                        <title>Knicks Champion Jalen Brunson Shares Insight Into Wrist Surgery Performed at HSS</title>

So I just wasn’t going to let something that seemed minor take me out. So, just whatever needed to be done, just focus on it afterwards.”</span></p><p><span>Brunson said his wrist would remain in a cast for roughly six more weeks before rehab begins, with a target of being ready by September. A member of the HSS Board of Trustees and Board of Advisors for over 15 years, she has made an indelible mark on the organization helping to shape the careers of young investigators, blaze new paths for scientific inquiry, and transform how HSS cares for those affected by musculoskeletal conditions.</span></p><p><span>Mrs. French has long been a champion of science and research at HSS, culminating her lead gift to Kellen Tower, which opened earlier this spring and marks a major milestone in the transformation of HSS’s Upper East Side campus. Mrs. French graduated from the Brearley School in New York City and the University of Lausanne, Switzerland, and attended the New York School of Interior Design.</span></p><p><span><strong>Robert N. Hotchkiss, MD</strong></span></p><p><span>Dr. Robert N. Hotchkiss served as the inaugural Joel and Anne Bick Ehrenkranz Chair in Hand and Upper Extremity Research and Director of Clinical Research at HSS, where he has devoted much of his professional career to advancing orthopedic surgery, clinical investigation, and innovation in musculoskeletal care. He also serves as Associate Professor of Orthopaedic Surgery at Weill Cornell Medical College and previously led the Hand and Upper Extremity Service at HSS for more than a decade.</span></p><p><span>Dr. Hotchkiss received his undergraduate education at the University of Colorado Boulder, majoring in both Chemistry and Slavic Languages and Literature, before earning his MD from Johns Hopkins University School of Medicine, where he also completed his surgical and orthopedic training.</span></p><p><span>Internationally recognized for his pioneering work in hand, elbow, and upper-extremity reconstruction, Dr. Hotchkiss has helped shape modern approaches to complex elbow trauma, instability, stiffness, and reconstructive surgery. Over a distinguished career spanning more than four decades, he has made contributions that extend beyond the operating room into biomechanics, implant design, translational research, and the emerging use of machine learning and advanced imaging technologies in medicine. Talk to the player, see what happened, get them in right away, and evaluate them. I work with my colleague Bob Hotchkiss, our hand and wrist expert and get all the imaging studies. Dr. Hotchkiss did the surgery and did a wonderful job as far as restoring anatomy. But the biggest issue in the recovery process is getting the motion back. Fingers and hands tend to get stiff," said Dr. Rodeo.</span></span></p><p><span style="background-color:rgb(255,255,255);"><span style="text-align:left;">“If you put him in a cast for three months, the bone would heal, but then he couldn't move his fingers. But if you move faster than the bone is healed, then that alignment issue that you are trying to protect becomes compromised. When I see a patient with radial nerve palsy after humerus fracture, I get advanced imaging of MR neurography and ultrasound to determine if the radial nerve is injured and if there is fascicular continuity throughout the zone of injury. I am also looking to see if there is any neuroma formation. Other questions I ask the radiologist is whether there is external compression on the nerve, impingement from bone fragments, scar tether, compression from hematoma or other findings. There is more suspicion for operative nerve injury if the fracture is open and if the palsy occurred after manipulation. I also use the advanced imaging gunshot wounds.                         <title>A Ukrainian soldier was told his legs could be amputated. An American hospital might help him walk again</title>

