Su, MD,</span></a><span> hip preservation surgeon at HSS. After two knee replacements, two hip replacements, a shoulder surgery and several ankle surgeries, he says, “everything works and I have a full, productive life.”</p><p style="text-align:justify;">The Long Island resident says sports are a vital part of his life. He has an activity for every season and most days of the week. Sailing, sculling, tennis, bicycling, snow skiing and open-water swimming keep him busy year-round.</p><p style="text-align:justify;">So, when he developed arthritis, he wasn’t about to let that stop him. “In my late 30s I felt the first twinge of pain in my right hip. Within a couple of years, it had worsened to the point that I needed care. I was told that I had arthritis likely from one of two serious falls as a young man,” he explained. “Because I was still young and active, I held off surgery until I hit 50, by which time I was using a cane.” He had his first hip replacement at HSS more than 20 years ago, and it’s still holding up. &nbsp;</p><p style="text-align:justify;">Years later, at age 66, he felt the same pain in his other hip, and he had a second hip replacement at HSS. This time, with advances in surgical technique and technology, the former attorney says, “I was up and about the following day, and in six weeks I had forgotten all about it.”</p><p style="text-align:justify;">“More aging meant more arthritis,” he explained, and in 2024 he had both knees replaced in two separate surgeries by<span>&nbsp;</span><a href="https://www.hss.edu/profiles/doctors/fred-cushner" target="_blank">Fred D. Austin, MD</span></p>]]></description><content:encoded><![CDATA[<p>Healio reported on new research from HSS that finds patients who reported an iodine-related allergy prior to total joint arthroplasty did not experience a hypersensitivity reaction if they received an iodine-based product during surgery.&nbsp;</p><p><a href="https://www.hss.edu/profiles/doctors/matthew-austin" target="_blank">Matthew S. Austin, MD</a>,<span>&nbsp;hip and knee replacement surgeon at HSS</span>, told Healio about results presented at the American Academy of Orthopaedic Surgeons Annual Meeting. “This study sheds light on the fact that if someone says they have an allergy to an iodine-related product one should not automatically avoid its use,” he<span>&nbsp;</span>said. <span style="text-align:left;">Dr. Austin and colleagues retrospectively reviewed data from 3,696 patients who reported an allergy to iodine, iodine-containing products or shellfish who either did (n = 1,378) or did not (n = 2,318) receive an iodine-based product during primary TJA.&nbsp;</span></p><p><span style="text-align:left;">“Instead of a blanket policy of if a patient says they have an allergy to an iodine-based product, they cannot receive any iodine-based product, institutions can have a protocol in place where you look at the patient’s history,” Dr. Austin continued. “What is the patient saying the reaction was? How severe was the reaction? How far in the past? Have they safely received an iodine-related product in the interim since they discovered they had this allergy?                         <guid>https://news.hss.edu/robot-use-lowers-opioid-use-for-thr-patients/</guid><pp:caseid>707836</pp:caseid><description><![CDATA[<p><span>Orthopedics This Week featuring </span>Periklis Giannakis, MD</p>]]></description><content:encoded><![CDATA[<p>Orthopedics This Week highlights an <span style="text-align:start;">HSS study </span>that won the<span><strong>&nbsp;</strong></span>President’s Choice award at the American Society of Regional Anesthesia and Pain Medicine meeting titled: “Opioid Consumption During Hospitalization Across Manual, Computer- and Robotic-Assisted Total Hip Replacement.” The study found <span style="text-align:start;">that using robot assistance in total joint arthroplasty LOWERS post-op opioid use.</span></p><p style="margin-left:0px;text-align:start;">The investigators found that patients who had robotic-assisted <a href="https://www.hss.edu/health-library/conditions-and-treatments/list/hip-replacement" target="_blank">total hip replacement</a> (THR) needed less opioid medication during hospitalization than patients who had computer-assisted or manual THR. It is the first report of robot-assisted THA’s superiority over computer-assisted THA.</p><p style="margin-left:0px;text-align:start;">Co-author <strong>Periklis Giannakis, MD</strong>, research fellow at HSS, explained, “This study is aligned with the goals of our newly-established Pain Prevention Research Center at HSS, which has given us the unique opportunity for collaborations with other HSS services, including the Adult Reconstruction and Joint Replacement Service. Chalmers, MD</span></a><span style="text-align:start;">, &nbsp;hip &nbsp;and knee surgeon at HSS, presented at the </span>American Association of Hip and Knee Surgeons annual meeting <span style="text-align:start;">on a study indicating proximal femoral replacement may be an effective salvage option in complex revision total hip arthroplasty cases.