The Upper East Side hospital welcomed 20 students from the New York tri-state area to a day-long event to introduce them to the field of orthopedic surgery.</p><p>They participated through the Perry Initiative's Perry Outreach Program. “The goal of our program is to introduce students to all the exciting careers in orthopedics on both the surgical and engineering sides,” explained Amelia Knarr, PhD, executive director of the Perry Initiative. “Students get a real hands-on feel for the different types of procedures in orthopedics and also get to hear from successful surgeons and engineers. This benefits the students by showing them they are wanted, welcome, and capable of joining this exciting field.”</p><p>Under the guidance of HSS physicians, students participated in hands-on activities using plastic replicas of bones and tools of the trade such as drills and casts. They learned techniques that orthopedic surgeons use to repair broken bones, fix torn ligaments, apply a cast and suture a wound.</p><p>“The activities highlighted various biomechanical principles in orthopedics, demonstrating the integration of engineering in this field,” said <a href="https://www.hss.edu/profiles/doctors/jessica-heyer" target="_blank">Jessica Heyer, MD</a>, a pediatric orthopedic surgeon at HSS and event organizer. “They were excited to use orthopedic tools and enjoyed casting each other and learning how to suture.”</p><p>In addition to hands-on learning, the students attended lectures to get an idea of what it takes to become an engineer or an orthopedic surgeon and what their jobs are like on a day-to-day basis. “The students demonstrated great enthusiasm, asking thoughtful questions,” Dr. Heyer said.</p><p><span>Aarti Shenoy, PhD, an instructor of applied biomechanics at HSS, delivered an engaging lecture, sharing key moments in her journey to become an engineer. She highlighted the collaborative nature of her work – describing how she and her colleagues in the Biomechanics Department partner with HSS surgeons to design research studies to advance the field of musculoskeletal medicine.</span></p><p><span>“The Perry Outreach workshop was a valuable experience for the participants, as they were able to gain insight into what a career in orthopedics might entail,” she said. “Students gained a sense of accomplishment from completing tasks that may have seemed challenging at first. Through the hands-on activities, they were encouraged to envision themselves as surgeons or engineers.”</span></p><p><span>“</span>I learned how to use a drill today, and then we drilled a bunch of screws into different bone models,” said Lina Bernatska, a junior at Staten Island Technical School, who gave the event a thumbs up. “It really inspired me as a woman to want to go into these fields where women are underrepresented, and it's so refreshing to see all these educated young women tell their stories and tell us about their journeys…to get to where they are in life.”&nbsp;<span>&nbsp;</span></p><p>Her classmate, Ava Glatman, said it was cool to learn about different orthopedic procedures, such as total knee replacements, and how to fix a broken ankle. Reflecting on the event, she said, “I think it's really meaningful, especially understanding that this is such an underrepresented field for women. It is an autoimmune disease, or a condition in which the body attacks its own tissues—in this case, the joints.</span></p><p style="margin-left:0in;"><span>Kids with oligoarticular JIA have swelling, pain, and stiffness in large joints like their knees, ankles, and elbows. They may have trouble walking, and one leg could be longer than the other because bones near inflamed joints grow too quickly or slowly, per NIAMS.</span></p><p style="margin-left:0in;text-align:start;"><span>Keep in mind that your child might not tell you their joint hurts. It’s not that they don’t feel pain, but that they react to it differently than adults might, said Dr. Onel. “I think it happens slowly and they get used to it. One main difference is that the EULAR recommendations included timelines for treatment targets, while the ACR's did not, she continued.&nbsp;</span></p><p style="margin-left:0px;text-align:start;">"It's great to have these timelines in there," <span style="color:#000000;">cited </span>Dr. Onel though there are still some unknowns. They probably have thought that the child has growing pains and is fine. It’s never normal for kids to have swollen joints.” She continued, “It’s easy to blame it on an injury, but the reality is that especially for little ones, they don’t fall hard enough, or twist enough so that they get swelling from an injury. Swelling is never normal, stiffness is never normal.”<span>&nbsp;</span></p><p>Dr. Onel added, “Nobody knows children the way their parents do. When parents see that there’s a change, a difference, even if it’s something the doctor might not be familiar with, they’re the ones who know. &nbsp;I think parents have to be key advocates for their kids, but also we [physicians] have to make sure that we listen.”<span>&nbsp;</span></p><p>When asked if juvenile arthritis goes away as kids get older, Dr. Onel noted, “About 40% of the kids have active arthritis into adulthood,” stating “we have incredibly effective medications - so when people say will they have this forever? The answer is they may or may not.”&nbsp;</p><p>Dr. Onel concluded, “On the medications, they’ll be able to grow up and do everything they want to do. In fact, for systemic JIA the guidelines suggest using biologic DMARDs as a first line. The results were presented today at the American College of Rheumatology (ACR) annual meeting in a poster titled &ldquo;Use of Total Hip Arthroplasty in Patients Under 21 Years Old: A U.S. Population Analysis&rdquo; and recently published in <a href="https://www.sciencedirect.com/science/article/abs/pii/S0883540321006446?dgcid=coauthor" style="text-decoration:underline"><em>The Journal of Arthroplasty</em></a>.</span></span></span></p><p><span><span><span>Between 2000 and 2016, the number of total hip arthroplasty (THA) procedures performed in patients under 21 years of age increased from 347 to 551, while the total pediatric population in the United States remained stable, the researchers found.</span></span></span></p><p><span><span><span>Traditionally, most young patients undergoing THA suffer from developmental bone or joint ailments or juvenile inflammatory arthritis. However, the new study revealed that the upswing in THA procedures in young patients is not being driven by a rise in the number of children with inflammatory joint disease, suggesting that nonsurgical therapies to control these conditions are working, said <a href="https://www.hss.edu/physicians_mehta-bella.asp" style="text-decoration:underline">Bella Mehta, MBBS, MS, MD</a>, a rheumatologist at HSS and the senior author of the study.</span></span></span></p><p><span><span><span>&ldquo;Our study shows that although THA procedures are increasingly being performed in young people, we aren&rsquo;t seeing more of these patients seeking surgery for inflammatory arthritis,&rdquo; Dr. Mehta said. &ldquo;We&rsquo;re doing a better job at treating these individuals so they don&rsquo;t develop end-stage joint damage. Twenty years ago, we didn&rsquo;t have access to effective pharmacologic treatments for these conditions, and now we&rsquo;re using them well and helping these patients live a better life.