                        <title>Biologic Therapy Significantly Improves Pregnancy Outcomes in Women with Antiphospholipid Syndrome at High Risk for Serious Complications</title>

The institution is collaborating with medical centers and other organizations to advance the quality and value of musculoskeletal care and to make world-class HSS care more widely accessible nationally and internationally. </span><a href="http://www.hss.edu"><span>www.hss.edu</span></a><span>.</span></p>]]></pp:boilerplate><description><![CDATA[<p>A landmark clinical trial co-led by Hospital for Special Surgery (HSS) has found that blocking inflammation with the drug certolizumab significantly reduces the risk of serious adverse pregnancy outcomes in women with antiphospholipid syndrome (APS).&nbsp;<br><br>The phase 2 IMPACT Trial (IMProve Pregnancy in APS with Certolizumab Therapy) was co-led by <a href="https://www.hss.edu/physicians_salmon-jane.asp" target="_blank">Jane E. Salmon, MD</a>, a rheumatologist and the Collette Kean Research Chair at HSS and D. Ware Branch, MD, an obstetrician/gynecologist at University of Utah Health. It is the first clinical trial to evaluate a biologic therapy to prevent serious adverse outcomes in pregnant women with APS and their babies. The study results were published online in <a href="https://www.sciencedirect.com/science/article/abs/pii/S0003496725002432" target="_blank">Annals of the Rheumatic Disease</a>s.&nbsp;<br><br>“The IMPACT study represents a bold and very successful partnership over many years between government, industry, foundations, and academic health research institutions,” said Dr. Salmon, the senior study author. “It is exciting to see our preclinical work in the laboratory, which began more than a decade ago, translate into such promising results for patients. I hope our findings will allow the drug to become widely available so that more pregnant women with APS and high-risk pregnancies can benefit.”&nbsp;<br><br>APS is a rare autoimmune disorder affecting up to 0.05% of people. It is frequently associated with lupus, a disease most prevalent in women during their reproductive years. In APS, the body produces autoantibodies that react with blood vessels and circulating cells, causing dangerous blood clots throughout the body, which can lead to strokes, heart attacks, and phlebitis. During pregnancy, APS raises the risk of serious complications arising from issues with the placenta, including fetal death, preeclampsia, and restricted fetal growth.&nbsp;<br><br>Previous research led by Dr. Salmon identified lupus anticoagulant (LA), an autoantibody produced by some APS patients, as the strongest predictor of poor pregnancy outcomes in APS patients. Historically, 39% to 86% of pregnant women with APS who are LA positive have faced severe complications, despite treatment with standard-of-care blood thinners like heparin and aspirin. These patients have the greatest need for better treatments.&nbsp;<br><br>Dr. Salmon’s preclinical research in experimental models showed that inflammation in the developing placenta, not blood clots, was the main cause of pregnancy complications in APS. This pivotal discovery led her team to investigate whether TNF-alpha inhibitors, a class of drugs used to treat inflammatory conditions like rheumatoid arthritis, Crohn’s disease and plaque psoriasis, could offer better protection to APS pregnancies than blood thinners alone.&nbsp;<br><br>The IMPACT trial enrolled 51 pregnant women ages 18 to 40 with high-risk APS, defined as positive for lupus anticoagulant. All participants were treated with certolizumab, a TNF-alpha inhibitor that does not cross the placenta, in addition to standard-of-care heparin and aspirin, starting at week eight of pregnancy. Medication was stopped at week 28, the point at which the placenta is well-developed. The unique design of the IMPACT study made it possible for pregnant patients from 16 states and one Canadian province to participate.&nbsp;<br><br>The 20% complication rate in patients treated with certolizumab was dramatically lower than their prior pregnancies in which 69% to 79% had severe adverse outcomes, despite standard treatment with heparin and aspirin. The average gestational of age at delivery was 36.5 weeks in certolizumab-treated pregnancies, compared to 24 weeks in patients’ prior pregnancies. Among IMPACT participants, 93% brought home a healthy baby—a remarkable improvement over the 38% survival rate for their previous pregnancies. Notably, none experienced serious infections or lupus flares, a concern when the study began.&nbsp;<br><br>The trial supports the concept derived from basic laboratory studies that targeting inflammation, rather than blood clotting, is effective in preventing pregnancy complications in high-risk pregnant patients with APS. It also shows the power of collaboration between rheumatologists and obstetricians.&nbsp;<br><br>“Our study heralds a new era for trials with biologics to prevent adverse pregnancy outcomes. We have shown that it is possible to gain the confidence of regulators and the trust of pregnant women who will enroll in clinical trials,” said Dr. Salmon. “We are grateful to the patients and their care providers, who were committed partners in this trial, and to our funders who watched our trial with anticipation and excitement,” said Dr. Salmon.&nbsp;<br><br>“These results also open a window for studying whether TNF-alpha blockade could help prevent preeclampsia in women without autoimmune disorders,” she added. “Preeclampsia is the most common cause of morbidity and mortality of pregnant women and their babies, and there are currently no effective therapies.”&nbsp;<br><br>The study was supported by funding from the National Institutes of Health, the Lupus Foundation of America, the Morris and Alma Schapiro Fund, the James R. and Jo Scott Research Endowment at the University of Utah and UCB Inc., the maker of certolizumab (Cimzia).&nbsp;<br>&nbsp;</p>]]></description><category><![CDATA[pressrelease,Salmon,Rheumatology,APS,antiphospholipid-syndrome]]></category>