Pinned in the back seat, Orlov <span style="color:#000000;">said </span>he was initially unable to get out of the vehicle – his feet had been crushed by the car and his legs had been wounded by the explosion. Once he finally did, he and his team laid in the nearby grass watching the flames to figuring out their next steps.</p><p><span style="background-color:rgb(255,255,255);color:#000000;"><span style="text-align:start;">Orlov was taken to a Ukrainian hospital and told he may need to have at least one leg amputated or that he may never walk again.&nbsp;</span></span></p><p style="margin-left:0px;text-align:start;"><span style="color:#000000;">Ashley Matkowsky, Orlov’s American girlfriend and a videographer who had been working in Ukraine, recorded what he looked like after the attack.</span></p><p style="margin-left:0px;text-align:start;"><span style="color:#000000;">That video caught the attention of U.S. volunteers and made its way to<span>&nbsp;</span>Gary Wasserson, a retired American businessman from New York who was already coordinating volunteer aid resources to the region. Wasserson asked HSS &nbsp;if they’d be able to save Orlov’s legs and, optimistic they could, he was admitted.</span></p><p style="margin-left:0px;text-align:start;"><span style="color:#000000;">Dr. Fufa, who is now helping care for Orlov, described his injuries as extraordinarily complex.</span></p><p style="margin-left:0px;text-align:start;"><span style="color:#000000;">“The complexity comes from the fact that he had both soft tissue wounds as well as bone defects or missing bone from the blast injuries and the multiple fractures in each of the feet,” she said.</span></p><p style="margin-left:0px;text-align:start;"><span style="color:#000000;">Since he arrived in the U.S. in January, Orlov has already undergone “two very lengthy procedures to begin the major step for reconstruction of both his right and his left foot” with care that has involved multiple specialists, Dr. Fufa explained. Wolff, EdD, OT, CHT</span></p>]]></description><content:encoded><![CDATA[<p>NBC News Now highlights the patient story of Emily Duncan, a lifelong musician who experienced a hand injury that impacted her ability to perform. Duncan sought treatment from <a href="https://www.hss.edu/rehab-staff_wolff-aviva.asp"><span>Aviva L. Wolff, EdD, OT, CHT</span></a>, occupational hand therapist at HSS, after enduring countless doctor’s visits, physical therapy appointments, nerve tests and steroid injections.<span>&nbsp;</span></p><p>Wolff had pioneered an innovative approach to treating sore hands and arms. In her lab, she attaches motion sensors to each musician and analyzes the biomechanics of their playing with multiple cameras, an approach akin to what athletic trainers and conditioning coaches do with professional athletes to tweak a golf swing, a basketball jump shot or pitching form. With the musicians, most of their pain is chronic and the high-tech motion analysis allows Wolff to detect abnormal hand or body placements that are exacerbating their injuries.</p><p>While similar in some ways, musicians’ and athletes’ bodies do differ in one key aspect of their performance. “Unlike athletes, musicians have to play without tension,” noted Wolff. “With professional musicians who are using their body optimally, you can see how light they look. It seems effortless, and that’s what you want to see.”</p><p>Wolff is currently developing an injury prevention and treatment curriculum for conservatories and orchestras that includes anatomy lessons, stretches and exercises designed for each instrument. She’s also educating her medical colleagues with research from the HSS Motion Analysis Lab that shows specific ways to get injured musicians back to playing in the short-term and in better shape for the long-term. “We are not educated enough in the demands of musicians,” said Wolff. “The musculoskeletal system and our bodies are healing machines. There are solutions out there.”</p><p>Wolff created a customized “return to play” plan for Duncan, using the data she captured through her motion analysis studies. One surprise: Though Duncan felt pain and numbness in her hand, Wolff pinpointed exercises for an area no one else had detected as a possible source of the pain — the upper back muscles. “It was really illuminating for her to say, ‘You’re feeling the symptoms here in your hands and your wrist but actually if you strengthen these muscles in the back, that will help your symptoms,” cited Duncan.