</span></p><p>“It is important to try to do everything we can to mitigate the common causes of re-revision in these complex patients,” said Dr. Chalmers.&nbsp;</p><p>Dr. Chalmers and colleagues retrospectively analyzed data for 49 patients (50 hips) who underwent revision THA to evaluate the survivorship of proximal femoral replacement. The mean follow-up was 5 years, with a minimum follow-up of 2 years. Dr. Chalmers measured included all-cause reoperation, all-cause re-revision, survivorship free from dislocation and risk factors for poor outcomes.</p><p>Outcomes measured included all-cause reoperation, all-cause re-revision, survivorship free from dislocation and risk factors for poor outcomes. Dr. Chalmers said survivorship free from all-cause reoperation was 78% at both 2 and 5 years. In addition, he said survivorship free from all-cause re-revision was 87% at 2 years and 82% at 5 years. Survivorship free from dislocation was 84% at both 2 and 5 years, according to Dr. Chalmers. He said indications, bearing designs, as well as construct length and body length ratios, were not found to be risk factors for reoperation. However, he noted there was a trend toward increased rates of dislocation in patients who had a standard or elevated liner vs. a constrained liner. If the injury does not extend into his acetabulum or disrupt his pelvic ring his recovery will be much more reliable and quicker. He will likely be unable to return to the court for at least 3-6 months, but the injury and recovery is variable. Returning too soon increases the risk for refracture,” explained Dr. Page.&nbsp;</span></p><p><span style="text-align:start;">“His potential return later in the season will depend on the nature of the injury, however it is difficult to assess without reviewing his imaging studies. The initial diagnosis could have meant surgery, but that wasn't on Baker's mind.</p><p style="margin-left:0px;text-align:start;">Determined to run the 2023 race, Baker and the team at HSS gathered to meet her goal -- all without going the route of surgery.</p><p style="margin-left:0px;text-align:start;">"I had wonderful support through all the professionals here, and I felt good because it was a very manageable plan," she added.</p><p style="margin-left:0px;text-align:start;"><a href="https://www.hss.edu/physicians_mcgrath-todd.asp" target="_blank">Todd McGrath, MD</a>, sports medicine physician at HSS explained that surgery would have cost Baker the chance to run last year's event and this year's because of the long rehab process. The two cohorts did not differ significantly in terms of preoperative demographics, with the exception of travelers having had lower average Harris Hip Scores. There were no differences in postoperative PROs, complications, readmissions or reoperations. The hip joint, a ball-and-socket structure cushioned by cartilage, is one of the body’s largest weight-bearing joints and plays a crucial role in our mobility, she explained.</span></span></p><p style="margin-left:0px;text-align:start;">Building muscle strength<span>&nbsp;</span>around the hip<span>&nbsp;</span>is one of the most effective ways to ease and prevent hip joint pain when walking, said Dr. Barsoum. And one of the best moves to try is squats, she explained.</p><p style="margin-left:0px;text-align:start;">Using a wall for support can help maintain proper form and reduce the risk of knee injury. “The key is to imagine sitting back into a chair,” Dr. Barsoum said. “You’re sticking your butt out to the back and lowering yourself down as if you were going to sit down into a chair.”&nbsp;</p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:start;">Walking, using an elliptical machine and running are also good for hip strength. HSS investigators presented two retrospective studies at the American Academy of Orthopaedic Surgeons (AAOS) Annual Meeting in San Francisco analyzing outcomes in both knee and hip replacement surgeries performed with robotic assistance.</span></p><p style="text-align:justify;"><span>“We have found that robotic assistance leads to improved implant positioning, alignment and ligament balance, which enable us to better replicate the patient’s natural anatomy,” said </span><a href="https://www.hss.edu/physicians_mayman-david.asp"><span>David J. Mayman, MD</span></a><span>, chief of the Adult Reconstruction and Joint Replacement Service at HSS. “With our vast registry of patient data and strong commitment to research, HSS is in a unique position to continue leading the evolution of robotic assistance in the operating room.”