                        <guid>https://news.hss.edu/the-hidden-cost-of-wearing-too-small-shoes-an-hss-expert-explains-the-cinderella-complex/</guid><pp:caseid>787062</pp:caseid><description><![CDATA[<p><span>New York Post featuring Rock Positano, DPM, MSc, MPH, DSc</span></p>]]></description><content:encoded><![CDATA[<p><span>Many people assume their shoe size stays the same for life, but that’s a common misconception, according to </span><a href="https://www.hss.edu/profiles/doctors/rock-positano" target="_blank" rel="noreferrer noopener"><span>Rock Positano, </span>DPM, MSc, MPH, DSc</a>, <span>Founder and Director of the </span><a href="https://www.hss.edu/departments/non-surgical-foot-ankle-service">Non-surgical Foot and Ankle Service</a><span> and the </span><a href="https://www.hss.edu/departments/non-surgical-foot-ankle-service/heel-pain-center">Heel Pain Center</a><span> at HSS.</span></p><p><span>Research has found that many women wear shoes that are too small and may need a larger size as they age. Dr. Positano refers to this as the “Cinderella complex”: trying to fit a larger foot into a smaller shoe size.</span></p><p><span>“It is unlikely that your shoe size at age 20 is going to be the same at age 50,” said Dr. Positano.</span></p><p><span>Wearing ill-fitting shoes can contribute to foot pain, tendonitis, blisters, ankle swelling, and even discomfort in the knees, hips, and lower back. After four decades of racing cars, Hutchings was faced with a difficult decision – either have surgery to correct a lifelong foot issue or risk being sidelined from the sport he loves. His </span><a href="https://www.hss.edu/health-library/conditions-and-treatments/list/flatfoot" target="_blank"><span>flatfoot</span></a><span> issue worsened over time, making it hard to drive a car – let alone a race car.</span></p><p><span>"With this problem, the heel kind of goes out to the side and foot collapses down," said </span><a href="https://www.hss.edu/profiles/doctors/elizabeth-cody" target="_blank"><span>Elizabeth A. Cody, MD</span></a><span>, foot and ankle surgeon at HSS. Hutchings explained that high-speed driving and intense pain do not mix on a racetrack."There's a technique we call heal and toe. You put the ball of your foot on the brake pedal, and you roll your foot to touch the gas pedal to raise the revs to match when we downshift and I actually go to do it and my foot won't move," he said.</span></p><p><span>Dr. Cody proposed that Hutchings have reconstruction surgery."His deformity was so bad, I had to fuse some of the bones together. Ellis, MD</a>, foot and ankle surgeon at HSS about results of a study that indicate the cervical ligament in the subtalar joint may need further examination in foot reconstruction surgery, especially in patients with flatfoot deformity.&nbsp;</p><p>“Basically, what we found is that the flatfoot patients had compromise of the spring ligament more than our control patients and the worse the cervical and spring ligaments became, the worse deformed the foot was, particularly in abduction,” Dr. Ellis said about the study, which was a Roger A. Mann award finalist at the American Orthopaedic Foot and Ankle Society Annual Meeting. &nbsp;</p><p>“The take-home is that, going forward, we're going to have to start looking at the cervical ligament,” Dr. Ellis added. We need to study it more, and we're going to need to develop techniques to recreate or reconstruct that ligament as part of our flatfoot reconstructive surgeries. Blanco, MD</p>]]></description><content:encoded><![CDATA[<p style="margin-left:auto;text-align:start;">Health Day highlights tips for buying back to school shoes for your kids.&nbsp;</p><p style="margin-left:auto;text-align:start;">Buying back-to-school shoes should be more about the right fit than the right look — but with luck you can combine the two.</p><p style="margin-left:auto;text-align:start;">Shoes play a big part in how kids' feet function and should be selected with care.</p><p style="margin-left:auto;text-align:start;">"When parents bring their children in with foot pain or an injury, the first thing I do is ask about their shoes. The problem is often inappropriate or poorly fitting footwear," said <a href="https://www.hss.edu/physicians_blanco-john.asp" target="_blank">John S. Blanco, MD,</a> pediatric orthopedic surgeon at HSS.&nbsp;</p><p style="margin-left:auto;text-align:start;">"Footwear has a lot to do with how their feet function day to day, especially during athletic activities," said Dr. Blanco.&nbsp;</p><p style="margin-left:auto;text-align:start;">"If we could make sure shoes were the right size, had proper support and were laced up appropriately, we would probably solve 80% of the basic foot problems we see in kids," explained Dr. Blanco.</p><p style="margin-left:auto;text-align:start;">Shopping for shoes at a specialty store can be helpful because staff there can measure the foot and help select the best shoe for a child's foot type, said Dr. Blanco. That doesn't mean the shoes need to cost a lot.