&rdquo;</span></span></span></p><p><span><span><span>&ldquo;These procedures are still pretty rare in hospitals, and some surgeons might not see any young patients,&rdquo; added Alex Gibbons, a research assistant in Dr. Mehta&rsquo;s group and a co-author of the study. &ldquo;Although some site-specific data on THAs in adolescents exist, it wasn&rsquo;t known how frequently these procedures were being performed in the United States and if that frequency was growing or not.&rdquo;</span></span></span></p><p><span><span><span>For the study, Dr. Mehta and her colleagues used the Kids&rsquo; Inpatient Database, a repository of information on patients under 21 years of age from approximately 4,200 hospitals in 46 states. The researchers looked for elective and non-elective primary THA procedures.</span></span></span></p><p><span><span><span>The mean age of young patients undergoing THA was just over 17 years and remained constant throughout the duration of the study. Most patients were white (55.3%) and had private insurance (56.5%). The majority (80%) of THAs were performed in urban teaching hospitals.</span></span></span></p><p><span><span><span><a href="https://www.hss.edu/physicians_figgie-mark.asp" style="text-decoration:underline">Mark P. Figgie, MD</a>, chief emeritus of the Surgical Arthritis Service at HSS and a co-author of the study, said the finding that most THA procedures in young patients are performed in urban and teaching hospitals reflects a trend that is beneficial for these patients. As other studies have demonstrated in conditions like inflammatory arthritis, when hip replacements are performed by surgeons with significant experience, patients have better outcomes. &ldquo;And for younger patients, the nuances require the expertise and knowledge to avoid complications and optimize outcomes,&rdquo; Dr. Figgie said.</span></span></span></p><p><span><span><span>The most common diagnoses for young patients undergoing THA were osteonecrosis, osteoarthritis and juvenile idiopathic arthritis (JIA)/inflammatory arthritis, the researchers found. Over the course of the study, the frequency of THA for osteonecrosis rose from 24% to 38%, while that for arthritis fell from 27% to 4%&mdash;likely a reflection of recent improvements in medical management for the condition, they added.</span></span></span></p><p><span><span><span>Dr. Figgie noted that improvements in implant technology and materials have made the devices far more durable than they were 20 years ago. As a result, surgeons now feel more comfortable offering THA to young patients because their implants are likely to withstand the wear and tear of decades of activity.</span></span></span></p><p><span><span><span>Dr. Mehta said the findings could be of value both to clinicians and young patients, for whom, in many cases, the surgery is their first major operation. &ldquo;I would use these results to say to a young person: &lsquo;There are a lot of people who get these procedures; you&rsquo;re not alone.&rsquo; I find that, especially for young patients, knowing they&rsquo;re not the only ones to experience something really helps. ]]></description><content:encoded><![CDATA[<p>Healio Rheumatology reports clinicians and researchers continue to debate the etiology and treatment paradigms of multi-inflammatory syndrome in children (MIS-C), which impacts some children with COVID-19, according to a presentation at the American College of Rheumatology (ACR) State-of-the-Art Clinical Symposium by <a href="https://www.hss.edu/physicians_onel-karen.asp">Karen Brandt Onel, MD</a>, chief of pediatric rheumatology at HSS.&nbsp;</p><p>It is important to understand who may be impacted by MIS-C. Children aged 6-12 years are more likely than their younger counterparts than to develop this complication, as are children with cardiorespiratory or cardiovascular without respiratory involvement, those mucocutaneous infection, neutrophil or lymphocyte ratios greater than five and low platelet counts. It is most likely to occur in non-Hispanic Black children.</p><p>As for treatment, early research conducted by Dr. Onel and colleagues showed that intravenous immunoglobulin (IVIG), along with aspirin, administered before day 10 of infection, could be most effective. Dr. Onel advised it is key to intervene as early as possible. ]]></description><content:encoded><![CDATA[<p>In this Chatting With Betsy podcast, <a href="https://www.hss.edu/physicians_onel-karen.asp">Karen Brandt Onel, MD</a>, chief of pediatric rheumatology at HSS, discussed chronic recurrent multifocal osteomyelitis (CRMO), current treatment options&nbsp;and work being done&nbsp;to refine the&nbsp;approaches for management.&nbsp;</p><p>&ldquo;[CRMO] Chronic recurrent multifocal osteomyelitis or chronic noninfectious osteomyelitis is part of a group of disorders that we call autoinflammatory disorders,&rdquo; said Dr. Onel. &ldquo;In autoinflammatory diseases the immune system is just turned on and goes crazy. Those of us who have been hearing about COVID and conversations about cytokine storm - cytokine storm is a little bit of what we&rsquo;re discussing here. In these kids, for reasons we don&rsquo;t understand, the immune system turns on and they make a lot of cytokine. They get very inflamed in the bone which is very painful but you can&rsquo;t see it from the outside. That&rsquo;s part of the problem.&nbsp;Its invisible because it's inside of the bone. The immune system just turning itself on is causing all of this problem,&rdquo; she explained.</p><p>&ldquo;The reality is this disease is very heterogeneous. It&rsquo;s important to think about it because if it&rsquo;s not on your mind you&rsquo;re not going to make the diagnosis,&rdquo; she continued. &ldquo;If we don&rsquo;t treat these children what ends up happening is the bones get deformed. And if the bones become deformed as a child the bones will always be deformed. We would say we want to treat these children aggressively because hopefully at some point they&rsquo;ll either go into remission or we&rsquo;ll figure out the answer of how we&rsquo;ll actually make them go into remission or treat them better. We want their bones, joints everything to be perfect &ndash; we have all the reason in the world to treat them aggressively and keep them functional.&rdquo;</p><p>While there are different approaches to treatment, Dr. Onel cited, &ldquo;We don&rsquo;t know what&rsquo;s best for any one person. Not yet. That&rsquo;s part of the reason why we want to get organized. We want to be able to come to an answer on what&rsquo;s the best way to do that but we can&rsquo;t unless we work together. <p><span><span><span>The research focuses on the diagnosis of renal disorders, the risk of venous thromboembolism after total knee replacement (TKR), and the care of pediatric and young adult patients with rheumatologic diseases. There are also studies related to the care of rheumatology patients during the COVID-19 pandemic.</span></span></span></p>

