Reif, MD</a>, orthopedic surgeon at HSS shared, "<span style="text-align:left;">Within the last 5 to 10 years there has been a lot more industry that has been capable of quickly turning around custom titanium implants with a textured surface that can achieve osseointegration and be made whatever shape we need it to be made. Seeing this trend happening within orthopedics, we applied it to osseointegration. Osseointegration is a surgical procedure that enables a prosthetic limb to be attached directly to the bone of someone who has had an amputation, eliminating the need for a traditional socket prosthesis.</p><p>The studies, which looked at safety and outcomes in both femur-level (above-the-knee) and tibia-level (below-the-knee) OI, were presented at the 2026 Annual Meeting of the American Academy of Orthopaedic Surgeons (AAOS) in New Orleans this week.</p><p>“Osseointegration represents a significant advancement in the field of prosthetics for patients who have had a limb amputation, and interest is building rapidly,” said <a href="https://www.hss.edu/profiles/doctors/jason-hoellwarth">Jason Hoellwarth, MD</a>, director of research in the Limb Lengthening and Complex Reconstruction Service and the Osseointegration Limb Replacement Center. “At HSS, we have performed more OI surgeries than any other hospital in the United States, treating more than 300 patients since 2017. We have found that osseointegration provides improved mobility, comfort and quality of life for patients when compared to a standard socket prosthesis.”&nbsp;<span> &nbsp;&nbsp;</span></p><p><strong>Study 1: Femur and Tibia Press-Fit Osseointegration - A Comparison of Safety and Outcomes</strong></p><p style="margin-left:0in;">“There are no studies in the scientific literature focusing on differences in safety and outcomes of patients who underwent transfemoral osseointegration as compared to those who had transtibial osseointegration,” said <a href="https://www.hss.edu/profiles/doctors/taylor-reif">Taylor J. Reif, MD</a>, an orthopedic surgeon in the Osseointegration Limb Replacement Center at HSS who presented the research. Dr. Reif and colleagues set out to compare safety, clinical, and quality-of-life outcomes in these two patient groups.</p><p>Dr. Hoellwarth, co-author of the study, noted that osseointegration was originally considered primarily for patients with femur-level amputations, since they fared much worse with a socket prosthesis than individuals with tibia-level amputations. “It was widely believed that patients with a below-the-knee amputation did well enough with their socket prosthesis and therefore would not derive significant benefit from osseointegration. Our study directly challenged that perspective.”</p><p>Researchers reviewed 147 OI procedures: 83 femur-level and 64 tibia-level. Both groups experienced substantial improvements in mobility and overall patient satisfaction, and there were no significant differences in the rate of adverse events.</p><p>“Contrary to widespread assumptions, our research found that tibia osseointegration patients have as much to gain from the surgery as femur OI patients,” Dr. Reif noted. “Therefore, any individual with a lower-limb amputation who is dissatisfied with their current socket prosthesis should be evaluated for osseointegration if they would like to pursue that option.”</p><p><strong>Study 2:</strong> <span><strong>Primary Amputation with Osseointegration Versus Osseointegration for Existing Amputation</strong></span></p><p>HSS researchers set out to compare safety and outcomes in patients who had osseointegration performed at the same time they had an amputation versus the more commonly performed osseointegration in individuals with an existing amputation.</p><p>The study included patients who had lower-extremity osseointegration with a minimum of one-year follow-up at HSS. There were 139 OI procedures: 81 femur-level and 58 tibia-level. Fifteen patients had simultaneous amputation and OI (4 femoral and 11 tibial), while 124 patients had OI for an established amputation (77 femoral and 47 tibial).<span>&nbsp;</span></p><p>The researchers found that both groups of patients—those undergoing osseointegration at the time of their amputation and those who had osseointegration with an existing amputation—achieved comparable gains in mobility and quality of life. There was no significant difference in adverse events between the two groups.