</p><p style="margin-left:auto;text-align:start;">"The price of the sneaker has nothing to do with the quality of the shoe," Dr. Blanco explained. Levine, MD</a>, to shed light on the typical prognosis and recovery from a Lisfranc injury, in wake of Oklahoma City Thunder NBA player Chet Holmgren's foot surgery.</p><p>According to Dr. Levine, who did not treat Holmgren, “A Lisfranc injury is a sprain (tear) of the ligaments that tightly hold the multiple Midfoot bones together. Normally, the Midfoot is a stable, immobile part of the foot when compared to the mobile/flexible Hindfoot (Ankle) or the Forefoot (ball of the foot/toes). If this injury is a minor/low-grade sprain, the Midfoot remains stable and surgery is not necessary. However, when the injury is a high-grade sprain, the stability of the Midfoot has been compromised. These injuries do not do well left alone and surgical treatment is necessary to restore the normal Midfoot stability.”</p><p>He explained, “While there are still those Lisfranc injuries that occur in car accidents and falls from a height, more and more these injuries are seen in sports. Classically, an athlete is running, jumping, leaping or landing and comes down ‘awkwardly’ (on someone else’s foot, for example). The Midfoot ‘buckles’ tearing the Lisfranc ligaments. The result in an ‘unstable’ Midfoot which is painful and unable to bear weight.”</p><p>Dr. Levine added, “The risk to future performance is the development of arthritis in the Midfoot. Midfoot arthritis causes pain and an inability to efficiently transmit power from the muscles of the leg to the ball of the foot for push-off.<span>&nbsp; </span>This can manifest as a negative impact on quickness. Elliott, MD</p>]]></description><content:encoded><![CDATA[<p><i>The Boston Globe </i>reports on the Lisfranc injury of NBA player Chet Holmgren, who plays for Oklahoma City Thunder, and includes commentary from HSS foot and ankle surgeon <a href="https://www.hss.edu/physicians_elliott-andrew.asp">Andrew J. Elliott, MD</a>, who did not treat Holmgren.</p><p>According to Dr. Elliot, the injury is significant, occurring when the mid-foot ligaments are torn or ruptured when the foot is pulled in an awkward direction.</p><p>He continued, “The foot has to, in order for running and pushing off and jumping, has to act like a rigid structure. In acting like a rigid structure to hold all these bones together, the ones in the mid-arch region are very, very strong. There are three different things that can happen. One, the ligament can pull off the bone, and the second one is when the ligament tears right in the middle, or you can get fractures as well as tearing of the ligaments.”</p><p>Dr. Elliott said the Thunder aren’t being overly cautious by holding Holmgren out for the entire season, even though the season doesn’t begin for nearly two months.</p><p>“In basketball, I would equate them more as a wide receiver,” he said. “They’re doing a lot of running on a hard surface. And you’re probably looking at the earliest, depending on which joint was injured, six months, but more likely between nine to 12 months.</p><p>“The basic premise is you’re rupturing the ligament because the ligament is seeing a load to it that’s higher than what it can handle,” he added.</p><p>Dr. Elliott said he watched the play during which Holmgren was injured on video and believed it was caused by all of Holmgren’s weight landing on his foot as he tried to defend LeBron James.</p><p>“He’s done that a million times, but obviously this had a little bit more weight,” Dr. Elliott explained. “And it was a different vector of force that was going through it than he normally had seen. That’s the same for most people. Positano, DPM, MSc, MPH, DSc</a>, director of the Non-surgical Foot and Ankle Service at HSS.<br><br>Dr. Positano explained he has seen an increase in foot pain – nearly 20 to 30 percent — and calls the phenomenon “pandemic foot.”<br><br>Some of the most common foot ailments occur simply because the foot is under increased strain during the pandemic. He noted, “People don’t realize how much mileage they put on walking and standing in their houses.”<br><br>If foot injuries are not addressed they have the potential to result in other pain. “People think they are falling apart, but they are not,” he added. “They are overusing their feet.”<br><br>According to Dr. Positano, heel pain should get checked by a doctor before stretching. Ellis, MD</a>, finding most people who have had surgery to correct progressive collapsing foot deformity are able to return to the activities they enjoy, but it may take a long time.</p><p>Dr. Ellis said, "[It's] a debilitating condition marked by several progressive deformities that require a combination of extensive reconstructive procedures, from cutting and reshaping bones in the foot to rerouting tendons in the fallen arch.”