DT helps to eliminate trial-and-error because physicians can monitor a patient’s “twin” in real time, then experiment with treatment strategies without traditional safety risks.</span></p><p><span style="text-align:start;">At HSS, the multidisciplinary team working on the DT project consists of surgeons, radiologists, biomechanical engineers, and artificial intelligence and data specialists. Using the DT tool, they’ll focus initially on the knee. 3D imaging will be used to create a patient’s anatomic replica and then treat musculoskeletal injuries. Surgeons can then run simulations driven by data in real time to plan surgery, make decisions intraoperatively, and predict how well a patient will do, which in turn will allow patients to better understand their conditions.&nbsp;</span></p><p><span style="text-align:start;">“In our pipeline, we take an MRI of the knee and autosegment it to create a 3D model,” said&nbsp;</span>Dr. Pearle.<span style="text-align:start;"> “You can integrate this 3D MRI data with a physics-based computational model, essentially adding virtual ligaments to the knee to simulate how it will behave in the real world. When an athlete goes to cut or to pivot, they plant and their foot goes one way and their knee goes the other. That puts the ACL at risk.&nbsp;</span></span></p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">Dr. Pearle continued. “Classically people feel a pop. They can feel it or hear it. People usually go down immediately afterwards. If they try and get up, the knee feels loose or unstable. &nbsp;Usually people can't continue playing. &nbsp;That day or that night, the knee will often swell. If you have a swollen knee, &nbsp;that usually means you've damaged the internal structure of the knee. If you can't bear weight, it's certainly time to see a doctor. ”</span></span></p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">It's not just professional athletes that are at risk. “A lot of the spotlight has been on elite athletes, but the population that's most vulnerable is high school athletes, particularly girls. They have a 2 to 8 times higher risk of ACL injury than boys. &nbsp;If you tear your ACL in high school, there's about a 50 to 70 percent chance that you're going to get arthritis in your thirties,” said Dr. Pearle.&nbsp;</span></span></p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">“We have solutions for this. Neuromuscular training can help. Pearle, MD</span></span></p>]]></description><content:encoded><![CDATA[<p>CNN reports on <span style="background-color:rgb(255,255,255);"><span style="text-align:start;">why female soccer players suffer more ACL injuries than male counterparts and what can be done about it.</span></span></p><p style="margin-left:0px;text-align:start;">After<span>&nbsp;</span>seeing the headlines<span>&nbsp;</span>of how many of the world’s top players are missing out on this year’s Women’s World Cup — including two from the US team — it’s hard not to&nbsp;think of this injury as an inevitable part of the sport, especially as a woman.</p><p style="margin-left:0px;text-align:start;">Different studies have found female athletes are two to eight times more likely than their male counterparts to tear their ACLs.</p><p style="margin-left:0px;text-align:start;">“This issue has been around for a long time, it’s gotten worse and worse,” said <a href="https://www.hss.edu/physicians_pearle-andrew.asp" target="_blank"><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">Andrew D. Pearle, MD</span></span>, </a>chief of sports medicine at HSS.&nbsp;</p><p style="margin-left:0px;text-align:start;">Over a 10-year period, Dr. Pearle says he saw a<span>&nbsp;</span>300%&nbsp;increase in ACL injuries in adolescents.</p><p style="margin-left:0px;text-align:start;">So while the injuries among top professional players have brought this issue into the spotlight, Dr. Pearle said the highest incidences are still in high school athletes —&nbsp;and he is seeing increasing numbers in middle school-age athletes.</p><p style="margin-left:0px;text-align:start;">Dr. Pearle said this is why parents need to be aware that these injury prevention programs exist.</p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">“If you got a girl’s soccer team, you’d say to the coach or the sports organization, OK, what’s your neuromuscular training plan? Since 2021, at least 87 players from eight of the world’s top women’s soccer leagues have torn their ACLs. Some of the sport’s biggest stars will miss the World Cup because of this injury.