Griffin Research Accelerator, which will work to increase the personalization of knee replacement surgery.</span></p><p style="margin-left:0in;"><span>The gift will also support the HSS Kellen Tower, a 94,000-square-foot (8,733-square-meter) building currently under construction at 70th Street and FDR Drive. The tower, which is expected to open in 2025, will be the headquarters for joint replacement surgery and house the accelerator. Steel, Co-Chair of the HSS Board of Directors. Haas, MD</a>, c<span style="background-color:rgb(255,255,255);"><span style="text-align:left;">hief of the knee service at HSS</span></span><span> .&nbsp;</span></p><p style="margin-left:0in;"><span>Historically, posterior stabilized and cruciate-retaining knee bearings have been popular bearing types among orthopedic surgeons for total knee arthroplasty.</span></p><p style="margin-left:0in;"><span>Data from the 2022 American Joint Replacement Registry Annual Report showed posterior stabilized implants were used in more than half of all primary TKA procedures until 2019 when the rate decreased to less than 50%, while cruciate-retaining designs increased annually since 2016, reaching 49.7% in 2021.</span></p><p style="margin-left:0in;"><span>However, the report also showed the use of ultracongruent components increased 57% between 2012 and 2021. A study led by <a href="https://www.hss.edu/biomechanics-research-staff-fernando-quevedo-gonzalez.asp"><span>Fernando J. Quevedo-González, PhD,</span></a> assistant scientist in the Department of Biomechanics at HSS, and HSS hip and knee surgeon <a href="https://www.hss.edu/physicians_vigdorchik-jonathan.asp">Jonathan M. Vigdorchik, MD</a>, used these kinds of models to determine how best to minimize the risk of complications in cementless TKA. The model was developed using preoperative scans from 12 female patients about to undergo robotic TKA and was used to ask questions about how sizing the tibial baseplate (the metal part of the prosthesis that connects to the shin bone) affects outcomes. The investigators found <span>that, in general, decreasing the size of the baseplate increased the amount of bone at risk of failure, but this risk was small compared with normal variability of bone quality among patients and how their individual knee joints were loaded as they used their knee in performing activities of daily living. Vigdorchik, MD</span></a><span>.</span></p><p><span><strong>Posterior Tibial Slope in Total Knee Arthroplasty: The Transmalleolar Sagittal Axis Underestimates Slope Compared to Traditional Intramedullary Axis</strong></span></p><p>The tibial slope, one of multiple angles used to place a TKA, defines how the tibial component meets the knee and impacts the knee’s ability to bend and the strain that’s put on the ligaments throughout range of motion. A study led by HSS hip and knee surgeons <a href="https://www.hss.edu/physicians_chalmers-brian.asp">Brian P. Chalmers, MD</a>, and <a href="https://www.hss.edu/physicians_haas-steven.asp">Steven B. Haas, MD</a>, looked at how robotics and computer navigation influence the tibial slope in TKA. Historically, knee implants were designed using what is called the intramedullary axis to measure the tibial slope, but technology-assisted knee replacement operations determine the slope in a different way. The investigators retrospectively reviewed preoperative CT scans from 40 TKA procedures and reconstructed the 3-D geometry of the tibia and fibula to determine the slope based on the intramedullary axis. They determined that in many cases, the alternate calculation of the slope used in technology-assisted procedures resulted in a different slope in the newly implanted knee. A novel study led by <a href="https://www.hss.edu/biomechanics-jonathan-glenday.asp">Jonathan Glenday, PhD,</a> a postdoctoral fellow, and <a href="https://www.hss.edu/biomechanics-research-staff-fernando-quevedo-gonzalez.asp">Fernando J. Quevedo-González, PhD</a>, assistant scientist, both in the Department of Biomechanics at HSS, looked at the intricate relationship between attachment to the bone and joint motion, and how these two aspects of joint mechanics are related. The investigators used a computational model that included gait mechanics and anatomic information from CT scans to look at whether the anterior-posterior (AP) translation (front-to-back movement) of the femur with respect to the tibia while walking was correlated with bone-implant micromotion after surgery. Based on their models, they determined tha<span>t the AP translation significantly related to