</span></p><p><span>The first study found that the use of a surgical robot during total knee replacement surgery reduced the need for manipulation under anesthesia (MUA), an outpatient procedure that may be prescribed when physical therapy (PT) after surgery becomes too painful for the patient due to joint stiffness caused by scar tissue. During the course of care, approximately 3.5% of knee replacement patients undergo MUA, according to </span><a href="https://www.hss.edu/physicians_jerabek-seth.asp"><span>Seth A. Jerabek, MD</span></a><span>, hip and knee surgeon at HSS and co-author of this study, typically within 90 days of their surgery. After MUA, patients are typically better able to participate in PT and progress in their recovery.</span></p><p><span>Investigators at HSS set out to determine if the use of computer navigation or robotic-arm assistance during surgery reduced the need for MUA. Previous studies have shown that the use of either tool leads to a more precise knee replacement.</span></p><p><span>The HSS researchers conducted a retrospective chart review of 21,893 knee replacement surgeries performed between April 2008 and December 2022. They found that the use of the robotic arm or computer navigation led to a significant decrease in the number of patients who needed MUA after surgery (2.7% of patients with robotic-assisted surgery; 2.8% of patients whose surgery was performed with computer navigation; 3.7% of patients whose surgery was performed without either technology).</span></p><p><span>“These results highlight the potential benefits of incorporating technological advancements into knee replacement surgery, offering improved outcomes and reducing the need for additional interventions such as MUA,” Dr. Jerabek said.</span></p><p><span>The second study compared short-term outcomes of the anterior versus the posterior approach in robotic-assisted total hip replacement (THR) surgery. “There is no consensus on which approach has a lower risk of postoperative complications,” said </span><a href="https://www.hss.edu/physicians_gausden-elizabeth.asp"><span>Elizabeth B. &nbsp;Gausden, MD, MPH</span></a><span>, a hip and knee surgeon at HSS and principal investigator of the study. “This can often lead to confusion on the part of the patient as to which approach may be best for them. Our objective was to compare rates of dislocation, re-operation, revision and patient-reported outcome measures of each approach performed with robotic assistance.”</span></p><p><span>Dr. Gausden and colleagues identified 2,040 consecutive robotic-assisted THRs for osteoarthritis in the HSS patient registry between 2017 and 2020. A total of 497 hip replacements were performed using the anterior approach; 1,542 were performed with the posterior approach.</span></p><p><span>At mean follow-up of 18 months, the researchers found no difference in the rate of dislocation, re-operation or revision surgery between the two approaches. At early follow-up of six weeks, the anterior approach was associated with a somewhat faster functional recovery. Most of it was concerned about fracture or implant failure, but now we are really becoming much more aggressive about what we are letting people get back to.” “If you’re a runner, and it’s something that brings you joy and it’s your passion, is it the best exercise on your hip replacement? Probably not, but I’m okay with you getting back to it in smaller doses,” Dr. Gausden explained.</span></p><p><span>Dr. Gausden explains the traction that robotic surgery has gained in recent years. One thing we haven’t shown yet is that the clinical outcomes are superior. We are working on that. We do know that a robot when added to a surgery does add precision compared to manual instrumentation.”</span></p><p><span>“Its an exciting time. The addition of robotics has made it easy to collect data on a large scale. Collecting additional data from each case and add that in with post operative data. There’s more wearables that people are using after surgery, so their watch is collecting data. Their phone might be collecting data. They may have censors around their knee or hip collecting data. It’s going to be really exciting to see if those things can help us do a better job of their hip or knee replacement and minimize the risk of complications. Su, MD</a>, hip and knee surgeon at HSS.&nbsp;</p><p>Resurfacing — an alternative to a full hip replacement that involves shaving damaged bone and cartilage from the femur, capping that bone with metal and popping it back into a lined socket — has high-profile test cases in tennis and pro wrestling.</p><p>“I think the thing that got me,” Kane said, “was the fact that Dr. Su believes that I won’t have to retire because of my hip now.”