</p><p>He added, "Although patients can expect to eventually resume at least light to moderate physical activity, the recovery period can take many months to a year or more, and some of the most vigorous sports may not be possible.”</p><p>HSS clinical research scientist <strong>Robert Fuller</strong> said, "We think this study gives us the ability to say to a patient, 'If you're interested in doing yoga again, you can expect to return in about six to nine months, and be back to full form in about a year.' The institution is collaborating with medical centers and other organizations to advance the quality and value of musculoskeletal care and to make world-class HSS care more widely accessible nationally and internationally. </span><a href="http://www.hss.edu"><span>www.hss.edu</span></a><span>.</span></p>]]></pp:boilerplate><description><![CDATA[<p><span>Many patients who undergo surgery to correct a collapsing foot share the question: Will I be able to get back to doing the things I love?</span></p><p><span>A new study by researchers at Hospital for Special Surgery (HSS) in New York City provides attainable goals for return to sports and related physical activities for patients who have undergone procedures to correct progressive collapsing foot deformity (PCFD), a severe musculoskeletal pathology that can drastically inhibit a patient’s daily function. The findings were presented today at the American Academy of Orthopaedic Surgeons (AAOS) 2022 Annual Meeting.</span></p><p><span>“This procedure is the epitome of big surgery,” said </span><a href="https://www.hss.edu/physicians_ellis-scott.asp"><span>Scott J. Ellis, MD</span></a><span>, an orthopaedic foot and ankle surgeon at HSS and the senior researcher on the study. “It is associated with a long recovery period, and patients are quite immobile even before they reach the operating room. As a result, even surgeons want to know: Is this going to be worth it?”</span></p><p><span>Dr. Ellis added, “PCFD is a debilitating condition marked by several progressive deformities that require a combination of extensive reconstructive procedures, from cutting and reshaping bones in the foot to rerouting tendons in the fallen arch. Although patients can expect to eventually resume at least light to moderate physical activity, the recovery period can take many months to a year or more, and some of the most vigorous sports may not be possible.”</span></p><p><span>This study is the first attempt to quantify the recovery period for individual activities. The HSS research team, led by clinical research scientist Robert Fuller, analyzed patient-reported data from 82 patients, aged 18 to 60 years, who had undergone reconstructive foot surgery at the hospital between February 2016 and May 2019. About three-fourths of the patients were women, and the average time since surgery was approximately 3 years (range, 2 to 5.4 years).</span></p><p><span>Patients were asked about their ability to return to 21 physical activities, such as yoga, biking, walking and running, as well as their overall level of function, pain and other outcomes using the HSS Orthopaedic Foot and Ankle Registry, a rich repository of data that helps clinicians and researchers better guide treatment decisions.&nbsp;&nbsp;</span></p><p><span>According to the findings, patients said that half of the 21 activities had become easier for them to perform after surgery, while 33% experienced no difference and 17% said the activities had become more challenging. Improvements in physical function (</span><i><span>P</span></i><span>=0021), pain (</span><i><span>P</span></i><span>=0.001) and global physical health (</span><i><span>P</span></i><span>=0.001) were associated with statistically significant increases in patient satisfaction with respect to sports and physical activities.</span></p><p><span>No patients surveyed said they had stopped doing activities they had performed before surgery, but many said they had taken up new activities—mainly low-impact pursuits such as walking, biking and swimming. Overall, 74 of the 82 patients (90%) reported being somewhat to very satisfied with their ability to be active, while just eight (10%) said they were unsatisfied with the results of the surgery in that regard.</span></p><p><span>The most exciting part of the study, Fuller said, was that the researchers were able to collect enough data on individual physical activities that they can now advise patients not only on the types of activities they can expect to carry out, but also on the length of time they can expect to be off their feet after surgery.</span></p><p><span>“We think this study gives us the ability to say to a patient, ‘If you're interested in doing yoga again, you can expect to return in about six to nine months and be back to full form in about a year,’” Fuller said. “It can help provide clarity for a lot of people showing these surgeries can be life-changing in terms of getting them active again, but a substantial amount of people will face some extended recovery.” Indeed, the researchers found that 22% of patients said they never reached their maximum preoperative level of participation in physical activities, and 26% said the difficulties they experienced in performing activities had increased after surgery.