APS ACTION conducts large, multicenter clinical studies and trials in patients with positive antiphospholipid antibodies (aPL).&nbsp;<br><br>APS is a systemic autoimmune disorder characterized by the production of aPL, which attack proteins binding the cell walls of blood cells located on the inner layer of veins and arteries. APS increases the risk of dangerous blood clots, strokes, heart attacks and pregnancy complications.&nbsp;<br><br>APS ACTION created the APS ACTION Clinical Database and Repository (APS ACTION Registry) to study the natural course of disease in patients with persistently positive aPL, with or without other systemic autoimmune diseases. Investigators worldwide collect blood samples and clinical data annually and follow patients for 10 years. As of November 2023, the registry included samples and data for about 1,200 patients.&nbsp;<br><br>“Since the registry began in 2010, APS ACTION investigators have published 28 studies in peer-reviewed journals and delivered 50 podium and poster presentations at international congresses,” said <a href="https://www.hss.edu/physicians_erkan-doruk.asp" target="_blank">Doruk Erkan, MD, MPH</a>, a physician-scientist at the Barbara Volker Center for Women and Rheumatic Diseases and rheumatologist at HSS. Dr. Erkan is also a founding member of APS ACTION and the current chair of its Executive Committee. “It’s exciting to see six new studies presented at ACR Convergence 2023 that advance our understanding of APS.”&nbsp;<br><br>These six studies were based on the analysis of registry clinical data alone, registry clinical data in combination with patient samples and efforts of APS ACTION International Core Laboratories to improve the reliability of aPL tests. Highlights are as follows:&nbsp;<br><br>Three Studies Based on APS ACTION Registry Clinical Data&nbsp;<br><br><strong>First and Recurrent Thrombosis Risk after 4,454 Patient-Years of Follow-Up&nbsp;</strong><br><br>The study updated the incident first and recurrent thrombosis risk, previously reported in 2021 as 1.02 and 2.09 per 100 patient-years, respectively. Based on 4,454 patient-years of follow-up, the risk of first and recurrent thrombosis events remained relatively low at 1.00 and 2.13 per 100 patient-years in aPL-positive patients with and without a history of thrombosis, respectively. A secondary analysis revealed potential differences in medications and cardiovascular risk factors at enrollment between patients with and without thrombosis at follow-up. The researchers concluded these findings should be interpreted cautiously, given the multifactorial nature of thrombosis and the relatively small number of new events during follow-up. New APS ACTION studies are already in progress to better define independent thrombosis risk and protective factors in this patient population.&nbsp;<br><br>HSS authors: Jonathan Thaler, MD (presenting), Doruk Erkan, MD, MPH (senior).&nbsp;<br>Complete author list: Refer to ACR Convergence 2023 Abstract 1604 (podium presentation).&nbsp;<br><br><strong>Clinical Characteristics of Patients Presenting with Transient Ischemic Attack&nbsp;</strong><br><br>Transient ischemic attack (TIA) can occur in aPL-positive patients. However, diagnosing TIA is challenging due to the need to rule out other conditions, such as complex migraine with aura or seizure. This study analyzed the clinical characteristics of persistently aPL-positive patients with reported TIA before enrolling in the registry and/or during follow-up. The investigators retrospectively compared patients’ baseline characteristics, including those with and without a history of thrombosis before they joined the registry. Next, the researchers prospectively assessed patients with new onset TIA. The study found that 8% of patients had a history of TIA at registry entry and the recurrence rate during follow-up was 8%. However, the new TIA rate was less than 1%. About two-thirds of TIA patients without a history of thrombosis were treated with anticoagulants, and all patients with new or recurrent TIA during follow-up were on anticoagulation. The researchers analyzed reported causes of death and compared the clinical and laboratory characteristics of patients who died with patients still living. The study found that the mortality rate among 963 patients was 5% after a median follow-up of five years. The five-year survival probability declined with age and was lowest for patients over 60 when they joined the registry. A history of arterial thrombosis, catastrophic APS, cardiovascular disease risk factors and co-existing systematic autoimmune diseases independently predicted future mortality. The investigators emphasized that a better understanding of the predictors of mortality is critical to help tailor treatment plans to improve patient outcomes.&nbsp;<br><br><br>Presenting author: Yasaman Ahmadzadeh, MD (Roger Williams Medical Center).&nbsp;<br>HSS author: Doruk Erkan, MD, MPH (senior author).&nbsp;<br>Complete author list: Refer to ACR Convergence 2023 Abstract 0098 (poster presentation).&nbsp;<br><br>Two Studies Based on APS ACTION Registry Clinical Data and Patient Samples&nbsp;<br><br><strong>Complement Activation as a Marker of Thrombosis Risk&nbsp;</strong><br><br>Recent studies suggest additional markers of thrombosis risk are present in patients with severe APS, such as catastrophic APS (CAPS), a life-threatening condition that involves the formation of multiple blood clots in a very short period. The study evaluated plasma levels of three complement activation markers—C5b-9, C4d and Bb fragment—using ELISA and the modified HAM assay that measures complement-dependent cell killing. The investigators compared results for patients who experienced thrombosis with those who did not over a median prospective follow-up time of 4.6 years. In general, the investigators found that elevated C4d levels and a positive modified HAM result were associated with a higher risk of new thrombosis. They concluded these complement markers might be useful tools for stratifying patients by risk.&nbsp;<br><br>Presenting author: Cecile Yelnik, PhD (Lille University).&nbsp;<br>HSS authors: Jane Salmon, MD, Doruk Erkan, MD, MPH (senior author).&nbsp;<br>Complete author list: Refer to ACR Convergence 2023 Abstract 1605 (podium presentation).