</p><p>The belief, held by the player, the team and its medical staff, is still that Kane can be that person, even at 35. It’s shared by Dr. Su, who has performed thousands of hip resurfacings over nearly two decades, including . Success stories are important to him; the procedure accounts for less than 1 percent of all yearly hip replacements in the United States, and he believes that number should be higher, especially, he says, now that lower-quality implants and less committed surgeons are no longer in the mix.&nbsp;</p><p>Kane, in the six months since his surgery, “has reset the whole recovery timeline” — and not just for hockey players. &nbsp;“(Kane) really is, like, kind of freakish,” Dr. Su told The Athletic. “He’s just blowing it out of the water. Things that we expected to be accomplished by three months, he did in six weeks. Things that we expected in four months, he did at two months. Hip and knee surgeries are fantastic surgeries. They have great outcomes. Patients are very satisfied after the surgery. That has been the case for a while. The biggest advances we have made in the past decade are in improving the pain control around the time of surgery. Previously, patients were staying in the hospital for up to a week at a time after a hip or knee replacement. They'd require really strong pain medications which carry their own risks and side effects and the pain persisted for weeks after the surgery. Now, patients are able to come in, get their surgery, go home often the same day or next day. We use different smaller medications to help settle down the pain. We don't need the super strong pain medications any longer. The amount of time you need the pain medications is also less, so that's been a big improvement,” explained Dr. Summers.&nbsp;</p><p style="margin-left:0px;">“During the surgery, we have new tools we get to use during the operation itself, so we have different planning software that we use where we use computers to help find out exactly where we should put your hip or knee replacement. During the surgery, we have robots and other advanced tools that help us place those components into that position that we previously planned for. By 1923, doctors were adept at using plaster of Paris to immobilize limbs and x-ray machines could see broken bones underneath the skin.</p><p style="margin-left:0px;text-align:start;">Even so, doctors sometimes had to treat patients with the methods known to the ancients: Identify a fracture, stretch and straighten the limb to align the ends of broken bones, and make a splint. Open breaks carried serious risks of blood loss and infection.</p><p style="margin-left:0px;text-align:start;">“If you think about what orthopedics was 100 years ago, it was plaster and sticks and casts and traction. That was it,” said <a href="https://www.hss.edu/physicians_ast-michael.asp" target="_blank">Michael P. Ast, M.D.,</a> orthopedic surgeon and chief medical innovation officer at HSS. Founded in 1863, HSS is the oldest orthopedic hospital in the nation.</p><p style="margin-left:0px;text-align:start;">“If you go back two generations, a broken hip — everyone says, how did your grandma die? She died of a broken hip. I mean, you can’t die from breaking a bone right?” Dr. Ast said.</p><p style="margin-left:0px;text-align:start;">“But what happens is that because we didn't have a way to fix them reliably, these patients would be unable to walk,” Dr. Ast explained. “They would be bed-bound, and then bad things happen when older patients are bed-bound. What you eat may help. That’s especially true for people who have a history of mistrust in the health care system, as many Black Americans do, said Dr. Parks.</span></span><br><br><span style="color:#000000;"><span style="text-align:start;">His and others’ research has found that African Americans — especially those from poor communities or who have less education are more likely to have poor outcomes from surgery and to require additional surgery resulting from complications. Research also has found that Black Americans are less likely to receive knee or hip replacement surgery and less likely to receive pain management than white patients.</span></span></p><p><span style="color:#000000;"><span style="text-align:start;">That’s partly due to providers’ inherent biases, poor&nbsp;</span></span><a href="https://www.arthritis.org/health-wellness/treatment/treatment-plan/you-your-doctor/talking-with-your-doctor"><span style="color:#000000;">communication</span></a><span style="color:#000000;"><span style="text-align:start;">&nbsp;and because people in these communities are less likely to seek treatment, said Dr. Parks.&nbsp;</span></span><br><br><span style="color:#000000;"><span style="text-align:start;">“We've done studies where we looked at patients and how their perception of their interaction with the physician was, and many particularly African American patients who are women felt that their complaints were dismissed. So we have to think about that. What are our implicit biases or explicit biases when we're dealing with patients, and hopefully check those at the door and meet the patients where they are so that we can again treat them equitably,”&nbsp;he continued.