<span>&nbsp;</span></p><p>Now Duncan feels secure about her future and her ability to continue to do what she loves best, in the place she loves best. “Slowly but surely as New York is coming back, I’m coming back,” she concluded. “It’s nice to be able to play music again.”</p><p>This segment was live streamed via NBC News Now on July 5, 2022. The institution is collaborating with medical centers and other organizations to advance the quality and value of musculoskeletal care and to make world-class HSS care more widely accessible nationally and internationally. </span><a href="http://www.hss.edu"><span>www.hss.edu</span></a><span>.</span></p>]]></pp:boilerplate><description><![CDATA[<p><span>Males presented with more severe carpal tunnel syndrome and were offered surgery more often than females with the condition, according to work by researchers at Hospital for Special Surgery (HSS) featured today in a poster presentation at the </span><a href="https://www.aaos.org/annual"><span>American Academy of Orthopaedic Surgeons 2022 Annual Meeting</span></a><span>.</span></p><p><span>The study adds to a growing body of research by HSS investigators examining gender, racial/ethnic and socioeconomic disparities in diagnoses, treatment approaches and outcomes in a range of musculoskeletal conditions, including hip and knee replacement, lupus and rheumatoid arthritis.</span></p><p><span>“Our study aimed to investigate if disparities in treatment for carpal tunnel syndrome exist,” said </span><a href="https://www.hss.edu/physicians_fufa-duretti.asp"><span>Duretti Fufa, MD</span></a><span>, HSS hand and upper extremity surgeon and senior author of the study. “Patients and surgeons should always be aware of potential biases when discussing and making treatment decisions, especially for patients with moderate disease symptoms where the decision to pursue surgery is more subjective.”</span></p><p><span>The carpal tunnel is the narrow space within the wrist that contains bones, tendons, ligaments and the median nerve responsible for transmitting sensations from the fingers and thumb. Compression of the median nerve from prolonged wrist flexion or extension, trauma, arthritis or inflammation of wrist tendons can cause numbness, weakness and tingling in the hand and arm, resulting in </span><a href="https://www.hss.edu/conditions_carpal-tunnel-syndrome-myths-facts-diagnosis-treatment.asp"><span>carpal tunnel syndrome</span></a><span>.</span></p><p><span>Electrodiagnostic testing to confirm a diagnosis of carpal tunnel syndrome consists of nerve conduction studies that assess how electrical signals move through the median nerve to hand muscles and </span><a href="https://www.hss.edu/conditions_emg-testing-a-patient-guide.asp"><span>electromyography (EMG) testing</span></a><span> that evaluates whether the muscles activate correctly in response to those signals. Test results indicate whether disease severity is mild, moderate or severe.</span></p><p><span>Patients with mild symptoms are typically offered nonsurgical treatments such as bracing or steroid injections. Patients with severe symptoms are often offered </span><a href="https://www.hss.edu/reliability-compare-carpal-tunnel-release.htm"><span>carpal tunnel release</span></a><span>, a minimally invasive surgical procedure that relieves pressure on the median nerve by cutting the carpal ligament. “However, for patients with moderate symptoms, it’s not as obvious whether a nonsurgical or surgical approach is best,” said Dr. Fufa. “There is more decision-making involved, and therefore a higher potential for patient factors and potentially subconscious biases to influence recommendations for surgery.”</span></p><p><span>For the study, Dr. Fufa and colleagues reviewed hospital records for 949 patients with carpal tunnel syndrome treated at HSS between February 2016 and October 2020. All patients had been diagnosed by HSS hand surgeons and received electrodiagnostic testing. The researchers grouped patients’ records according to disease severity and looked for differences in treatment recommendations, completion of treatment and the time interval between being offered surgery by gender, race/ethnicity and age.