&nbsp;<br><br><strong>Plasma Proteomic Profiling of Different APS Phenotypes&nbsp;</strong><br><br>APS is a heterogeneous autoimmune disease with complications arising from interaction between the innate immune system and coagulation. Patients may present with different phenotypes, such as thrombotic, obstetric or catastrophic/microvascular APS, while others may be aPL-positive without the presence of disease. To better understand the mechanisms driving immunothrombotic events, the investigators performed multiplex proteomic profiling of about 7,000 unique proteins found in plasma samples of 40 patients and compared results with 10 healthy controls. The plasma proteome of APS subtypes revealed alteration in several pathways, particularly receptor signaling, signal transduction, regulation of cellular differentiation, neutrophil, complement, coagulation and cytokine activation notable in all individuals with aPL-positivity. However, variability in test results is challenging, especially in anticoagulated samples. The two commonly used tests as part of LA are the activated partial thromboplastin time and dilute Russell’s viper venom time (DRVVT). Based on a standardized protocol, four APS ACTION Core Laboratories assessed the performance of the LA test using the prothrombin-activating Taipan snake venom time (TVST) test and the Ecarin clotting time (ECT) confirmatory test, which are insensitive to vitamin K antagonists. The investigators compared the agreement in LA status between the DRVVT and the TVST/ECT. The results were presented today in a poster session at American College of Rheumatology (ACR) Convergence 2023, the ACR’s annual meeting.<br><br>“There are a lot of unanswered questions about how APS affects pediatric patients since evidence to date has been very limited,” said <strong>Jheel Pandya, MD,</strong> a pediatric rheumatology fellow at HSS and lead author of the research. “Our study reveals that an initial high-risk antiphospholipid antibody profile is unlikely to be transient in pediatric patients, indicating their risk of serious health problems remains elevated and they should be followed carefully.”&nbsp;<br><br>APS is a rare autoimmune disorder in which antibodies made by the immune system, called antiphospholipid antibodies (aPL), attack proteins that bind the phospholipid cell walls of blood cells located on the inner layer of arteries and veins. The disorder increases the risk of dangerous blood clots inside blood vessels, strokes, and heart attacks, and can cause pregnancy problems.&nbsp;<br><br>For patients with symptoms, rheumatologists use three laboratory tests to evaluate the presence of aPL: lupus anticoagulant (LA), anticardiolipin antibody (aCL) and anti-beta-2-glycoprotein-1 antibody (aβ2GPI). An international team of rheumatologists, including HSS investigators, recently published new criteria for classifying adult APS patients for research purposes using these tests,2 but there are not yet established criteria for classifying pediatric patients.&nbsp;<br><br>Dr. Pandya and colleagues analyzed electronic medical records for aPL-positive patients ages 10 to 18 who were treated at HSS between 2016 and 2022. They divided patients with initially positive results into high- and low-risk groups as follows: those with a positive LA test and/or higher level aCL and aβ2GPI antibodies into the high-risk group; and those with a negative LA test and/or lower levels of aCL and aβ2GPI antibodies into the low-risk group. The investigators assessed subsequent aPL results, as well as the demographic and clinical characteristics of patients with persistently positive aPL results reported at least 12 weeks apart.&nbsp;<br><br>The analysis revealed that aPL persisted for 25 of 27 patients (93%) in the higher-risk group. By comparison, aPL persisted for only seven of 15 patients (47%) in the lower-risk group.&nbsp;<br><br>Among 32 patients in either group with aPL results that remained positive over time, blood clots occurred in 9 (28%) patients in the high-risk group, while none occurred in the low-risk group. For 26 patients who did not have persistently positive antibody results, none experienced blood clots, one had a skin rash, and two experienced migraines.&nbsp;<br><br>“The more we can learn about differences in APS development and progression in pediatric patients compared with adults, the more we can optimize their diagnosis and treatment,” said co-author <a href="https://www.hss.edu/physicians_onel-karen.asp" target="_blank">Karen Brandt Onel, MD</a>, chief of Pediatric Rheumatology at HSS. “This study underscores the importance of continuing to follow pediatric patients with higher-risk profiles and transitioning them to adult care after the age of 18.”&nbsp;<br><br>“Our results highlight the need for a large-scale, international effort to better understand how APS affects pediatric patients,” said senior author <a href="https://www.hss.edu/physicians_erkan-doruk.asp" target="_blank">Doruk Erkan, MD, MPH</a>, a physician-scientist at the Barbara Volker Center for Women and Rheumatic Diseases, attending rheumatologist at HSS and professor of medicine at Weill Cornell Medicine. “An international effort has been initiated to pool pediatric APS data and ultimately guide the development of classification criteria for pediatric patients.” Dr. Erkan was co-principal investigator for the recently published 2023 ACR/EULAR Adult APS Classification Criteria.2&nbsp;<br><br>HSS is one of the leading centers in the United States providing care for adults and children with APS, with a team of 18 rheumatologists who see APS patients, including five who see pediatric patients. Patients accumulating at least 3 points each from the clinical and laboratory domains are classified as having APS. Salmon, MD,</span></a><span> is conducting a groundbreaking, interventional trial funded by the NIH and Lupus Foundation of America (LFA) to help patients with lupus and antiphospholipid syndrome (APS) (often seen in lupus patients) have successful pregnancies. Women with lupus and APS are at increased risk for preeclampsia and other serious pregnancy complications. The IMPACT clinical trial is the first trial to use a biologic therapy to prevent serious adverse outcomes in high-risk pregnancies.</span></p><p><span>Dr. Salmon and her team completed an observational study of 775 pregnancies to determine the strongest predictors of pregnancy complications in patients with lupus and/or APS. The presence of certain autoantibodies in women with APS with or without lupus is associated with a nearly 50% risk for serious adverse pregnancy outcomes. The IMPACT trial will determine whether treatment with Certolizumab, a monoclonal anti-TNF antibody that blocks inflammation, will prevent serious pregnancy complications. Studies in experimental animal models show that blocking this inflammatory mediator prevents preeclampsia, fetal death and fetal growth restriction – all problems seen in patients with lupus and with APS.