                        <title>Making Sense of the Connection Between Vaccines and Lupus Flares</title>

Mandl, MD, MPH</a>.&nbsp;</p><p><span>In 1978, a team of Harvard Medical School researchers examined whether people with lupus responded to the influenza vaccine differently from the way a group of “normal” control subjects did. They looked at vaccine side effects, as well as post-vaccination lupus symptoms and autoantibody activity.&nbsp;</span></p><p><span>“Symptoms after vaccination were somewhat more frequent in the lupus patients than in the normal subjects,” they reported. “However, all symptoms were minor and no major flare of illness occurred. No significant induction or increase of pre-existing autoantibodies among the patients was detected after vaccination.”</span></p><p><span style="text-align:start;">“This is a big concern we hear from patients—whether their rheumatic disease will flare after a vaccine,” said Dr. Barbhaiya. “What the data suggests is that most people do well, and severe flares are very uncommon.”</span></p><p><span>Intuitively, it makes sense that a vaccine—something that is designed to stimulate an immune response—may occasionally trigger a reaction in people who experience overactive or disordered immune activity, such as those with SLE.&nbsp;</span></p><p><span>&nbsp;</span><span style="text-align:start;">People with lupus are at risk for these adverse events just like anyone else. Are they at greater risk? That doesn’t seem to be the case. “There is no substantial evidence that patients with lupus have adverse events beyond what’s reported in the general population,” Dr. Barbhaiya explained.&nbsp;</span></p><p style="margin-left:0px;text-align:start;">In 2021, Dr. Barbhaiya and colleagues, including Dr. Lisa Mandl, conducted a study of COVID-19 vaccination responses among people with SLE. “The COVID pandemic offered this unique opportunity where we could assess vaccine outcomes in real time on a very large scale,” Dr. Mandl said.&nbsp;</p><p style="margin-left:0px;text-align:start;">Their group’s study of 183 patients found that roughly 8% of SLE patients experienced a flare within two weeks of vaccination with one of the three available SARS-CoV-2 vaccines. “These results were very reassuring,” Dr. Mandl said. “Of the few who did report a flare, most were mild or moderate.” In fact, only one patient in the study experienced a severe flare following vaccination. More than 300 patients responded to the survey, and the analysis reported at the meeting included data from 122 respondents with systemic lupus erythematosus (SLE) or rheumatoid arthritis (RA) — 49 with SLE and 73 with RA.&nbsp;<br><br>Overall, 39% reported an association between their menstrual cycle and disease flares, with similar rates in both the SLE and RA groups. Those who experienced these flares usually had them within the week before or during their periods. The investigators say these findings could inform physician counseling and that closer monitoring of patients during vulnerable phases of the menstrual cycle could enable more timely intervention and better disease control. Certain drugs prescribed for rheumatic disease are teratogenic, or can damage a fetus. Data were collected on 249 women with reproductive capacity, including 147 with inflammatory arthritis and 102 with lupus-like disorders.&nbsp;<br><br>The 52 participants taking teratogenic medication were more likely to discuss contraception with their rheumatologist compared with those not taking teratogenic medication (44.2% vs. 21.8%), and more frequently reported using permanent contraception (8.0% vs. 1.1%). Those taking teratogenic medication were no more likely to use effective contraception than those who were not: 44.0% reported using less effective methods such as barrier methods and fertility awareness/withdrawal or using no contraception. Those with lupus were less likely to use more effective methods containing estrogen (31.5% vs. 14.7%), even though these methods are safe for some (but not all) lupus patients. Sammaritano, MD</a>.&nbsp;<br><br><strong>Demographic and Clinical Factors Associated with HPV Vaccination in Young Adults with Rheumatic Disease&nbsp;</strong><br>Although the HPV vaccine is known to be safe and effective in people with rheumatic disease (RD), many do not get vaccinated. This study used data from the electronic health record of patients diagnosed with a rheumatic disease in adolescence, seen at HSS, to look at factors associated with HPV vaccination. Overall, the investigators found that among 321 individuals with rheumatic disease between the ages of 16 and 22, half were not fully vaccinated against HPV (2 or more doses).