.edu/pediatric-orthopedic-surgery.asp">Lerner Children&rsquo;s Pavilion</a>.</p>

<p>&ldquo;Many of our pediatric patients who are treated for scoliosis need x-rays several times a year. We take every safety precaution with all imaging tests, and the low-dose system is another way we can meet the needs of our patients and provide high quality care,&rdquo; noted <a href="https://www.hss.edu/physicians_widmann-roger.asp">Dr. Roger Widmann</a>, chief of Pediatric Orthopedic Surgery at HSS.</p>

<p>&ldquo;Parents of our Westchester and Connecticut patients have been willing to travel one or two hours to our Manhattan location for the low-dose x-rays,&rdquo; he added. &ldquo;The new imaging system at HSS Westchester will make it more convenient for these families.&rdquo;</p>

<p>Vera Scavone, who lives in Armonk in Westchester County, was pleased to hear that the lower-dose imaging system is now available in White Plains. Dr. Widmann successfully treated her daughter, Grace, for scoliosis, and the new location will be more convenient for any future spine x-rays she may need.</p>

<p>Ms. Scavone learned about low-dose x-rays for the first time two years ago when Grace became Dr. Widmann&rsquo;s patient at age 15. &ldquo;I definitely felt better, since Grace needed a lot of x-rays,&rdquo; recalls Ms. Scavone, who traveled to HSS in Manhattan each time x-rays were needed. &ldquo;It&rsquo;s absolutely reassuring for parents to know that the dosage of radiation is minimized.&rdquo;</p>

<p>&ldquo;Our goal is to provide high quality patient care. Lower-dose x-ray imaging and other cutting edge technologies enable us to do this,&rdquo; said Dr. John A. Carrino, vice chairman of the <a href="https://www.hss.edu/radiology-imaging.asp">Department of Radiology and Imaging</a> at HSS.</p>

<p>When choosing an imaging modality to make a diagnosis and plan treatment, physicians must balance the need for accurate measurements with the need to minimize radiation, Dr. Carrino explains. This concept, known as ALARA (as low as reasonably achievable), is particularly important in children. Physicians at HSS seek to ensure that the lowest amount of radiation is used to produce images based on the patient&rsquo;s size and the diagnostic task at hand.</p>

<p>The lower-dose imaging facility is open to anyone who has a prescription for x-rays from his or her physician. One does not need to be a patient of an HSS doctor. Lower-dose imaging is most often used for patients who have scoliosis. The system is not used to diagnose a broken bone; regular x-rays remain the gold standard if a fracture is suspected.</p>

<p>In addition to scoliosis, the lower-dose system is used to assess leg length discrepancies in children. These patients generally require frequent imaging tests to monitor growth, plan for surgery and determine the effectiveness of treatment. A previous study at HSS found that the lower-dose imaging system performed as well as a conventional CT scan to assess leg length.</p>

<p>&ldquo;HSS was an early adopter in using the low-dose imaging technology and is the only hospital in the U.S. with four units,&rdquo; notes Christopher Smith, administrative director of Radiology & Imaging at HSS. &ldquo;The advanced imaging system is the only one available in Westchester County and the Lower Hudson Valley Region.&rdquo;</p>

<p>Although originally acquired primarily for imaging exams in pediatric patients at HSS, the system is being used increasingly by orthopedic surgeons treating adults who need a hip or knee replacement or spine surgery. The system provides 3D frontal and side weight-bearing images that aid in pre-surgical planning.</p>

<p>The test takes about 25 seconds. Patients must be capable of standing still in the spacious unit and able to follow directions from an x-ray technologist. An objective diagnosis through imaging can also help with longitudinal monitoring to see how nerves improve, or don't improve, over time."</p>

<p>When looking at the growing subfield of MR neurology, Dr. Carrino explains that not all readers are created equal. "There are specialized protocols we use at HSS that require interaction with other multidisciplinary providers for optimal outcomes. This involves understanding imaging and understanding the disease process—that specific interaction is key. I would advise pain providers to seek out professionals who really know how to image the peripheral nerves rather than sending patients to the closest or most convenient location, which may not result in the most accurate information."</p>

<p>"It is not uncommon that objective outcome variables do not correlate with more subjective patient-reported outcome measures (PROMs)," she explains. The discordance between clinical (subjective) and imaging (objective) results is often a result of individuals who report good function and limited symptoms despite the presence of suboptimal graft appearance on imaging.</p>