</p><p>“Our research challenges the current paradigm that individuals with amputations should first try conventional socket prosthetics and only consider osseointegration if they are dissatisfied with their prosthesis,” said Dr. Reif, who presented the study. “It would be reasonable to offer simultaneous amputation and osseointegration to select, well-informed patients who prefer to bypass the trial of a socket prosthesis.”</p><p><span><strong>Study 3: Safety and Early Experience of Osseointegration Limb Replacement with Custom-Fit Implants</strong></span></p><p><span>HSS researchers set out to review the early outcomes of osseointegration with custom-made 3D-printed implants, which may be required for certain patients to obtain an ideal fit.</span></p><p><span>“</span>Some designs of press-fit osseointegration implants have standard curvatures and contours with patient-specific length, leading to difficulty fitting the shape of the residual bone in some cases,” Dr. Hoellwarth explained.</p><p><span>Dr. Reif, who presented the study at the AAOS meeting, noted that additive manufacturing using electron beam melting (EBM) allows the production of precision osseointegration implants to achieve an optimal fit, making it possible </span>to reconstruct nearly any residual bone segment to meet an individual’s anatomical needs.</p><p><span>The HSS researchers retrospectively reviewed the records of 19 patients at HSS who underwent osseointegration with custom 3D-printed implants planned from a preoperative CT scan. Patients included those who had above-the-knee, below-the-knee and above-the-elbow osseointegration procedures between May 2024 and March 2025.</span></p><p><span>The investigators found large, statistically significant improvements in patient-reported outcome scores following OI, and a significant increase in prosthetic use was observed in those who had used a socket prosthesis before OI surgery. &nbsp;</span></p><p><span>Dr. Reif noted that the custom EBM osseointegration implants avoided intraoperative distal chip fracture, which can sometimes occur when using standard OI implants, and there was no loosening of the custom implants. “We found that short-term functional outcomes were similar to standard osseointegration implants,” he added. “Further studies are warranted to assess the long-term outcomes and durability of custom implants.”</span></p><p><span><strong>Study 1 - </strong>Femur And Tibia Press-Fit Osseointegration - A Comparison of Safety and Outcomes (Zachary Glassband BA, Taylor J. He chose to have the surgery after learning that osseointegration enables a prosthetic limb to be attached directly to the remaining bone of someone with an amputation. The bone grows into a metal implant, creating a strong and stable connection.</p><p style="text-align:justify;">The procedure provides improved mobility, comfort, and quality of life compared to a standard socket prosthesis, according to&nbsp;<a href="https://www.hss.edu/physicians_hoellwarth-jason.asp">Jason Hoellwarth, MD,</a>&nbsp;a highly specialized orthopedic surgeon at Hospital for Special Surgery (HSS) in NYC and at HSS Long Island.&nbsp;&nbsp;&nbsp;</p><p style="text-align:justify;">Nick, who is 45 and lives in Saint James, Long Island, credits Dr. Hoellwarth and the team at the<span>&nbsp;</span><a href="https://www.hss.edu/osseointegration-limb-replacement.asp" target="_blank">HSS Osseointegration Limb Replacement Center</a><span>&nbsp;</span>with giving him his life back. A congenital condition that caused a leg bone deformity and a severe ankle injury when he was 11 years old led to two major reconstructive surgeries with lengthy recoveries. As time went by, he started to experience pain that got progressively worse, hindering his ability to walk, work out, and do his job. By age 40, he says it was "hard to express the level of pain." A motorcycle accident when he was 43 was the final blow that “annihilated” his left ankle.</p><p style="text-align:justify;">By the time Nick found out about osseointegration, he says the pain had become almost unbearable,&nbsp;and he was in a “very dark place.” He had seen several specialists who ordered x-rays, CT scans and MRIs. “The doctors all said my condition would require extensive and complex reconstructive surgery with a recovery of up to two years. I didn’t want to go down that path again, and the long-term outcome could not be guaranteed,” Nick explains.