</span></p><p style="margin-left:0in;"><span>In women, the intercondylar notch, the area in the femur where the ACL sits, is narrower; additionally, women’s pelvises are wider, creating a higher angle between the hips and knees that can put greater stress on the quadriceps and the ACL. Female athletes tend to land flat-footed with knees inward on jumps and have more strength in the quads rather than the hamstrings. Those circumstances put female athletes at risk before even taking the field. In movement-heavy sports, a sudden change of direction or an improper landing could spell a tear.&nbsp;</span></p><p>“The quintessential ACL-busting cohort is in female soccer players,” said Dr. Pearle. “The numbers are really kind of tragic.”</p><p>Across soccer, pushes have been made to make injury prevention programs accessible. In May, The Aspen Institute Sports & Society Program and &nbsp;HSS </span></span>created<span style="background-color:rgb(255,255,255);"><span style="text-align:start;">&nbsp;the National ACL Injury Coalition, a group of experts and leaders that will work together through 2026 to identify and implement practices, partnerships, and resources that can</span></span><span>&nbsp;</span>reduce injuries<span style="background-color:rgb(255,255,255);"><span style="text-align:start;">&nbsp;in the high school population.</span></span></p><p style="margin-left:auto;text-align:start;">Step one: Get coaches to build better injury-prevention exercises into team activities. That includes neuro-muscular training, which develops muscle memory that optimizes athletic movement and leads to more control, agility, and strength during sudden changes of direction. Such exercises include squats and side lunges and can<span>&nbsp;</span>eliminate<span>&nbsp;</span>50% to 80% of non-contact ACL injuries. As a side benefit, they improve athletic performance and reduce the risk of other injuries like ankle sprains and even concussions.</p><p style="margin-left:auto;text-align:start;">Team warm-ups should include the FIFA-developed protocol<span>&nbsp;</span>11+. Or coaches could ask players to do these exercises at home and track their participation through an app called <a href="https://www.riipreps.com/" target="_blank">RIIP Reps.</a>&nbsp;</p><p style="margin-left:auto;text-align:start;">“It’s there, it’s free,” said Dr. Pearle. “It’s seven minutes a day, four days a week. ACL injuries are as much as six times more common among female high school soccer players than among male players. A girl who plays soccer year-round has a 16% chance of injuring her ACL during her high school years, explained Dr. Pearle.</span></p><p><span>The good news for amateur players is that half or more ACL injuries are preventable with regularly performed exercises called neuromuscular training, Dr. Pearle said. HSS and the Aspen Institute recently formed the&nbsp;National ACL Injury Coalition&nbsp;to encourage their adoption.</span></p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">In general, several factors are thought to put female athletes at higher risk of ACL injuries, &nbsp;explained Dr. Pearle. Female athletes tend to move their knees inward when landing from a jump rather than keeping them in line with their toes. They’re more knee-dominant in their change-of-direction movements rather than engaging the muscles around their knees and hips. Their joints are generally looser than those of male athletes. Anterior cruciate ligament (ACL) injuries, in particular, bring physical, mental and financial impacts that can last a lifetime. Research suggests that prevention programs can reduce the risk of ACL injury among young athletes. However, widespread implementation of prevention programming in high school sports remains limited. To address this complex challenge, Hospital for Special Surgery (HSS) and the Aspen Institute have partnered to form the first national coalition to reduce ACL injuries. The National ACL Injury Coalition – which held its first in-person planning session in late March – envisions a sports ecosystem in which high school participants learn to move better and face fewer threats to serious injury, so they can remain active for life. The coalition understands that the risk of non-contact ACL injury can be dramatically reduced through the adoption of exercises that improve athletic movement and allow athletes to change speed and direction with greater strength and body control. Research now demonstrates that this neuromuscular training can help protect athletes against many types of sports-related injuries, including ACL tears. Convening leading medical, sport, insurance, public health, and educational experts will create the type of coalition we need to develop a bold plan for action and achieve systems-level progress.”