<p><span><span><a href="https://acrabstracts.org/abstract/geographical-variations-in-covid-19-perceptions-and-patient-management-a-national-survey-of-rheumatologists/"><b><span>Geographical Variations in COVID-19 Perceptions and Patient Management: A National Survey of Rheumatologists</span></b></a> </span></span></p>

<p><span><span><span><span>A study led by HSS rheumatologist</span> <a href="https://www.hss.edu/physicians_mehta-bella.asp"><span>Bella Mehta, MBBS, MS</span></a><span>, looked at <span><span>the perceptions and behaviors of rheumatologists in the United States regarding the risk of COVID-19 for patients with autoimmune disorders and how those patients were managed with immunosuppressive and anti-inflammatory medications.&nbsp;The</span></span> <span>271 respondents were asked whether patients with rheumatic diseases were at higher risk of COVID-19 and whether the pandemic had led them to reduce the use, dosage, or frequency of biologics or steroids. Overall, the survey found geographical variations regarding perceptions of patients&rsquo; risk of COVID-19, with responses that were significantly different in the Northeast region of the United States compared with other regions.</span></span></span></span></span></p>

<p><span><span><span><a href="https://acrabstracts.org/abstract/can-a-clinical-disease-activity-index-based-on-patient-reported-joint-counts-pt-cdai-be-used-to-inform-target-based-care-in-telemedicine-an-analysis-of-2-early-ra-cohort-studies/"><b><span>Can a Clinical Disease Activity Index Based on Patient-Reported Joint Counts (PT-CDAI) Be Used to Inform Target-Based Care in Telemedicine? An Analysis of 2 Early RA Cohort Studies</span></b></a>&nbsp;</span></span></span></p>

<p><span><span><span>A team that included HSS rheumatologist</span> <a href="https://www.hss.edu/physicians_bykerk-vivian.asp"><span>Vivian P. Bykerk, BSc, MD, FRCPC,</span></a> <span>looked at data from two cohort studies on patients with early rheumatoid arthritis (RA) that compared patient and physician assessment of tender and swollen joints. This research is important because COVID-19 has forced rheumatologists to shift from in-person clinical visits to telemedicine, which limits their ability to carry out complete joint exams. The study found that patient and physician assessments had good agreement when it came to identifying active versus controlled disease activity. In addition, the researchers reported that predictors of higher discrepancies between patient and physician assessments may help identify patient subsets that could benefit the most from more physician-guided training at joint self-assessments as well as more probing questioning during telehealth visits to confirm active synovitis.</span></span></span></p>

<p><span><span><a href="https://acrabstracts.org/abstract/characterization-of-antiphospholipid-antibody-associated-nephropathy-an-international-survey-of-renal-pathology-society-members/"><b><span>Characterization of Antiphospholipid Antibody-Associated Nephropathy: An International Survey of Renal Pathology Society Members</span></b></a> </span></span></p>