</span></p><p><span>The grants from the NIH and Lupus Foundation of America come at a pivotal time in the study as they have recently enrolled 40 patients, and, so far, they are seeing promising results. The trial is spearheaded by the HSS Research Institute and University of Utah, where Dr. David Ware Branch serves study Co-Principal Investigator. &nbsp;Patients across the nation are eligible to enroll in the trial. Detailed results and consensus discussions will be presented in future manuscripts.</span></span></p><p style="text-align:start;">“Today we present our international, multidisciplinary effort to develop new, rigorous, consensus- and evidence-based, highly specific classification criteria to identify patients with high likelihood of APS for clinical research,” said Dr. Barbhaiya.</p><p style="text-align:start;">Dr. Barbhaiya, Dr. Erkan, and Stéphane Zuily, MD, PhD, p<span style="background-color:rgb(255,255,255);"><span style="text-align:start;">rofessor of medicine, at Lorraine University, France,</span></span> presented the methodology, novel aspects, and application of the draft criteria on Sunday, 11/13.&nbsp;</p><p style="text-align:start;">“The goal for classification is to identify a well-defined study sample representative of the majority,” said Dr. Barbhaiya. “We tried to achieve high specificity, sometimes at the cost of sensitivity.”</p><p style="text-align:start;">The methodology involved four phases—item generation, item reduction, weighting of items and threshold identification, and validation—each balancing expert-based and data-driven processes.</p><p style="text-align:start;">Dr. Barbhaiya listed several strengths of the methodology, such as rigorous clinical and laboratory criteria definitions and international cases, and the inclusion of international cases that captured the APS spectrum, reducing the risk of selection bias and increasing generalizability.</p><p><span>Dr. Erkan demonstrated the draft criteria in action, starting with the entry criteria of at least one documented clinical criterion plus a positive aPL test within three years of the clinical criterion. But he offered an important reminder: “Do not count a criterion if there is an equally or more likely explanation than APS. Within each domain, only count the highest weighted criterion toward the total score.”</span></p><p><span>Dr. Erkan also highlighted the point system for domains presented by Dr. Zuily, noting that the final classification criteria manuscript includes specific definitions and instructions.</span></p><p style="text-align:start;">If a case does not meet the APS classification criteria, the case may still be uncertain or equivocal rather than “not APS,” Dr. Erkan said. Each year, the ACR and ARP recognize members who have made outstanding contributions to the field of rheumatology. The ceremony took place at the ACR Convergence meeting in Philadelphia on November 12.</span></p><p><a href="https://www.hss.edu/physicians_lockshin-michael.asp"><span>Michael Lockshin, MD</span></a><span>, received the ACR Distinguished Clinician Scholar Award, which honors a rheumatologist who has made outstanding contributions in clinical medicine, clinical scholarship or education.</span></p><p><a href="https://www.hss.edu/physicians_smith-melanie.asp"><span>Melanie Smith, MD, PhD</span></a><span>, received the ACR Distinguished Fellow Award, which recognizes clinical and research fellows who are in a rheumatology fellowship training program and have performed meritoriously.</span></p><p><span><strong>Adena Batterman, LCSW, MSW</strong>, received the ARP Distinguished Clinician Award. It is presented to an ARP member who is engaged in clinical practice and demonstrates outstanding clinical expertise in arthritis and the rheumatic diseases.</span></p><p><span>"HSS is extremely proud of our team members who have been honored with these notable awards,” said <strong>Louis A. Shapiro</strong>, president and CEO of HSS. “We warmly congratulate them for their contributions and achievements in patient care, research, support and education.”</span></p><p><span><strong>Michael Lockshin, MD</strong></span></p><p><span>Dr. Lockshin’s award follows a career spanning more than four decades at HSS. When asked about receiving the honor, he said, “I think it honors those who surrounded and challenged me more than it does me—patients who asked hard questions; the team led by Charles Christian that trained me and with whom I was privileged to work; intellectually curious colleagues, students and fellows; Hospital for Special Surgery, which gave me freedom to pursue goals I thought were worthwhile; and my wonderful family.”</span></p><p><span>Director emeritus of the Barbara Volcker Center for Women and Rheumatic Disease at HSS, Dr. Lockshin is a preeminent expert in the long-term care of chronically ill patients. His research enabled him to develop special expertise in solving health-care issues associated with systemic lupus erythematosus, antiphospholipid antibody syndrome and other autoimmune diseases that predominantly affect women.</span></p><p><span>Dr. Lockshin continues to conduct research and engage in mentoring activities at HSS. His current work focuses on diagnostic uncertainty, the topic of an international conference he organized last year. He has also published a white paper and written a book on the subject.</span></p><p><span><strong>Melanie Smith, MD, PhD</strong></span></p><p><span>Dr. Smith describes her Distinguished Fellow award as an incredible honor. “It is a testament to the opportunities I have had during my fellowship training and the amazing group of mentors that have supported my development as both a physician and a scientist,” she said. “I am excited to embark on a career dedicated to understanding mechanisms of disease with the goal of improving care for our patients.”</span></p><p><span>Dr. Smith, now a staff rheumatologist at HSS, specializes in treating inflammatory arthritic conditions such as rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, Sjogren's syndrome and gout/pseudogout. She also treats patients with osteoarthritis. Her research focuses on how cells that make up the healthy joint lining change when an individual develops rheumatoid arthritis.</span></p><p><span><strong>Adena Batterman, LCSW, MSW</strong></span></p><p><span>Adena Batterman, recognized with the ARP Distinguished Clinician Award, is senior manager of the Inflammatory Arthritis Support and Education Programs at HSS. “I feel honored and grateful to be recognized for clinical work and research that enhances the lives, and elevates the voices, of patients,” she said. “I’m grateful to be engaged in work made possible by colleagues and mentors that continues to have deep personal and professional meaning to me.”