&nbsp;<br><br>Those who were white and non-Hispanic/Latino and those who had commercial insurance were less likely to be fully vaccinated —factors that are associated with lower vaccination rates in the general population. Those who were unvaccinated were more likely to have inflammatory arthritis versus other connective tissue diseases. Those who were fully vaccinated were also more likely to have received flu and COVID-19 vaccines. Sammaritano, MD</a>.&nbsp;<br><br><strong>Educational Intervention to Increase Contraception Screening and Documentation for Reproductive-Aged Women Seen in an Academic Rheumatology Clinic&nbsp;</strong><br><br>Only one-third of reproductive-aged women with rheumatic disease are prescribed effective contraception, despite the widespread use of teratogenic medications which can harm a fetus. This initiative aimed to increase the rate of provider screening and documentation of contraception use in the electronic health record for female rheumatology patients ages 18 to 45.&nbsp;<br><br>Interventions from the research team included educational presentations and motivating reminders for clinical staff. Over the 24-week study period, the rate of contraception documentation for these patients increased from 11% to 54%. Sammaritano, MD</a>.&nbsp;<br><br><strong>Experiences and Attitudes Related to Assisted Reproductive Technologies Among Women with Systemic Rheumatic Disease&nbsp;</strong><br>Because systemic rheumatic diseases may be associated with decreased fertility in women, those with such diseases may pursue assisted reproductive technologies such as oocyte cryopreservation (OOC, or egg freezing). This study evaluated how sociodemographic factors and medical history influence OOC experiences and overall attitudes toward reproductive technologies among women with these conditions.&nbsp;<br><br>Among the 265 surveyed patients of reproductive age, 21 had done OOC (most before 35 years of age) and 102 had considered OOC. The factors that increased openness to OOC included having no previous pregnancies, having greater infertility concerns, lacking a spouse or partner, having a bachelor’s degree or higher, and having a history of taking certain immunosuppressive drugs. Among those who did not already have fertility issues, 64% reported that, if necessary, they would consider using these technologies only if their rheumatologist advised it was safe. Sammaritano, MD</a>.&nbsp;<br><br><strong>HPV Vaccination in Adolescent and Young Adult Patients Seen in an Academic Rheumatology Center&nbsp;</strong><br>This study looked at rates of HPV vaccination in adolescents and young adults living in New York State who had received care from a rheumatologist. The investigators reviewed data from the electronic health record, including vaccination information from a New York State database, for 534 HSS patients ages 16 to 22. Less than half of patients had been vaccinated for HPV. The study found that HPV vaccination was more common in people with a rheumatic disease diagnosis compared to those who did not have a rheumatic disease, but not after accounting for demographic variables and receipt of other elective vaccines. Additionally, those who had received flu and COVID-19 vaccines were more likely to have been vaccinated for HPV.&nbsp;<br><br>The overall low rate of HPV vaccination was concerning because young people with rheumatic diseases may be at increased risk for HPV infection and HPV-associated cancers due to immune dysregulation, immunosuppression, and gaps in preventive healthcare delivery. Sammaritano, MD</a>.&nbsp;<br><br><strong>Knowledge and Misconceptions about Contraception Among Individuals with Inflammatory Arthritis and Lupus-like Diseases&nbsp;</strong><br>This study looked at what patients with systemic rheumatic disease know about the use of contraceptives, comparing the knowledge of women with inflammatory arthritis to those with lupus-like diseases. This analysis included female patients aged 18-49 years who were seen by a rheumatologist at HSS two or more times between 2020 and 2022, excluding those who reported menopause, oophorectomy, hysterectomy, or other causes of infertility.