<p><span><span><span>A survey led by HSS rheumatologist</span> <a href="https://www.hss.edu/physicians_barbhaiya-medha.asp"><span>Medha Barbhaiya, MD, MPH</span></a><span>, sought to determine whether pathologists worldwide use uniform criteria to distinguish antiphospholipid antibody-associated nephropathy (aPL-N) from other forms of thrombotic microangiopathy. The study was done in parallel with an international effort to develop new classification criteria for antiphospholipid syndrome. The web-based survey included 780 members of an international Renal Pathology Society; 111 renal pathologists responded, representing 33 countries. The participants were asked to determine whether two acute and eight chronic aPL-N features were consistent with acute or chronic aPL-N without and were not provided with serology information. Results from the survey indicated that more than 90% of pathologists worldwide agreed that the renal pathologic features most specific for aPL nephropathy are noninflammatory glomerular or small arterial microthrombi and organized microvascular thrombi with recanalization. In the absence of serologic data, more than 75% of pathologists indicated lack of specificity of chronic glomerular or small arterial changes. These findings indicate the importance of serologic test results in biopsy interpretation and suggest higher specificity for certain acute or chronic features.</span></span></span></p>

<p><span><span><a href="https://acrabstracts.org/abstract/the-risk-of-venous-thromboembolism-after-septic-total-knee-replacement-tkr-revision-is-double-the-risk-after-aseptic-tkr-revision-an-analysis-of-administrative-discharge-data/"><b><span>The Risk of Venous Thromboembolism After Septic Total Knee Replacement (TKR) Revision Is Double the Risk After Aseptic TKR Revision: An Analysis of Administrative Discharge Data</span></b></a></span></span></p>

<p><span><span><span><span>A team led by HSS rheumatologist</span></span> &nbsp;<a href="https://www.hss.edu/physicians_bass-anne.asp"><span>Anne R. Bass, MD</span></a><span>, <span>assessed the 90-day risk of postoperative venous thromboembolism (VTE) in people who underwent revision TKR surgery. Although it was already known that infection can increase the risk of thrombosis by triggering coagulation pathways, the impact of infection in this context was not well understood. The study, which included 25,441 of these surgeries performed between 1998 and 2014, used data from the New York Statewide Planning and Research Cooperative System. Overall, 69% of these revisions were for mechanical causes (aseptic), 28% were for infection (septic), and 3% were related to a periprosthetic fracture. The researchers found that the risk of VTE after septic revision surgery was double the risk after aseptic revision surgery, and was 2.6-fold higher after revision surgeries for fracture. These findings point to the importance of taking infection and fracture status into account when planning VTE prophylaxis for patients having revision surgery.</span></span></span></span></p>

<p><span><span><a href="https://acrabstracts.org/abstract/using-a-patient-engaged-approach-to-identify-cross-cutting-disease-factors-impacting-mental-health-in-youth-with-rheumatologic-disease-2/"><b><span>Using a Patient-Engaged Approach to Identify Cross-Cutting Disease Factors Impacting Mental Health in Youth with Rheumatologic Disease</span></b></a></span></span></p>

<p><span><span><span>A team including HSS chief of pediatric rheumatology</span> <a href="https://www.hss.edu/physicians_onel-karen.asp"><span>Karen Brandt Onel, MD</span></a><span>,</span> <span><span><span>examined the mental health experiences of those with juvenile arthritis, juvenile dermatomyositis, or systemic lupus erythematous. It included patients aged 14 to 24 and parents of patients between the ages of 8 and 24. The primary outcome was the presence of any clinical or self-diagnosed mental health problem. The survey included information on cross-cutting disease factors such as disease duration, active disease status, current steroid medication, history of disease flare following remission, and appearance-altering comorbidities (such as psoriasis, stretch marks, alopecia, skin ulceration, and visible scarring). The researchers also examined results by mental health problem (depression, anxiety, and self-harm/suicidal ideation). Overall, they found that certain rheumatologic disease factors, including those that alter appearance, are predictive of mental health problems such as depression or anxiety. The findings will help identify targets for mental health screening in youth with rheumatologic diseases.</span></span></span></span></span></p>

<p><span><span><a href="https://acrabstracts.org/abstract/new-juvenile-idiopathic-arthritis-quality-measure-set-for-the-pediatric-rheumatology-care-and-outcomes-improvement-network-2/"><b><span>New Juvenile Idiopathic Arthritis Quality Measure Set for the Pediatric Rheumatology Care and Outcomes Improvement Network</span></b></a> </span></span></p>