</span></span><br><br><span style="color:#000000;"><span style="text-align:start;">“The whole concept of shared decision-making is where you give a patient an array of options,” explained Dr. Parks. “I try to open a dialogue and make the patient feel comfortable with their questions. My role is to educate them, to tell them where I think they are, to give them my recommendations. And then I always tell them, "It's between you and that painful joint. It will often make you do something. Since your hip can be sore and sometimes even painful, you rely more on your core and upper body to help you get around.”</p><p>He added, “Exercise in one form or another starts right away. Usually these are very simple exercises meant to get your hip moving and begin your recovery. The most important exercise to do during that time is simply walking.”. The goal is to gradually increase your physical activity post-surgery.</p><p>Dr. Ast advised doing hip replacement post-op exercises that target the glute muscles that surround and stabilize the hips. These exercises work to restore joint mobility and stability.</p><p>Eannucci recommended a series of four exercises alongside video demonstrations for strengthening glutes and improving hip flexibility, doing 3 sets of 10 reps per exercise three times a day for the first four weeks post-op. (Before incorporating these recommendations, make sure you talk to your doctor and/or physical therapist about the best hip replacement exercises for you.)</p><p>She said a ‘sit to stand’ exercise “builds strength to do this activity while strengthening your thigh and buttock muscles.”</p><p>Eannucci noted a ‘bent knee fall out’ exercise works hip rotation. “Hip rotation is a motion that typically is lost due to hip arthritis. People find they can't put their shoes on and off, for example, as a result. Ultrasonography provides its operator with the ability to visualize intraarticular effusions, a key advantage over fluoroscopy. However, it is also critical to note that ultrasound is more of a technically demanding procedure which can depend on operator experience, as well as patient factors, including BMI, body habitus and if revision surgery has been previously performed.”</p><p>He added, “The study highlights the evident need in the periprosthetic joint infection clinical literature for a standardized method by which aspiration results and the technique utilized to be reported.                         <title>Use of Nerve Blocks for Hip Fracture Surgical Patients is Increasing Across the United States</title>

This could indicate that positive strides are being made to reduce opioid use in these patients. These findings were presented at the 2022 Spring American Society of Regional Anesthesia and Pain Medicine&nbsp;(ASRA) Annual Meeting.<sup>1</sup></span></p><p><span>Because patients with hip fractures have significant comorbidities, they are often at a higher risk of experiencing adverse effects from opioids. Using nerve blocks for pain relief in these patients can help to not only reduce pain, but also opioid use and confusion. Some emergency medicine and acute pain societies are attempting to increase the use of ultrasound guided regional anesthesia.</span></p><p><span>“This was the first attempt to look at how hip fracture patients are treated with local anesthetics prior to surgery. It is a descriptive study from a nationally representative database,” explained <strong>Alexander Stone, MD</strong>, regional anesthesia/acute pain medicine (RA/APM) fellow at HSS, who received one of three</span><a href="https://www.asra.com/events-education/ra-acute-meeting/abstracts---eposters"><span> Resident/Fellow Travel Awards</span></a><span> from ASRA for this study.&nbsp;</span></p><p><span>The researchers identified hip fracture patients in the Premiere Healthcare Database, excluding patients under 18 years of age, who had missing data on sex or on discharge date. The researchers identified the use of injectable local anesthetics from medication billing records and physician order entry on the day of admission.&nbsp;</span></p><p><span>This study included 864,416 patients with an emergency admission due to a hip fracture from 2009-2019 from 1,012 different hospitals. Of these patients, 1,126 (0.11%) underwent surgery on the day of hospital presentation.</span></p><p><span>From 2009-2019 the rates of local anesthetic use on the day of admission increased as follows:&nbsp;</span></p><ul><li><span>Bupivacaine (13.3% to 18.6%)</span></li><li><span>Ropivacaine (2.0% to 8.4%)</span></li><li><span>Mepivacaine (0.05% to 0.14%)</span></li><li><span>Lidocaine (28.4% to 41.0%)</span></li></ul><p><span>Limitations of the study include the inability to tell from medical reports whether the medication was administered as a nerve block, field block, or local infiltration. Additionally, investigators could not determine who administered the medication.