</span></p><p><span>Ms. Batterman develops and oversees support and education programs designed to address the multifaceted needs of patients with inflammatory arthritis during the many stages of their illness. She underscored the importance in counseling women to have inactive disease when they contemplate pregnancy, as others have shown that active disease is associated with poor outcomes such as preeclampsia, fetal death, and maternal flares of lupus. Phase I and II involved generating 261 candidate criteria for defining APS and reducing the list to 27 finalist criteria, grouped into six specific categories: laboratory tests, macrovascular, microvascular, obstetric, cardiac and hematologic. The ultimate goal is to develop a threshold-based scoring system that will allow researchers worldwide to determine which patients most likely have APS and should be included in research studies.</span></span></p><p><span><span><a href="https://www.hss.edu/condition-list_antiphospholipid-syndrome.asp">APS</a> is an autoimmune disorder in which the immune system produces antibodies against specific normal proteins in the blood, causing clots in blood vessels, low platelet counts, heart valve abnormalities and pregnancy complications. It is the leading cause of stroke in young people, causing one in three strokes in people under the age of 50, one in five recurrent miscarriages and up to 20 percent of all <a href="https://www.hss.edu/condition-list_deep-vein-thrombosis.asp">deep vein thromboses</a>. The disease is often found in patients with other autoimmune conditions, especially <a href="https://www.hss.edu/condition-list_lupus-sle.asp">lupus</a>.</span></span></p><p><span><span>The current APS Sapporo Classification System was published in 1999, validated in 2000, and revised in 2006. &ldquo;The previous classification system was based on the available evidence at the time,&rdquo; says lead author <a href="https://www.hss.edu/physicians_barbhaiya-medha.asp">Medha Barbhaiya, MD, MPH</a>, rheumatologist at the <a href="https://www.hss.edu/barbara-volcker.asp">Barbara Volcker Center for Women and Rheumatic Diseases</a> at HSS and assistant professor of medicine and population health sciences at Weill Cornell Medicine. &ldquo;In the last decade, researchers have identified additional features of APS that deserve inclusion. We also now have more rigorous methods for developing and validating disease classification criteria.&rdquo;</span></span></p><p><span><span>The international initiative is jointly funded by the American College of Rheumatology (ACR) and the European League Against Rheumatism (EULAR), in recognition of the need for improving the method of classifying patients for APS research to be used worldwide. <a href="https://www.hss.edu/physicians_erkan-doruk.asp">Doruk Erkan, MD, MPH</a>, rheumatologist at the <a href="https://www.hss.edu/barbara-volcker.asp">Barbara Volcker Center for Women and Rheumatic Diseases</a> at HSS and associate professor of medicine at Weill Cornell Medicine, is the senior author and co-principal investigator together with Stephane Zuily, MD, MPH, PhD, a vascular medicine specialist and professor of medicine at the Universit&eacute; de Lorraine, Nancy, France.</span></span></p><p><span><span><span><span>Emphasizing the difference between</span></span> <a href="https://www.hss.edu/conditions_top-ten-points-classification-versus-diagnosis-antiphospholipid-syndrome-how-are-they-different.asp"><span>classification versus diagnosis criteria for APS</span></a><span><span>, Dr. Erkan says, &ldquo;Classification criteria are intended to be used in a research setting whereas diagnostic criteria are used in a clinical setting for the purpose of managing diseases; the two sets of criteria may have overlapping elements, but they have separate goals. The goal of classification criteria is to ensure that patients with common characteristics, thought to be specific for a certain disease, are included in appropriate research studies.</span></span> <span><span><span>In contrast, the</span></span></span> <span><span><span>goal of diagnostic criteria is to identify, as accurately as possible, whether patients have that particular disease.&rdquo;</span></span></span></span></span></p><p><span><span>The next phase of the new APS classification criteria methodology involves defining, refining, and weighting the candidate criteria and assigning a threshold score for identifying APS using real-world cases and a computer-based decision tool. Following the final validation phase of the project, researchers plan to report their results in 2021. Salmon, MD</a>&nbsp;discussed new evidence derived by small numbers of the Phase II IMPACT (Improve Pregnancy in APS with Certolizumab Therapy) trial, which suggests treatment with the TNF-inhibitor certolizumab could help improve high-risk pregnancy outcomes in women with antiphospholipid syndrome (with or without systemic lupus erythematosus) and lupus anticoagulants.</span></span></p><p><span><span>Dr. Salmon, who is also the co-lead of the IMPACT trial, presented this new line of research during the Rheumatology Research Foundation Memorial Lectureship session, to honor Shaun Ruddy, MD, at the virtual American College of Rheumatology annual meeting.</span></span></p><p><span><span>Certolizumab is already approved for the treatment of rheumatoid arthritis (RA), psoriatic arthritis, ankylosing spondylitis and Crohn&rsquo;s disease, and isn&rsquo;t transferred across the placenta. Doses of certolizumab were similar to those used in RA and Crohn&rsquo;s disease, with the first dose at eight weeks and six days of gestation to target abnormal development of the placenta, which starts early and is usually found at 12&ndash;15 weeks. Certolizumab is discontinued at 28 weeks, after which benefit is unlikely.</span></span></p><p><span><span>With a trial target of 50 participants, 27 patients have been enrolled, 23 of whom have completed pregnancies that can be evaluated. Only three&mdash;or 13%&mdash;of the women experienced a primary outcome, far lower than the 44% expected. One fetal death occurred at 10.3 weeks of gestation, along with two cases of preeclampsia with births at less than 34 weeks.</span></span></p><p><span><span>&ldquo;Though it&rsquo;s a small number, we are quite optimistic and looking forward to continuing the trial,&rdquo; said Dr. Salmon. &ldquo;Treatments to prevent poor pregnancy outcomes require an understanding of mechanisms of injury, and one needs animal studies and human observational studies to develop that understanding. <p><span><span><span>The research focuses on the diagnosis of renal disorders, the risk of venous thromboembolism after total knee replacement (TKR), and the care of pediatric and young adult patients with rheumatologic diseases. There are also studies related to the care of rheumatology patients during the COVID-19 pandemic.</span></span></span></p>