&nbsp;</p><p>High rates of ACL injuries among high school athletes can have an adverse effect on sports participation and performance in the near term as well as on overall health and physical activity across the lifespan. Those who suffer ACL injuries are seven times more likely to develop end-stage osteoarthritis that requires knee replacement. The median ACL reconstruction procedure costs $9,400 and total lifetime costs exceed $38,000, with the costs even higher ($88,000) for those who receive non-operative treatment.&nbsp;</p><p>However, not all athletes are at equal risk. Research shows female athletes are 1.5 times more likely to suffer an ACL injury than males. Adolescents playing team sports that require sudden change of direction (cutting, landing, stopping), like soccer, basketball, lacrosse, field hockey, ultimate frisbee, and flag and tackle football, face substantially heightened risk of ACL injuries. Meanwhile, public schools in low-income communities have fewer resources to implement ACL injury prevention programs. Youth from lower-income families are also less likely to have health care insurance as well as access to adequate medical care after an injury.&nbsp;</p><p>A flood of recent ACL injuries in professional women’s soccer highlights the urgency of this public health crisis. Internationally, 57 ACL injuries occurred among the six major women’s professional leagues in 2022. Within the U.S. national team that defends its title in the FIFA Women’s World Cup starting in July, Catarina Macario, Christen Press and Tierna Davidson have suffered an ACL injury in the past six months, while Alex Morgan, Megan Rapinoe, Ashlyn Harris and Ali Kriefer tore the ligament in prior years.&nbsp;</p><p>“If we’re going to build healthy people and communities, we need to reckon with the underappreciated problem of athlete knee injuries that could be prevented,” said Tom Farrey, executive director of the Aspen Institute’s Sports & Society Program. “The problem needs the same attention that has gone to reducing brain injuries, given the importance of healthy knees to moving well and the physical, mental and emotional benefits that flow to those whose bodies stay in motion throughout adulthood.”&nbsp;</p><p>The Aspen Institute is a global nonprofit organization committed to realizing a free, just, and equitable society. Pearle, MD</p>]]></description><content:encoded><![CDATA[<p>HealthDay reports on preliminary research presented at the American Orthopaedic Society for Sports Medicine (AOSSM) 2022 annual meeting comparing repair and reconstruction procedures of anterior cruciate ligament (ACL) tears and includes commentary from <a href="https://www.hss.edu/physicians_pearle-andrew.asp">Andrew D. Pearle, MD</a>, chief of sports medicine at HSS, who was not involved in the study.&nbsp;<br><br>Dr. Pearle said ACL repair is “an important tool to have in our armamentarium,” but advised that the risk of re-injury in young patients should be taken seriously.<br><br>According to Dr. Pearle, “For young athletes, repair has what I consider an unacceptable revision rate.”<br><br>A repeat rupture, he noted, can damage other tissue in the knee — including cartilage cushioning the joint.<br><br>Dr. Pearle added that for adults who are not heavily into “cutting” sports, like soccer and basketball, ACL repair can be an option.<br><br>Additionally, another choice for the relatively older, non-athletic crowd: Physical therapy, without surgery.<br><br>Dr. Pearle explained that some people are “copers.” That is, their knee joint is stable enough for their usual activities, even without an ACL. Other, not-so-lucky people are “non-copers,” and without an ACL they end up with a “trick knee” that routinely buckles. (Unfortunately, Pearle noted, there's no way to predict which patients will be copers.)<br><br>One question the new findings do not address, Dr. Pearle said, is how ACL repair compares against physical therapy alone for relatively older patients. HSS will also provide priority access to a new injury prevention training app for coaches. Any high school in the nation can apply to join the program, as long as they do not have a full-time athletic trainer.</span></p><p><span>“High school sports have long been an important community pillar as they provide children with important social skills and health benefits,” said <strong>Joseph Janosky, DrPHc, MSc, PT, AT, </strong>Director, Injury Prevention Programs, HSS. “While injuries are common, the good news is that proper training can change this. Here at HSS, we have developed evidence-based injury prevention programs so kids can reap the benefits of sports with a reduced risk of getting hurt.”</span></p><p><span>Approximately 7 million students play high school sports with over 2 million injuries each year. Nearly 30,000 of those injuries result in hospital stays. The CDC believes that up to half of these injuries could be prevented with proper training and resources.