</span></p><p><span>There were two interesting findings within the moderate disease severity group of 141 females and 90 males: Females were 23 percent less likely to be offered surgery than males, and Hispanic and Black females were four times less likely to be offered surgery than their male counterparts. These trends were strong but not statistically significant, given the size of the study.</span><br><br><span>“Overall, our findings suggest that increased utilization by males was largely explained by the fact that men were more likely to present with more severe carpal tunnel syndrome,” Dr. Fufa said. “I suspect that with larger numbers of patients to study, differences based on gender and race may prove to be significant. Carlson, MD</a>, hand and upper extremity surgeon at HSS.</span></span></span></p><p><span><span><span>According to Dr. Carlson, osteoarthritis (OA), the most common type of arthritis, is a condition caused by the natural wearing away of the cartilage that covers the bone. In healthy joints, the cartilage coverings at the ends of the bones match up and glide smoothly against one another.</span></span></span></p><p><span><span><span>Cartilage cannot not be built back, but motion can be preserved by keeping the soft tissue in the joint as supple as possible.</span></span></span></p><p><span><span><span>Dr. Carlson advised incorporating moves to help maintain hand function, such as a shoulder stretch. Maroldi explained, &ldquo;Oftentimes, tendons or ligaments are overloaded or repetitively strained during certain exercises, leading to tendinitis, tenosynovitis, or ligament sprains or tears.&rdquo;</span></span></p><p><span><span>Although repetition is a common practice for building muscle, it&rsquo;s not ideal for hand health. Maroldi noted, &ldquo;Repetitive stresses from exercise can also lead to ganglion cysts, which most commonly appear on the back of the wrist.&rdquo;</span></span></p><p><span><span>&ldquo;Other exercises that involve dynamic movement and loading the wrists, such as snatches and clean and jerks, can lead to ligament tears in the wrist, most commonly the scapholunate ligament (SL ligament),&rdquo; added Maroldi.</span></span></p><p><span><span>Another watch-out is weight-bearing exercises. &ldquo;The most common exercises that cause wrist/hand injuries are those that involve weight-bearing or dynamic loading through extended wrists such as pushups and burpees,&rdquo; said Maroldi.</span></span></p><p><span><span>Maroldi offered tips for avoiding injury during exercise, such as maintaining a neural wrist position. &ldquo;Neutral wrist position means that the wrist is in slight extension (make a tight fist and the position your wrist is in is your neutral wrist position),&rdquo; he explained.</span></span></p><p><span><span>Maroldi also advised to avoid overgripping. &ldquo;Your wrists and hands are constantly working and under tension, and it is therefore not recommended to do specific wrist- or grip-strengthening exercises. "Hands, wrists, fingers, and thumbs are a complex part of the body!" she said. "There are a lot of different types of injury, or causes of pain, that need completely different types of treatment. Some injuries need rest, some injuries do better with exercise.” A certified hand therapist will be able to work with you to give you the best advice for your specific injury and workout preferences, added McLeod.</p><p>McLeod noted some modification examples include doing push-ups on a closed fist or push-up bar instead of your palms, and using resistance bands or machines to replace free weights. Easing up on intensive also falls under the category of modification. "If you haven't worked out in a while, be sure to pace yourself!" she advised. "If you've had an injury, you probably won't be able to lift the same weights as before. Start with a weight and a workout intensity that feels a little challenging but still manageable. Then, gradually build up."</p><p>McLeod also suggested cardio exercise with the permission from a medical expert. "Walking, jogging, and running are generally safe after most hand injuries. The design is the culmination of three decades of award-winning research by <a href="https://www.hss.edu/physicians_wolfe-scott.asp">Scott Wolfe, MD</a>, a hand surgeon at Hospital for Special Surgery (HSS), and Joseph J. Crisco, III, PhD, director of the Bioengineering Laboratory at Brown University and Rhode Island Hospital.</p>