&nbsp;</p><p style="text-align:justify;">In his online research, Nick found HSS, the most experienced hospital in the United States for osseointegration to treat people with a transtibial (below the knee) amputation, which is what he would have. After an initial meeting with a highly experienced foot and ankle specialist at HSS who also explained that multiple surgeries and lengthy recoveries would be required, Nick scheduled a second consultation – this time with Dr. Hoellwarth – to see if osseointegration would be right for him.&nbsp;</p><p style="text-align:justify;">“I informed Nick that a single surgery of amputation with osseointegration would provide a rapid return to all-day pain-free mobility. So Nick went forward, had the surgery, and never looked back,” Dr. Hoellwarth says. &nbsp;</p><p style="text-align:justify;">After his recovery and a short period of physical therapy, Nick says he regained the life he had longed for − pain-free and walking without a limp for the first time in 30 years. My growth has been exponential, and I imagine that’s the kind of explosive growth the country will see over the next 10 years.”</span></span></p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">When Dr. Rozbruch got started as one of the first OI surgeons in the country, the operation was performed almost exclusively on above-knee amputees at the younger, fitter end of the spectrum. But successful OI outcomes are now becoming commonplace in patients who fall outside that fairly narrow profile—including patients who are over 70 years old, who have diabetes, who have below-knee amputations, or who face other adverse indications for OI.</span></span></p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">Dr. Rozbruch described himself as “the kind of surgeon who is willing to go on the journey” with a patient—meaning he will operate on someone with a less-than-perfect risk profile, as long as the patient understands the odds and accepts their share of responsibility for optimizing outcomes. The institution is collaborating with medical centers and other organizations to advance the quality and value of musculoskeletal care and to make world-class HSS care more widely accessible nationally and internationally. </span><a href="http://www.hss.edu"><span>www.hss.edu</span></a><span>.</span></p>]]></pp:boilerplate><description><![CDATA[<p><span><span><span>Leg amputees who receive bone-anchored prosthetics report high levels of satisfaction compared with those who receive conventional devices, according to new study findings published in <a href="https://journals.lww.com/jbjsoa/Fulltext/2021/09000/Early_Experience_with_Femoral_and_Tibial.18.aspx" style="text-decoration:underline"><em>JBJS Open Access</em></a> by researchers at Hospital for Special Surgery (HSS) in New York City.</span></span></span></p><p><span><span><span>Osseointegration prostheses are a relatively new form of prosthetic technology. The approach involves surgically installing a titanium rod into a patient&rsquo;s remaining bone&mdash;such as the tibia or femur in the leg&mdash;which in turn attaches to a prosthesis that allows ambulation. The technology avoids the need for traditional sockets against which limb stumps rest, an interface that produces friction that can generate significant pain and discomfort for amputees and frequently requires repair or revision.</span></span></span></p><p><span><span><span>Although osseointegration devices are available to patients in Europe and elsewhere, the U.S. Food and Drug Administration has yet to approve the technology. But the agency allows a limited number of patients to receive the implants through humanitarian exceptions.</span></span></span></p><p><span><span><span><span>&ldquo;This is a disruptive technology to improve the life quality of amputees,&rdquo; said study co-author</span> <a href="https://www.hss.edu/physicians_rozbruch-s.asp" style="text-decoration:underline">S. Robert Rozbruch, MD</a>, Chief of the <a href="http://www.hss.edu/limblengthening" style="text-decoration:underline">Limb Lengthening and&nbsp;Complex Reconstruction&nbsp;Service</a> and Director of the <a href="http://www.hss.edu/LSARC" style="text-decoration:underline">Limb Salvage and Amputation Reconstruction Center</a> at HSS. &ldquo;<span>With a direct skeletal connection of the prosthetic limb, amputees report big improvements in function, comfort, balance, proprioception and even emotional connection with their limb.