&nbsp;<br><br>The participants were surveyed about the effectiveness of various contraceptives and about misconceptions related to specific methods in the context of having a rheumatic disease. The researchers found that despite being highly educated (93.2% held a bachelor’s degree or higher), about half of all patients responded incorrectly to questions about the effectiveness of various contraceptive methods. Those with lupus-like diseases were more likely to report that certain hormone-based methods have contraindications, in response to both accurate and inaccurate statements. Sammaritano, MD</a>.&nbsp;<br><br><strong>Knowledge Related to Fertility and Infertility Treatments Among Women with Systemic Rheumatic Disease&nbsp;</strong><br>Systemic rheumatic diseases can be associated with decreased fertility, but patients are not always aware of this connection. This study looked at patients’ understanding of these issues, taking into consideration the patient’s race, ethnicity, and socioeconomic status.&nbsp;<br><br>In an interim analysis of 142 reproductive-aged women treated at HSS for rheumatic disease who completed the Fertility & Infertility Treatment Knowledge Score (FIT-KS) test, the investigators found that — compared with women in the general population previously administered the FIT-KS — women with rheumatic diseases were more likely to be non-White, Hispanic/Latina and highly educated, and less likely to have had successful pregnancies. This population was more aware of the risks of infertility due to smoking, sexually transmitted infections and being underweight. These women also had a better understanding of success rates for vitro fertilization in older women but were less knowledgeable about the effectiveness of oocyte cryopreservation (OOC, or egg freezing). Sammaritano, MD</a>.&nbsp;<br><br><strong>Reproductive History and HPV Vaccination Awareness Among Women with Systemic Lupus Erythematosus and Rheumatoid Arthritis&nbsp;</strong><br>Women with systemic rheumatic diseases, particularly those with systemic lupus erythematosus (SLE), may be more vulnerable to HPV infection and HPV-related cervical cancer, but HPV testing and vaccination rates are low in these patients. This study compared HPV vaccination status and awareness in female patients with SLE to female patients with rheumatoid arthritis (RA).&nbsp;<br><br>The analysis included 295 participants with similar demographics; notably, those with SLE were slightly younger than those with RA. Both groups were similarly likely to be sexually active and had similar rates of having regular Pap smears; however, women with lupus were more likely to have persistent abnormal smears (13.3% vs. 1.7%). While not statistically significant, two women with lupus reported a cervical cancer diagnosis compared to none with rheumatoid arthritis. HPV vaccination rates were similar for the two groups, with 37% of 158 women eligible for the vaccine (including those made eligible given extended eligibility approved in 2019 for those aged ≤45 years if clinically appropriate) reporting having received full vaccination. Among the 79 women who had never received the vaccine yet were eligible, 90% reported that it had never been offered. HSS researchers developed the Sexual Function in SSc Questionnaire (SFSScQ) to anonymously survey patients on whether they had problems with their sexual function and, if so, whether they had ever discussed it with their rheumatologist. Participants who completed the SFSScQ were also asked to complete the already-established Female Sexual Function Index or the International Index of Erectile Function Questionnaire.&nbsp;<br><br>More than half of those who completed both surveys (26 out of 41) reported that they had sexual problems. The researchers note that while the number of survey participants was small, age, sex, race, SSc subtype and time since SSc diagnosis did not appear to be predictors of sexual dysfunction. The institution is collaborating with medical centers and other organizations to advance the quality and value of musculoskeletal care and to make world-class HSS care more widely accessible nationally and internationally. </span><a href="http://www.hss.edu"><span>www.hss.edu</span></a><span>.</span></p>]]></pp:boilerplate><description><![CDATA[<p><span><span><span><span>Despite clear directives outlined in the updated</span></span> <span><span><span>guidelines published by the American College of Rheumatology/National Psoriasis Foundation (ACR/NPF) in 2018, there is limited data regarding medication use in real-world clinical practice and patient medication preferences for the treatment of psoriatic arthritis (PsA).