Born with a limb difference, Apgar came to HSS to collaborate with <a href="https://www.hss.edu/profiles/rehab/chrysta-irolla" target="_blank" rel="noreferrer noopener">Chrysta Irolla, MS, MSPO, CPO</a>, Director of Prosthetics and Orthotics at HSS, on building a custom prosthetic.</p><p style="margin-left:0in;"><span>Irolla is building a prosthetic for Apgar with a gel liner and a hook extension that is designed to fit around the bat and provide a more secure attachment. Doctors used some of the bones in Apgar’s left leg and foot to help strengthen her arm when she was a kid, so HSS also is creating custom foot orthotics for her with built-in rigid plates.</span></p><p style="margin-left:0in;"><span>“She’s very open to trying new things. She just wants to perform at the highest level and everything that she can do to get there,” Irolla said. “It’s really nice to have somebody be that open and creative and motivated.”</span></p><p style="margin-left:0in;"><span>Apgar has no feeling in her nub. During one visit with Irolla, she tried on a prosthetic that mimicked the sensation of touch.</span></p><p style="margin-left:0in;"><span>“It was the first time that I had held a bat and it felt like I was holding it with two hands. These sessions help identify potential issues that could contribute to shin pain. But the number that you’re seeing might not present your fitness level as accurately as assumed. <span>Research questions whether the most common way that we measure&nbsp;</span>VO₂<span><strong> </strong>max&nbsp;– that is, based on bodyweight – actually provides the best way to compare aerobic efficiency across different body types.&nbsp;</span></p><p style="text-align:start;">Elite marathoners tend to have high relative VO₂ max values, partly because of their smaller, lighter frames, said Baird. But having stable knees is super important in a bunch of different scenarios—from running a<span>&nbsp;</span>marathon<span>&nbsp;</span>to climbing the stairs to<span>&nbsp;</span>squatting<span>&nbsp;</span>down to pick up a child—which means doing these types of moves on the reg is actually a really smart idea.</p><p style="text-align:start;">“Everybody could probably benefit from some sort of strengthening to improve stability of the knee,”<span>&nbsp;</span>said Whitelam.</p><p><span style="text-align:start;">Knee stability can encompass a lot of different things, but to boil it down, it’s “the ability to maintain alignment and positioning of your knee during movement and activity,” Whitelam shared. There are passive structures in the body that help with knee stability, which include certain ligaments, Whitelam explains–such as the ACL and MCL. Moody underwent a procedure late last week that will require anywhere between nine to 12 months of rehab. <a href="https://www.hss.edu/profiles/rehab/ryan-kennelly" target="_blank"><span>Ryan Kennelly, </span>PT, DPT</a>, physical therapist at HSS, weighed in on what the next year will require from Moody.</p><p><span style="text-align:start;">“This is not something that you want to rush him back from,” Kennelly said. “The top cause of injury, or in this case a re-tear, is returning to play too early. That’s always a risk you want to avoid. Strength training also helps maintain your resting metabolic rate (how much energy you burn at rest), promotes stability throughout your joints, and can help lower injury risk in day-to-day life, she adds. It’s no surprise then that Hall recommends regular strength training to her patients on GLP-1s.</p><p style="text-align:start;">To complement the strength training, Hall also recommends flexibility work. “Flexibility training plays an important role in maintaining quality movement patterns during rapid weight loss, which can help to reduce the risk of injury,” she said. That’s because when you lose weight, your movement mechanics can change. Flexibility training can keep your joints and muscles moving through their full range of motion, which helps you lift weights with safe, effective form. Neglecting your mechanics on top of that can lead to overloading certain joints and tissues, which can lead to injuries, she explained.</span></p><p><span style="text-align:start;">It’s unrealistic to think you can focus on improving both speed and form all year round, every single year, Baird said. You may also need to build up your fitness to tackle 12-3-30, said Carter. See how the workout feels at different inclines and paces, he said, and strive for a sweet spot of challenging but sustainable.</span></p><p style="margin-left:auto;"><span>If you need to hold on to the handrails to support yourself, that’s a sign you should decrease the incline, the speed or both, Carter shared. Otherwise, you’re “reducing the amount of work that you are doing and therefore the potential benefits.” </span><span style="text-align:start;">Carter also recommended leaning forward slightly when walking at a steep incline, both for balance and to generate more force. But higher-intensity exercise may burn more calories total, he added.</span></p><p style="margin-left:0px;text-align:start;"><span>"If the goal is weight loss, it's about the total amount of calories you’re burning," Machowsky added. “You need to be in a calorie deficit to promote actual reduction of fat stores off your body.”</span></p><p><span>“Some people find the act of exercising in the morning makes them more mindful of their eating choices the rest of the day, so it can have a positive ripple effect,” said Machowsky.</span></p><p><span>Ultimately, the impact of the 30-30-30 method will depend on a person's baseline activity level and other habits, the experts emphasize. The most common heart rate formula people use is<span><strong>&nbsp;</strong></span>220 minus your age<span><strong>&nbsp;</strong></span>to get your max heart rate, then they calculate zones based on percentages. This isn’t a very accurate measure for most people as age is one of a number of factors that determines your heart rate, says Baird.</p><p style="margin-left:0px;text-align:start;">“There is incredible variation in heart rate among individuals due to factors such as sex, age, fitness level, health considerations, and lifestyle. These all contribute to a person’s heart rate at rest, during exercise, and maximally,” she explained.</p><p style="margin-left:0px;text-align:start;">Genetics may be another reason. <span style="background-color:rgb(255,255,255);"><span style="text-align:start;">Some people just have higher heart rates, said Baird. One solution is to run by RPE (rate of perceived exertion) instead. </span></span><span>Running by RPE is paying attention to your breath, sweat rate, heart rate, and muscle fatigue when running and adjusting your pace to stay in range.</span></p><p><span>To run easy, you should notice how hard you are breathing and whether you can talk, said Baird. And it’s an intrinsic human motion we learn when we’re really young. So for all those reasons, it’s really hard to change your running form, especially in real time.”</span></span></p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">Even if you are able to maintain a form cue over the course of a run, your body might not be able to handle multiple miles with a new technique. “If you go from a heel strike to a mid-foot strike in the moment, chances are your body’s not developed in a way to tolerate this change in the repetitive loading,” explained Baird. “You’ll end up stressing a different area that you’re not used to stressing.