<p>The HSS Innovation Institute worked closely with Drs. Wolfe and Crisco to obtain a patent for their research and design ideas. The patent was then licensed to Extremity Medical, LLC, a privately funded medical device company in New Jersey.</p>

<p>&ldquo;We believe the new wrist replacement, known as the KinematX&trade; Total Wrist Implant, has advantages over traditional implants,&rdquo; said Dr. Wolfe, chief emeritus of the Hand and Upper Extremity Service at HSS. &ldquo;Our extensive research into how the wrist moves helped us design a replacement that more closely matches the anatomy and motion of a normal wrist. This should allow for more natural motion and increased durability compared to currently available implants.&rdquo;</p>

<p>&ldquo;In our view, the KinematX&trade; will be a game changer in the field of wrist replacement surgery,&rdquo; said <strong>Doug Leach</strong>, managing director of Biomechanical Innovation at the HSS Innovation Institute. &ldquo;Current wrist replacement devices are relatively dated. The new implant design leverages the clinical and bioengineering experience of Drs. Wolfe and Crisco, their seminal research, and modern-day engineering and design principals of total joint replacement in general.&rdquo;</p>

<p>Wrist arthritis is one of the most common and debilitating conditions treated by hand surgeons, affecting about five million people in the United States. A fusion of the wrist bones can alleviate pain, but patients are often limited in performing some activities. Wrist joint replacement surgery was proposed five decades ago as an option to relieve pain and restore function.</p>

<p>&ldquo;Most people are familiar with total joint replacement in the knee or hip, which is much more common and highly successful,&rdquo; said Dr. Wolfe. &ldquo;Total wrist joint replacement has not enjoyed the same degree of success. Historically, some studies demonstrate failure rates of near 50% within 5 to 10 years, mainly due to loosening of implant components.&rdquo;</p>

<p>Dr. Wolfe and his longtime colleague, Dr. Crisco set out to design a better wrist replacement 30 years ago. The first step was to unravel the complexity of how individual bones move in a normal wrist and in one that has sustained an injury. Their research led to the design and validation of a noninvasive three-dimensional motion analysis system to measure wrist kinematics, which describes the wrist motions necessary for a wide range of activities. Armed with that information, they sat down at Dr. Wolfe&rsquo;s kitchen table and started sketching out their idea for a novel wrist replacement based on their research.</p>

<p>Over the years, Drs. Wolfe, Crisco and colleagues have received more than $10 million in research grants from the National Institutes of Health to study the wrist. They have published hundreds of papers in peer-reviewed journals. In 2017, they received the Kappa Delta Award from the American Academy of Orthopaedic Surgeons and the Orthopaedic Research Society, considered one of the highest honors for an orthopedic researcher.</p>

<p>The wrist is highly complex, made up of more than a dozen individual joints formed by eight small bones that collectively make up the wrist. It was previously believed that the wrist moved the hand in two different planes &ndash; up and down or side to side. The research by Drs. Wolfe and Crisco demonstrated that during many activities, such as throwing a ball, hammering a nail or pouring a glass of water, the wrist doesn&rsquo;t move in just one of these directions. It accomplishes the action by combining movements in both planes.</p>

<p>&ldquo;Traditional wrist replacements often constrain the wrist to move in one plane at a time, and this puts stress on surrounding joints," said Dr. Wolfe. &ldquo;The increased loads on the implant-bone interface often lead to prosthesis loosening and mechanical failure. In addition, traditional implants often make it difficult or impossible to return to some activities, such as tennis or golf.&rdquo;</p>

<p>The KinematX&trade; is the only wrist replacement that is computer-designed to mimic the kinematics, or motion, of a human wrist &ndash; movements that are instrumental in throwing, hammering and other high-performance activities, according to Dr. Wolfe. The unique design should enable the implant to be more durable than traditional wrist replacements.</p>

<p>Surgery with the KinematX&trade; entails replacing the proximal carpal row bones of the wrist, which are those most severely affected by osteoarthritis. The device utilizes modular components in various sizes to replicate the complex anatomy of the wrist and allow the motion necessary for occupational, recreational and household activities.</p>

<p>The Innovation Institute at HSS played a key role in the development of the new wrist replacement. &ldquo;Our goal is to work with talented HSS physicians, inventors and entrepreneurs to empower them to develop innovations in musculoskeletal health that could improve or even revolutionize patient care,&rdquo; Mr. Leach said.</p>

<p>Wrist replacement surgery with the KinematX&trade; will be performed on an outpatient basis. Dr. Wolfe says he already has a waiting list of individuals who are good candidates. A patient registry will be established at HSS and other leading medical centers across the country to track data on outcomes.</p>

According to Dr. Osei, most younger patients will have improvement in the ability to feel the top of the hand over time.&nbsp;</p>

<p>"He could have numbness or tingling or even burning pain from the recovering nerve, but his hand and wrist range of motion will be unaffected," said Dr. Osei.</p>

]]></description><content:encoded><![CDATA[<p><em>SF Examiner</em> reports Golden State Warriors&rsquo; Stephen Curry underwent surgery to repair a broken second metacarpal in his left hand.</p>