</span></span></span></span></span></p><p><span><span><span><span>According to a new study conducted by researchers at Hospital for Special Surgery (HSS) in New York City and presented at the ACR annual meeting, there has been a significant increase in anti-IL17 and anti-phosphodiesterase-4 (anti-PDE4) medications being used as initial treatment for PsA since 2018, which may reflect their inclusion as potential initial therapy in the ACR/NPF guidelines, as well as the importance placed by patients on medication side effects.</span></span></span></span></p><p><span><span><span><span>&ldquo;Regarding patient medication preferences, we found patients highly value preservation of their joint and function as well as safety,&rdquo; said lead study author <strong>Monica Schwartzman</strong>, MD, MS, a rheumatologist at HSS. &ldquo;We can use these findings to help us tailor our therapy to achieve the best outcomes for our patients.&rdquo;</span></span></span></span></p><p><span><span><span>PsA is a very heterogenous disease with five distinct patterns of arthritis, which can make it challenging to treat. The disease is associated with a myriad of extra articular manifestations, including skin psoriasis, uveitis, and enthesitis, as well as comorbidities such as metabolic syndrome and depression.</span></span></span></p><p><span><span><span>The investigators sent surveys to patients in the HSS PsA registry. <span><span>Preferences were ranked on a five-point Likert scale ranging from &ldquo;not at all important&rdquo; to &ldquo;extremely important.&rdquo;</span></span> The median age of the 137 respondents was 60 years, and the median duration of PsA skin symptoms, joint symptoms, and PsA diagnosis by a physician was 19, 12, and 8 years respectively.</span></span></span></p><p><span><span><span>The survey solicited information regarding phenotypic features and characteristics of the respondent&rsquo;s PsA, demographics, initial and current therapy, and medication preferences. Preference domains included medication efficacy for skin and joint disease, time to onset of efficacy, mode and frequency of administration, cost and insurance coverage, need for laboratory monitoring, side-effect profile, ability to affect function, and advice from physicians, peers, or commercial entities.</span></span></span></p><p><span><span><span><span>The researchers found that 62% of patients were first treated with non-steroidal anti-inflammatory drugs (NSAIDs). The most common initial immunomodulatory medications were anti-tumor necrosis factor (TNF)-&alpha; (35%), followed by methotrexate (19%), anti-PDE4 (12.4%), other conventional synthetic disease modifying anti-rheumatic medications (csDMARDs) (11.7%), anti-IL17 (5.1%), and anti-IL23 (2.9%). At survey administration, the most common immunomodulatory therapies were anti-TNF-&alpha; (30%), followed by anti-IL17 (20.4%), methotrexate (10.2%), anti-PDE4 (8.8%), other csDMARDs (8.0%), Janus kinase inhibitors (2.2%), and anti-cytotoxic T-lymphocyte-associated protein 4 (CTLA 4) (1.5%), while 28% of patients were not on any immunomodulatory therapy. Following the publication of the 2018</span></span> <span><span><span>ACR/NPF</span></span></span> <span><span>updated guidelines for PsA, a significantly higher percentage of patients&rsquo; first medications was an anti-IL17 compared to 2018 or earlier (30% vs 3.5% <em>P</em><0.001). This pattern was also seen with anti-PDE4 medications (40% vs 11.5% <em>P</em><0.012).</span></span></span></span></p><p><span><span><span>Medication preferences most respondents ranked as &ldquo;extremely important&rdquo; were prevention of joint damage (80%), ability to perform daily activities (71%), prevention of pain (70%), rheumatologist recommendation (63%), and medication side effects (62%).</span></span></span></p><p><span><span><span>&ldquo;At HSS, more patients with PsA are being started on an anti-TNF before methotrexate.&nbsp;This likely reflects the fact that many of these patients have axial disease that methotrexate cannot treat effectively,&rdquo; said</span> <a href="https://www.hss.edu/physicians_mandl-lisa.asp" style="text-decoration:underline"><span>Lisa A. Mandl, MD, MPH</span></a><span>, a rheumatologist at HSS and a study coauthor.&nbsp;&ldquo;Patients with PsA, who are often younger, are telling us they want to be treated with medication which will keep them mobile and active.&nbsp;It is important to take into consideration when making therapeutic decisions that alleviating pain is not the only concern. Medications like NSAIDs, which may be effective for pain relief, are not aligned with patient&rsquo;s long-term goals, as they are not an effective disease-modifying agent.