&rdquo;</span></span></span></span></p><p><span><span><span>For the new study, <a href="https://www.hss.edu/physicians_reif-taylor.asp" style="text-decoration:underline">Taylor J. Reif, MD</a>, an orthopedic surgeon and a member of the Limb Lengthening and Complex Reconstruction Service, Dr. Rozbruch and their colleagues reviewed the medical records of 31 men and women who underwent osseointegration procedures at HSS starting in October 2017 under compassionate use exceptions. Of those, 18 had implants in the femur, or thigh bone, and 13 had implants in the tibia, the bone below the knee.</span></span></span></p><p><span><span><span>The primary outcome of the study assessed changes in the Questionnaire for Persons with a Transfemoral Amputation (QTFA), which patients complete before and after surgery. The researchers used the same questionnaire for patients with tibial amputations. They also gathered data on a range of quality-of-life measures including patient-reported functional activity, mental health, pain and self-image, as well as adverse events such as serious infections, fractures at the site of the implant and other complications of the procedure.</span></span></span></p><p><span><span><span>Researchers found that scores across all domains of the QTFA improved significantly throughout the study period. Prosthetic mobility rose from 49.7 to 81.4, while prosthetic problems fell from 46.4 to 9.1. Patients also experienced substantial gains in function with the implants, as measured by 2- and 6-minute walking tests.</span></span></span></p><p><span><span><span>Dr. Reif said the benefits of the devices extend beyond the raw numbers. <span>&ldquo;The scores going up is great, but that doesn&rsquo;t necessarily capture what we&rsquo;re seeing in the clinic, which is smiles on people&rsquo;s faces and the expressions of gratitude for how much their lives have changed. It&rsquo;s totally life-altering.&rdquo;</span></span></span></span></p><p><span><span><span>Although some patients did experience early complications, nearly all (93%) were managed without removing the implant. Encouragingly, while some patients developed minor infections where the titanium post exits the skin (an opening called a stoma), most of these resolved with the use of oral antibiotics. Only one patient had an infection in the bone itself leading to removal.</span></span></span></p><p><span><span><span><span>The bond between the titanium implant and the bone is so strong that patients are able to rapidly resume physical activity. In some cases, patients can return to vigorous, weight-bearing exercise such as running within six months to one year after the procedure.</span></span></span></span></p><p><span><span><span>As the data showed, patients who seek osseointegration prostheses tend to be young, and most had sustained catastrophic injuries to their legs in vehicular accidents or other traumatic events. &ldquo;The older population of patients have become accustomed to their sockets or using a wheelchair,&rdquo; explained Dr. Reif. &ldquo;But as this technology expands and more surgeons start to employ it, I think the age that these are implanted also will expand in both directions.&rdquo;</span></span></span></p><p><span><span><span>He added, &ldquo;<span>No implant has a perfect track record, but when you combine the functional gains that these patients are experiencing with improvements in their quality of life, a small risk of infection or fracture is acceptable to them, as well as to the orthopedic community.&rdquo;</span></span></span></span></p>]]></description><category><![CDATA[pressrelease,Rozbruch,Reif,Institute for Limb Lengthening and Complex Reconstruction,Research Clinical,osseointegration]]></category>

]]></description><content:encoded><![CDATA[<p>PIX11 reports a former science teacher will be one of the first in the nation to receive a myoelectric arm that she will control with her mind.</p><p>A year and a half ago, amputee Sondra Stieber met with an HSS physician and learned she was a candidate for the prosthesis. <a href="https://www.hss.edu/physicians_rozbruch-s.asp">S. Robert Rozbruch, MD</a>, chief of Limb Lengthening and Complex Reconstruction Service at HSS, started performing the <a href="https://www.hss.edu/condition-list_osseointegration.asp">osseointegration</a> procedure two years ago, mostly on leg amputees, as hands are more intricate. Stieber is one of just a few patients to receive a myoelectric arm following osseointegration surgery.