<p><span><span><span><span>A study led by HSS rheumatologist</span> <a href="https://www.hss.edu/physicians_mehta-bella.asp"><span>Bella Mehta, MBBS, MS</span></a><span>, looked at <span><span>the perceptions and behaviors of rheumatologists in the United States regarding the risk of COVID-19 for patients with autoimmune disorders and how those patients were managed with immunosuppressive and anti-inflammatory medications.&nbsp;The</span></span> <span>271 respondents were asked whether patients with rheumatic diseases were at higher risk of COVID-19 and whether the pandemic had led them to reduce the use, dosage, or frequency of biologics or steroids. Overall, the survey found geographical variations regarding perceptions of patients&rsquo; risk of COVID-19, with responses that were significantly different in the Northeast region of the United States compared with other regions.</span></span></span></span></span></p>

An Analysis of 2 Early RA Cohort Studies</span></b></a>&nbsp;</span></span></span></p>

<p><span><span><span>A team that included HSS rheumatologist</span> <a href="https://www.hss.edu/physicians_bykerk-vivian.asp"><span>Vivian P. Bykerk, BSc, MD, FRCPC,</span></a> <span>looked at data from two cohort studies on patients with early rheumatoid arthritis (RA) that compared patient and physician assessment of tender and swollen joints. This research is important because COVID-19 has forced rheumatologists to shift from in-person clinical visits to telemedicine, which limits their ability to carry out complete joint exams. The study found that patient and physician assessments had good agreement when it came to identifying active versus controlled disease activity. In addition, the researchers reported that predictors of higher discrepancies between patient and physician assessments may help identify patient subsets that could benefit the most from more physician-guided training at joint self-assessments as well as more probing questioning during telehealth visits to confirm active synovitis.</span></span></span></p>