&rdquo;</span></span></span></p><p><span><span><span>Dr. Schwartzman said the</span> <span><span>2018 guidelines on PsA from the ACR/NPF</span></span> <span>support a wider range of medications as initial therapy. &ldquo;Over the last decade or so, our treatment armamentarium has expanded in an incredible way for this disease, and it includes medications with novel mechanisms of action.&rdquo; She added the increasing use of newer agents with novel mechanisms of action may &ldquo;indicate a therapeutic need unfulfilled by traditional medications.&rdquo;</span></span></span></p><p><span><span>The researchers say more research is needed on the 28%</span></span> <span><span><span>of patients who were not receiving any immunomodulatory therapy.</span></span></span> <span><span>Left untreated, PsA can lead to functional impairment, poor quality of life, and significant morbidity and mortality.</span></span></p>]]></description><category><![CDATA[pressrelease,psoriatic-arthritis,Rheumatology,Mandl,Research Clinical]]></category>

The institution is collaborating with medical centers and other organizations to advance the quality and value of musculoskeletal care and to make world-class HSS care more widely accessible nationally and internationally. </span><a href="http://www.hss.edu"><span>www.hss.edu</span></a><span>.</span></p>]]></pp:boilerplate><description><![CDATA[<p><span><span><span>A new study by researchers at Hospital for Special Surgery (HSS) in New York City demonstrates over half of patients with rheumatic diseases who contracted COVID-19 during the pandemic and completed a COVID-19 survey, experienced so-called &ldquo;long-haul&rdquo; COVID, or prolonged symptoms of the infection, including loss of taste or smell, muscle aches and difficulty concentrating, for one month or longer.</span></span></span></p><p><span><span><span>Findings identified long-haul COVID was particularly high for smokers, patients with comorbidities such as asthma or lung disease, cancer, chronic kidney disease, diabetes, congestive heart failure or myocardial infarction, and those taking corticosteroids.</span></span></span></p><p><span><span><span>&ldquo;Knowing the impact of this problem is critical,&rdquo; said <a href="https://www.hss.edu/physicians_barbhaiya-medha.asp" style="text-decoration:underline"><span>Medha Barbhaiya, MD, MPH</span></a><span>, a rheumatologist at HSS who led the study. &ldquo;</span>For rheumatology patients, long-haul COVID may be particularly challenging as these patients already have significant chronic health issues and warrants further investigation.&rdquo;</span></span></span></p><p><span><span><span>Dr. Barbhaiya and her colleagues presented their study, &ldquo;Risk Factors for &lsquo;Long Haul&rsquo; COVID-19 in Rheumatology Outpatients in New York City,&rdquo; at the American College of Rheumatology (ACR) annual meeting.</span></span></span></p><p><span><span><span>For the study, Dr. Barbhaiya&rsquo;s group emailed surveys to 7,505 men and women aged 18 years and older who had been treated at HSS for rheumatologic complaints between 2018 and 2020. Participants were asked if they had received a positive test for COVID-19 or if they had been told by a healthcare provider that they had contracted the infection.</span></span></span></p><p><span><span><span>The researchers defined long-haul COVID-19 infections as those with symptoms lasting one month or longer, whereas limited-duration cases were considered those with symptoms lasting less than one month.</span></span></span></p><p><span><span><span><span><span>Among the 2,572 individuals who completed</span></span> the survey, nearly 56% of patients who reported having contracted COVID-19 said their symptoms lasted at least one month. Only two patients in the study had a previous diagnosis of fibromyalgia &mdash; a condition marked by fatigue, muscle aches and other symptoms that have been associated with long-haul COVID &mdash; suggesting that overlap between the two disorders is minimal.</span></span></span></p><p><span><span><span>&ldquo;Our findings do not suggest that symptoms of fibromyalgia are being misinterpreted as long haul COVID in patients with rheumatic diseases, which is something that has been raised as a possibility,&rdquo; said <a href="https://www.hss.edu/physicians_mandl-lisa.asp" style="text-decoration:underline">Lisa A. Mandl, MD, MPH</a>, a rheumatologist at HSS and senior author of the new study.