the peak micromotion throughout gait, especially in the lateral portion of the knee; greater AP translation resulted in larger micromotions. Offset of components, which is related to the width of the hip, directly affects the impingement of the hip after surgery. Surgeons have multiple ways to alter offset during THA surgery; however, these alterations have only been studied in two dimensions. A team led by HSS hip and knee surgeon </span><a href="https://www.hss.edu/physicians_vigdorchik-jonathan.asp">Jonathan M. Vigdorchik, MD</a><span>, and former clinical fellow <strong>Eytan Debbi, MD, PhD</strong>, used 3-D models created from pre- and postoperative CT scans of 16 patients who underwent the procedure to retrospectively analyze the relationship between offset and impingement. They compared cup/liner (acetabular) offset to head/stem (femoral) offsets and looked at the effects that these configurations had on the patient’s range of motion and incidence of impingement after surgery. The investigators found that in all cases, increasing the offset reduced the amount of impingement, and increased the full range of motion of the hip. Haas, MD</a>, chief of the knee service at HSS.</p><p>When asked if joint replacement surgery can be done safely during the pandemic, Dr. Haas noted, &ldquo;I think the answer is yes &mdash; as long as the right protocols are in place.&rdquo; It all starts with a conversation with your doctor about factors such as the severity of your joint damage as well as your risk for severe COVID-19 infection. &ldquo;For the right patient with the right level of disease, and with the right protections, it can be done safely,&rdquo; he added.</p><p>When the time does come for surgery, there are a number of protocols in place at medical facilities around the country to protect against COVID-19. According to Dr. Hass, HSS has installed HEPA air filters in operating rooms, which may help reduce airborne transmission of the coronavirus.</p><p>The approach to post-surgery recovery and physical therapy has also changed since the pandemic. Some practices are also finding success with teletherapy. HSS has a telemedicine program called HSS At Home. &ldquo;With this home physical therapy program you have the therapist on the [screen] doing personalized physical therapy and monitoring exercise with the patients. The results have been outstanding. Our post-op patients during COVID are doing just as well as those before COVID,&rdquo; said Dr. Haas. &ldquo;We had only small amounts of telemedicine before COVID. But I&rsquo;ve found through telemedicine you can make a good connection with people, even if you can&rsquo;t do physical exams.&rdquo; He continued, &ldquo;The patient often has family members with them, they&rsquo;re comfortable at home, I get to see their environment, and I&rsquo;m not distracted by having to enter data on my computer.&rdquo;</p><p>When considering joint replacement surgery, Dr. Haas provided guidance on items to discuss with your physician, including questions about the facility where the surgery will be performed. &ldquo;If it&rsquo;s at a hospital, does it have safety protocols set up to do COVID-19 testing and monitoring?&rdquo; advised Dr. Haas. However, not all decisions are straightforward and there are gray areas. &ldquo;If you&rsquo;re an older adult with comorbidities, but you can barely walk around the house and you&rsquo;re deteriorating and possibly getting depressed as a result, we have to have those individual conversations to see if surgery is the right thing,&rdquo; cited Dr. Haas. &ldquo;Patients just need to have the talk with their provider. <p>As more orthopedic groups across the country are putting this procedure in place, they are finding that it not only reduces costs but also has better patient outcomes. &ldquo;My belief is this: When you do a surgical integration, especially a quality of life one, we want to disrupt the patient&rsquo;s life as little as possible,&rdquo; said Dr. Haas. HSS started its shift toward outpatient surgery for joint replacement nearly four years ago, one of the first in the nation to do so. Dr. Haas explained HSS was not only driven by cost cutting, but more so by how they believed this would improve both the experience and outcome for the people undergoing the surgery. &ldquo;There is no doubt there were multiple drivers,&rdquo; he said. &ldquo;But if you look at the bigger picture, this really hits the sweet spot of decreasing costs while improving care and the patient experience. It&rsquo;s the right thing to do.&rdquo;</p>