</p><p>&ldquo;A very, relatively short prosthetic has to be very sophisticated,&rdquo; noted Dr. Rozbruch. The patient controls the prosthesis just by thinking about it. Stieber was tested by physicians to make sure she could do it. &ldquo;The band that goes around the arm and the level of those muscles it transmits those signals to the myoelectric prosthesis,&rdquo; he explained.</p><p>Stieber and her husband recently spent a week in New York City at HSS to understand how the arm works.</p><p>Watch the segment on <a href="https://pix11.com/video/teacher-making-history-with-bionic-arm/6455123/">Pix11.com</a>. ]]></description><content:encoded><![CDATA[<p><i>The New York Times Magazine </i>features an essay written&nbsp;by U.S. Army Veteran and HSS patient Elana Duffy discussing the injuries she incurred to her right leg and ankle during her Iraq deployment in 2005.</p>

<p>Duffy wrote, &ldquo;What I didn&rsquo;t know at the time was that the injuries I suffered in these two incidents had inflicted such complex and significant damage to my body that they would end my military career and leave me in nearly constant, and sometimes excruciating, pain. My daily life and my personal relationships were overtaken by my injuries and the new ones they repeatedly caused.&rdquo;</p>

<p>Upon her return to the U.S.,&nbsp;Duffy noted she underwent an ankle assessment which indicated several ligament tears and a chip broken away from the bone. For more than a year, Army doctors tried to repair her ankle, however over time the muscles atrophied. By 2019, the damage was so extensive and surgery was unavoidable. Duffy met with <a href="https://www.hss.edu/physicians_rozbruch-s.asp">S. Robert Rozbruch, MD</a>, chief of Limb Lengthening and Complex Reconstruction Service at HSS, who recommended the option of <a href="https://www.hss.edu/condition-list_osseointegration.asp">osseointegration surgery</a>, which would entail amputating her right leg at midcalf. After much&nbsp;consideration, Duffy decided to undergo the surgery.</p>

<p>It&rsquo;s been more than 12 months since Duffy&rsquo;s surgery and she is living pain free, returning to fundamental activities such as climbing the stairs and her interest of climbing.</p>

<p>Prior to his accident Winograd enjoyed fishing, exercising and traveling. After being struck by a box truck in 2018, the left limb below his knee could not be saved, and he underwent osseointegration surgery performed by <a href="https://www.hss.edu/physicians_rozbruch-s.asp">S. Robert Rozbruch, MD</a>, chief of Limb Lengthening and Complex Reconstruction Service at HSS. Since his surgery, Winograd can do things &lsquo;as if it was [his] regular leg.&rsquo;</p>

<p>&ldquo;It feels like I&rsquo;m a kind of a pioneer in my own little area of surgery,&rdquo; he said. &ldquo;I would like to get the word out there and I know there&rsquo;s a lot of other amputees - especially on Staten Island - who probably have never even heard of osseointegration.&rdquo;</p>

<p><span>"It will be a relatively short prosthetic that's going to be very, very sophisticated," explained Dr. Rozbruch. Stieber will be able to control the hand by just thinking about it. "There's a band that goes right around the arm at the level of those muscles, and it transmits the signals to the myoelectric prosthesis," added Dr. Rozbruch.</span></p>

He had no choice but to amputate&nbsp;after an infection. Yet his residual leg was not fitting well with the traditional prosthetic and caused him pain.</p><p>Giaimo found <a href="https://www.hss.edu/physicians_rozbruch-s.asp">S. Robert Rozbruch, MD</a>, Chief of <a href="https://www.hss.edu/limblengthening.asp">Limb Lengthening and Complex Reconstruction Service</a> at HSS, who offered a new solution.</p><p>"My last amputation prior to David, that was the same way amputation was done 100 years ago. This is the first new development in amputation surgery, that's huge,"&nbsp;said Dr. Rozbruch.</p><p>Dr. Rozbruch amputated the infected portion of his leg, implanted a 3D printed titanium rod into his femur and then implanted a post to the femur that attaches to the new prosthetic.</p><p>Dr. Rozbruch is one of four surgeons in the U.S. approved by the FDA to study this approach.</p><p>"It's connected to my bone, so I have a sense of it being attached to me. It's a prosthetic but it's now my leg," said Giaimo.