<p><em>SF Examiner</em> spoke to <a href="https://www.hss.edu/physicians_osei-daniel.asp">Daniel A. Osei, MD, MSc</a>, hand and upper extremity surgeon at HSS, who has not treated Curry, about the recovery and repair for this type of injury. While Curry&rsquo;s injured hand is not his dominant shooting hand, defense mechanisms are a concern. &ldquo;It&rsquo;s all the other stuff - the defending, the swatting of balls, the landing on your hand again, and frankly, trying to make sure that you don&rsquo;t re-injure the area,&rdquo; explained Dr. Osei. &ldquo;At three months, I will say that most of these fractures go from, &lsquo;It&rsquo;s healed, but you could reinjure yourself,&rsquo; to healed enough that you basically can&rsquo;t.&rdquo;</p>

<p>Regarding&nbsp;methods for surgical repair (plates versus screws), Dr. Osei noted this is dependent on the pattern of the fracture. &ldquo;Plates are used because they&rsquo;re a little bit stronger, so it allows for more initial strength, so basically, you can get somebody back a little bit faster.&rdquo; Dr. Osei added, &ldquo;Screws are great because they&rsquo;re more low-profile, and may not bother someone as much, because you can&rsquo;t feel screws as much as you can feel a plate, but the downside is that they&rsquo;re not as strong, so the chance of re-injury is a little bit higher. Really, it&rsquo;s a combination of the location, the pattern of the injury and issues related to how quickly you&rsquo;re trying to get a guy back,&rdquo; he said.</p>

<p><span>&ldquo;In general, the index metacarpal is a good bone to break if you have to break one,&rdquo; said&nbsp;</span><a href="https://www.hss.edu/physicians_carlson-michelle.asp">Michelle G. Carlson, MD</a>, hand and&nbsp;<span>upper extremity surgeon at HSS, who has not treated Curry. &ldquo;It is a little easier to come back from than the other metacarpals.&rdquo;</span></p>

<p><span>Dr. Carlson explains that treatment typically includes elevating the hand to avoid swelling and beginning motion therapy soon after surgery. If surgery is not required, a cast or splint would be used for up to a month.</span></p>

<p>Wolff wrote while its unlikely for musicians to cut down on the number of practices and&nbsp;performances which involve highly repetitive and coordinated movements, she underscored the importance of physically warming up prior to playing (i.e. stretches to the forearm muscles). &ldquo;Warming up before playing increases the blood flow to the muscles and allows musicians to play longer with less pain and fatigue,&rdquo; she cited.&nbsp;Wolff suggested taking 10-15 minute breaks every hour, doing a few gentle stretches or simply walking around to relieve any muscle tension and give the muscles a chance to recover.</p>

<p>Wolff concluded it&rsquo;s while common for musicians to give in to poor posture as they are playing for long periods of time in unusual positions, it is advised to practice good body mechanics to help place less stress on the joints and muscles.</p>

]]></description><content:encoded><![CDATA[<p>Inside Edition reports on the dangers of handling sharp tools when carving pumpkins, and spoke to <a href="https://www.hss.edu/physicians_fufa-duretti.asp">Duretti Fufa, MD</a>, hand and upper extremity surgeon at HSS, who provided safety tips to consider to&nbsp;prevent hand injuries.</p>

<p>Dr. Fufa recommended the use of specially designed pumpkin carving kits versus a kitchen knife, which could lead to sliced fingers and hands. &ldquo;Have the knife blade down, and your hand on top of the knife so that when we&rsquo;re exerting the downward force, our hand will not slip down the knife.&rdquo; Dr. Fufa advised using sawing not stabbing-like motions when carving, and to never cut toward the hand. &ldquo;The blade could inadvertently cut the inside of the palm. Instead you should stabilize the pumpkin and be cutting away from the hand.&rdquo; Furthermore, never put your hand inside the pumpkin when carving. &ldquo;You may inadvertently plunge inside and cut the hand when you&rsquo;re not seeing it,&rdquo; said Dr. Fufa. &ldquo;To prevent yourself from being tempted to put your hand in the pumpkin you can put the top back inside while you&rsquo;re working on making your outside cuts,&rdquo; she cited.</p>