<p><span><span>A team that included HSS pediatric rheumatologist</span></span> <span><span><a href="https://www.hss.edu/physicians_pan-nancy.asp"><span>Nancy Pan, MD</span></a></span></span><span><span>, reported highlights of new quality measures for the treatment of juvenile idiopathic arthritis. The study also provided information about the performance of these new measures, which were created by the Pediatric Rheumatology Care and Outcomes Improvement Network. The measures take into consideration clinical health importance, scientific validity, and feasibility, as well as best practices. They include both clinical evaluations based in part on the clinical Juvenile Arthritis Disease Activity Score and patient-reported outcomes such as pain, physical function, and overall well-being. ]]></description><content:encoded><![CDATA[<p><i>Healio Rheumatology</i> reports the authors of the 2020 American College of Rheumatology juvenile idiopathic arthritis (JIA) guidelines emerged with 47 recommendations, emphasizing the early use of biologics, regular immunizations and reduced reliance on corticosteroids. HSS chief of pediatric rheumatology, <a href="https://www.hss.edu/physicians_onel-karen.asp">Karen Brandt Onel, MD</a>, and others, aimed to develop a document that was timely, relevant and feasible in clinical practice. The guidelines cover&nbsp;oligoarthritis, temporomandibular joint (TMJ) arthritis and systemic JIA with or without macrophage activation syndrome (MAS).</p><p>&ldquo;The strength of those recommendations is largely conditional,&rdquo; explained Dr. Onel. &ldquo;The quality of supporting evidence is largely very low,&rdquo; she added. &ldquo;Similar to the RA [rheumatoid arthritis] guidelines, although we know that corticosteroids are important, especially for children as regards growth stunting and weakened bones, we really try to avoid their use whenever possible so that was a critical part of what we presented,&rdquo; noted Dr. Onel. &ldquo;Secondly, strong recommendations for immunizations. Thirdly, early use of biologics, especially for children with systemic juvenile arthritis. And our first toe into the water of recommending stopping medications when children [exhibited inactive disease]. This chronic autoinflammatory syndrome, which can be debilitating, is underdiagnosed and is characterized by multiple foci of painful swelling of bones, mainly in the metaphyses of the long bones, in addition to the pelvis, the shoulder girdle and the spine. Because CNO often mimics a bone infection or cancer, children with the condition often see various specialists before getting an accurate diagnosis. The new</span> <a href="https://acrabstracts.org/abstract/comparison-of-clinicopathologic-and-imaging-features-between-chronic-nonbacterial-osteomyelitis-and-its-mimickers-a-multi-national-450-case-control-study/"><span>study</span></a> <span>was presented at the virtual annual meeting of the American College of Rheumatology.</span></span></span></p>

<p><span><span><span>&ldquo;The reason we conducted this study is that for patients with CRMO or CNO, there is often an enormous delay between first symptoms to diagnosis, because it is confused with many other things,&rdquo; said study coauthor</span> <a href="https://www.hss.edu/physicians_onel-karen.asp"><span>Karen Brandt Onel, MD</span></a><span>, Chief of Pediatric Rheumatology and Professor of Clinical Pediatrics at Hospital for Special Surgery (HSS) in New York City. &ldquo;It is not uncommon to have children who have a two, or three-year span, or even as much as a 15-year span between the first symptoms and when they are ultimately diagnosed and treated.&rdquo;</span></span></span></p>

<p><span><span><span>In the new study, the researchers identified 264 cases of CNO and 145 mimicking conditions from 20 centers in 7 countries on 4 continents, using the REDCap online database. Cases were included if they had at least 12 months of followup. The researchers compared clinical and investigational features of CNO patients with those of patients who had conditions that mimicked CNO.</span></span></span></p>

<p><span><span><span>When compared to mimicker diagnoses, CNO patients were predominantly female, more frequently exhibited intermittent versus continued pain, but less commonly had a fever. Clavicular swelling was more common in CNO than in mimickers. Symmetric patterns of bone lesions were also more common in CNO. CNO frequently involved the thoracic spine, clavicle, sternum/manubrium, pelvic bones, bilateral femur, bilateral tibia, unilateral fibula, and foot bones. Imaging features including cortical bone disruption, disorganized bone formation, mass structure, marrow infiltrate, abscess or geographic appearance were less common in CNO.</span></span></span></p>

<p><span><span><span>The researchers say that clinicians can use the new information to help improve the diagnosis of CNO patients. &ldquo;We are working slowly and steadily towards classification criteria, so we can make the diagnosis quickly and get treatment instituted quickly. We are trying to work through this disease to identify clear as a bell diagnostic criteria, outcomes measures, and treatment,&rdquo; said Dr. Onel. &ldquo;This is a diagnosis that is not in everyone&rsquo;s brains. Especially if there are typical upper extremity lesions, with the clavicle remaining number one, if there is no fever, and there are symmetric lesions, both tibias, both fibulas, both femurs etc., those are things that should push us in the direction of a diagnosis of CNO or CRMO. If you have symmetric lesions and especially if you have upper extremity lesions, you have to think CRMO or CNO.&rdquo;</span></span></span></p>

Parents and physicians should be aware that morning joint stiffness may indicate early disease symptoms of polyarticular JIA and serve as a more reliable indicator than pain.</span></span></span></p>

<p><span><span><a href="https://www.hss.edu/condition-list_juvenile-idiopathic-arthritis.asp"><span>Juvenile idiopathic arthritis</span></a> <span>(JIA) is a condition involving joint inflammation that lasts more than six weeks in children or adolescents under 16 years of age. It is the most common form of childhood arthritis, affecting about 1 in 1,000 children and teens in the United States. Scientists believe a combination of genetic predisposing factors and the environment causes JIA but the exact cause is unknown.</span></span></span></p>

<p><span><span><span>Polyarticular JIA is a chronic form of the disease with symptoms of pain, swelling, stiffness or warmth in four or more joints. Younger children may develop a limp or have difficulty using their fingers yet not complain about pain. Symptoms are typically worse first thing in the morning or after a period of rest and improve throughout the day as the child becomes more active.</span></span></span></p>

<p><span><span><span>&ldquo;Polyarticular JIA looks a lot like rheumatoid arthritis in adults. Early diagnosis is essential to help keep the disease in remission, ensure children meet developmental milestones and delay or prevent joint destruction as they age,&rdquo; says<strong> Nayimisha Balmuri, MD</strong>, a third-year pediatric rheumatology fellow at HSS and lead author of the study. &ldquo;Reported morning joint stiffness may be a better indicator of early disease symptoms than pain since reporting pain can be influenced by cultural factors. Our findings underscore the importance of early referral of suspected cases for expert diagnosis and treatment.&rdquo;</span></span></span></p>