<p>Dr. Haas noted outpatient surgery isn&rsquo;t for everybody - a patient needs to be motivated and have the right attitude. &ldquo;There is no doubt there are people who are a good choice for this and those who are not,&rdquo; he cited. &ldquo;I don&rsquo;t envision 100 percent of [total joint replacement patients] going home the same day. That&rsquo;s never going to happen.&rdquo; The patient must be willing to do their at home physical therapy daily, stay up to speed on their medications, and be willing to push themselves. &ldquo;If you are not a motivated patient, this is not for you. I tell every patient: This is a team effort, and I have the easiest part on that team. I give them the equipment. They have to use it.&rdquo;</p>

<p>Dr. Haas added part of the reason this works is that at home, people must get up and do things. &ldquo;If you lay someone down for a day, they will want to lay down the next day too,&rdquo; he said. &ldquo;At home, you have to get up and do things that seem hard, but once you do them, you see you can.&rdquo;</p>

<p>Dr. Haas explained, &ldquo;It's less to do with ambulatory surgery, but the big changes for reimbursement had to do with an acceptance of telemedicine during the pandemic. Prior to the pandemic, to even see a new patient or take care of patients via telemedicine, you have to be licensed in the state that you're providing care. New York is a tristate area. We are bordered by Connecticut and we're bordered by New Jersey; those are our catchment area. At the beginning of COVID, neither the payers would pay for anybody through telemedicine and we were legally not allowed to do patients who literally were as close as Queens, but they were in New Jersey, actually probably closer. The insurers all opened up to allow that.&rdquo;</p>

Haas, MD</a>.</p><p>As patient selection is a top consideration for the success of outpatient total joint replacement, Dr. Haas explained he is a big proponent of putting patients on comprehensive prehab programs and optimizing their pre-op nutritional status. This prepares the&nbsp;body&nbsp;for the trauma of major surgery and improves the ability&nbsp;to recover quickly. &ldquo;Prehab is important because the better condition patients are in heading into surgery, the better condition they&rsquo;ll be coming out,&rdquo; he said.</p><p>Is it important for surgeons to understand and commit to an approach that prepares patients for same-day discharge. Dr. Haas has developed a surgical technique that involves operating through small incisions and eliminates the need to cut the quadriceps tendon, factors that limit post-op pain and help to prepare patients for same-day discharge.</p><p>The quality of care that patients receive in the weeks and months after surgery and the amount of work they put into their rehab regimens are essential to ensuring they achieve the outcomes they expect. Digital platforms now allow patients to follow prescribed physical therapy programs at their convenience and from the comfort of their homes. HSS launched a telehealth physical therapy program prior to&nbsp;the COVID-19 pandemic for&nbsp;select patients, but has since expanded its use. Patients go home, get online and work with a physical therapist remotely. When Craig Williamson, an active 44 year-old father was unable to keep up with his kids, or take the stairs due to bone-on-bone osteoarthritis in his right knee, the only solution was a total knee replacement. Williamson&rsquo;s surgeon&nbsp;<a href="https://www.hss.edu/physicians_haas-steven.asp">Steven B. Haas, MD</a>, chief of the Knee Service at HSS, confirmed he was a good candidate for outpatient knee replacement.</p>

<p>CBS New York spoke to Williamson about his&nbsp;recovery post-surgery, and Dr. Haas who explained the recent advances that have made this procedure possible. &ldquo;We developed less invasive or minimum invasive techniques. It&rsquo;s involved less cutting of the muscle and tendons around the knee,&rdquo; said Dr. Haas. &ldquo;That was followed by innovations in pain control. [We] got better at controlling the pain, better nerve blocks, and injections around the knee,&rdquo; he added. The improved pain control allows the patient to walk and even climb the stairs within a few hours after surgery.</p>

<p>It&rsquo;s been three months since his surgery and Williamson is doing well. &ldquo;I feel great. I go to the gym all the time again and I am taking spin classes a couple times a week, doing the elliptical. I actually ran up the stairs the other day,&rdquo; he said.</p>

                        <title>Barrack V. Haas: Patella Resurfacing: Rarely, If Ever, Necessary</title>

                        <guid>https://news.hss.edu/barrack-v-haas-patella-resurfacing-rarely-if-ever-necessary/</guid><pp:caseid>321946</pp:caseid><description><![CDATA[<p>In a recent Orthopaedic Crossfire debate for <em>Orthopedics This Week</em>, HSS chief of knee service <a href="https://www.hss.edu/physicians_haas-steven.asp">Steven B. Haas, MD</a>, argued that patella resurfacing during total knee arthroplasty is necessary.</p>

<p>"Initially not resurfacing the patella leads to higher revision rates and equal or more pain. So, why are we doing it? A meta-analysis (Nizard et al., CORR 2005) shows that un-resurfaced patellas had increased pain, greater reoperations, greater anterior knee pain," said Dr. Haas during the debate at the Annual Current Concepts in Joint Replacement (CCJR) Winter meeting in Orlando, Florida.</p>

<p>According to recent studies, Dr. Haas stated that non-resurfaced patellas required re-operation compared to those with a resurfaced patella.&nbsp;&nbsp;</p>

<p>"If 3-5% [of patients] are going to go back, my patients are not going to be happy. I think that if you resurface them you're going to be happier with that result and not have to go back and re-operate," he said.</p>

<p>The debate was moderated by HSS surgeon-in-chief emeritus <a href="https://www.hss.edu/physicians_sculco-thomas.asp">Thomas P. Sculco, MD</a>.</p>