</span></p><p><span>Not surprisingly, Fuller said, people who undergo foot reconstruction can resume low-impact activities—faster than those who practice more demanding sports such as dancing, jogging, skiing or basketball. However, “we observed great progress in both high- and low-impact activities,” he added.</span></p><p><span>Although the new research highlights the effectiveness of the reconstructive procedure, Fuller and Dr. Ellis said more work is needed to help standardize good outcomes for patients. At HSS, Dr. Ellis and his colleagues are using new technologies such as weight-bearing CT scanning to help identify which bones in the affected foot are collapsing and which ligaments are failing to better understand the anatomy of the deformities and optimize the surgical plan. They also are incorporating a computer modeling technique called finite element analysis to simulate how the foot will appear after the bones and other tissues are cut.</span></p><p><span>&nbsp;“We plan to use the data from this study to provide patients with realistic expectations and timelines regarding their return to sports activities after collapsing flatfoot reconstruction, and as a resource for patients at the time when surgery is being discussed,” Dr. Ellis concluded.</span></p>]]></description><category><![CDATA[pressrelease,Ellis,Foot and Ankle,Foot,Research Clinical,flatfoot]]></category>

There is debate as to which is better. However, ORIF is generally done for lesser and more ligamentous injuries whereas fusion is indicated in injuries with bone fractures and more severe ligamentous injuries.”<br><br>Dr. Ellis noted on a typical return, “The timetable can vary depending on the severity of injury, with worse injuries and those requiring fusions probably taking longer to recover. With ORIF and screw removal, it is possible that players could return to play around 5 to 6 months after the injury. However, the average return to play demonstrated in studies is 10 to 11 months which I think is a more realistic timetable.”<br><br>He added, “If the bone heals well and in the correct position, Hill probably can make a return. Studies show that roughly 80 to 90 percent of NFL players will return to full competition after a Lisfranc injury. The condition, which affects at least 3% of women over age 40 and more than 10% of people over age 65, results from weakening, stretching or other alterations to the tendons and ligaments in the foot and can lead to severe pain and significantly restricted mobility. Women are more prone to the problem than men; and being overweight is considered an increased risk factor.</span></span></span></p><p><span><span><span>A mix of stretching, physical therapy and the use of orthotic devices may help people with milder cases. But for the more severe cases of PCFD, surgeons can operate to reshape and restore function to the affected extremity.</span></span></span></p><p><span><span><span>&ldquo;Once the foot starts collapsing, it never uncollapses, and the worry is that over time the ligaments will continue to stretch, the foot will flatten, and the pain will increase,&rdquo; said <a href="https://www.hss.edu/physicians_ellis-scott.asp" style="text-decoration:underline">Scott J. Although nonsurgical or conservative management appears to help prevent the progression of PCFD, Dr. Ellis noted that hasn&rsquo;t been conclusively proven. &ldquo;Roughly half of patients will need surgery. If the foot continues to hurt and limits quality of life and function, and there&rsquo;s pain from walking, travel and other activities, then we say it&rsquo;s time to perform surgery.&rdquo;</span></span></span></p><p><span><span><span>Determining which patients stand to benefit most from foot reconstruction &mdash; and who would do better with more conservative management &mdash; has been unclear.</span></span></span></p><p><span><span><span>For the new study, Dr. Ellis and his colleagues sought to determine if instruments from the Patient-Reported Outcomes Measurement Information System (PROMIS) could help physicians predict which patients would experience the most improvement from foot surgery. PROMIS has multiple components, including scores for physical function, pain scale and depression, and is considered an objective measure of a patient&rsquo;s physical and mental health specific to a given condition.</span></span></span></p><p><span><span><span>The HSS team, including <strong>Matt Conti, MD</strong>, a chief resident and the first author on the paper, identified 71 men and women (75 feet in total) who underwent surgery for PCFD between February 2016 and April 2018, and for whom PROMIS data were also available. The average follow-up time for the patients was roughly 27 months.