<p><span><span><span>The</span> <a href="https://acrabstracts.org/abstract/who-ordered-the-stiff-one-characteristics-of-polyarticular-juvenile-idiopathic-arthritis-patients-associated-with-the-presence-and-increased-duration-of-joint-stiffness/"><span>study abstract</span></a> <span>is featured today in a poster presentation at</span> <a href="https://www.rheumatology.org/Annual-Meeting"><span>ACR Convergence 2020</span></a><span>, the American College of Rheumatology&rsquo;s annual conference, which is being held virtually this year.</span></span></span></p>

<p><span><span><span>Dr. Balmuri and colleagues, including senior author</span> <a href="https://www.hss.edu/physicians_onel-karen.asp"><span>Karen Brandt Onel, MD</span></a><span>, chief of pediatric rheumatology at HSS, identified 1,684 American patients diagnosed with polyarticular JIA using the</span> <a href="https://carragroup.org/research-registry/registry-faq"><span>Childhood Arthritis and Rheumatology Research Alliance (CARRA) Registry</span></a> <span>from July 2015 through February 2020. The CARRA Registry is a collection of information provided by North American patients and families on childhood-onset rheumatic diseases, including JIA.</span></span></span></p>

<p><span><span><span>The investigators extracted data on the presence and duration of morning joint stiffness. They also examined reported family income, the highest level of guardian-completed education and community poverty level, collectively called social determinants of health, using zip codes and the 2014&ndash;2018 American Community Survey. Finally, Dr. Balmuri and colleagues included the rheumatoid factor (RF) and anti-cyclic citrullinated peptide (anti-CCP) antibody status from patients&rsquo; diagnostic lab results as indicators of disease severity. The patients were seven years of age, on average.</span></span></span></p>

<p><span><span><span>The researchers found that children with polyarticular JIA from less affluent families, defined as having an income of $25,000 to $49,000, were twice as likely to report more than an hour of morning joint stiffness compared to children with the condition from families with an income of more than $100,000. This association between reported morning joint stiffness held even after the researchers adjusted the data for age at diagnosis, and RF and CCP status.</span></span></span></p>

<p><span><span><span>&ldquo;Family income as an indicator for insurance status may indicate that more affluent families can navigate the healthcare system better, ensuring their children see pediatricians sooner and therefore obtain earlier referrals to pediatric rheumatology sub-specialists,&rdquo; says Dr. Balmuri. &ldquo;We can&rsquo;t prove that with our present study, but our findings raise important questions for further research.&rdquo;</span></span></span></p>

<p><span><span>&ldquo;We hope our findings encourage physicians to ask patients about morning joint stiffness, not just pain, which may streamline the referral process,&rdquo; Dr. Balmuri says. &ldquo;Time to referral affects how soon we can initiate multidisciplinary care, which may include medications, physical therapy and occupational therapy that are crucial for improving mobility and joint function over these children&rsquo;s lifetimes.&rdquo;</span></span></p>]]></description><category><![CDATA[pressrelease,Pediatric Rheumatology,Research Clinical,Onel,juvenile-arthritis]]></category>

<p>If you&rsquo;re tense or upset, it&rsquo;s likely your child will pick up on this and become anxious too, explained Dr. Onel. &ldquo;I can&rsquo;t tell you how many times the parent is crying while the child is getting their labs done,&rdquo; she said. Even subtle signs of parental distress, like a grimace or a furrowed brow, can be enough to affect your child, she added.&nbsp;</p>

<p>When you&rsquo;re a kid, hearing that you&rsquo;ll be on medication for the rest of your life can be tough. It&rsquo;s important to instill hope in your child, as Dr. Onel cited that finding a cure in her patients&rsquo; lifetime is very possible. &ldquo;My patients are very little. If you think about how far we&rsquo;ve come in the last 30 years, [it's] one hundred percent,&rdquo; she noted. In the meantime, the vast majority of kids with JIA do find a treatment that works for them, preventing any permanent damage from occurring. And, many go into remission and lead long, healthy normal lives, she noted.</p>

<p>As a child gets older and starts to develop an identity, he or she probably won&rsquo;t want to be known as &ldquo;the kid with arthritis.&rdquo; But making their childhood as normal as possible can mean not giving into your own anxieties. &ldquo;Your first instinct is you want to hold them close. You don&rsquo;t want them to do anything that gets them injured,&rdquo; said Dr. Onel. But the fact is, overprotected children may not perform as well in life, she added, if they never get a chance to develop the resilience and confidence needed to succeed. She encouraged parents to ask their child&rsquo;s physician about their limitations. If given the green light, children should try different activities and go out with friends. &ldquo;You want that real life waiting for them,&rdquo; Dr. Onel noted. &ldquo;I say this as a mother: Some people just know how to let their kids soar, even when they&rsquo;re scared. We could all learn a lesson from them.&rdquo;</p>

<p>&ldquo;Doctors are still learning about what causes MIS-C, but some children have had clinical and laboratory evidence of a phenomenon known as cytokine storm,&rdquo; said Dr. Taber. &ldquo;In cytokine storm, the normal control that the body has over the immune system is compromised, leading to an over-activation that can cause fevers, rashes, and damage to the organs,&rdquo; she explained.</p>