</span></p><p><span>When schools have access to a full-time athletic trainer, their students see lower overall and recurring injury rates. Unfortunately, approximately two-thirds of U.S. high schools lack access.</span></p><p><span>Kids who endure sports injuries are more likely to miss school, suffer from mental health issues such as depression, and develop chronic orthopedic issues like arthritis that could increase risk of opioid dependency.</span></p><p><span>“We understand the devastating effects that injuries can have on our youth and we believe that we can help kids stay safe playing the sport of their choice by providing our resources and expertise directly to their schools,” said Janosky.</span></p><p><span>“Move Better Play Better” lives within the HSS Sports Medicine Institute’s </span><a href="https://www.hss.edu/injury-prevention-programs.htm"><span>Youth Sports Safety Program</span></a><span>, which brings together sports medicine doctors, physical therapists and other experts to develop injury prevention programs aimed to keep youth athletes safe. Goolsby, MD</a>, and sports medicine surgeon <a href="https://www.hss.edu/physicians_shubin-stein-beth.asp">Beth E. Shubin Stein, MD</a>, are the newly designated co-directors of the <a href="https://www.hss.edu/womens-sports.htm">HSS Women’s Sports Medicine Center</a> (WSMC).</p><p>&nbsp;The WSMC was a first of its kind focused on women and sports medicine and was co-founded in 1997 by primary sports medicine physician <a href="https://www.hss.edu/physicians_callahan-lisa.asp">Lisa R. Callahan, MD</a>, and sports medicine surgeon <a href="https://www.hss.edu/physicians_hannafin-jo.asp">Jo A. Hannafin, MD, PhD</a>, who served as co-directors ever since. The multidisciplinary center now consists of a team of sports medicine-trained clinicians including physicians, nutritionists, exercise physiologists, physical therapists and sports psychologists.</p><p>&nbsp;The change in leadership comes as Dr. Hannafin has recently retired her clinical practice while continuing to be active in the WSMC and as vice chair of the Institutional Review Board at HSS, which is responsible for the evaluation of current and newly submitted research projects. As a practicing physician, Dr. Callahan will also continue to be actively involved in the WSMC while serving in her role as chief medical officer of Madison Square Garden Sports.</p><p>&nbsp;“The WSMC is a key reason why I feel excited to go to work each day. I am constantly inspired by my colleagues and the patient care that they provide, which motivates me to take the very best care of my own patients,” said Dr. Goolsby.</p><p>&nbsp;Approximately 80,000 recreational and elite athletic patients a year receive collaborative care through the WSMC. Every clinician aims to empower each patient to achieve success based on his or her individual goals.</p><p>&nbsp;“Being part of the WSMC has helped me in all facets of life. It’s been invaluable for my career, provided spectacular comradery, and guided me to find a work/life balance,” said Dr. Shubin Stein, who noted how Drs. Callahan’s and Hannafin’s successes have opened so many doors for all women. “They have helped us all to dream big.”<span>&nbsp;</span></p><p>&nbsp;Drs. Callahan and Hannafin have led distinguished careers and have served as team physicians for numerous professional and Olympic teams. Notably, Dr. Callahan was the first female head team physician in the National Basketball Association and president of the NBA Physicians Association. Dr. Hannafin was the first female president of the American Orthopaedic Society for Sports Medicine (AOSSM).<br><br>“We want to ensure we carry on the legacy and support all of the work that Drs. Callahan and Hannafin have done as true pioneers in our field,” added Dr. Goolsby, who explained that she and Dr. Shubin Stein have spent time individually with all members to determine new, impactful goals for the group.</p><p>&nbsp;WSMC clinicians have provided care for numerous Olympic and professional teams and organizations including the New York Knicks, Westchester Knicks, New York Liberty, TCS New York City Marathon, Champions Series Tennis, USA Basketball, USA Gymnastics, UFC, US Biathlon, US Rowing and US Lacrosse.</p><p>&nbsp;“First and foremost, our priority is patient care. ]]></description><content:encoded><![CDATA[<p>WABC-TV reports Lori Vollkommer (53) has returned to the world of competitive gymnastics after recovering from surgery performed by HSS orthopedic trauma surgeon <a href="https://www.hss.edu/physicians_difelice-gregory.asp">Gregory S. DiFelice, MD</a>, to repair her ruptured anterior cruciate ligament (ACL) in 2017. Five months after her surgery, Lori was back on the mat training.</p>