<p><span><span><a href="https://acrabstracts.org/abstract/characterization-of-antiphospholipid-antibody-associated-nephropathy-an-international-survey-of-renal-pathology-society-members/"><b><span>Characterization of Antiphospholipid Antibody-Associated Nephropathy: An International Survey of Renal Pathology Society Members</span></b></a> </span></span></p>

<p><span><span><span>A survey led by HSS rheumatologist</span> <a href="https://www.hss.edu/physicians_barbhaiya-medha.asp"><span>Medha Barbhaiya, MD, MPH</span></a><span>, sought to determine whether pathologists worldwide use uniform criteria to distinguish antiphospholipid antibody-associated nephropathy (aPL-N) from other forms of thrombotic microangiopathy. The study was done in parallel with an international effort to develop new classification criteria for antiphospholipid syndrome. The web-based survey included 780 members of an international Renal Pathology Society; 111 renal pathologists responded, representing 33 countries. The participants were asked to determine whether two acute and eight chronic aPL-N features were consistent with acute or chronic aPL-N without and were not provided with serology information. Results from the survey indicated that more than 90% of pathologists worldwide agreed that the renal pathologic features most specific for aPL nephropathy are noninflammatory glomerular or small arterial microthrombi and organized microvascular thrombi with recanalization. In the absence of serologic data, more than 75% of pathologists indicated lack of specificity of chronic glomerular or small arterial changes. These findings indicate the importance of serologic test results in biopsy interpretation and suggest higher specificity for certain acute or chronic features.</span></span></span></p>

<p><span><span><a href="https://acrabstracts.org/abstract/the-risk-of-venous-thromboembolism-after-septic-total-knee-replacement-tkr-revision-is-double-the-risk-after-aseptic-tkr-revision-an-analysis-of-administrative-discharge-data/"><b><span>The Risk of Venous Thromboembolism After Septic Total Knee Replacement (TKR) Revision Is Double the Risk After Aseptic TKR Revision: An Analysis of Administrative Discharge Data</span></b></a></span></span></p>