<p>&ldquo;There is no evidence at this time that children with juvenile idiopathic arthritis (JIA) or other pediatric inflammatory diseases are at increased risk,&rdquo; cited Dr. Taber. &ldquo;Recent evidence also suggests that most of the medications used to treat JIA do not place a child at increased risk of developing MIS-C or other severe manifestations of COVID.&rdquo; Healthy children with a family history of autoimmune diseases are also not at an increased risk at this time, she added.</p>

<p>&ldquo;Some autoimmune diseases, including systemic onset JIA and lupus, may have symptoms which are similar to MIS-C, such as fever or rash,&rdquo; said Dr. Taber.</p>

<p>According to the article, teens often struggle with psychosocial stressors involving mental health, substance abuse and sexuality, apart from any medical conditions. An especially vulnerable population of teens is the lesbian, gay, bisexual, transgender and questioning/queer (LGBTQ) community. Physicians have the unique opportunity to build relationships to help them navigate this challenging period of life.</p>

<p>Due to the lack of access to quality healthcare, gender nonconforming youth may resort to self-treatment by purchasing illicit parenteral and enteral growth hormones, estrogens and androgens, all of which &ldquo;have been shown to intensify immune responses; therefore, inappropriate doses of these medications that are not being overseen by a physician may have increased consequences, such as exacerbating systemic autoimmune or inflammatory diseases&rdquo;.</p>

<p>The writers conclude that the use of the HEADSS (home, education/employment, activities, drugs, sexuality, suicide/depression) assessment, including sexual orientation and gender identity-related questions, &ldquo;can help normalize conversations with patients, offer more patient-specific and holistic treatment modalities for their rheumatologic illness and help establish an open dialogue regarding issues that may affect their overall well-being&rdquo;.</p>

<p>Dr. Onel discussed her background and interest in ethics, stating&nbsp;&ldquo;I&rsquo;ve always been fascinated by issues related to ethics and, in particular, the ethics of proxy consent, such as when a parent agrees to have [their] child in a study. I have served on institutional review boards (IRBs) for most of my career and was vice chair of the IRB at the University of Chicago. Over time, in addition to my interest in medical ethics, my interest in the effects of conflicts of interest has grown &ndash; along with the rest of the world&rsquo;s interest, it seems: It is so common now to hear conflicts of interest discussed on the news and [see the issues] on the front page of the paper."</p>

<p>When it comes to current ethics-related challenges for rheumatologists, Dr. Onel explained, &ldquo;We need to think about conflicts&mdash;not just real conflicts, but apparent conflicts&mdash;in order to retain the public trust. Without that trust, the doctor-patient relationship can be damaged and interfere with best practices.&rdquo;</p>

It has received excellent reviews from health professionals around the country who are providing the book to their patients.</p>

<p>Judy Campbell, a volunteer at HSS who herself has lupus, wrote it. The hospital&rsquo;s Department of Social Work Programs approached her about the project when they learned she had written a children&rsquo;s book some years back. &ldquo;Chita Shares a Secret,&rdquo; geared toward ages 6 to 11, explains lupus in terms a child could understand. The story comes alive with a cast of jungle characters and bright, colorful illustrations.</p>

<p>Ms. Campbell says she aimed to convey a positive message for young patients &ndash; and their parents &ndash; so they would gain the courage to cope with a difficult illness. &ldquo;I set out to write a positive, upbeat book that would provide a message that it&rsquo;s OK to have lupus,&rdquo; Campbell explained.</p>

<p>Lupus causes the body&rsquo;s immune system, which is supposed to protect us from illness and infection, to attack healthy cells and organs. It affects people differently and is often unpredictable. Common symptoms include fatigue, headaches, joint pain or swelling, and a rash on the face or elsewhere on the body. In severe cases, it can affect the kidneys, heart and lungs.</p>

<p>The book has been well-received by <a href="https://www.hss.edu/pediatric-rheumatology.asp">pediatric rheumatologists</a> at HSS and other hospitals around the country. &ldquo;I love it. It is bright, cheerful and easy to read, but addresses a really big problem,&rdquo; said <a href="https://www.hss.edu/physicians_onel-karen.asp">Karen Brandt Onel, MD</a>,&nbsp;chief of the Division of Pediatric Rheumatology at HSS. &ldquo;Kids are so afraid to share with their friends, as they think they will be rejected. The book just tackles it right on.&rdquo;</p>

<p>Research shows that children are being diagnosed with lupus at a younger age, and there are limited age-appropriate resources to educate them and their families about the condition, according to <strong>Priscilla Toral</strong>, manager of LupusLine&reg; at HSS, which provides free telephone counseling for people with lupus and their families. &ldquo;The book provides physicians and the healthcare team with an age-appropriate tool to discuss a diagnosis of lupus, its symptoms and treatment with young patients,&rdquo; she said. &ldquo;Adult lupus patients can also use the book to explain their condition to their children.&rdquo;</p>

<p>The&nbsp;book helps young lupus patients come to terms with a new and difficult diagnosis, while giving them the courage to broach the subject with their family and friends, says <a href="https://www.hss.edu/physicians_taber-sarah.asp">Sarah Faith&nbsp;Taber, MD</a>,&nbsp;pediatric rheumatologist at HSS. &ldquo;It is written in straight-forward, basic language that even young children can understand and illustrated with eye-catching, engaging drawings,&rdquo; she explains. &ldquo;Families have been delighted to receive it."</p>