<p>Dr. DiFelice credits Vollkommer&rsquo;s quick recovery to the surgery in which he reattached her ACL vs. reconstructing it. However, he noted Vollkommer was a good candidate for the surgery based on the particulars of her injury and age. The surgery is not recommended for people under 30 years of age, particularly teenagers, said Dr. DiFelice. &ldquo;It doesn&rsquo;t work for everybody. You have to have a specific type of tear,&rdquo; he said.</p>

<p><a href="https://www.hss.edu/physicians_pearle-andrew.asp">Andrew D. Pearle, MD</a>, chief of the HSS Sports Medicine Institute added, "HSS pioneered the ACL repair technique over 40 years ago, and we are still exploring the most appropriate indications for its use. It may be appropriate for treating extremely rare types of knee injuries among older patients."</p>

<p>In 2018, Vollkommer won five gold medals at the International Association of Independent Gymnastics Clubs World Championships in Orlando, and she continues to train three days a week.</p>

<p>Reconstruction is the current standard of care surgical treatment for tears of the anterior cruciate ligament (ACL). This procedure typically uses a graft, or a piece of tissue, placed in the knee minimally invasively, using small incisions. About 300,000 ACL reconstructions are performed annually in the United States with well documented rates of success.</p>

<p>ACL repair is an older technique that involved suturing (sewing) torn ACL tissue, not rebuilding it with a graft. ACL repair was performed in the 1970s at select institutions, including Hospital for Special Surgery, but was abandoned due to unacceptably high failure rates of up to 50%.</p>

<p>Today, ACL repair has been modernized and can be performed through a minimally invasive approach. Proponents suggest that modern ACL repair techniques may be performed safely and may lead to a quicker recovery than ACL reconstruction. However, the limited studies to date raise questions that must be discussed in detail with your surgeon.</p>

<h2><strong>1. It is critically important to get ACL surgery right the first time.</strong></h2>

<p>When ACL surgery fails, surgeons must do a revision procedure to correct or revise the problems that were not fully addressed the first time. Revision requires reconstructive ACL surgery with a graft. (ACL repairs cannot be done as revisions.) Redoing ACL surgery may result in higher rates of failure, lower rates of return to sports, and increased risk of osteoarthritis.</p>

<p>The importance of successful initial ACL surgery holds true for people of all ages, but it is particularly critical for young athletes, for whom a failed surgery can have devastating consequences, including years lost from sport in the short term as well as chronic pain and loss of function in the long term.</p>

<h2><strong>2. Failure rates for ACL repair appear to be significantly higher than those for ACL reconstruction in people of all ages.</strong></h2>

<p>Current studies suggest that modern ACL repair techniques have a failure rate that may be five to ten times higher than that of ACL reconstruction<sup>1,2,3</sup>. One major recent study in a prominent sports medicine journal found that arthroscopic ACL repair has unacceptable outcomes in young athletes, with short-term failure rates of close to 50%<sup>2</sup>.</p>

<p>It has been suggested that ACL repair may be appropriate for older adults who have a type of ACL tear called a proximal avulsion and who do not participate in &ldquo;cutting and pivoting&rdquo; sports such as basketball, volleyball, and soccer. For these patients, failure rates in the short term appear to be much higher than ACL reconstruction in this age group<sup>1,2,3</sup>.</p>

<h2><strong>3. There is still much we don&rsquo;t know about ACL repair.</strong></h2>

<p>There has been no scientific evidence that shows a difference in postoperative pain between ACL repair and reconstruction. Rehabilitation and return-to-play protocols for ACL repair have not yet been established. Proper guidelines for rehabilitation and return to sports are essential to keep people safe from re-injury.<br />

<p>Dr. Pearle started at HSS as a surgical intern in 1998, and began practicing at the Hospital in 2005. He currently sees patients at the main HSS campus in New York City, and at HSS Westchester in White Plains, NY, where he holds the position of medical director. He developed one of the first computer-assisted surgery programs in the US and performed the first robotic partial knee replacement in New York in 2007.</p>

In addition to his responsibilities at HSS, Dr. Pearle is Head Team Orthopedic Surgeon for Rugby United New York, an Associate Team Physician for the New York Mets, and helps coordinate care for the Mets' minor league affiliates including the Brooklyn Cyclones.</p>