<p><span><span><span><span>A team led by HSS rheumatologist</span></span> &nbsp;<a href="https://www.hss.edu/physicians_bass-anne.asp"><span>Anne R. Bass, MD</span></a><span>, <span>assessed the 90-day risk of postoperative venous thromboembolism (VTE) in people who underwent revision TKR surgery. Although it was already known that infection can increase the risk of thrombosis by triggering coagulation pathways, the impact of infection in this context was not well understood. The study, which included 25,441 of these surgeries performed between 1998 and 2014, used data from the New York Statewide Planning and Research Cooperative System. Overall, 69% of these revisions were for mechanical causes (aseptic), 28% were for infection (septic), and 3% were related to a periprosthetic fracture. The researchers found that the risk of VTE after septic revision surgery was double the risk after aseptic revision surgery, and was 2.6-fold higher after revision surgeries for fracture. These findings point to the importance of taking infection and fracture status into account when planning VTE prophylaxis for patients having revision surgery.</span></span></span></span></p>

<p><span><span><a href="https://acrabstracts.org/abstract/using-a-patient-engaged-approach-to-identify-cross-cutting-disease-factors-impacting-mental-health-in-youth-with-rheumatologic-disease-2/"><b><span>Using a Patient-Engaged Approach to Identify Cross-Cutting Disease Factors Impacting Mental Health in Youth with Rheumatologic Disease</span></b></a></span></span></p>

<p><span><span><span>A team including HSS chief of pediatric rheumatology</span> <a href="https://www.hss.edu/physicians_onel-karen.asp"><span>Karen Brandt Onel, MD</span></a><span>,</span> <span><span><span>examined the mental health experiences of those with juvenile arthritis, juvenile dermatomyositis, or systemic lupus erythematous. It included patients aged 14 to 24 and parents of patients between the ages of 8 and 24. The primary outcome was the presence of any clinical or self-diagnosed mental health problem. The survey included information on cross-cutting disease factors such as disease duration, active disease status, current steroid medication, history of disease flare following remission, and appearance-altering comorbidities (such as psoriasis, stretch marks, alopecia, skin ulceration, and visible scarring). The researchers also examined results by mental health problem (depression, anxiety, and self-harm/suicidal ideation). Overall, they found that certain rheumatologic disease factors, including those that alter appearance, are predictive of mental health problems such as depression or anxiety. The findings will help identify targets for mental health screening in youth with rheumatologic diseases.</span></span></span></span></span></p>

<p><span><span>A team that included HSS pediatric rheumatologist</span></span> <span><span><a href="https://www.hss.edu/physicians_pan-nancy.asp"><span>Nancy Pan, MD</span></a></span></span><span><span>, reported highlights of new quality measures for the treatment of juvenile idiopathic arthritis. The study also provided information about the performance of these new measures, which were created by the Pediatric Rheumatology Care and Outcomes Improvement Network. The measures take into consideration clinical health importance, scientific validity, and feasibility, as well as best practices. They include both clinical evaluations based in part on the clinical Juvenile Arthritis Disease Activity Score and patient-reported outcomes such as pain, physical function, and overall well-being. ]]></description><content:encoded><![CDATA[<p>The American College of Rheumatology (ACR) has published its first-ever Reproductive Health in Rheumatic and Musculoskeletal Disease guideline, which aims to provide valuable guidance for the evaluation and management of reproductive health challenges for women across the spectrum of rheumatic diseases.</p>

<p>Autoimmune and inflammatory diseases are especially prevalent among women, particularly those in their childbearing years. For example, systemic lupus erythematosus (SLE) is approximately nine times more common in women, and three times as many women as men are diagnosed with rheumatoid arthritis.</p>

<p>Even though an increasing number of patients are having successful pregnancies, certain complications are more common in patients with rheumatologic disorders, and certain interactions between the diseases and pregnancy can be problematic. One such concern is preeclampsia, which occurs in some mothers with SLE or antiphospholipid syndrome and can be life-threatening. Another is increased risk of fetal death or low birth weight. All of these medical challenges require expert knowledge and vigilance by rheumatologists, in collaboration with obstetricians and gynecologists. Yet until now, no formal guidance had been available to support the management of the spectrum of reproductive health issues encountered by women with rheumatic diseases.</p>

<p>The guideline was created by a group of experts, including members of the HSS faculty, who conducted a systematic review and evaluation of the literature in the area of reproductive health and rheumatic and musculoskeletal diseases. The group voted on hundreds of specific recommendations. Pregnancy is a major focus of the guideline, which also addresses contraception, assisted reproductive therapies, fertility preservation therapies and use of hormone replacement therapy. The recommendations also include conditional guidance for clinical situations that have not yet been sufficiently addressed in research studies.</p>

<p>An important message from the guideline is that patients with rheumatic diseases should plan for pregnancy accordingly, which means using safe and effective contraception during periods of active disease flares. Planning a pregnancy should be done when rheumatic disease, especially lupus, is quiet and has been quiet for six months.</p>

<p>Like with other ACR guidelines, this new <a href="https://www.rheumatology.org/Portals/0/Files/Reproductive-Health-Guideline-Final-2020.pdf">guideline</a> will be revised and updated as future studies provide more information.&nbsp;</p>