]]></description><content:encoded><![CDATA[<p><em>The Times Picayune</em> reports on the thumb injury of New Orleans Saints quarterback, Drew Brees, to which experts have speculated this to be his ulnar collateral ligament (UCL).</p>

<p><em>The Times Picayune</em> spoke to <a href="https://www.hss.edu/physicians_carlson-michelle.asp">Michelle G. Carlson, MD</a>, hand and upper extremity surgeon at HSS, who does not treat Brees, but commented on the recovery of a UCL injury. Dr. Carlson cited this injury will likely cause him [Brees] to miss a good chunk of the season. &ldquo;If you were a lineman, you could put your hand in a cast and deal with this at the end of the season,&rdquo; said Dr. Carlson. She added, &ldquo;If you need to throw the football, you need to fix it now.&rdquo;</p>

<p>With regard to return to sport, Dr. Carlson explained it typically takes ligaments 8-12 weeks to fully heal, however it could be as few as five weeks, with the use of suture tape, as the&nbsp;tape acts as an internal brace, offering extra protection while the ligament heals. Dr. Carlson concluded, &ldquo;Results are excellent, certainly long-term, in terms of return to play.&rdquo;</p>

                        <title>Physician Education and Guidelines Lead to Big Drop in Opioids Prescribed after Ambulatory Hand Surgery</title>

The educational session was mandatory for all HSS staff involved in prescribing controlled substances. The hospital also conducted extensive research to develop guidelines for opioid prescription.&nbsp;</p>

<p>The study, "Opioid Prescriber Education and Guidelines Decrease Opioids Prescribed after Ambulatory Hand Surgery," was presented today at the American Academy of Orthopaedic Surgeons Annual Meeting in New Orleans.</p>

<p>"Prescription opioid misuse is an epidemic in the United States with almost 2 million Americans having abused prescription opioid medication. It&rsquo;s now the leading cause of death in adults under 50," said <a href="https://www.hss.edu/physicians_osei-daniel.asp">Daniel Osei, MD</a>, a hand surgeon at HSS and senior investigator. "Recent studies demonstrate a consistent over-prescription of opioid medications nationwide. As physicians, we always strive to provide adequate pain control for our patients. But we are also increasingly aware that when there are excess amounts of unused medication, it can be diverted to uses other than treating pain."</p>

<p>Currently, there is no national standardized prescriber education course or postoperative opioid guidelines for ambulatory hand surgery. However, prescriber education previously has been shown to decrease over-prescription on a small scale.</p>

<p>The HSS researchers set out to evaluate the effect of the education session and the hospital&rsquo;s new opioid guidelines on physician prescribing practices after ambulatory hand surgery.</p>

<p>HSS mandated a one- to two-hour education program in November 2016 for all employees qualified to prescribe controlled substances. The hospital also formulated prescribing guidelines after an extensive review of the medical literature on the amount of pain medication commonly needed after hand surgery. The new guidelines, which recommended a specific number of pills based on the type of hand surgery performed, were disseminated to staff in February 2017.</p>

<p>To see if there was a change in prescribing practices, investigators reviewed postoperative opioid prescriptions for patients who underwent ambulatory hand surgery in the two months prior to the mandatory education session; in the two months following physician education; and during the two-month period that followed the dissemination of prescribing guidelines.</p>

<p>A total of 731 ambulatory hand surgeries with postoperative opioid prescriptions for patients were included in the study. All three time periods had a similar ratio of surgery types. Researchers found a 45 percent reduction in opioids prescribed after the education session and distribution of guidelines.</p>

<p>"It would be difficult to overstate the importance of this study. A 45 percent reduction is a huge amount and demonstrates the success of the program in reducing the amount of prescribed narcotics," said Dr. Osei.&nbsp;"Our results suggest that similar standardized education and prescribing guidelines on a national level could reduce the amount of opioids prescribed after ambulatory hand surgery nationwide."</p>

<p>The next step for Dr. Osei and his colleagues at HSS will be an analysis of how well the prescribing practices worked in terms of pain control and patient satisfaction for those in the study.