</span></span></span></p><p><span><span><span>Patients with the lowest PROMIS scores for physical function (PROMIS-PF) before surgery &mdash; indicating the most severe impairment &mdash; were most likely to experience clinically meaningful improvements two years after the procedure, according to the researchers. Those with a PROMIS-PF score of 45.7 had roughly a 14% chance of improvement, while patients with a score below 40.9 had nearly a 98% chance of clinical gains.</span></span></span></p><p><span><span><span>Armed with the new information, surgeons now have a more objective way of informing patients of their chances of a successful operation. &ldquo;When a patient walks in the door, you can get an idea of where they stand on that scale, and we know based on our study where patients need to land to achieve clinically meaningful improvements from the surgery,&rdquo; Dr. Ellis said. &ldquo;It could be a real-time decision with the patient and something they could follow over time to allow them to see the trajectory of their recovery and where they are heading.&rdquo;</span></span></span></p><p><span><span><span>Every patient scheduled for surgery at HSS completes the PROMIS survey, and the results become part of the electronic medical record. Metzl, MD</a>, sports medicine physician at HSS.</span></span></p><p><span><span>Dr. Metzl said that plantar fasciitis sufferers typically feel a sharp stab or deep ache in the heel or along the arch of the foot.</span></span></p><p><span><span>Plantar fasciitis-related heel pain tends to strike those who overtrain, neglect to stretch their calf muscles, or overdo hill workouts and speed intervals. &ldquo;When you have very tight calf muscles, they will pull on the plantar fascia and cause a lot of pain,&rdquo; noted Dr. Metzl.</span></span></p><p><span><span>According to Dr. Metzl it is critical to address plantar fasciitis as soon as there is pain. &ldquo;When the fascia comes off the bone, it gets chronically inflamed and can heal exceptionally slowly. Plantar fasciitis can be sore for months because the healing response is proportionate to blood flow. This can cause inflammation which leads to pain in the in the heel and Achilles tendon. Dr. Quirolgico explained, “Over time, if this keeps on happening, microtears in the plantar fascia can occur, and as it heals, each time the tissue becomes thicker and stiffer.”</span></p><p><span>Flat feet, higher arches, sudden weight gain and some exercises can put people at higher risk for plantar fasciitis. Dr. Quirolgico said possible causes could be running too far or too fast without building up your mileage or speed, and doing repetitive movements, like running on a hard surface and in unsupportive shoes.</span></p><p><span>“The first lines of treatment [for plantar fasciitis] include calf stretching, massage at the bottom of the foot or arch, over-the-counter anti-inflammatories like ibuprofen, and wearing gel cups for cushioning of the heel,” suggested Dr. Quirolgico.</span></p><p><span>Dr. Quirolgico recommended stretches and massage to help alleviate pain and tightness in the plantar fascia. For a massage she advised, “Sit up tall in a chair. Place a frozen water bottle in front of you. Place a towel over the bottle so the cold doesn’t harm your skin. <p>RunnersWorld.com spoke to <a href="https://www.hss.edu/physicians_metzl-jordan.asp">Jordan D. Metzl, MD</a>, primary care sports medicine physician at HSS, who detailed what runners should know about stress fractures. &ldquo;Like almost every overuse injury, the worse the symptoms get and the more you try to run through the pain, the worse you&rsquo;ll make this injury,&rdquo; explained Dr. Metzl. Dr. Metzl cited there are three ways to identify a stress fracture, which include point tenderness, swelling, and changes in your running form. MRIs are typically done to definitively diagnose a stress fracture, and you may want to stay off the bone to give it enough time to heal.</p>

<p>Regarding stress fracture prevention, Dr. Metzl advised adding mileage gradually to long runs to prevent injuries, shortening running stride and quickening the cadence.</p>

Positano, DPM, MSc, MPH</a>, director of the non-surgical Foot and Ankle Service at HSS, who explained, &ldquo;[Joe] DiMaggio&rsquo;s&nbsp;injury and a failed surgery during his playing career were the big impetuses for non-surgical musculoskeletal foot and ankle interventions in this country.&rdquo; Dr. Positano advised those who are experiencing foot pain to look for an expert that is affiliated with a podiatry or medical school and who specializes in nonsurgical interventions.</p>

<p>Five solutions provided that can build "happy feet" include finding shoes that fit appropriately; exercises that maintain foot function; maintaining a diet inclusive of omega-3 and omega-6 fatty acids; walking around barefoot at home; and rolling the foot on a lacrosse or golf ball for five minutes a day to provide the stimulation your foot requires.</p>