</span></span></span></p><p><span><span><span>HSS researchers plan to use the data as part of a longitudinal analysis of rheumatology patients with long-haul COVID to determine if the lingering symptoms of the infection interfere with their rheumatologic conditions. The study, by researchers from Hospital for Special Surgery (HSS), in New York City, appears online first in the journal <a href="https://onlinelibrary.wiley.com/doi/10.1002/acr.24547"><i>Arthritis Care & Research</i></a>.</p><p>&ldquo;The results may provide some reassurance to physicians who are taking care of women with rheumatic diseases who become pregnant or who are contemplating pregnancy during the pandemic, because there does not seem to be an increased risk of COVID-19, but the findings need to be interpreted with caution given that in other national datasets including the Centers for Disease Control and Prevention data, pregnant patients who get COVID-19 have been shown to have potentially more severe outcomes,&rdquo; said lead study author <a href="https://www.hss.edu/physicians_barbhaiya-medha.asp">Medha Barbhaiya, MD, MPH</a>, rheumatologist at HSS and assistant professor of medicine and healthcare policy & research, Weill Cornell Medicine.</p><p>For their study, Dr. Barbhaiya and her colleagues emailed an online COVID-19 survey to 26,045 patients in the Division of Rheumatology at HSS. 7,094 responded with 1,547 in the targeted population of women age 18 to 50. 4% reported pregnancy.</p><p>The prevalence of self-reported COVID-19 was similar in pregnant and non-pregnant women (8% vs. 9% respectively; <i>P</i>=0.76). Among women with COVID-19, pregnant women had a shorter duration of symptoms (<i>P</i><0.01) and were more likely to experience loss of smell or taste (<i>P</i>=0.02) than non-pregnant women. A systemic rheumatic disease was present in approximately three-fourths of women, with no differences when stratified by pregnancy or COVID-19 status. While 67% of pregnant women reported changes to prenatal care during the pandemic, 23% of postpartum women stated the pandemic affected delivery.</p><p>&ldquo;These findings are novel and should be investigated in other cohorts. Future studies will hopefully assess the long-term outcomes of rheumatology patients and their offspring who were exposed to COVID-19 during pregnancy, which our cross-sectional study could not address,&rdquo; said Dr. Barbhaiya.</p><p>&ldquo;Why pregnant patients are more likely to present with these symptoms warrants further study. This unique finding may help us better understand the immunology of both conditions,&rdquo; said senior study author <a href="https://www.hss.edu/physicians_mandl-lisa.asp">Lisa A. Research in mice infected with SARS-CoV, a coronavirus similar to the one that causes COVID-19, has suggested that plasmacytoid dendritic cell precursors (pDCs) are key to the immune response to infection. These pDCs activate macrophages, which in turn secrete cytokines. In the SARS-CoV research, depletion of pDCs appeared to protect the mice from lethal lung injury. Using blood samples from donors, the investigators will study the pathway by which pDCs activate macrophages and look at ways to therapeutically block that process.</span></p><p><strong>Inhibiting RNA Polymerase II Transcription Complexes in Macrophages to Target COVID-19–Associated Cytokine Storm</strong></p><p><span><strong>Principal Investigator:</strong> </span><a href="https://www.hss.edu/research-staff_rogatsky-inez.asp"><span>Inez Rogatsky, PhD</span></a></p><p><span><strong>Co-Investigators:</strong> Steven Josefowicz, PhD, and Robert P. Fisher, MD, PhD</span></p><p>This pilot project will dissect the role of macrophages in SARS-CoV-2-induced acute respiratory distress syndrome (ARDS), the main driver of COVID-19-associated mortality. We will test small-molecule inhibitors of RNA Polymerase II (Pol II) transcription complexes for their ability to modulate type I interferon and inflammatory pathways in monocytes/macrophages. This research will be done using cultured macrophages as well as donor blood and blood from COVID-19 patients.