</span></p><p><span>These results show a trend towards increased use of local anesthetics before surgery in patients with hip fracture. There was an increase in the use of sustained pain relief being started early in admissions, specifically with longer acting anesthetics such as bupivacaine and ropivacaine.&nbsp;</span></p><p><span>“The primary takeaway is that it appears more hip fracture patients are being treated with longer-acting local anesthetics prior to surgery. This could reflect changes in opioid sparing and multimodal analgesia practices,” Dr. Stone noted. “It is important for anesthesiologists to be aware of this, and for increased communication and collaboration between specialties to reduce the risk of local anesthetic toxicity.”</span></p><p><span>When discussing follow-up research, Dr. Stone said more detailed information about this topic is needed.&nbsp;</span></p><p><span>“This study is not granular, which is a limitation of using large administrative databases. It’s not supposed to hurt.” Avoid overstretching your hips in splits or foot-in-hand extensions, because that motion will only exacerbate underlying inflammation.<br><br>“When you are stretching those hip flexors, make sure you’re focusing on the correct muscle group, doing it with the proper technique,” she added.<br><br>Finding other ways to strengthen your body outside of the studio is key to preventing injuries. Dr. Buza recommended focusing on supplemental activities, like Pilates and yoga, that target the core, back and the muscles surrounding the hips. Additionally, low-impact cardio exercise, like swimming or biking, is also an excellent choice because it builds stamina and endurance in the muscles surrounding the hips.<br><br>She advised to be mindful of posture and alignment throughout the day. “If you’re doing anything sustained for a long period of time in malalignment, then it’s going to place stress across the joint and increase pain and pressure,” she noted.<br><br>Read the full article at <a href="https://www.dancemagazine.com/hip-health/">Dancemagazine.com</a>. The results were presented today at the American College of Rheumatology (ACR) annual meeting in a poster titled &ldquo;Use of Total Hip Arthroplasty in Patients Under 21 Years Old: A U.S. Population Analysis&rdquo; and recently published in <a href="https://www.sciencedirect.com/science/article/abs/pii/S0883540321006446?dgcid=coauthor" style="text-decoration:underline"><em>The Journal of Arthroplasty</em></a>.</span></span></span></p><p><span><span><span>Between 2000 and 2016, the number of total hip arthroplasty (THA) procedures performed in patients under 21 years of age increased from 347 to 551, while the total pediatric population in the United States remained stable, the researchers found.</span></span></span></p><p><span><span><span>Traditionally, most young patients undergoing THA suffer from developmental bone or joint ailments or juvenile inflammatory arthritis. However, the new study revealed that the upswing in THA procedures in young patients is not being driven by a rise in the number of children with inflammatory joint disease, suggesting that nonsurgical therapies to control these conditions are working, said <a href="https://www.hss.edu/physicians_mehta-bella.asp" style="text-decoration:underline">Bella Mehta, MBBS, MS, MD</a>, a rheumatologist at HSS and the senior author of the study.</span></span></span></p><p><span><span><span>&ldquo;Our study shows that although THA procedures are increasingly being performed in young people, we aren&rsquo;t seeing more of these patients seeking surgery for inflammatory arthritis,&rdquo; Dr. Mehta said. &ldquo;We&rsquo;re doing a better job at treating these individuals so they don&rsquo;t develop end-stage joint damage. Twenty years ago, we didn&rsquo;t have access to effective pharmacologic treatments for these conditions, and now we&rsquo;re using them well and helping these patients live a better life.&rdquo;</span></span></span></p><p><span><span><span>&ldquo;These procedures are still pretty rare in hospitals, and some surgeons might not see any young patients,&rdquo; added Alex Gibbons, a research assistant in Dr. Mehta&rsquo;s group and a co-author of the study. &ldquo;Although some site-specific data on THAs in adolescents exist, it wasn&rsquo;t known how frequently these procedures were being performed in the United States and if that frequency was growing or not.&rdquo;</span></span></span></p><p><span><span><span>For the study, Dr. Mehta and her colleagues used the Kids&rsquo; Inpatient Database, a repository of information on patients under 21 years of age from approximately 4,200 hospitals in 46 states. The researchers looked for elective and non-elective primary THA procedures.