<p>HSS patient Elizabeth Lopez, who was diagnosed with lupus at age 11, said she showed the book to her friends to help them understand her illness.</p>

<p>Campbell, the author, understands lupus all too well. She was diagnosed in her 40s after years of doctor visits to determine what was causing her symptoms. She found a physician at HSS and firmly resolved to live a full and happy life. She has pushed through flare-ups, pain and several surgeries. Instead of dwelling on her illness, she considers herself fortunate for the many opportunities she has had throughout her life. And happy with the care she received at HSS, she says she wanted to give back.</p>

<p>Hospital for Special Surgery has printed more than 1200 copies of the book over the past year for health professionals and lupus organizations nationwide that have requested hard copies. The costs of production, printing and mailing are paid by HSS. <p>Comments from recipients around the country show it has made a difference. &ldquo;This is a wonderful resource; we use them (the books) for our patients and include them in our new patient packets,&rdquo; wrote Duke Pediatric Rheumatology staff. &ldquo;The story made me cry,&rdquo; said a health professional at Seattle Children&rsquo;s Hospital.</p>

<p>The story takes place in the jungle, where a cheetah named Chita has lupus. Dr. Hippopotamus is the kindhearted physician who explains the disease and gives good advice. Chita is sometimes too tired to play with her jungle friends and is afraid, at first, to tell them about her illness. She finally explains her symptoms to her friends and answers their questions. The animals are all very supportive, providing a message of acceptance and encouragement. In the end, they give Chita a tee shirt that says &ldquo;Chita the Brave.&rdquo;</p>

<p>Clever illustrations make the story come alive. &ldquo;The author wanted to convey a sense of belonging and security,&rdquo; explains <strong>Steven Portera</strong>, executive producer in the Creative Services Department at HSS, who illustrated the book. &ldquo;I know firsthand how hard it is to be a kid with a chronic illness and how difficult it can be most times to blend in with your friends. <p>Dr. Taber wrote the easiest way to avoid a tick bite is by staying away from places where ticks are typically found (i.e., wooded areas, tall grasses and brush). She suggested walking or biking in well-cleared paths, prohibiting children or pets from wandering off the trail, and wearing long sleeves and pants to limit skin exposure. Additionally, after spending time outside, Dr. Taber advised doing a tick check and checking every family member over (including pets). If a tick is found, Dr. Taber cited there is no reason to panic, as ticks must be attached for up to 3 days prior to transmitting bacteria that can cause Lyme disease. She noted daily tick checks and prompt removal might be enough to prevent infection.</p>

<p>Dr. Taber concluded Lyme disease is treatable with antibiotics in all three of its phases. The type of antibiotic and duration of treatment is chosen based on a number of factors, including the age of the patient and the symptoms. As a nation of over 300 million people, none of these diseases are exactly common: Juvenile idiopathic arthritis is the most common and estimated to affect about 300,00 children in the U.S.," said Dr. Onel at the Rheumatology Nurses Society Annual Conference.</p>

<p>She warned that children treated by rheumatologists who do not specialize in pediatrics may not receive the same level of appropriate care as those treated by pediatric rheumatologists.</p>

<p>Additionally, Dr. Onel noted that by staying current with recent management guidelines and increasing exposure to pediatric rheumatology in fellowship programs, physicians could help reduce treatment disparity and improve the care of children with rheumatic diseases.</p>

<p>"Whether anti-TNF treatment can be stopped in children with JIA is the really big question we'd like to answer in the next 10 years," said Dr. Onel at the Rheumatology Nurses Society annual meeting.</p>

<p>According to the article, sixty percent of the patients involved in the study experienced a flare after their anti-TNF therapy stopped.</p>

<p>"Currently, I view stopping anti-TNF therapy more like a long medicine holiday, and advise patients that there's a chance they will have to go back on the treatment. But if you're talking about children and injections, sometimes they do need a breather," added Dr. Onel.</p>

                        <guid>https://news.hss.edu/finding-hope-for-the-smallest-patients/</guid><pp:caseid>321774</pp:caseid><description><![CDATA[<p>In a collaborative effort with Weill Cornell Medicine, HSS rheumatologist <a href="https://www.hss.edu/physicians_onel-karen.asp">Karen Brandt Onel, MD</a>, identified an effective treatment for children with systemic onset juvenile arthritis.</p>

<p>"When I first started, these were the hardest children to treat. They really didn't respond to any medications… They suffered from tremendous side effects including gaining of weight, changing in their face, they would be severely immunosuppressed and specifically they got a lot of arthritis of their hips and many of them needed to have hip replacements," Dr. Onel explained.</p>

<p>According to the video, Weill Cornell Medicine and HSS developed a pilot clinical trial with a drug that was originally created to treat adult rheumatoid arthritis. While it didn't work for adults, they found that the medication helped pediatric patients.</p>

<p>"You have children in the hospital who are desperately ill with daily fevers, and then you give them the medication and it just stops. Instantaneously. It is one of the most remarkable things I have ever seen," Dr. Onel noted.</p>

<p>Eva Chi, a two-year-old HSS patient, felt her symptoms improve dramatically after receiving the drug therapy.</p>