</p><p><span><strong>Mechanisms of Cytokine Storm in Patients with COVID-19</strong></span></p><p><span><strong>Principal Investigator:</strong> </span><a href="https://www.hss.edu/physicians_crow-mary.asp"><span>Mary K. Ivashkiv, MD</span></a></p><p><span>The objectives of this study are to describe the process of the cytokine storm in people with COVID-19 and to identify biologic predictors of a favorable outcome in patients with severe cases of the disease. The project aims to characterize immune cell populations seen in COVID-19 patients who experience cytokine storm and compare them to those patients who do not, to compare the immune response before and after patients are given the anti-inflammatory drug anakinra, and to identify measures that suggest patients are more likely to decline and eventually require mechanical ventilation. This study will evaluate the response to and recovery from total knee arthroplasty (TKA) in people who have antibodies to COVID-19 — a marker of exposure. This study will include both patients who have COVID-19 antibodies and those who don’t, to act as controls. Patients will be followed for six weeks after surgery and evaluated for the presence of certain immune markers in the blood, as well as symptoms of inflammation including pain and stiffness in the joint.</span></p><p><span><strong>SARS-CoV-2 Exposure and the Role of Vitamin D Among Hospital Employees</strong></span></p><p><span><strong>Principal Investigator:</strong> </span><a href="https://www.hss.edu/physicians_stein-emily.asp"><span>Emily M. This study will investigate vitamin D status and associated immune markers as risk factors for COVID-19 infection in a cohort of healthcare workers. Healthcare workers are at higher risk of contracting COVID-19 than the general population, making them a good group to study. Vitamin D is critical for immune function and is known to be protective against respiratory-tract infection and tuberculosis. Retrospective data will be used to evaluate the incidence and severity of COVID-19 in these patients. This registry, along with COVID-19 screening procedures, will provide the tools to address specific research questions. As elective surgeries resume, those with prior exposure to SARS-CoV-2 will inevitably present for treatment, and some may have perioperative management considerations related to their risk of deep-vein thrombosis. This project will use, a noninvasive device that can determine clotting risks, to investigate whether people who have had COVID-19 have a more dysfunctional endothelium preoperatively and at 24 hours after surgery. The TRC will provide consultation on the design and implementation of COVID-19 research in the areas of orthopedics, rheumatology and metabolic bone disease; support for a COVID-19 biobank; and technical expertise and facilities required for clinical and translational researchers working on COVID-19–related projects. <p><em>CreakyJoints</em> spoke to <a href="https://www.hss.edu/physicians_mandl-lisa.asp">Lisa A. Mandl, MD, MPH</a>, rheumatologist at HSS, about the affects of osteoarthritis on your joints. &ldquo;OA isn&rsquo;t just about the cartilage. It&rsquo;s a disease of the whole joint. The ligaments are weak, the tendons are frayed, the muscles are weak,&rdquo; explained Dr. Mandl. Dr. Mandl cited age is the strongest non-modifiable risk factor for OA, and sex also plays a role. Knee and hand OA is more common in women than men, and the frequency of hip OA is the same in women and men, said Dr. Mandl. Additionally, weight is the biggest modifiable risk factor for developing OA. &ldquo;Even losing five pounds can make a difference. The heavier you are, the more likely you are to develop knee OA and for it to progress and become more severe. It is never too late to lose weight," noted Dr. Mandl.</p>

<p>Management strategies for OA have changed over time from more traditional approaches of pain control to interventions that can also improve functional activity and quality of life.</p>

<p>According to Dr. Mandl, the main goal is to improve mobility. "We should be engaged with our patients and partner with them to find the best nonpharmacologic plan."</p>

<p>Dr. Mandl references recent research in the field and points out that OA is not just a disease of cartilage but of the whole joint. "<span>Therefore, it is important for patients to engage in physical therapy because the muscles that surround the knee joint can act like a splint and help stabilize their joint. This allows for less pain when a patient moves. We have to remember that all components of the joints are important, not just the cartilage."</span></p>

<p>Dr. Mandl also states that it's important for rheumatologists to remember that joint replacements can be extremely effective for the right patient.&nbsp;"<span>It can take patients, especially women, a long time to decide if this operation is right for them. It is often helpful to let patients know that a joint replacement is a potential therapeutic option earlier rather than later so patients are aware this could be something down the road if they ever need it."</span></p>