</span></span></span></p><p><span><span><span>The mean age of young patients undergoing THA was just over 17 years and remained constant throughout the duration of the study. Most patients were white (55.3%) and had private insurance (56.5%). The majority (80%) of THAs were performed in urban teaching hospitals.</span></span></span></p><p><span><span><span><a href="https://www.hss.edu/physicians_figgie-mark.asp" style="text-decoration:underline">Mark P. Figgie, MD</a>, chief emeritus of the Surgical Arthritis Service at HSS and a co-author of the study, said the finding that most THA procedures in young patients are performed in urban and teaching hospitals reflects a trend that is beneficial for these patients. As other studies have demonstrated in conditions like inflammatory arthritis, when hip replacements are performed by surgeons with significant experience, patients have better outcomes. &ldquo;And for younger patients, the nuances require the expertise and knowledge to avoid complications and optimize outcomes,&rdquo; Dr. Figgie said.</span></span></span></p><p><span><span><span>The most common diagnoses for young patients undergoing THA were osteonecrosis, osteoarthritis and juvenile idiopathic arthritis (JIA)/inflammatory arthritis, the researchers found. Over the course of the study, the frequency of THA for osteonecrosis rose from 24% to 38%, while that for arthritis fell from 27% to 4%&mdash;likely a reflection of recent improvements in medical management for the condition, they added.</span></span></span></p><p><span><span><span>Dr. Figgie noted that improvements in implant technology and materials have made the devices far more durable than they were 20 years ago. As a result, surgeons now feel more comfortable offering THA to young patients because their implants are likely to withstand the wear and tear of decades of activity.</span></span></span></p><p><span><span><span>Dr. Mehta said the findings could be of value both to clinicians and young patients, for whom, in many cases, the surgery is their first major operation. &ldquo;I would use these results to say to a young person: &lsquo;There are a lot of people who get these procedures; you&rsquo;re not alone.&rsquo; I find that, especially for young patients, knowing they&rsquo;re not the only ones to experience something really helps. McLawhorn, MD, MBA</u></span></a><span>, hip and knee surgeon at HSS.</span></p><p><span>Tulberg said of the experience, “[Dr. McLawhorn] was great. HSS was very conservative in their approach, which I liked.”</span></p><p><span>Tulberg tried physical therapy, dry needling and acupuncture, and finally made the personal decision to schedule replacement surgery in September 2020.</span></p><p><span>Just three months after surgery, she was performing splits, and a year later she feels better than she has in years.</span></p><p><span>Tulberg said, “Now I’m auditioning again for shows. I’m having callbacks for shows, on hold for a couple of projects. I feel like I’m able to dance better than I was before. I have a full range of motion, if not better than before I had the replacement done.”</span></p><p><span>She continued about the shattering the stigma of injury in her field of dance, “The strangest thing has been figuring out who to talk to, if I should be vocal about having this surgery. There is certainly a stigma in the dance world that once you have a hip replacement, your career is over. Su, MD</u></span></a><span>, hip and knee surgeon, and colleagues.</span></p><p><span>The study found that patients with unilateral hip resurfacing arthroplasty had higher cobalt and chromium serum ion levels than those with unilateral ceramic on polyethylene bearings. Additionally, more patients who had a hip resurfacing arthroplasty developed adverse local tissue reactions (ALTR) or metallosis on MRI than did patients who got ceramic on polyethylene bearings.</span></p><p><span>Dr. Potter said, “While the majority of these high functioning individuals with HRA [hip resurfacing arthroplasty] had no adverse local tissue reaction, longitudinal yearly evaluation disclosed that more patients who received a HRA developed ALTR or metallosis on MRI than patients who had ceramic on polyethylene bearings (hazard ratio 4.8 (95% CI 1.2-18.4); p=0.02).”</span></p><p><span>Dr. Su explained, “This study highlights the value in using MRI to evaluate patients with metal-on-metal hip resurfacing. It demonstrates that metal levels or clinical performance do not always correlate with MRI findings of adverse local tissue reaction, and therefore all of this information is valuable in the evaluation of such a patient. Just as I would not recommend the sole use of metal ion levels in deciding management of a patient, similarly, I would not utilize MR information alone to make decisions.
